A Case-Control Study on Dietary Quality and Risk for Coronary Heart Disease in Korean Men (한국 남성에서 식사의 질과 관상동맥질환 발생 위험에 대한 환자-대조군 연구)

  • The purpose of this study was to investigate the association between dietary quality and the risk of coronary heart disease (CBD) among Korean men by a case-control study. Methods: The cases were 108 male patients with a first acute myocardial infarction or a new diagnosis of angina pectoris who were admitted to a university teaching hospital in Seoul. Controls were 142 age-matched patients admitted to the departments of ophthalmology and orthopedic surgery at the same hospital. Dietary information was assessed by a nutritionist using a semiquantitative food frequency questionnaire (FFQ), and dietary quality index (DQI) scores were calculated. Results: The intakes of total fat and cholesterol and body mass index (BMI) in cases were significantly higher than those in controls. The mean DQI scores were 8.6 for the cases and 9.4 for the controls. A higher DQI, which was indicative of a better quality diet, was inversely associated with the risk of CHD when comparing the highest to lowest quatiles, but borderline significant (odd ratios 0.60, 95% confidence interval 0.25-1.39, P for trend = 0.05). Conclusions: Our findings suggest that better dietary quality may reduce the risk of CHD among Korean men.

  • Anoctamin 1 (ANO1) is an anion channel that is activated by changes in cytosolic $Ca^{2+}$ concentration and noxious heat. Although the critical roles of ANO1 have been elucidated in various cell types, the control of its gating mechanisms by $Ca^{2+}$ and heat remain more elusive. To investigate critical amino acid residues for modulation of $Ca^{2+}$ and heat sensing, we constructed a randomized mutant library for ANO1. Among 695 random mutants, reduced $Ca^{2+}$ sensitivity was observed in two mutants (mutant 84 and 87). Consequently, the E143A mutant showed reduced sensitivity to $Ca^{2+}$ but not to high temperatures, whereas the E705V mutant exhibited reduced sensitivity to both $Ca^{2+}$ and noxious heat. These results suggest that the glutamic acids (E) at 143 and 705 residues in ANO1 are critical for modulation of $Ca^{2+}$ and/or heat responses. Furthermore, these findings help to provide a better understanding of the $Ca^{2+}$-mediated activation and heat-sensing mechanism of ANO1.

The Effect of Platform Screen Doors on PM10 Levels in a Subway Station and a Trial to Reduce PM10 in Tunnels

  • $PM_{10}$ concentrations were measured at four monitoring sites at the Daechaung station of the Seoul subway. The four locations included two tunnels, a platform, and a waiting room. The outside site of the subway was also monitored for comparison purposes. In addition, the effect of the platform screen doors (PSDs) recently installed to isolate the $PM_{10}$ in a platform from a tunnel were evaluated, and a comparison between $PM_{10}$ levels during rush and non-rush hours was performed. It was observed that $PM_{10}$ levels in the tunnels were generally higher than those in the other locations. This might be associated with the generation of $PM_{10}$ within the tunnel due to the train braking and wear of the subway lines with the motion of the trains, which promotes the mixing and suspension of particulate matter. During this tunnel study, it was observed that the particle size of $PM_{10}$ ranged from 1.8 to 5.6 ${\mu}m$. It was revealed that the $PM_{10}$ levels in the tunnels were significantly increased by the PSDs, while those in the platform and waiting room decreased. As a result, in order to estimate the effect of ventilation system on $PM_{10}$ levels in the tunnels, fans with inverters were operated. It was found that the concentration of $PM_{10}$ was below 150 ${\mu}g/m^3$ when the air flow rate into a tunnel was approximately 210,000-216,000 CMH.

  • Phase I of the Trials of Hypertension Prevention was a multicenter, randomized, controlled trial designed to determine the efficacy of seven nonpharmacologic interventions in reducing blood pressure among persons with high-normal diastolic blood pressure. the initial goal for recruitment was to enroll 2,100 participants over a nine-month period. The yield from the first screening visit to randomization was 13% overall, with clinic-specific yields ranging from 4.5% to 31.7%. After five months of recruitment, approximately 60% of the goal for that point in the recruitment timetable had been randomized. Clinical centers falling short of their goals at that time altered their recruitment strategies and intensified their efforts, and centers that had exceeded their goals recruited additional participants. As a result, 2,182 participants, or 104\% of the goal for recruitment, were randomized over a 13-month period. Those clinics using a cohort, or wave, type of enrollment were most successful in achieving their recruitment goals within the prescribed timetable.

A study on implementation of adult learner-targed college based on case study of foreign universities: Focused on cases of US and UK (해외 사례분석을 통한 성인전담 단과대학 운영방안: 미국과 영국 대학을 중심으로)

  • The purpose of this study is to seek practical implementation of adult learner-targeted college. For this, it first of all pointed out expanding opportunities of adult learners to learn and limitation of current lifelong education system at higher education level as well as lifelong learning centered university project. Then it selected and examined 3 university cases abroad, such as Harvard university, the university of Chicago in US and Warwick university in UK in terms of their curriculum and operating method for adult learners. In addition to the literature and case studies, this study did interviews on 5 professors with major of lifelong education and 5 managerial experts of university to suggest practical operating approach of adult learner-targeted college, which is as following. University's philosophy should be geared to meet public responsibility, specialization and persistency of the system and proper measurement be followed such as various range of admission track, prior education courses, recognition of prior learning experience, major, teaching method, curriculum and extra adult learner tailored service including financial support. Based on the study result, practical implications were suggested.

Long-term results of forward intensity-modulated radiation therapy for patients with early-stage breast cancer

  • Purpose: To observe long-term clinical outcomes for patients with early-stage breast cancer treated with forward intensity-modulated radiation therapy (IMRT), including local control and clinical toxicities. Materials and Methods: We retrospectively analyzed a total of 214 patients with stage I-II breast cancer who were treated with breast conserving surgery followed by adjuvant breast radiation therapy between 2001 and 2008. All patients were treated using forward IMRT. The whole breast was irradiated to a dose of 50 to 50.4 Gy followed by an 8 to 12 Gy electron boost to the surgical bed. Results: The median age was 46 years (range, 21 to 82 years) and the medial follow-up time was 7.3 years (range, 2.4 to 11.7 years). Stage T1 was 139 (65%) and T2 was 75 (35%), respectively. Ipsilateral breast recurrence was observed in 3 patients. The 5- and 10-year local control rates were 99.1% and 97.8%, respectively. The cosmetic outcome was evaluated according to the Harvard scale and 89.4% of patients were scored as excellent or good. Conclusion: The whole breast radiation therapy as an adjuvant treatment using a forward IMRT technique showed excellent long-term local control as well as favorable outcomes of toxicity and cosmesis.

Gender-specific cephalometric features related to obesity in sleep apnea patients: trilogy of soft palate-mandible-hyoid bone

  • Background: The aim of this study is to investigate the relationship between gender-specific and obesity-related airway anatomy in patients with obstructive sleep apnea (OSA) by using cephalometric analyses. Methods: We retrospectively evaluated 206 patients with suspected OSA undergoing polysomnography and anthropometric measurements such as body mass index, neck circumference, and waist-hip ratio. We checked lateral cephalometry to measure tissue landmarks including angle from A point to nasion to B point (ANB), soft palate length (SPL), soft palate thickness (SPT), retropalatal space (RPS), retrolingual space (RLS), and mandibular plane to hyoid (MPH). Results: Male with OSA showed significantly increased SPL (P = .006) compared with controls. SPL and MPH had significant correlation with apnea-hypopnea index (AHI) and central obesity. Female with OSA showed significantly increased ANB (P = .013) and SPT (P = .004) compared with controls. The receiver operating characteristic curves revealed that SPT in male and ANB and SPT in female were significant in model 1 (AHI ≥ 5) and model 2 (AHI ≥ 15). MPH was also significant for male in model 2. Conclusion: Male and female with OSA had distinct anatomic features of the upper airway and different interactions among soft palate, mandible, and hyoid bone.

Acupuncture Stimulation for Motor Cortex Activities: Evidence from 3T Functional MRI Study

  • The purpose of this study was to investigate whether or not acupuncture of GB34 produces a significant response of the modulation of somatomotor areas by functional magnetic resonance imaging (fMRI) study. The acupoint, GB34, located in the back of the knee, is known to be effective in recovering motor function after stroke. Using 3T MRI scanner, functional MR imaging of the whole brain was performed in 12 normal healthy subjects during two stimulation paradigms; acupuncture manipulation on GB 34 and sham points. This study investigates the activation of the mortor cortex elicited by a soft and an intensified stimulation of GB 34. Three different paradigms were carried out to detect any possible modulation of the Blood Oxygenation Level Dependent (BOLD) response in the somatomortor area to motor stimulation through acupuncture. Group analysis from seven individuals showed that bilateral sensorimotor areas (BA 3,4,6 and 7) showed stimulation related BOLD signal contrast of approximately 6% whereas very few areas were activated when sham stimulation is given. The present study shows that acupuncture fMRI study can be safely conducted in 3T MRI environment, and acupuncture stimulation in GB34 modulates the cortical activities of the somatomotor area in human. The present findings may shed light on the CNS mechanism of motor function by acupuncture and form a basis for future investigations of motor modulation circuits in the stroke patients.

Impact of Hunger Experiences in Childhood or Adolescence on Diabetes among Korean Elders (한국 노인의 당뇨병과 소년기 굶주림 경험)

  • Objectives: To investigate the relationship between experience of hunger in childhood or adolescence and diabetes mellitus in old age. Methods: Stratified national samples of 421 men and 554 women aged 60-89 in South Korea were analyzed. Diabetes diagnosed by a doctor was used as the dependent variable. Experience of hunger in childhood was assessed with the question, "In your childhood, have you ever gone hungry with skipping a meal?" and "In your childhood, have you ever eaten porridge or some bran cake as main dish instead of rice because of lack of food?" Odds ratios (ORs) and 95% confidence interval (CI) were calculated by multiple logistic regressions. Results: ORs of diabetes by experience of hunger in childhood were 1.64(95% CI 0.95-2.83) for men and 1.58(95% CI 1.01-2.47) for women. Significant association of experience of hunger in childhood with prevalence of diabetes persisted in men after adjustment for adulthood and old age socioeconomic position indicators and other risk factors(OR 1.88(95% CI 1.04-3.41)). The effect of experience of hunger in 10-19 years was more prominent than that in less than 10 years in both gender. Conclusion: Experience of hunger in childhood or adolescence may increase the risk of diabetes in Korean elders.

Characteristics of Nitrate (NO3-) Volatilization from Fine Particles (PM2.5) at 4 Measurement Sites in Seoul (미세입자(PM2.5)의 질산염 (NO3-) 휘발 특성 분석 -서울시 4개 측정지역을 중심으로-)

  • The purpose of this study is to understand the impact that temperature and relative humidity have on the volatilization loss of particulate nitrate $(NO_3^-)$ from Teflon filters during measurements of ambient fine particles $(PM_{2.5})$. Fine particles $(d_p<2.5{\mu}m)$ were measured using an annular denuder system (ADS) at four representive areas in Seoul. The measurements were made during 28 different days at 24-hr sampling intervals from February 14 to October 15, 1997. In this study, nitrate losses. calculated by the ratio of nitrate on the nylon filter to their sum in both Teflon and nylon titters, varied seasonally in the following order: summer (45.5%) > spring (23.8%) > fall (20.6%) > winter (19.7%). The results showed strong correlations with temperature, but we did not observe any significant effects of relative humidity. However, we observed that both temperature and relative humidity influenced the ambient gas/particle nitrate ratio in a different case study using a denuder.
