무지 외반증 환자의 hard insole 착용에 의한 중족지절관절 각도의 변화 (The Effect of Hard Insole on Metatarsophalangeal Joint in Patients With Hallux Valgus)

  • The purpose of this study was to investigate the effect of donning of a hard insole in patients with hallux valgus. Fourteen subjects were selected from patient with foot pain at Lee Chang-Heon Foot Clinic from August 4, 2000 to September 15, 2000. The hallux valgus angle and the first-second intermetatarsal angle were radiographically measured before and after donning the hard insole. Based on these two kinds of angles, a mild hallux valgus deformity group was characterized by the hallux valgus angle of less than 20 degrees, and a moderate hallux valgus deformity group was characterized by the hallux valgus angle of 20 to 40 degrees. After three weeks with the hard insole donned, the foot angles of the patients with hallux valgus were measured again. The data were analyzed by Wilcoxon signed ranks test, and the following results were obtained: 1) After the trial, both mild hallux valgus deformity group and moderate hallux valgus deformity group demonstrated that the hallux valgus angles were significantly decreased. 2) After the trial, mild hallux valgus deformity group demonstrated that the first-second intermetatarsal angle was significantly decreased. 3) After the trial, moderate hallux valgus deformity group demonstrated that the first-second intermetatarsal angle was not significantly decreased. The above findings revealed that according to donning hard insole, the hallux valgus angles of mild and moderate hallux valgus deformity groups and the first-second intermetatarsal angle of mild hallux valgus deformity group were significantly decreased. The results of this study have some limitation for generalization due to the limited number of subjects. Further studies are needed to evaluate the effect of hard insole on hallux valgus with more precise laboratory equipments and measurements in patients with hallux valgus.

Management for Gait Disturbance and Foot Pain in a Patient with Klippel-Trenaunay-Weber Syndrome : A case report

  • Background : Klippel-Trenaunay-Weber syndrome (KTS) is a rare congenital medical condition characterized by complex vascular malformation. KTS consists of a classic triad of capillary malformation (hemangioma), venous malformations and bone or soft tissue hypertrophy causing limb asymmetry. The aim of this report is to describe management for gait disturbance and foot pain in a Patient with KTS using custom-made total contact insole. Case presentation : A 32-year-old man with KTS presented with a 3-year history of gait disturbance on hard surface due to right first toe pain and Achilles tendon tightness. The patient had soft tissue hypertrophy, varicose veins and port-wine stains over the right lower limb associated with KTS. True leg length discrepancy was 2 cm. We prescribed custom-made total contact insole to protect his deformed foot and correct leg length discrepancy. The insole of right side included wedge shaped heel lift and the insole of left side included full length lift to add extra support on unaffected side. Also, we provided compression stocking and physiotherapy including manual lymphatic drainage for lymphedema and stretching exercise for tightness in right lower extremity. At 3 years follow-up, postural alignment including pelvic obliquity was improved using a custom-made total contact insole. The degree of scoliosis and foot pain were also reduced. Conclusion : An individualized and multidisciplinary approach is essential regarding the complexity of comorbidities in patients with KTS. For patients with KTS, orthotic management should be considered to prevent and correct deformities related to KTS. Active orthotic management, compression stocking and physiotherapy can enhance the quality of life and function in patients.

맞춤형 인솔이 산업체 근로자의 작업 후 신체불편지수와 통증에 미치는 영향 (The Effects of Customized Insoles on the Post-Work Discomfort and Pain)

  • In many manufacturing occupations, industrial workers reported foot or lower leg problems such as discomfort, pain or orthopedic deformities. This study investigated the effects of two different working conditions upon assembly worker's perception of discomfort and foot pain associated with various body parts. Twenty-three male volunteers performed work in the factory. Ergonomic intervention has been to modify the flooring in an attempt to alleviate the problems associated with constrained standing and walking work. The worker's standing conditions consisted of standing on a hard floor while wearing shoe insoles. Questions were asked regarding body discomfort and foot pain. Significant differences in body discomfort and foot pain were found when comparing the overall effects of wearing shoe insoles on a hard floor (p<.05). This investigation indicated that shoe insoles reduced body discomfort and foot pain (p<.05).

보행시 지반조건에 따른 팽이기초를 접목시킨 신발 족저압 분포 비교분석 (Top shoes foot pressure basis of the comparison analysis combine conical top foundation walking upon ground conditions)

  • 본 논문은 지반 조건에 따라 현재 개발 중인 팽이기초를 접목한 신발과 일반적인 신발인 워킹화의 보행시 압력분포의 비교분석에 대한 연구이다. 일반적인 신발, 현재 개발 중인 팽이기초를 접목한 신발 두 가지 카테고리의 신발이 본 연구에 사용되었으며, 실험 대상은 260mm를 착용하는 정상발의 조건에 만족하는 15명의 20대 남성을 대상으로 단단한 지반, 모래 지반에서의 실험을 실시하였다. 보행 시 압력의 측정은 Techstorm사의 무선 Insole System 을 사용하여 측정하였으며 발의 7 Zone에서 족저압을 측정하였다. 연구 결과 신발과 지반 조건에 따라 다른 최대 힘, 평균 압력, 압력 분포도를 나타냈으며, 본 연구 결과 단단한 지반 및 모래 지반에서 발 부위에 따라 족저압 분산 효과가 일반적인 신발과 현재 개발 중인 팽이기초를 접목한 신발이 서로 상이한 것을 확인할 수 있었다. 향후 보다 지속적인 연구를 통해 다양한 고무 소재를 선택하여 추가적인 실험을 통해 단단한 지반 및 모래 지반에서 모두 착용 가능한 신발의 개발에 유용하게 사용될 것으로 기대된다.

런닝화의 미드솔 경도가 하지 근육의 피로와 충격력에 미치는 영향 (The Effect of Midsole Hardness of Running Shoe on Fatigue of Lower Extremity Muscles and Impact Force)

  • Objective: The aim of this study was to investigate the effect of midsole hardness of running shoe on muscle fatigue and impact force during distance running. Method: Ten healthy college recreational runners who were performing distance running at least three times a week participated in this experiment. They were asked to run for 15 minutes in the treadmill at 10 km/h with running shoes having three different types of midsole hardness (Soft, Medium, Hard). EMG signal and insole pressure were collected during the first and last one minute for each running trials. Data were analyzed using a one-way analysis of variance (ANOVA) with repeated measures. Results: Midsole hardness did not affect the consistency of stride length. For the median frequency of the EMG signal, only VL was affected by midsole hardness; that of medium was greater than other midsoles (p<.05). The loading rate of impact forces increased by midsole hardness (p<.01). Conclusion: Although soft midsole could attenuate impact forces at heel contact, it might have a negative effect on the fatigue of muscle which could decelerate the body after heel contact. Therefore, it is necessary to select the optimum hardness of midsole carefully for both reduction impact forces and muscle fatigue.
