Changes in the Cross-Sectional Area of the Abductor Hallucis During the Toe-Spread-Out Exercise at Different Ankle Positions (다양한 발목각도에 따른 발가락벌리기 운동이 엄지벌림근 단면적에 미치는 영향)

  • PURPOSE: This study examined the changes in the cross-sectional area (CSA) of the abductor hallucis muscle during various ankle positions while performing toespread-out (TSO) exercise. METHODS: Thirty subjects with an average age of 22.1 years were recruited for this study. All subjects were firstly measured for the angle of their first metatarsophalangeal joint using a goniometer. Those with angles greater than 15° were allocated to the HV group. The remaining subjects were placed in the normal group. The CSA of the abductor hallucis was measured by ultrasound in the resting position with no movement and three ankle positions: neutral (0°), plantarflexion (30°), and dorsiflexion (30°). All data were analyzed using a two-way mixed analysis of variance between the groups (normal and HV group) and within the groups (resting, neutral, plantarflexion, and dorsiflexion) to determine the group x position interaction effects. RESULTS: During TSO exercise in the normal group, the CSA of the abductor hallucis was significantly greater in both the plantarflexion and neutral positions compared to the resting position (padj < .01), and plantarflexion was significantly greater than the dorsiflexion position (padj < .01). During the TSO exercise in the HV group, the CSA of the abductor hallucis for plantarflexion was significantly greater than it was for the resting, neutral, and dorsiflexion positions (padj < .01). CONCLUSION: Based on the above results, the TSO exercise in plantarflexion is an effective rehabilitation exercise for subjects with HV.

Radiographic Evaluation on the Forefoot: Based on the Two Different Data (1982 and 2004) (전족부의 방사선학적 관찰: 1982년과 2004년의 차이를 중심으로)

  • Purpose: We compared the bony arrangements of the forefoot in 2 different years, 1982 and 2004, to determine any changes with time period of 22 years. Materials and Methods: The radiographs of 200 normal Korean adults, 100 male and 100 female volunteers, were evaluated both in 1982 and 2004. The radiographic results were evaluated with as follows; hallux valgus angle (HVA), intermetatarsal angle (IMA), morphology of metatarsal head and relative anterior protrusion of the metatarsals and the phalanges. The mean ages were 38 years (23-52years) in 1982, and 37 years (24-50years) in 2004. Results: The mean of HVA decreased from 15.6 degrees in 1982 to 14.3 degrees in 2004 (p=0.047), and the mean of IMA increased from 8.0 degrees in 1982 to 9.4 degrees in 2004 (p=0.031). The morphology of metatarsal head and relative anterior protrusion of the metatarsals and the phalanges were not different between the two study years. Conclusion: Comparing with those of 1982 measurements, we found an increase of IMA and a decrease of HVA. A prospective study may be needed to illuminate course of the changes.

A Study on the Foot and Ankle Disease of Shelter-based Homeless People (노숙인 거주 시설의 족부 족관절 실태에 대한 조사)

  • Purpose: Foot and ankle disease (FAD) is a frequent cause of morbidity among the homeless population. Various conditions, exacerbated by malnutrition, poor lifestyle habits, psychiatric disorders, physical injuries, poor hygiene, and limited access to healthcare, have been described in this population. The purpose of this study was to investigate the cognition and management status of FAD in shelter based homeless people. Materials and Methods: Fifty-two male and twenty-five female volunteer homeless individuals were recruited from two homeless shelters. Each person completed a questionnaire assessing any presence of pain, pain management, as well as foot and ankle care status. A foot and ankle surgeon examined the physical status of the individual's foot and ankle, including tenderness and instability. A radiologic evaluation was done for 18 male and 11 female homeless people who agreed to participate in this test for the existence osteophyte or joint space narrowing representing osteoarthritis and some reference angles for hallux valgus, flatfoot and cavus foot. Results: Homeless people had higher prevalence of body mass index, diabetes, and smoking than the general population. The most prevalent infectious disease was fungal infection (male 78%, female 68%), with a low compliance of management for FAD. Conclusion: Although most of shelter-based homeless people showed an acceptable foot and ankle management status, the potential risk for FAD development and exacerbation of mild FAD was high.

Comparative Analysis of Plantar Pressure and Comfort Level in Developed-Domestic and Import Comfort Shoes for Elderly (고령층용 국내 개발 및 수입 컴포트화의 족저압 및 착화감 비교 분석)

  • This study was to compare a domestic comfort shoes to the popular import(SAS$^{(R)}$) to gain a better understanding in biomechanical characteristics for shoe design for the elderly. For each shoe type, morphologic dimensions, shock absorption, and flexibility were assessed. From subjects(n = 20, $72.4{\pm}5.5$ years, $67.7{\pm}7.9$ kg), mean peak pressure(MPP) and contact area(CA) at the plantar surface were analyzed. The domestic shoes reflecting anatomic contour of the plantar surfaces of Asians resulted in wider CA(by 30.4 $cm^2$), higher shock absorption(by 2.4%) and stiffness(by 0.5 N/mm) than the import. With the domestic shoes, significantly less MPPs were found at the forefoot(by 42~49%) regions(p < .05) and higher CA was noted additionally at hallux and lessor toes(by 26~63%). More anatomically-contoured insole and favorable mechanical characteristics may help reduce the plantar pressures more effectively and more evenly, especially across the central forefoot and midfoot regions of the plantar surface, especially for the design of the comfort shoes for the elderly.

The Effects of Asymmetric Bag Carrying during Walking on Plantar Pressure (보행 시 비대칭성 가방 휴대가 족저압에 미치는 영향)

  • PURPOSE: The purpose of the present study was to examine changes caused by asymmetric bag carrying methods to carry the bag with one shoulder only to plantar pressure during walking. METHODS: Twenty three normal adults without any gait problem participated in the present study. Experimental conditions used consisted of walking without carrying any bag(condition 1), walking wearing a bag on both shouders (condition 2), and walking wearing a bag on the right shoulder(condition 3) and the weight of the bag was set to 15% of each subject's body weight. All the subjects were instructed to participate in all experiments under these three conditions and plantar pressures were measured from the subjects' right and left feet using an F-scan system while the subjects were walking under the three conditions. To analyze the measured plantar pressure, the sole was divided into seven areas (Hallux, Toe, Met1, Met23, Met45, Mid foot and Heel) and maximum plantar pressures in individual areas were measured. RESULTS: The results of measurement of plantar pressures under three walking conditions did not show significant changes in any areas of the left and right feet except for the mid foot area of the right food. The asymmetry between the left and right feet was examined and the results showed significant differences only in area Met23 under condition 2 and did not show significant differences in any other areas. CONCLUSION: On comprehensively considering the results of the present study, it could be seen that asymmetric bag carrying did not have large effects on changes in plantar pressure during walking compared to symmetric carrying. The reason for this is considered to be posture adjusting mechanisms against load positions.

A Comparison of the Contact Area between Three Different Correcting Angles after Proximal Crescentic Osteotomy and Ludloff Osteotomy of the First Metatarsal (Preliminary Report) (제1 중족골 근위 반월형 절골술과 Ludloff 절골술 후 교정 각도에 따른 절골편간 접촉 면적 비교(예비보고))

  • Purpose: This study was performed to compare the contact area between three different correcting angles after the proximal crescentic and Ludloff osteotomies of the first metatarsal. Materials and Methods: We used the two sawbone models. Proximal crescentic (PCO) and Ludloff osteotomies (LO) were performed and secured using K-wires under the correcting intermetatarsal angle (IMA) $5^{\circ}$, $10^{\circ}$, and $15^{\circ}$. Then each 6 osteotomized model was scanned five times and measured the contact area using the calculating program. We excluded the highest and lowest values. Results: The mean area of cutting surface was 189 $mm^2$ in PCO, 863 $mm^2$ in LO. The mean contact area (contact ratio; contact area $\times$100/area of cutting surface) of PCO was 149 $mm^2$ (79%) in $5^{\circ}$, 139.5 $mm^2$ (74%) in $10^{\circ}$, 107 $mm^2$ (57%) in $15^{\circ}$ IMA. The mean contact area (contact ratio) of LO was 711 $mm^2$ (82%) in $5^{\circ}$, 535.5 $mm^2$ (62%) in $10^{\circ}$, 330 $mm^2$ (38%) in $15^{\circ}$ IMA. Conclusion: A significant decrease in the contact area and contact ratio according to increase in correcting IMA was noticed in LO. We recommend the PCO rather than LO, when the IMA is needed to correct over $15^{\circ}$.

Treatment of Bunionette Deformity with Diaphyseal Oblique Osteotomy (간부 사형 절골술을 이용한 소건막류의 수술적 치료)

  • Purpose: The purpose of this study is to evaluate the clinical and radiographic results of symptomatic bunionette treated with a diaphyseal oblique osteotomy. Materials and Methods: We retrospectively reviewed 12 feet of nine patients diagnosed as symptomatic bunionette and treated with diaphyseal oblique osteotomy. All patients were female and the average age at the time of surgery was 48 years. We checked the foot standing anteroposterior, oblique, and lateral images pre- and post-operatively. We measured the fourth intermetatarsal angle and fifth metatarsophalangeal angle and evaluated the clinical results using the American Orthopaedic Foot and Ankle Society (AOFAS) lesser metatarsophalangeal-interphalangeal (MTP-IP) scale preoperatively and six months postoperatively. Results: Of the nine patients, hallux valgus was combined with symptomatic bunionette in seven feet of five patients. In all of our cases, the average AOFAS lesser MTP-IP scale showed improvement after surgery. Painful callosity around the fifth metatarsophalangeal joint disappeared after surgery in all of our cases. The fourth intermetatarsal angle improved from $12.7^{\circ}$ to $3.1^{\circ}$ and the fifth metatarsophalangeal angle improved from $16.6^{\circ}$ to $2.3^{\circ}$. Conclusion: Diaphyseal oblique osteotomy of the fifth metatarsal appears to be a safe and satisfactory surgical procedure for treatment of symptomatic bunionette.

The Foot Pressure Change Caused by Functional Leg Length Having an Effect on the Foot Temperature (기능적인 하지길이 차이에 따른 족저압 변화가 족부체열에 미치는 영향)

  • Purpose : The purpose of the research was to analyze foot pressure, foot temperature, and correlation between foot pressure and foot temperature to grasp impact on foot pressure and body temperature distribution chart depending on functional difference of leg length. Method : After measuring leg length, put 15 students whose functional difference of leg length was over 10mm to difference group and 15 students whose functional difference of leg length was under 5mm to normal group and categorize soles of foot into 6 sections of hallux head, 1st metatarsal head, 2-4 metatarsal head, 5 metatarsal head, lateral heel, and then measure by foot pressure analyzer to analyze characteristic of pressure distribution and classify into front of the lower leg, back of the lower leg, soles of foot and measure by body temperature analyzer to analyze by checking body temperature. Result : Weight difference depending on foot pressure and body temperature was bigger when functional difference of leg length was bigger, and it could be confirmed that foot pressure and body temperature of short leg side were higher than those of short leg side. Thus, if difference exists in leg length, weight load on short leg side increases which results in higher foot pressure and body temperature, therefore enabling an assumption that mechanical problem will occur in short leg. Conclusion : When functional leg length inequality, weight bearing and pressure was getting high as a result, temperature was getting high in short leg.

천연기념물 제395호 진주 가진리 새발자국과 공룡발자국 화석산지의 새로운 해석

  • 천연기념물 제395호 진주 가진리 새발자국과 공룡발자국 화석산지는 1997년 5월 20일, 경남과학교육원 신축 공사장에서 당시 경남과학고등학교 교사로 근무하던 백광석(현재 서상중고등학교 교장)에 의해 최초로 발견되어, 수 백 점의 새발자국 화석과 공룡발자국 화석들이 학계에 알려지게 되었다. 지금까지도 세계 여러나라의 중생대 새 발자국 중에서 가장 밀도가 높은 화석산지로 손꼽히고 있으며 많은 학자들의 관심을 받고 있다. 이곳의 지질은 백악기 하양층군 함안층 하부의 적색 이암 또는 셰일과 사암의 호층으로 구성되어 있고, 새발자국 화석들은 적색 이암에서 발견된다. 현재, 이 화석산지는 경남과학교육원 건물 내부에 위치하고 있으며, '화석문화재전시관'이라는 명칭으로 보호 및 관리되고 있기때문에 자연재해와 풍화에 의한 침식을 1차적으로 막고 있다. 이 화석산지는 우리나라에서는 천연기념물 제394호 해남 우항리 화석산지와 함께 현장을 그대로 보존하면서 화석의 보존과 관리를 동시에 할 수 있는 건물을 지어 자연사교육을 진행할 수 있는 곳으로 많은 학생들을 대상으로 우리나라의 소중한 화석을 직접 관찰하고 학습하는 체험교육이 가능하다. 원격조종카메라 시스템을 활용하여 관람객이 직접 자신이 원하는 발자국 화석을 세부적으로 관찰할 수 있게 장치한 interactive system은 과학교육 효과를 증진시킨다. 선행 연구에 의하면, 본 화석산지에서 익룡의 발자국 화석(KS 071)도 공룡이나 새발자국과 같은 층리면에서 서로 겹쳐서 나타난다고 보고되었으나, 본 연구에 의해 다시 조사된 결과 전형적인 익룡의 발자국 화석에서 보이는 분명한 특징들을 발견할 수 없었다. 물갈퀴를 가진 새발자국 가운데에서 Uhangrichnus chuni 와 Jindongornipes kimi 로 기재된 표본들에 대하여서도 새롭게 분석하였다. Uhangrichnus chuni로 기재된 많은 표본들은 II-IV번 발가락 사이의 각, 물갈퀴의 형태, 뚜렷한 hallux 등의 형태적 특징을 근거로 할 때, 대부분이 Ignotorinis yangi로 판단된다. Jindongornipes kimi로 기재된 표본들은 이미 기재된 표본보다 크기가 적어도 10%-25%가 작으며, II-IV번 발가락 사이의 각에서 차이를 보인다.

Additional axial K-wire Fixation for Proximal Crescentic Metatarsal Osteotomy : A Biomechanical Study (반월형 근위 중족골 절골술에 대한 보강적 축성 K-강선 고정술 : 생체역학적 연구)

  • Purpose: Proximal crescentic metatarsal osteotomy(PMO) is one of the most common procedures for correcting moderate to severe degree hallux valgus deformity. Although screw fixation is used for osteotomy site stability, loss of reduction can occur. The purpose of this study is to compare the sagittal plane stability of the conventional crescentic PMO fixed with a single screw with that of the crescentic PMO fixed with 1 screw and 2 supplemental K -wires. Material and Methods: Ten matched pairs of cadaveric foot specimens were used for the proximal crescentic metatarsal osteotomy. For one foot specimen of each pair, crescentic osteotomy was fixed with 4mm long threaded cannulated screw, while the matched pair was prepared by adding two axial 1.6mm K-wires to the conventionally fixed 4mm screw. The extensometer was used to measure the osteotomy gap as the metatarsal head was loaded continuously until failure using a servohydraulic MTS Mini Bionix test frame. The strength of fixation was normalized with the bone mineral density (BMD) of the paired specimen $(N{\times}cm^{2}/gm)$, Result: The average strength of the crescentic PMO with axial K-wire fixation ($458.8cm^{2}/gm$, S.D. 434.3) was significantly higher than the standard crescentic PMO ($367.5cm^{2}/gm$, S.D. 397,9) (p=0.05). Conclusion: Supplemental fixation with two axial K-wires can be added to the crescentic PMO to enhance the initial fixation stability to prevent the loss of reduction or dorsal malunion.
