Effect of Pilocarpine Mouthwash on Xerostomia (구강건조증에 대한 필로카핀 구강양치액의 효과)

  • Xerostomia is subjective feeling of dry mouth, a symptom that may or may not be accompanied by hyposalivation, an objective decrease in salivary flow. There are many causes induced xerostomia like drugs, salivary gland diseases, radiation therapy to the head and neck region, Sjogren syndrome, emotional stress etc. Insufficient salivary flow creates complications with oral candidiasis, dental caries, periodontitis, halitosis, dysgeusia. So finally, these complications lead to an overall decline in quality of life. Managements of xerostomia are eliminating or alterating the etiologic factors, relieving symptoms, preventing or correcting the consequences of salivary dysfunction, treating underlying disease and stimulating salivation. One of the salivation stimulation agents studied to treat xerostomia was the pilocarpine muscarinic agonist. Pilocarpine is one of salivation stimulants, a parasympathomimetic drug and non-selective muscarinic receptor agonist. Systemic pilocarpine has been used to stimulate salivary secretion. But systemic administration of pilocarpine has limitations such as increased risk of side effects and contraindications. Side effects of systemic pilocarpine administration are sweating, urinary and gastrointestinal disturbance, risk of cardiovascular and pulmonary disorders. This drug must be used carefully by patients with controlled asthma, chronic bronchitis, pulmonary or cardiac disease. Patient with acute asthma, narrow angle glaucoma, iritis should not use pilocarpine. Like this, systemic pilocarpine has many limitations. So, many investigators also have looked at the effectiveness of topical pilocarpine. Here we present patients with xerostomia which was relieved by pilocarpine mouthwash.

The relationship between oral health behavior and dental services utilization in Korean adolescents (한국 청소년의 구강보건행태와 치과의료이용과의 연관성)

  • Objectives : This study examined the oral health behavior and dental services utilization of adolescents in order to provide information on how to disseminate the correct oral health behavior to the youth and develop programs for the approach. Methods : The raw data of 'The Third 2007 Youth Health Behavior Online Survey' carried out by the Korea Center for Disease Control and Prevention were analyzed. All survey data collected was processed using SPSS ver 17.0 for data analysis as Frequency analysis, chi-square and Logistic regression analysis. Results : 1.The adolescents who have parents with college degree are 70% higher of dental services utilization than the adolescents counterpart(whose parents don't have degrees). And also the adolescents living in cities are 1.3 times higher use of dental services than others living in countries. 2. In case of drinking soda, students having soda 4 days per week on average are 1.2 times higher use of dental services than who don't. 3.The students brushing teeth after lunch are 0.9 times lower use of dental service than who don't. And the students who have tooth ache whenever they have a meal are 1.6 times higher use of dental service than the students who don't have tooth ache. The students who had a dental health education are 1.4 times higher utilization of dental service than who don't. The difference of using dental service between the students who have gum disease and halitosis and the students who don't is almost none. 4. The students who drink alcohol and smoke 10 days more or less a month are 0.8 ~ 0.9 times lower use of dental service than who don't. Conclusions : The rate of utilizing dental service is higher at students who live with parents having college degrees and the more have oral health behavior, the less use of dental service for treatment. We concluded that more educational program should be developed and vitalized so that students can have oral health.

The Relationship Between Systemic Diseases and Oral Volatile Sulfur Compound (전신질환과 구취의 휘발성 황화합물 상관관계)

  • This study was conducted to investigate the relationship between systemic diseases and oral malodor. The author measured the volatile sulfur compound(VSC) of the patients who visited Pusan National University Health Promote Center for a comprehensive medical testing. The patients were examined gingival bleeding on probing, CPI index, tongue coating. Their systemic diseases were diagnosed by the specialist. 182 patients consisted of 112 males and 70 females. In this study, Oral $Chroma^{(R)}$ was used to measure oral malodor. This equipment could measure the concentration of intraoral VSC (hydrogen sulfide, methyl mercaptan, dimethyl disulfide). All data were analylized using Statistical Package for the Social Science $12.0^{(R)}$. The result of this study was the followings. 1. There was significant difference of numbers of patient who visited health care center according to the VSC concentration level and the Community Periodontal Index, bleeding on probing, tongue coating. 2. The subjects with hyperlipidemia showed the high level of $CH_3SH$ concentration (p=0.036). The concentration of $H_2S$ tends to be high in the group with abnormal findings on pulmonary fuction test(p=0.086). The concentration of $CH_3SH$ in the groups with abnormal findings on lipid profile test(p=0.130) and bone mineral density test(p=0.099) and abdominal ultrasonograpy(p=0.088) tends to be higher than the other group. 3. The concentration of $(CH_3)_2S$ in the group with abnormal findings on blood pressure test(p=0.113), hepatitis B virus serology(p=0.069), Abdominal ultrasonograpy(p=0.091) tend to be higher than the other group.

Consideration for the Importance of Diseases Associated with Oral Medicine by Analyzing the Dental Cyber Consultation (사이버 치과 진료 상담을 통해 살펴본 구강내과 질환의 중요성에 대한 고찰)

  • The moderners of industrial society suffer from various stresses, which bring about increase in prevalence of temporomandibular disorders, oral mucosal disease and chronic neuropathic pain, therefore, the number of patients seeking help of those symptoms tend to increase. The purpose of this study was to discuss the importance of oral medicine related disease by investigating questions that appeared in cyber consultation of Kangnung National University Dental Hospital web site. Among the nearly 2000 questions, the rate of questions related to oral medicine was 20.92%, and the rate of questions related to other departments were oral and maxillofacial surgery 16.87%, conservative dentistry 16.67%, orthodontics 14.02%, prosthodontics 12.25%, periodontics 8.36%, pediatric dentistry 4.93%, preventive dentistry 2.08% and otherwise questions 3.90%. Among the 403 questions related to oral medicine, the frequent questions were oral mucosal diseases 44.17%, temporomandibular disorders 41.19%, halitosis 4.47%, xerostomia 3.23%, other orofacial pain 2.23%, forensic dentistry 1.49% and otherwise questions 2.98%. From the higher rate of questions related to oral medicine compared with other fields of dentistry, we would consider that the people are considerably concerned about the oral medicine related disease, such as oral mucosal diseases and temporomandibular disorders.

$CO_2$ Laser Application for Treatment of the Hairy Tongue (이산화탄소 레이저를 이용한 모설의 치료)

  • Hairy tongue is a disease caused by hypertrophy of filiform papillae on the tongue dorsum. Clinically, it occurs on the posterior 1/3 of the tongue dorsum and is rarely observed on the lateral and tip of the tongue. Several types of colored pigmentation from white to brown and black appear depending on the ingested food and the existing pigmentation inducing bacteria. Although clinically asymptomatic, patients will visit the clinic for esthetic problems and at rare intervals experience nausea, halitosis, dysgeusia and unpleasant sensation. The exact etiology is yet not known but causes such as inappropriate oral hygiene control, a shift in oral bacterium, infection, radiotherapy, use of antibacterial drugs or immunosuppressants, excessive smoking or alcohol drinking and decrease of salivary flow rate may be a factor of the disease. Clinical symptoms are quite typical and diagnosis may be made simply by observation, not necessarily biopsy. Treatment of hairy tongue is done by eliminating the etiology factors, brushing the tongue gently with a tongue cleaner and in some cases using keratolytic agent. Although it is rare to treat hairy tongue surgically, we will introduce a case successfully treated using carbondioxide laser($CO_2$ laser), after no sufficient improvement had been made by conservative treatment.

Combined Therapy with Carboplatin and Meloxicam for Oral Squamous Cell Carcinoma in a Dog (구강 편평 세포암 이환견에서 Carboplatin과 Meloxicam의 병용투여 증례)

  • A 16-year-old, spayed female, Maltese dog weighing 4.3 kg was presented with a 2-week history of yellowish nose-and oral-discharge, anorexia and lethargy. In physical examination, indefinite ulcerative mass about $3{\times}3$ cm in size in the left sublingual region, severe gingivoperiodontitis, halitosis, moderate dental calculus, fever and swelling of mandible soft tissue were noted. In radiographic view, periodontitis and bone lysis findings on the left rostral part of maxilla and mandible were observed. In complete blood count and blood chemistry test, thrombocytosis, increased levels of serum $NH_3$, AST and ALP were obtained. Urinalysis revealed hematuria and proteinuria. In histological examination, squamous cell carcinoma was diagnosed. No surgery was performed, but combined treatment with carboplatin and piroxicam was initiated. Five days after initiation of medication, because of insistent vomiting, piroxicam was substituted to meloxicam. Although the therapeutic efficacy of meloxicam could not be known; administration of meloxicam might be less potential to gastrointestinal side-effects than piroxicam. The dog totally received 3 times of carboplatin infusion. Five days after third infusion of carboplatin, the dog was hospitalized with severe lethargy, vomiting and bloody diarrhea. In examination, severe renal failure signs were noted, and the dog was euthanized.

Antibacterial Activity of Sea-mustard, Laminaria japonica Extracts on the Cariogenic Bacteria, Streptococcus mutans (충치균 (Streptococcus mutans)에 대한 다시마 추출물의 항균활성)

  • In previous reports the authors have screened the inhibition effects of marine algae extracts on halitosis, and demonstrated that a brown algae, Eisenia bicyclis (' Daehwang') possess not only strong deodorant effect bug also considerable anticariogenic activities. In this study, we screened antibacterial effects of various marine algae, and measured minimum inhibitory concentration (MIC) value of them against mutans streptococci in vitro. Among the 27 species of marine algae, $80\%$ ethanol extract of dried sea-mustard, Laminaria japonica ('Dasima') showed the strongest inhibition activity against Streptococcus mutans KCTC 3300. The extracts of Ulva lactuca ('Galparae'), Codium fragile ('Cheonggak'), Ecklonia cava ('Gamtae'), E. stolonifera ('Gompi') and Undalia Pinnatifida ('Miyeok') showed slightly weaker inhibitory potency than L. japonica. Differences of MIC values in $80\%$ ethanol extract of some species of marine algae were observed depending on test bacterial species, i.e., S. mutans KCTC 3300 or S. sobrinus KCTC 3307. Eighty percent ethanol extract of dried L japonica was fractionated with diethyl ether, chloroform, ethyl acetate, n-buthanol and water successively, The ether-soluble fraction had inhibitory effect on S. mutans KCTC 3300, however the inhibitory effects were not found in the other fractions. The MIC values of $80\%$ ethanol extract and ether fraction were 180 and 105 $\mu$g/mL respectively, while no significant inhibition activity of water-soluble fraction was found even when the fraction was added up to 5,500 $\mu$g/mL.

Antibacterial Effect on Oral Normal flora of Phytoncide from Chamaecyparis Obtusa (구강 상주균에 대한 편백 피톤치드의 항균효과)

  • The present study was performed to observe the effect of phytoncide on oral normal microflora and the inhibitory effect of the surviving resident oral bacteria on F. nucleatum. In this study, saliva from each of 20 healthy subjects was treated with 1% phytoncide from Japanese Hinoki (Chamaecyparis obtusa Sieb. et Zucc.). The surviving salivary bacterium were isolated on blood agar plates and identified by 16S rDNA sequencing. In order to select inhibitory isolates against F. nucleatum, the isolates from the phytoncide-treated saliva were cultured with F. nucleatum. The results are as follows: 1. Among the 200 surviving resident oral bacterium, 70(35.0%) bacterium inhibit the growth of F. nucleatum on blood agar plates. 2. Among the 70 bacterium which inhibit F. nucleatum, Streptococcus salivarius was 41.3%(45/109), Streptococcus sanguinis was 28%.(7/25), Streptococcus mitis was 20%(3/15), Streptococcus parasanguinis was 33.3%(3/9), Streptococcus Alactolyticus was 100%(8/8), Streptococcus vestibularis was 28.6%(2/7) and Streptococcus sp. was 50%(2/4). Taken together, among the surviving resident oral bacterium, Streptococcus salivarius, Streptococcus sanguinis, Streptococcus mitis were mainly observed to inhibit F. nucleatum. and they may exert an additional inhibitory activity against the periodontopathic bacterium. Therefore, phytoncide can be used to prevent and cease the progress of periodontal disease, halitosis. Thus it is expected to promote oral health.

The Effect of the Phytoncide in Decreasing the Mouth Odor (피톤치드의 입냄새 제거효과)

  • Antimicrobial action of phytoncide in the mouth decrease odor-producing microorganisms. Also phytoncide has malodor effect by reaction with volatile sulfur compounds. Phytoncide has excellent malodor effect in microbiologically and chemically. This study prove the malodor effect of phytoncide by use ferrous sulfate. So I try to make new treatment method for halitosis. I get the results as follows. 1. The difference of mean value of absorbancy was 0.849 between the mean absorbancy of deposition by add phytoncide to saliva and the saliva only. 2. The difference of mean value of absorbancy was 0.701 between the mean absorbancy of deposition by add phytoncide to distilled water and the distilled water only. 3. The difference of mean value(0.849) in saliva by existence of phytoncide was larger than in double distilled water(0.701) by existence of phytoncide. Therefore, phytoncide make more deposition in saliva than double distilled water by reaction with sulfur compounds. As the results, phytoncide reaction with sulfur compounds in saliva. It take malodor action in liquid state effectively. It is thought, only the toothpaste it knows from in the limit which does not have a side effect by the human body it adds in the oral cavity of the mouth rinse and with the fact that it will be able to use positively in clinic.

A Study on the Health Status and Oral Health-Related Quality of Life in the Elderly Patients with Long-Term Care (노인장기요양 환자의 건강상태와 구강건강관련 삶의 질)

  • This study intended to examine the health status and quality of life among the elderly patients who stayed in geriatric hospitals for a long period of time. The study involved 222 elderly who were older than 65-year-old, an oral examination was conducted in 10 geriatric hospitals located in the metropolitan area and Chungcheong-do. Cross-tabulation, one-way ANOVA, t-test, and Stepwise multiple regression were performed for analysis. The average number of teeth among the subjects was 11.22 and that of dental caries was 1.17. Also, the average number of teeth mobility was 0.93 and halitosis was detected among 81.9%. Only 29.70% reported absence or a kind of systemic disease and 70.31% of the subjects was shown to have more than two kinds of complex disease. In addition, 87.8% of the subjects had depression and among them, 55.40% showed serious depression. The average score on quality of life was 2.62 points and as for the related factors, the number of remaining teeth and dental caries were counted along with care grade, hospitalization period, and age. According to such results, the oral health status among the elderly patients staying long in geriatric hospitals were not good in general; also they showed high level of systemic disease and depression and low quality of life.
