7구역진단기의 Factor AA 제4구역 유형과 심박변이도(HRV)와의 상관성 연구 (A Study on the Correlation between the Patterns of the Zone 4 of Factor AA in 7-Zone-diagnostic System and Heart Rate Variability)

  • Objectives : The 7-zonediagnostic system is a diagnostic device to predetermine bodily locations by measuring the energy of body. This study was to investigate the relation between the different patterns of Zone 4 of Factor AA in VEGA DFM 722 (VEGA, Germany), 7-zone-diagnositic system and heart rate variability. Methods : We made three groups according to the Factor AA patterns of VEGA DFM 722. The Factor AA pattern of Group A is that the red bar graph of zone 4 was higher than the normal range. The Factor AA patterns of Group B was that the red bar graph of zone 4 was located at the normal range. The Factor AA patterns of Group C was that the red bar graph of zone 4 was lower than the normal range. We investigated how to difference of the index of heart rate variability(HRV, LX-3202, LAXTHA, Korea) according to each groups. Results : Complexity, HRV-index, RMSSD, SDSD values of Group B were higher than other Groups. pNN50 values of Group B were lower than other groups. And Ln(TP), Ln(VLF), Ln(LF), Ln(HF) values of Group B were higher than other groups. Conclusions : We presumed that Group B was healthier than other groups for the stress.

만성 외상 후 스트레스 장애 환자에서 심박변이도와 증상과의 상관관계 : 외상증상과 심박변이도 관계 (The Relationship between Heart Rate Variability and Symptoms in Subjects with Chronic Posttraumatic Stress Disorder)

  • Objective : Heart rate variability (HRV) is known to reflect autonomic nervous system activity. Individuals with posttraumatic stress disorder (PTSD) are reported to have lower HRVs. We attempted to find HRV indices with head up tilt position that reflect the symptoms well in order to evaluate PTSD symptoms. Methods : Sixty-seven patients with PTSD and 72 patients without PTSD were assessed using the PTSD Checklist for DSM-5 (PCL-5), the Beck Depression Inventory, the Beck Anxiety Inventory and the Pittsburgh Sleep Quality Index. HRV was measured in the head-up tilt position. We collected data regarding heart rate (HR), standard deviation of the NN intervals (SDNN), the square root of the mean squared differences of successive NN intervals (RMSSD), log low-frequency (LNLF) and log high-frequency (LNHF). Results : The value of LNHF was different according to presence or absence of PTSD after head-up tilt position. In the findings of the association between PTSD symptoms and HRV indices as based on head-up tilt, LNHF had a significant correlation with the total score of PCL-5. Conclusion : The reduction of the high-frequency component of HRVs in the PTSD group might reflect more PTSD symptoms.

요통 환자의 심박변이도 특성에 대한 임상적 연구 (Clinical Study for Characteristics of Heart Rate Variability in Low Back Pain Patients)

  • Objectives : To study autonomic nervous system dysfunction of Low Back Pain(LBP) patients, using spectral analysis of Heart Rate Variability(HRV). Methods : HRV of 190 patients was measured and seperated into two groups, those with LBP(n=95) and healthy controls(n=95). HRV was measured by SA-6000(Medicore, Korea) for 5 minutes after 5 minutes' resting. Results : 1. Mean heart rate(MHRT) of the experimental group was slightly higher than that of the control group, but did not show significant difference(P=0.428). The square root of the mean squared differences of successive normal-to-normal intervals(RMSSD), logarithmic very low frequency power(Ln VLF) and low frequency power/high frequency power ratio(LH/HF ratio) were not significantly low between experimental group and control group(P=0.16, 0.130, 0.537). 2. The standard deviation of all the normal-to-normal intervals(SDNN), logarithmic total power(Ln TP), logarithmic low frequency power(Ln LF) and logarithmic high frequency power(Ln HF) were significantly low between experimental group and control group(P=0.03, 0.005, 0.001, 0.007). 3. Ln LF of acute group was significantly low compared with those of chronic group(P= 0.039). Conclusions : This study suggests the activity and imbalance of autonomic nervous system in LBP is low. Also sympathetic nervous system of acute LBP is lower than that of chronic LBP. Further study of HRV related to LBP is needed in the clinical medicine.

Reduced Heart Rate Variability in Somatic Symptom Disorder: Associations with Alexithymia

  • Objectives : We investigated heart rate variability (HRV) patterns in patients with somatic symptom disorder (SSD) and the relationships of these patterns with alexithymia. Methods : In total, 42 patients with SSD and 33 healthy controls were enrolled in this study. Demographic, psychological, and HRV data were assessed at baseline, and 24 patients with SSD were reassessed after 6 months of treatment. The psychological data included somatic symptoms and levels of depression, anxiety, and alexithymia as indicated by the somatic symptom subscale of the Symptom Checklist 90-Revision (SCL-12), Beck Depression Inventory-II (BDI-II), Beck Anxiety Inventory (BAI), and the Toronto Alexithymia Scale 20 (TAS-20), respectively. Results : Patients with SSD had a lower standard deviation of normal-to-normal R-R intervals (SDNN) and lower proportions of adjacent R-R intervals greater than 50 milliseconds (pNN50) compared with controls. These HRV parameters were negatively correlated with alexithymia severity. After treatment, patients exhibited significantly decreased levels of somatic symptoms and reduced anxiety and depression, but there were no significant differences in the HRV parameters. In patients with alexithymia, a high baseline SDNN and pNN50 were associated with a decrease in somatic symptoms. Conclusions : Patients with SSD have different HRV patterns, and several HRV parameters are associated with alexithymia severity. These findings suggest that ANS regulation is involved in the pathophysiology of SSD, mediated by alexithymia. Furthermore, these results suggest that certain HRV parameters may be associated with clinical outcomes of SSD.

카페인이 HRV에 미치는 영향과 내관 자침의 효과에 대한 예비연구 : 무작위 이중맹검시험 (Short Time Effect of Caffeine on Heart Rate Variability and the Effect of Acupuncture at Neiguan (PC6): A Randomized Double Blind Pilot Study)

  • Objectives : This study was to investigate the short time effect of caffeine on heart rate variability(HRV) and the effect of Neiguan(PC6) acupuncture stimulation on HRV. Methods : 27 healthy adult volunteers were randomly allocated to two groups: Neiguan group (N=13) or placebo group (N=14). The study was carried out under a randomized double-blinded placebo-controlled trial method. Each group orally received the same tablets with 200 mg caffeine. After 1 hour, acupuncture was applied to the Neiguan(PC6) points for the Neiguan group, and for the placebo group was applied to a non-acupuncture point. Both groups were estimated with HRV 3 times, before and after caffeine ingestion, 20 minutes after acupuncture stimulation. Results : After taking caffeine, pulse rate, mean-HRV, and pNN50(the proportion derived by dividing NN50 by the total number of NN intervals) decreased, SDNN(standard deviation of all normal-to-normal (NN) intervals), RMSSD (the root square of successive differences), TP log, HF(high frequency), and HRV index was increased. There were significant changes to the autonomic nervous system after taking caffeine. There were no significant differences between the two groups after acupuncture at Neiguan. Conclusion : Caffeine could induce general activation of the autonomic nervous system. Neiguan acupuncture stimulation may not have significant influence on the autonomic nervous system.

Effects of Head-Up Tilt on Nonlinear Properties of Heart Rate Variability in Young and Elderly Subjects

  • In the present study, our aim is to investigate whether responses to the head-up tilt (HUT) on nonlinear properties of heart rate variability (HRV) in young and elderly subjects are different or not. Thirteen young-healthy subjects ($24.5{\pm}3.7$ years) and 18 old-aged healthy subjects ($74.5{\pm}7.4$ years) participated in this study. An electrocardiogram (ECG) in the supine posture, at $0^{\circ}$, and in the standing posture, at $70^{\circ}$ of head-up tilt, was recorded. Detrended fluctuation analysis (DFA) and approximate entropy (ApEn), measures of short-/long-term correlation properties and overall complexity of heart rate (HR) respectively, along with spectral components of HR variability (HRV) were analyzed for both the supine and HUT postures. We observed that the short-term fractal exponent ${\alpha}_1$ increased during HUT posture (F(1, 29) = 39.79, P = 0.000), especially, the young subjects showed a significantly higher values compared to the elderly subjects. ApEn significantly decreased (F(1, 29) = 8.61, P = 0.006) during HUT posture. HUT posture decreased the complexity in HR dynamics and increased short-term fractal exponent values in young subjects but not in elderly subjects. These results imply that there are differences of response to HUT on nonlinear properties between young and elderly subjects.

구강작열감증후군 환자의 임상 특징에 따른 심박변이도 분석 (Heart Rate Variability Analysis According to Clinical Characteristics in Patients with Burning Mouth Syndrome)

  • Objectives: This study aimed to identify the characteristics of the heart rate variability (HRV) index in patients with burning mouth syndrome (BMS) and to evaluate the predictive value of HRV for Korean medical treatment responsiveness to tongue pain in BMS. Methods: Among BMS patients who visited the Oral Diseases Clinic of Kyung Hee University Korean Medicine Hospital from January 1, 2018, to May 31, 2021, we reviewed the clinical records of 217 women aged 50 years and over. HRV and unstimulated saliva flow rate (USFR) measurements were carried out, and all patients were asked to indicate their degree of tongue pain using the visual analog scale (VAS). The subjects were classified into subgroups according to the presence or absence of hyposalivation and VAS improvement, and each subgroup was compared using the HRV index. Logistic regression analysis was conducted to confirm whether cause-effect relationships were present between statistically significant values. Results: BMS patients had lower LF and HF than healthy people. Subgroup analysis demonstrated that there were some statistically significant differences in the HRV index according to salivation rate and degree of pain. In addition, as the LF/HF ratio increased, the pain improvement rate decreased after treatment. Conclusion: The autonomic nerve activity of BMS patients was lower than that of healthy people; however, autonomic balance was not impaired. In addition, tongue pain in BMS patients responded more favorably to Korean medical treatment when patients fell within a normal range of the LF/HF ratio, suggesting that autonomic imbalance could be used as one of the predictable factors in clinical practice.

  • 졸음 운전 등 운전자의 상태 변화에 따른 자동차 사고가 급증하고 있으며 이를 방지하기 위한 시스템 구축 및 운전자의 상태를 판단하는 알고리즘 개발이 요구되어 지고 있다. 본 논문에서는 모의 주행 시스템을 통한 운전자의 심박변이도, 산호 포화도 (SPO2), 체온을 측정하여 운전자의 상태를 알려 주는 실험을 수행하였다. 즉, 심박변이도 (Heart rate variability, HRV) 분석을 위해 운전자의 심전도(Electrocardiogram, ECG) 신호를 획득 한 후 심전도 P,QRS, T 파형 중 R peak 을 자동으로 검출하였고 이를 통해 구한 R-R interval을 이용하여 HRV의 주요 파라메타를 시간영역(time domain)으로 해석하여 작업 환경에 따른 운전자의 상태를 비교 분석하였다.

완전몰입형 가상현실(FIVR) 프로그램이 심박변이도에 의한 연령별 자율신경계의 변화 (Fully Immersive Virtual Reality Program Changes in the Autonomic Nervous System by Age According to Heart Rate Variability)

  • Purpose: This study aimed to investigate the fully immersive virtual reality (FIVR) program changes in the autonomic nervous system (ANS) by age according to heart rate variability (HRV). Methods: A total of 44 subjects were classified into group 1(aged 20-30), group 2(aged 40-60), and group 3(aged 70 and above). The study analyzed the HRV using a pulse wave analyzer and compared the ANS changes before and after applying the FIVR of the group. The LF, HF, RMSSD, and SDNN were measured. Fifteen minutes of virtual reality applications were applied to all subjects, and 1 min of rest was given in the middle of the session. A was used for anteroposterior comparisons of the ANS. Results: The HF, LF, RMSSD, and SDNN values were not statistically significant in all groups, but they all increased. The RMSSD value was statistically significant because it increased in group 3 (p<0.05). Those of the other two groups were not statistically significant (p<0.05). Conclusion: An FIVR program does not have a negative effect on the ANS response.

미주신경 전기자극이 심박변이도와 스트레스호르몬 분비에 미치는 효과 - 파일럿 연구 (Effect of Vagus Nerve Electrical Stimulation to Cortisol Level Control and Heart Rate Variability(HRV) - Pilot Study)

  • Purpose: Stress tends to cause sympathetic hyperactivity and increase blood cortisol levels. The vagus nerve is a parasympathetic nerve that is involved in relaxing the body. The purpose of this study was to investigate the effects of electrical stimulation of the vagus nerve on stress hormone (cortisol) levels and heart rate variability (HRV) in humans. Methods: A total of 10 healthy subjects participated in the pilot experiment. All subjects underwent electrical vagus nerve stimulation for 20 minutes in the cymba conchae of the left ear. An electro cardiogram meter was used to measure HRV. Blood samples were collected and analyzed for cortisol levels. The standard deviation of all normal N-N intervals (SDNN), root mean square of successive differences (RMSSD), low frequency, high frequency, and cortisol levels were compared in pre and post mean values after the intervention. Data were analyzed using the Wilcoxon's signed-rank test. Results: Coritsol levels were observed to decrease after vagus nerve electrical stimulation. The mean SDNN and RMSSD values were increased after the intervention, but not significantly. Conclusion: Vagus nerve electrical stimulation has been shown to increase the ability of the parasympathetic nerve to adapt to upward regulation and stress. Vagus nerve electrical stimulation could thus be an effective treatment for modern social psychological stress control.
