Human Papilloma Virus 18 Detection in Oral Squamous Cell Carcinoma and Potentially Malignant Lesions Using Saliva Samples

  • Background: Oral cancer has become one of the most prevalent cancers worldwide and human Papillomavirus is one of the risk factors for developing oral cancer. For this study HPV18 was chosen as it is one of the high risk HPV types and may lead to carcinogenesis. However, prevalence of HPV18 infection in Oral Squamous Cell Carcinoma in Malaysia remains unclear. Objective: This study aimed to investigate the viral load of HPV18 DNA in OSCC and potentially malignant lesions using saliva samples. Materials and Methods: Genomic DNAs of thirty saliva samples of normal subjects and thirty saliva samples compromised of 16 samples from potentially malignant lesions and 14 of OSCC patients were amplified for HPV18 DNA using a nested polymerase chain reaction analysis. All PCR products were then analyzed using the Bioanalyzer to confirm presence of HPV18 DNA. Result: From thirty patients examined, only one of 30 (3.3%) cases was found to be positive for HPV18 in this study. Conclusion: The finding of this study revealed that there is a low viral detection of HPV18 in Malaysian OSCC by using saliva samples, suggesting that prevalence of HPV18 may not be important in this group of Malaysian OSCC.

Neutralization of Human Papillomavirus by Specific Nanobodies Against Major Capsid Protein L1

  • The human papillomavirus (HPV) is the main cause of cervical cancer in developing countries. Rapid diagnosis and initiation of treatment of the HPV infection are critical. Various methods have been employed to reduce the immunogenicity of antibodies targeting HPV serotypes. Nanobodies are the smallest fragments of naturally occurring single-domain antibodies with their antigen-binding site compromised into a single domain. Nanobodies have remarkable properties such as high stability, solubility, and high homology to the human VH3 domain. In this study, a phagemid library was employed to enrich for nanobodies against the L1 protein of the human papilloma virus. Binding reactivity of the selected clones was evaluated using phage enzyme-linked immunosorbent assay (phage-ELISA). Finally, two nanobodies (sm5 and sm8) with the best reactivity against the Gardasil vaccine and the purified HPV-16 L1 protein were expressed and purified using a $Ni^+$-NTA column. The accuracy of expression and purification of the nanobodies was confirmed by sodium dodecyl sulfate polyacrylamide gel electrophoresis (SDS-PAGE) and immunoblotting assays. In vitro studies demonstrated that neutralization was achieved by the selected nanobodies. The ease of generation and unique features of these molecules make nanobodies promising molecules for the new generation of HPV diagnosis and therapy.

Risk Stratification of Early Stage Oral Tongue Cancers Based on HPV Status and p16 Immunoexpression

  • Background: Recent epidemiological data have implicated human papilloma virus (HPV) infection in the pathogenesis of head and neck cancers, especially oropharyngeal cancers. Although, HPV has been detected in varied amounts in persons with oral dysplasia, leukoplakias and malignancies, its involvement in oral tongue carcinogenesis remains ambiguous. Materials and Methods: HPV DNA prevalence was assessed by PCR with formalin fixed paraffin embedded sections (n=167) of oral tongue squamous cell carcinoma patients and the physical status of the HPV16 DNA was assessed by qPCR. Immunohistochemistry was conducted for p16 evaluation. Results: We found the HPV prevalence in tongue cancers to be 51.2%, HPV 16 being present in 85.2% of the positive cases. A notable finding was a very poor concordance between HPV 16 DNA and p16 IHC findings (kappa<0.2). Further molecular classification of patients based on HPV16 DNA prevalence and p16 overexpression showed that patients with tumours showing p16 overexpression had increased hazard of death (HR=2.395; p=0.005) and disease recurrence (HR=2.581; p=0.002) irrespective of their HPV 16 DNA status. Conclusions: Our study has brought out several key facets which can potentially redefine our understanding of tongue cancer tumorigenesis. It has emphatically shown p16 overexpression to be a single important prognostic variable in defining a high risk group and depicting a poorer prognosis, thus highlighting the need for its routine assessment in tongue cancers. Another significant finding was a very poor concordance between p16 expression and HPV infection suggesting that p16 expression should possibly not be used as a surrogate marker for HPV infection in tongue cancers. Interestingly, the prognostic significance of p16 overexpression is different from that reported in oropharyngeal cancers. The mechanism of HPV independent p16 over expression in oral tongue cancers is possibly a distinct entity and needs to be further studied.

남녀 대학생의 자궁경부암에 관한 지식 및 건강신념이 인유두종바이러스 예방접종에 미치는 영향 (The Influence of Cervical cancer, HPV knowledge and health beliefs on HPV Vaccination among Undergraduate Students)

  • 본 연구는 남녀 대학생의 자궁경부암 지식 및 건강신념이 인유두종바이러스 예방접종 이행에 미치는 영향을 확인하고자 시도하였다. 2014. 4.28-6.4 C지역 남녀 대학생을 편의 추출하여 연구에 동의한 278명에 대해 구조화된 설문지로 조사하였으며, 271부를 최종분석에 사용하였다. 연구결과, 자궁경부암 및 HPV에 관한 지식수준은 매우 낮았다(평균 5.34/26점). HPV에 관한 건강신념은 지각된 유익성 2.58, 지각된 장애성 2.52, 지각된 심각성 2.49순이었고, 지각된 민감성은 1.77점으로 매우 낮았다. 자궁경부암 및 HPV에 관한 지식이 HPV 예방접종에 미치는 영향은, 자궁경부암 지식 점수가 한 단위 높아질 때 예방접종을 받을 확률은 1.229배(p<.01), 건강신념의 지각된 유익성이 한 단위 상승할 때 예방접종을 받을 확률은 1.567배(p<.05) 올라가는 것으로 나타났다. 결론적으로 대학생의 HPV 예방접종율 향상을 위해서는 자궁경부암 및 인유두종 바이러스에 관한 지식을 높이고, 유익성을 강조하는 교육 전략으로 접근하는 것이 효과적일 것이다.

  • Purpose: The primary prevention for cervical cancer, the human papilloma virus (HPV) vaccination, has been available in South Korea and its importance has been emphasized publicly. The purpose of this study was to investigate the knowledge regarding HPV vaccination and identify the factors associated with HPV vaccination in female university students. Methods: A sample of 200 women among university students in Seoul was asked to answer a questionnaire on HPV-related knowledge and attitude, and influencing factors on HPV vaccination. Results: Among the respondents, 12.0% were HPV vaccinated. Overall HPV-related knowledge was low, and knowledge was not different between the vaccinated and unvaccinated groups. The vaccinated group demonstrated a higher score on the knowledge about the place where people could receive HPV vaccination and the cost of the vaccination than that of the unvaccinated group. The major influencing factor on vaccination was the parent's recommendation and the major barrier for vaccination was the cost of the vaccination. Conclusion: A broadened public campaign is recommended to increase the knowledge and positive attitude towards HPV vaccination for university female students as well as their parents.

Evaluation of Human Papilloma Virus Infection in Patients with Esophageal Squamous Cell Carcinoma from the Caspian Sea Area, North of Iran

  • Introduction: HPV has been found repeatedly in esophageal squamous cell carcinoma (ESCC) tissues. However, reported detection rates of HPV DNA in these tumors have varied markedly. Differences in detection methods, sample types, and geographic regions of sample origin have been suggested as potential causes of variation. We have reported that infection of HPV DNA in ESCC tumors depends on anatomical sites of esophagus of the patients from Mazandaran, north of Iran. Materials and Methods: HPV DNA was examined in 46 upper, 69 middle and 62 lower third anatomical sites of esophageal squamous cell carcinoma specimens collected from Mazandaran province in north Iran, near the Caspian Littoral as a region with high incidence of ESCC. HPV L1 DNA was detected using Qualitative Real time PCR and MY09/MY11 primers. Results: 28.3% of upper, 29% of middle and 25.8% of lower third of ESCC samples were positive for HPV DNA. 13.6% for males and 14.1% for females were HPV positive in all samples. Conclusions: HPV infection is about one third of ESCC in this area. Findings in this study increase the possibility that HPV is involved in esophageal carcinogenesis. Further investigation with a larger sample size is necessary.

HPV, Cervical Cancer and Pap Test Related Knowledge Among a Sample of Female Dental Students in India

  • Background: The present study was designed to ascertain knowledge about HPV, cervical cancer (CC) and the Pap test among female dental students of Panineeya Institute of Dental Sciences and Hospital, Hyderabad, India. Materials and Methods: A self-administered questionnaire covering demographic details, knowledge relating to human papilloma virus (HPV) (8 items), cervical cancer (4 items) and the Pap smear (6 items) was employed. Responses were coded as "True, False and Don't Know". Mean and standard deviation (SD) for correct answers and levels of knowledge were determined. Results: Based on the year of study, significant differences in knowledge of HPV werenoted for questions on symptoms (p=0.01); transmission from asymptomatic partners (p=0.002); treatment with antibiotics (p=0.002); start of sexual activity (p=0.004); and recommended age for HPV vaccination (p=0.01). For knowledge regarding CC, significance was observed for the age group being affected (p=0.008) and symptoms of the disease in early stages (p=0.001). Indications for Pap smear tests like symptoms' of vaginal discharge (p=0.002), marital status (p=0.01) and women with children (p=0.02) had significant difference based on the year of study. Based on religion, transmission of HPV via pregnancy, HPV related diseases except CC and preventive measures except condom use and oral contraceptives showed significant differences. However, significant variation with religion was observed only for two preventive measures of CC (Pap test; p=0.004) and HPV vaccination (p=0.003). Likewise, only the frequency of Pap test showed a significant difference for religion (p=0.001). Conclusions: This study emphasizes the lack of awareness with regard to HPV, CC and screening with pap smear even among health professionals. Hence, regular health campaigns are essential to reduce the disease burden.

Genotype Frequency of Human Papillomavirus Determined by PCR and DNA Sequencing in Korean Women

  • 파필로마바이러스(Human papilloma virus; HPV)는 자궁경부암의 주요한 원인균으로 30종 이상의 여성성기감염과 관련된 유전자형이 보고되었으며 자궁경부암과 관련성이 높은 고위험군과 관련성이 낮은 저위험군으로 나뉘어 진다. 최근 HPV 유전자형의 임상적 활용이 높아짐에 따라 신속하고 정확하게 HPV 유전자형을 선별할 수 있는 방법이 요구되고 있다. 본 연구의 목적은 여러 가지 분자생물학적 방법 중에서 정확도가 높은 DNA 염기서열분석을 이용하여 한국인 여성에서 HPV의 유전자형분포와 빈도를 구하고자 하였다. 전국 각 지역의 3,978명으로부터 채취한 자궁경부 검체에서 DNA를 추출하고, HPV L1 유전자 영역에서 PCR을 실시하였다. PCR 양성이 나온 경우 DNA 염기서열분석을 실시하였으며 GenBank BLAST program을 이용하여 HPV 유전자형을 분석하였다. 검사대상의 평균 년령은 37.6세였으며 년령 범위는 20-73세였고, 30대 여성이 검사를 가장 많이 실시하였다(42.2%). 총 3.978명 중에서 1,174명(1,174/3,978, 29.5%)이 HPV 양성을 보였으며 136명(11.6%)이 중복감염을 보여, 총 1,310개의 HPV 유전자를 분석하였다. 본 연구에서는 21종의 고위험군, 16종의 저위험군을 포함하여 총 37종의 HPV 유전자형이 검출되었으며, HPV 고위험군의 빈도는 69.8%(914/1,310), 저위험군은 26.0% (340/1,310)로 나타났다. 년령은 20대에서 HPV 양성률이 가장 낮았으며(69.5%), 60대 이상의 검체에서 발견된 HPV는 대부분이 고위험군이었다. 고위험군에서는 HPV 16형이 13.21%로 가장 높게 나타났으며, HPV 53형이 9.62%, 58형이 9.24%로 높게 나타났다. 다음으로 HPV 70(5.50%), 33(4.73%), 66(4.20%), 18(4.05%), 52 (4.05%), 31(3.97%), 56(3.51%)의 순으로 나타났다. 저위험군에서는 HPV 62(4.20%), 61(3.89%), 6(3.59%), 81(3.59%), 84(3.51%), 11(2.6%)의 순으로 검출되었다. DNA 염기서열분석을 이용한 한국인 여성의 HPV 유전자형빈도 분석 결과는 HPV의 역학적 연구와 백신개발을 위한 자료로 유용할 것이며, 자궁경부암의 치료와 관련한 특이적 HPV 유전자형 관련 연구에 도움을 줄 것으로 사료된다.

Mothers' Knowledge and Attitudes about HPV Vaccination to Prevent Cervical Cancers

  • Background: Cervical cancer which is one of the most preventable cancers is an important public health problem worldwide, and especially in developing countries. The aim of this study was to determine knowledge and attitudes about the HPV vaccination of mothers with 0- to 18-year old children. Materials and Methods: Written approval was taken from the local authorities. The study subjects consisted of 799 mothers who agreed to participate. The data were collected via a "Personal Information Form" which included 30 questions that were prepared by the researchers themselves in line with the literature. The data were collected by face to face interviews with the mothers. Analyses were performed using commercial software. Results: The mean age of the mothers who participated in the study was $32.0{\pm}6.52$, and 88.1% reported no information about HPV, and 83.5% no information about HPV vaccination. Only 0.7% of the mothers had daughters who had HPV vaccination, and 44.3% of the mothers who had sons were found out to be indecisive about having HPV vaccination. There was a significant corelation between the educational status of the mothers and their knowledge about HPV vaccination (p<0.05). However, there was no significant correlation in terms of economic conditions (p>0.05). Conclusions: This study suggested that mothers had very little information on HPV and HPV vaccination. Knowledge of the disease and its vaccination is an essential factor for the success of the vaccination program. It is of great importance that mothers are trained in this subject by health professionals.

Association between RASSF1A Promoter Hypermethylation and Oncogenic HPV Infection Status in Invasive Cervical Cancer: a Meta-analysis

  • Cervical carcinoma is the main cause of cancer-related mortality in women and is correlated with more than 15 risk cofactors, including infection of cervical cells with high-risk types of HPV (hrHPV). Indeed, both aberrant methylation of the RASSF1A promoter and hrHPV infection are often observed in cervical carcinomas. The purpose of our meta-analysis was to evaluate the role of RASSF1A promoter methylation and hrHPV infection in cervical cancer. Our meta-analysis involved 895 cervical cancer patients and 454 control patients from 15 studies. Our results suggested that RASSF1A promoter hypermethylation increased the risk of cervical cancer (OR=9.77, 95%CI=[3.06, 31.26], P=0.0001, $I^2=78%$). By grouping cases according to cancer subtypes, we found that HPV infection was higher in cervical squamous cell carcinomas (SCCs) than in cervical adenocarcinomas/adenosquamous cancers (ACs/ASCs) (OR=4.00, 95%CI=[1.41, 11.30], P=0.009, $I^2=55%$). Interestingly, HPV infection tended to occur in cervical cancers with relatively low levels of RASSF1A promoter methylation (OR=0.59, 95%CI=[0.36, 0.99], P=0.05, I2=0%). Our study provides evidence of a possible interaction between HPV infection and RASSF1A promoter methylation in the development of cervical cancers.
