To evaluate age and gender differences in the relation of cardiovascular risk factors, cognitive impairment, and subclinical carotid atherosclerosis from aged people using by a cross sectional method. Sixty-nine healthy elders living in the community who had not previously undergone carotid ultrasonography were included. We conducted life style surveys, and cognitive function tests including Korean-mini-mental state examination (K-MMSE) and clinical dementia rating-Korean. Various biomarkers from blood were assessed; fasting insulin-like growth factor-1, lipid-profile, high sensitivity C-reactive protein, total homocysteine, glucose, insulin, Homeostasis model assessment (HOMA) for insulin resistance index, vitamin B12, and folate level. Carotid intima-media thickness (C-IMT), and plaques were measured using carotid ultrasonography and aortic ultrasonography, a valid index of atherosclerosis. For the elderly subjects (aged 65-82 years), cognition impairment was more prevalent in females while subclinical atherosclerosis was more prevalentin males. Increased C-IMT has been kept in males, and C-IMT shows increasing trend and the peak at about 80 year-old in females with increasing age. The significant correlations between C-IMT and many vascular risk factors including age, triglyceride, abnormal homocysteinein male, and K-MMSE, insulin, HOMA index and abnormal aortic ultrasonography in female were different in each gender, with the exception of homocysteine (p<0.05). This data suggests that there were differences of age and gender characteristics in terms of subclinical atherosclerosis, cognitive impairment and vascular risk factors in community-living elders. Further larger and longitudinal studies across entire age are required to better understand the effects of risk factors on subclinical atherosclerosis.
We investigated the effect of Rhus verniciflua Stokes extract ingestion with exercise training on plasma lipids and insulin resistance for 8 weeks. Forty SD (Sprague-Dawley) male rats were used as the experimental animals that were divided into CON (control), RVS (Rhus verniciflua Stokes), EXE (exercise training), and RVS-EXE (Rhus verniciflua Stokes with exercise training), respectively. Body weight gain in EXE (202%) and RVS-EXE (203%) was significantly lower than CON (253%) and RVS (239). Stored fats were significantly lower with RVS and/or exercise training. Blood lipids were enhanced in RVS and RVS-EXE compared to CON. Blood glucose was significantly high in CON compared to the other groups. Insulin and HOMA index has the same tendencies with glucose, however, the synergic effect was found in RVS-EXE. In conclusion, Rhus verniciflua Stokes extract ingestion with exercise training has the effect of lowering amount of stored fats, reducing blood lipids, and enhancing insulin resistance, therefore, metabolic syndrome, diabetes, obesity, and hyperlipidemia might be prevented.

Purpose: Lipopolysaccharide-binding protein (LBP) is a 65-kDa acute phase protein, derived from the liver, which is present in high concentrations in plasma. Data regarding the association between circulating plasma LBP levels and obesity-related biomarkers in the pediatric population are scarce. We aimed to determine whether there was a difference in plasma LBP levels between overweight/obese and normal-weight adolescents and to assess the correlation of circulating LBP levels with anthropometric measures and obesity-related biomarkers, including insulin resistance, liver enzyme levels, and lipid profiles. Methods: The study included 87 adolescents aged 12-13 years; 44 were overweight/obese and 43 were of normal-weight. We assessed anthropometric and laboratory measures, including body mass index (BMI), blood pressure, insulin resistance, liver enzyme levels, and lipid profiles. Plasma LBP levels were measured using an enzyme-linked immunosorbent assay. Results: The mean age of the participants was $12.9{\pm}0.3$ years. Circulating plasma LBP levels were significantly increased in overweight/obese participants compared with those in normal-weight participants ($7.8{\pm}1.9{\mu}g/mL$ vs. $6.0{\pm}1.6{\mu}g/mL$, P<0.001). LBP levels were significantly and positively associated with BMI, systolic blood pressure, aspartate aminotransferase, alanine aminotransferase, total cholesterol, low density lipoprotein-cholesterol, fasting glucose and insulin, and insulin resistance as indicated by the homeostatic model assessment of insulin resistance (HOMA-IR) (all P<0.05). In multivariate linear regression analysis, BMI and HOMA-IR were independently and positively associated with plasma LBP levels. Conclusion: LBP is an inflammatory biomarker associated with BMI and obesity-related insulin resistance in adolescents. The positive correlation between these parameters suggests a potentially relevant pathophysiological mechanism linking LBP to obesity-related insulin resistance in adolescents.
Obesity has been reported to be a cluster of risk factors in the pathological ecology, In particular, there is increasing evidence that inflammation-related factors are associated with diabetes. This study examined the relationship between the hs-CRP level and FBG, fructosamine, and $HbA_1c$ in 4,734 non-diabetic adults aged 20 years or older, who were approved by the National Health and Nutrition Survey in 2015. The results showed that the FBG, fructosamine, and $HbA_1c$ levels increased with increasing BMI; the hs-CRP levels were the highest in the obese group, and HOMA-IR, an index of insulin resistance, was also significantly higher in the obese group. The hs-CRP level was the highest in obese adults. The levels of FBG, fructosamine. and $HbA_1c$, which are involved in blood glucose control, increased with increasing hs-CRP level. The FBG, fructosamine, and $HbA_1c$ levels increased significantly with increasing hs-CRP level after adjusting for various related variables. These results suggest that the obesity-induced increase in hs-CRP is a risk factor for diabetes mellitus in non-diabetic adults. Therefore, proper dietary habits and regular exercise should prevent diabetes by preventing obesity in non-diabetic adults.

1. Objectives This study was to designed investigate the relationship between hypertension and its risk factors and the prevalence of hypertension according to Sasang Constitution. 2. Methods Five hundred and twenty six people were subjects out of 666 people, over 40 years old, who participated in the community-based cohort in Wonju, South Korea from July 2nd to August 30th in 2006. Hypertensive group was 263 peoples and normal group was 263 people, selected randomly among non hypertensive people who had same age and sex with hypertensive groups. Sasang Constitutional Diagnosis was carried out using PSSC(Phonetic System of Sasang Constitution), face and tongue photos and a checkup list. Risk factors from blood samples, physical measurements and social indices were analyzed using SPSS. 3. Results The prevalence of hypertension in Taeeumin was 63.1%(N=166), that of Soeumin was 22.4%(N=59) and that of Soyangin was 14.4%(N= 38). Considering risk factors of hypertension, there weren't any differences between two groups in social support indices and Framingham type A score. But scores of Soeumin's and Soyangin's were significantly high compared with that of Taeeumin in female normal group. There were significantly high results in Adiponectin of Soeumin's and HDL-cholesterol of female Soyangin's and HOMA-IR of Taeeumin's than any other groups. Crude OR of Taeeumin was 2.18 as compared with that of Soeumin in terms of risk of hypertension, and OR of Taeeumin was 2.02 as compared with that of Soeumin after drinking, smoking, total cholesterol, fasting blood sugar and HOMA-IR were adjusted. But after BMI was adjusted there wasn't a significance between Soeumin and Taeeumin and the OR of more than 25 was 2.42 as compared with that of less than 23 in BMI. 4. Conclusions Sasang Constitution is thought to be the reasonable variable to control hypertension in terms of prevention, treatment and regimen. And constitution is needed as a good variable to make a cohort study concerning chronic diseases, especially hypertension.
The purpose of this study was to provide obese middle school girls with walking and behavior modification program(WBMP) by investigating effects of the exercise on their body composition(BC), physical fitness(PF) and metabolic syndrome(MS). Analysis result of the difference between before and after the exercise(paired and independent t-test) was acquired by measuring BC, PF and MS for 12 weeks(60min/day, three a week). 24 Volunteers were 14 years over ${\geq}223.64kg/m^2$ and 15 years over ${\geq}24.00kg/m^2$. In the body composition, waist circumference(WC), body fat mass(BFM), %fat, arm muscle circumference(AMC), total body water(TBW), physical score(PS) were significant difference between the exercise group(EG) and the control group(CG), also including all factors in PF. The components of metabolic syndrome were significant difference between EG and CG, but HDL-C, systolic & diastolic blood pressure were no difference. The metabolic syndrome factors by separate of waist girth were no difference between EG and CG, but BMI, diastolic blood pressure were significant difference. The prevalence of metabolic syndrome was 4 of 24(16.7%) before intervention, but it dropped from 2 to 1 after intervention in WBMP. The components of metabolic syndrome was distributed a big WC>low HDL-C>high TG>hypertension. The changes of AMC, We, PS, BW, 50m run and BFM effect the factors on BMI(35%), WHR(waist-hip ratio, 69%), HDL-C(42%), fasting blood glucose(65%), HOMA-IR index(34%) and systolic blood pressure(39%) respectively. There were significantly increased in breakfast frequency, walking steps, and meal eating period, but decreased in snacks and eating out in exercise group.
Purpose: Rice bran is a byproduct of the hulling of rice and contains a variety of bioactive components. Various studies have reported on the antioxidative, anticancer, immune-enhancing, and hypocholesterolemic effects of rice bran. However, few studies about the physiological activity of stabilized rice bran supplement on dietary intake of sugars is limited. The aim of this study was to investigate the effect of stabilized rice bran supplement on blood glucose in C57BL/6 mice fed a high sucrose diet. Methods: Animals were randomly divided into three groups respectively, and were fed a normal diet (ND group), a high sucrose diet (HSD group) or a high sucrose diet containing 20% stabilized rice bran (HSD-SRB group) for 12 weeks. Results: In the oral glucose tolerance test (OGTT), after seven weeks of feeding on the experimental diets, a significantly lower result was observed for HSD-SRB than for HSD at 30 and 60 minutes after oral administration in glucose solution (2 g/kg body weight). The incremental area under the curve (IAUC) of HSD-SRB was significantly lower than that of HSD. After 12 weeks, fasting blood glucose level of HSD-SRB was significantly lower than that of HSD. No significant difference in the serum insulin level was observed between HSD and HSD-SRB. However, HOMA-IR was significantly decreased in HSD-SRB compared to HSD. In addition, HOMA ${\beta}$-cell was significantly increased in HSD-SRB compared to HSD. Triglyceride in liver of HSD-SRB was significantly lower than that of HSD. Conclusion: Feeding diets containing 20% rice bran improved insulin resistance and insulin secretion by decreasing triglyceride in liver. Thus, rice bran has a positive effect on glycemic control. In addition, the results are expected to be utilized as a basis for human study and development of food products with added rice bran.
Purpose: This study was conducted to identify the association between insulin resistance and the major dietary patterns of Korean adults. Methods: This study used data from the 2015 Korea National Health and Nutrition Examination Survey. The subjects were 2,276 adults aged 19 to 64 years old. Based on the food frequency questionnaire data, 112 food items were reclassified into 30 food groups. The principal component analysis method was applied to identify major dietary patterns. We used homeostatic model assessment of insulin resistance (HOMA-IR) and quantitative insulin sensitivity check index (QUICKI) value as indicators of insulin resistance. The association between major dietary patterns and insulin resistance was investigated using logistic regression analysis. Results: Three major dietary patterns were identified and assigned descriptive names based on the food items with high loadings: 'healthy Korean meal pattern', 'western meal pattern', and 'white rice, alcohol, meat pattern'. As the 'white rice, alcohol, meat pattern' score increased, significant increasing trends for fasting glucose concentration and HOMA-IR and a significant decreasing trend for QUICKI were observed after adjusting for age and sex. The odds ratio of insulin resistance according to the 'healthy Korean meal pattern' and the 'western meal pattern' were not statistically significant. the 'white rice, alcohol, meat pattern' showed a significant positive association with the risk of insulin resistance after adjusting for covariates. Conclusion: These results suggest that the 'white rice, alcohol, meat pattern' is positively associated with the risk of insulin resistance. The white rice, alcohol, meat pattern was related to the high consumption of alcohol together with rice or meat. This pattern was also associated with the high intake of sodium and low intakes of vitamin C, calcium, potassium, and dietary fiber. To confirm the association, further longitudinal studies are required.
BACKGROUND/OBJECTIVES: Diabetes-specific oral nutritional supplements (ONS) have anti-hyperglycemic effects, while D-allulose exerts anti-diabetic and anti-obesity effects. In this study, we investigated the efficacy and safety of diabetes-specific ONS, including allulose, on glycemic and weight changes in overweight or obese patients with type 2 diabetes mellitus (T2DM). SUBJECTS/METHODS: A single-arm, historical-control pilot clinical trial was conducted on 26 overweight or obese patients with T2DM (age range: 30-70 yrs). The participants were administered 2 packs of diabetes-specific ONS, including allulose (200 kcal/200 mL), every morning for 8 weeks. The glycemic profiles, obesity-related parameters, and lipid profiles were assessed to evaluate the efficacy of ONS. RESULTS: After 8 weeks, fasting blood glucose (FBG) level significantly decreased from 139.00 ± 29.66 mg/dL to 126.08 ± 32.00 mg/dL (P = 0.007) and glycosylated hemoglobin (HbA1c) improved (7.23 ± 0.82% vs. 7.03 ± 0.69%, P = 0.041). Moreover, the fasting insulin (δ: -1.81 ± 3.61 μU/mL, P = 0.017) and homeostasis model assessment for insulin resistance (HOMA-IR) (δ: -0.87 ± 1.57, P = 0.009) levels decreased at 8 weeks, and body weight significantly decreased from 67.20 ± 8.29 kg to 66.43 ± 8.12 kg (P = 0.008). Body mass index (BMI) also decreased in accordance with this (from 25.59 ± 1.82 kg/m2 to 25.30 ± 1.86 kg/m2, P = 0.009), as did waist circumference (δ: -1.31 ± 2.04 cm, P = 0.003). CONCLUSIONS: The consumption of diabetes-specific ONS with allulose in overweight or obese patients with T2DM improved glycemic profiles, such as FBG, HbA1c, and HOMA-IR, and reduced body weight and BMI.

BACKGROUND/OBJECTIVES: Cocoa consumption is associated with health benefits due to its high content of polyphenols. However, the effects of short-term cocoa consumption remain unclear. We aimed to determine the effects generated by cocoa consumption (for 7 days) in young adults in normoweight and class II obesity. SUBJECTS/METHODS: Before-and-after study was carried out in normoweight (NW) (n = 15) and class II obesity (CIIO) (n = 15) young adults. The NW and CIIO participants consumed 25 and 39 g of cocoa, respectively, per day for 7 days. The effect of cocoa consumption was evaluated on the lipid profile, insulin resistance (IR), and inflammation. Oxidative damage was also examined by assessing the biomarkers of oxidative damage in plasma. In addition, recombinant human insulin was incubated with blood obtained from the participants, and the molecular damage to the hormone was analyzed. RESULTS: Cocoa consumption resulted in decreased low-density lipoprotein-cholesterol in both groups (P = 0.04), while the total cholesterol, high-density lipoprotein cholesterol, and triglycerides were maintained at the recommended levels. Initially, IR was detected in the CIIO group (homeostasis model assessment [HOMA] = 4.78 ± 0.4), which is associated with molecular damage to insulin. Interestingly, intervention with cocoa resulted in improved IR (HOMA = 3.14 ± 0.31) (P = 0.0018) as well as molecular damage to insulin. Finally, cocoa consumption significant decreased the arginase activity (P = 0.0249) in the CIIO group; this is a critical enzymatic activity in the inflammatory process associated with obesity. CONCLUSIONS: The short-term consumption of cocoa improves the lipid profile, exerts anti-inflammatory effects, and protects against oxidative damage. Results of this study indicate that cocoa consumption can potentially improve IR and restore a healthy redox status.
