Role of HER2 in Brain Metastasis of Breast Cancer: a Systematic Review and Meta-Analysis

  • Background: Breast cancer is one of the most common cancers among women worldwide and the HER2 receptor plays an important role in its development and progression. This systematic review aimed to summarize the role of HER2 in brain metastasis in patients with breast cancer. Materials and Methods: We conducted a literature search by advanced search in title field using the Scopus, Pubmed, and Google scholar databases until the end of June 2014. With metastasis, metastatic, HER2, brain, and breast cancer, as terms of search we selected 31 articles, which were reviewed by two independent and blinded expert reviewers. The studies were first selected according to their titles and abstracts. Quality of the studies were then assessed using the STROBE (Strengthening the Reporting of Observational Studies in Epidemiology) protocol for observational studies and CONSORT(Consolidation of Standards for Reporting Trials) protocol for clinical trials. For statistical analyses, we used STATA, version 11.0 software. Forest and funnel diagrams were drawn and for heterogeneity, index was also considered. Also we used meta regression analysis. Results: Finally, we reviewed 10 studies. The prevalence of brain metastasis in HER2-positive breast cancer patients was 24.9%. There was publication bias in the reviewed studies. Meta regression analysis showed that follow up time had no significant effect (p=0.396) on the prevalence of brain metastasis. Conclusions: The results showed a high prevalence of brain metastasis in HER2 positive breast cancer patients.

A Study on the urban housewives wedding behavior and satisfaction - focus on the housewives who have been married for less than five years - (도시주부의 혼례행동 및 혼례만족에 관한 연구 - 결혼 5년 이내의 주부를 중심으로 -)

  • The purpose of this study is to investigate (1)the level of the urban housewives’behavior and satisfaction of wedding, (2)the influential factors related to the two dependent variables above mentioned. So that provides some fundamental materials to improve the level of sound wedding culture and the whole home living. The subjects were 356 housewives, in April, 1997, Seoul. The data obtained were analyzed by Mean, Pearson’s correlation, Stepwise Multiple Regression and Path Analysis. The major findings were as follows: 1) The general tendency of the housewives’wedding behavior and satisfaction was reasonable. 2) According to the background variables(ie: marital form, the existence of job, the recognition degree of her husband’s family’s living standards, the recognition degree of her parents’home’s living standards, the perception of marital transactions), the housewives’wedding behavior was significantly different. 3) According to (1)the background variables(ie: communication frequency in household, self-acceptance, the adequacy of household income, educational level), (2)intermediated variable(ie: articles essential to a marriage), the housewives’wedding satisfaction was significantly different. 4) The indirect variable of the positive influence for housewives’satisfaction of wedding was marital form, the existence of job. the indirect variable of the negative influence for housewives’satisfaction of wedding was the recognition degree of her husband’s family’s living standards, the recognition degree of her parents’home’s living standards, the perception of marital transfactions.

Molecular Types and Neoadjuvant Chemotherapy in Patients with Breast Cancer- While Molecular Shifting is More Common in Luminal a Tumors, The Pathologic Complete Response is Most Frequently Observed in Her-2 Like Tumors

  • Background: Pathologic complete response (pCR) is one of the most important target end-points of neoadjuvant chemotherapy (NACT) in patients with breast cancer (BC). In present study, we aimed to investigate the relationship between molecular subtypes and NACT in patients with BC. Materials and Methods: Using the Akdeniz University database, 106 patients who received NACT for operable breast cancer were retrospectively identified. Prognostic factors before and after NACT were assessed. According to the molecular subtypes, molecular shifting after NACT and tumoral and nodal response to NACT were analyzed. Results: The distribution of subtypes was: Luminal A, 28.3% (n=30); Luminal B, 31.1% (n=33); HER2-like, 24.5% (n=26); and basal like/triple negative (BL/TN), 16.0% (n=17). According to molecular subtypes, pCR rates in both breast and axillary were 0%, 21.4%, 36.4% and 27.3% for luminal A, luminal B, HER2-like and BL/TN, respectively (p=0.018). Molecular subtype shifting was mostly seen in luminal A type (28.6%) after the NACT. The pCR rate in breast and axillary was significantly higher in patients with HER2-like type BC. Conclusions: In patients with HER-2 like type BC, NACT may be offered in early stages. Additionally, due to molecular shifting, adjuvant treatment schedule should be reviewed again, especially in the luminal A group.

Molecular Profiling of Breast Carcinoma in Almadinah, KSA: Immunophenotyping and Clinicopathological Correlation

  • Purpose: To subtype breast cancer (BC) in Saudi women according to the recent molecular classification and to correlate these subtypes with available clinicopathological parameters. Materials and Methods: Estrogen receptor (ER), progesterone receptor (PR) and human epidermal growth factor receptor (Her2/neu) immunostaining was semi-quantitatively assessed to define molecular subtypes of luminal A and B, HER-2 and triple negative (basal-like) in BC paraffin embedded sections from 115 Saudi female patients diagnosed between 2005 to 2015 at the Department of Pathology, King Fahd Hospital, Almadinah, Saudi Arabia. Results: The most common subtypes were luminal A (47%), followed by luminal B (27.8%) and basal like subtypes (18.3%), whereas HER-2 was the least common subtype (6.9%). Luminal A was predominantly found in the old age group, with low tumor grade (p<0.001) and small tumor size, whereas HER-2 and basal-like subtypes were significantly associated with young age, high tumor grade, lymph node metastasis and lymphovascular invasion (p<0.03, 0.004, 0.05 and 0.04 respectively). All subtypes showed advanced clinical stage at the time of presentation. Conclusions: Molecular subtypes of Saudi BC patients in Almadinah region are consistent with most of the worldwide subtyping. The biological behaviour of each molecular subtype could be expected based on its characteristic clinicopathological features. Along with other prognostic indicators, molecular subtyping would be helpful in predicting prognosis and management of our BC patients. We recommend screening and early diagnosis of BC in our population.

Virtual Screening Approaches in Identification of Bioactive Compounds Akin to Delphinidin as Potential HER2 Inhibitors for the Treatment of Breast Cancer

  • Small molecule tyrosine kinase inhibitors targeting HER 2 receptors have emerged as an important therapeutic approach in inhibition of downstream proliferation and survival signals for the treatment of breast cancers. Recent drug discovery efforts have demonstrated that naturally occurring polyphenolic compounds like delphinidin have potential to inhibit proliferation and promote apoptosis of breast cancer cells by targeting HER2 receptors. While delphinidin may thus reduce tumour size, it is associated with serious side effects like dysphonia. Owing to the narrow therapeutic window of delphinidin, the present study aimed to identify high affinity compounds targeting HER2 with safer pharmacological profiles than delphinidin through virtual screening approaches. Delphinidin served as the query parent for identification of structurally similar compounds by Tanimoto-based similarity searching with a threshold of 95% against the PubChem database. The compounds retrieved were further subjected to Lipinski and Verber's filters to obtain drug like agents, then further filtered by diversity based screens with a cut off of 0.6. The compound with Pubchem ID: 91596862 was identified to have higher affinity than its parent. In addition it also proved to be non-toxic with a better ADMET profile and higher kinase activity. The compound identified in the study can be put to further in vitro drug testing to complement the present study.

A Case Report of a Guillain-Barre Syndrome Patient Suspected of Having Acute Transverse Myelitis Treated with Korean Medicine (급성 횡단성 척수염으로 의심된 길랑바레증후군 환자에 대한 한방 치료 증례보고 1례)

  • A 67-year-old female patient diagnosed with acute transverse myelitis visited our hospital in February 2023. Consultation papers indicated that she was diagnosed with acute transverse myelitis and that she complained of quadriparesis and numbness in both hands and feet. However, her spine magnetic resonance imaging (MRI) did not clearly indicate that she suffered from acute transverse myelitis. She was then sent to a high-level hospital and diagnosed with Guillain-Barre Syndrome through electromyography, cerebrospinal fluid test, and spine MRI. On March 2023, she visited our hospital again, still complaining about weakness in both hands and inability to walk. She was treated with Korean medicine, including acupuncture, electroacupuncture, pharmacopuncture, and herbal medicine such as Yanggyeoksanhwatang and Sipimijihwang-tanggami. After three months, she was discharged on cane walking. The manual muscle test (MMT) grade of her upper limbs increased from 3-/3-/3-/3- (shoulder/elbow/wrist/fingers) to 4/4/4/3+, and the MMT grade of her lower limbs increased from 2/2/2/2 (pelvis/knee/ankle/feet) to 4/4/4-/4-. Her numbness in both feet disappeared, and her numbness in both hands decreased from a numeric rating scale of 7 to 2. No adverse events were reported during treatment. This case implies the therapeutic potential of Korean medicine for Guillain-Barre Syndrome, especially for those who had delayed diagnosis and missed the opportunity to get plasma exchange.

CYP2D6 Genotype and Risk of Recurrence in Tamoxifen Treated Breast Cancer Patients

  • Background: Despite consistent pharmacogenetic effects of CYP2D6 on tamoxifen exposure, there is considerable controversy regarding the validity of CYP2D6 as a predictor of tamoxifen outcome. Understanding the current state of evidence in this area and its limitations is important for the care of patients who require endocrine therapy for breast cancer. Materials and Methods: A total of 101 patients with breast cancer who received tamoxifen therapy for at least 3 years, were genotyped for common alleles of the CYP2D6 gene by nested-PCR and restriction fragment length polymorphism PCR. Patients were classified as extensive or poor metabolizers (PM) based on CYP2D6*4 alleles in 3 different groups according to the menopause, Her2-neu status, and stage 3. Results: The mean age of the patients with the disease recurrence was $50.8{\pm}6.4$ and in non recurrent patients was $48.2{\pm}6.8$. In this study 63.3% (n=64) patients were extensive metabolizers and 36.6% (n=37) were poor metabolizers. Sixty four of the 101 patients (63.3%) were Her2-neu positive. For tamoxifen-treated patients, no statistically significant difference in rate of recurrence observed between CYP2D6 metabolic variants in stage 3 and post-menopausal patients. However, there was a significant association between CYP2D6 genotype and recurrence in tamoxifen-treated Her2-neu positive patients. Compared with other women with breast cancer, those with Her2-neu positive breast cancer and extensive metabolizer alleles had a decreased likelihood of recurrence. Conclusions: This study for the first time demonstrated significant effects of CYP2D6 extensive metabolizer alleles on risk of recurrence in Her2-neu positive breast cancer patients receiving adjuvant tamoxifen therapy. Therefore, CYP2D6 metabolism, as measured by genetic variation, can be a predictor of breast cancer outcome in Her2-neu positive women receiving tamoxifen.

Exploring Science Teacher Agency at the Age of the New Normal after the COVID-19 Pandemic: The Case of Second Year of Distance Learning Practice (코로나 뉴노멀 시대의 과학교사 행위주체성 탐색 -COVID-19에 따른 원격 수업 2년 차 실행 사례를 중심으로-)

  • The COVID-19 pandemic has changed everything, even education. Last year when distance learning was introduced, science teachers faced many challenges to overcome. However, teachers adapted quickly, and in this year, it became the 'new normal'. In this situation, teachers are likely to habitually repeat past practices, but the context of the second year of distance learning is changing constantly, and teachers are asked to interpret the problem occurring in a situation and to adjust their practice for solving the problem with their teacher agency. In this study, we explore the emergence of science teacher agency and factors shaping teacher agency in the second year of distance learning and we focus on teachers' agentic practice that did not follow their past practice without reflection. For this purpose, we mainly analyze the semi-constructed interview of three science teachers. In the first year of distance learning, two teachers maintained their practice, not much different to face-to-face learning. However, one teacher reflected upon herself and started to recognize and solve her problems. Reflection for her practice can support this process. Another teacher changed her practice due to external suggestions, but it evolved her practice to fit the situation better, and her experience of last year helped her to adapt to the change. The other teacher who modified her practice to persist her professional purpose last year was consistently practicing in the second year and collaboration and autonomy can support her. This study shows the teachers' dynamic change of agency and the emergence for the relational interaction between teacher and context.

  • The number of Frida Kahlo's works is 143, 55 of which are her own portraits. She spent a great deal of time on the production of her works, a large portion of which is taken up by her portraits. Art and creation by artist are the expressive process of artist's unconsciousness. Certain works express personal unconsciousness and collective, archetypal unconsciousness. These works evoke certain impressive image or unknown emotion in their mind impulsively. Artist could talk with their own work in this process. The producted symbolic images want to seek harmony with the artist's consciousness. This study has examined, from the viewpoint of analytical psychology, the psychological changes of Frida Kahlo by way of interpreting 4 of her own realistic portraits. These particular 4 works were finished in the most critical life time of Frida Kalho. We tried to find out what the purpose of her work would be. From the examination the following characteristics were gound. 1) Her portraits are not only concerned with her personal life and personal unconscious, but they also contain elements of the collective unconscious. 2) She experienced a psychological healing for herself in the process of transforming shocking events into painful portraits. 3) Her self portraits are the expression of the creative transformation she underwent.

Measurement for the Tuning Circle of the Stern Trawler HAELIM-3 by the Differential GPS (DGPS에 의한 선미트롤선 해림 3호의 선회권측정)

  • The methods by turning circle test and maneuvering indices have been used to study and evaluate the maneuverability of a ship. However recently many studies utilizing the GPS are made on the measurement of the turning circle and in the fishery and hydrographic survey. In this paper, the author carried out the turning circle test using the differential GPS and dumb card together, and compared the data measured by them and analyzed the accuracies of them to obtain the utility basic ones on the measurement of the turning circle by the DGPS. The main results area s follows : 1) To check the accuracies of the GPS, the circling experiments of 50m radius by the DGPS were made on the ground. The accuracies of turning circle measured by the DGPS were found to be very high as the errors of 1.5m. 2) the turning circle by the DGPS could be measured very accurately, by the seed, rudder angle, starboard and port respectively. 3) The turning circle measured by the dumb card was found to be measured accurately as much as the DGPS, when using large rudder angle, the turning circle was large, the turning circle by the dumb car could not be measured accurately on account of large error of bearing of compass. 4) The tactical diameters by the DGPS in case of the rudder angle 35。~5。, were found to be 2.6。15.0 times the Lpp of S.T HAELIM-3 at her slow speed 2.8~16.6 times her Lpp at her half speed, 3.1~17.4 times her Lpp at her full speed. The tactical diameter by the dumb card was found to be 2.4~9.5 times, 2.6~9.6 times, 3.2~12.2 times her Lpp respectively, in the above case and speed.
