건강검진자료에 의한 일반 성인들의 혈압과 혈청지질 및 비만지표와의 관련성 (Relationships Between Blood Pressure and Serum Lipids, Obesity Indices in Adults using Health Examination Data)

  • 본 연구의 목적은 건강검진을 받은 일반 성인들의 혈압과 혈청지질치 및 비만지표와의 관련성을 검토하고자 하였다. 조사대상은 2012년 한 대학교병원에서 건강검진을 받았던 20세 이상 일반 성인 2,734명(남자 1,344명, 여자 1,390명)을 대상으로 하였다. 조사는 해당 대학병원 연구윤리위원회(IRB)의 승인을 받은 후 대상자들의 종합건강검진 결과표와 문진표로부터 자료를 수집하였으며, 조사항목으로는 피조사자들의 성별, 연령, 안정 시 혈압(SBP, DBP), 혈청지질치(TC, HDL-C, LDL-C, TG, AI) 및 비만관련지표(BMI, 체지방률, 허리둘레) 등이었다. 연구결과, TC, TG, AI는 남녀 모두 고혈압군이 비고혈압군보다 유의하게 높았으며, HDL-C는 여자에서 비고혈압군이 고혈압군보다 유의하게 높은 반면, LDL-C는 고혈압군이 비고혈압군보다 유의하게 높았다. BMI, 체지방률, 허리둘레는 남녀 모두 고혈압군이 비고혈압군보다 유의하게 높았다. SBP와 DBP는 남녀 모두 연령, TC, TG, AI, BMI, 체지방률 및 허리둘레와 유의한 양의 상관관계를 보였다. 그러나 HDL-C와 LDL-C는 여자에서만 각각 음의 상관관계와 양의 상관관계를 보였다. 연령을 조정한 고혈압발생위험비는 남녀 모두에서 TC, TG, BMI, 허리둘레가 정상인 군보다 비정상인 군에서 유의하게 증가하였으며, HDL-C는 여자에서만, 체지방률은 남자에서만 유의하게 증가하였다. 이상과 같은 결과는 혈압이 여러 혈청지질치 및 비만지표들과 유의한 관련성을 갖고 있음을 시사한다.

비타민 B6 부족이 정기적인 운동 훈련시 연료의 이용과 혈액 콜레스테롤 성상에 미치는 영향 (The Effect of Vitamin B6 Deficiency on the Utilization of Fuel and Blood Cholesterol Profile with Regular Exercise-Training in Rats)

  • The purpose of this study was to determine whether vitamin B6(B6) deficiency affects fuel utilization and blood cholesterol profile with exercise-training. Twenty-four rats were fed a B6 deficient(-B6) diet or a control (+B6) diet for 5 weeks and either exercised(EX) or nonexercised (NE). EX rats were exercised on treadmill(10$^{\circ}$, 0.5-0.8km/h) for 20 minutes everyday. Glucose(GLU), glycogen (GLY), protein(PRO), trglyceride(TG), free fatty acid(FFA), total cholesterl(TC), HDL-cholesterol(HDL-C) and LDL-choleterol(LDL-C) were compared in plasma(P), liver(L) and skeletal muscle(M) of rats. There was a vitamin effect on the level of P-GLU, P-TG, M-TG, L-GLY, L-PRO and an exercise effect on the level of P-PRO, P-FFA, M-PRO, L-GLY, L-TG, P-TC, P-HDL-C, P-LDL-C. Compared to +B6 rats were lower and there were no differences in P-GLU, P-FFA, P-TG. M-GLY, L-TG, P-TC and P-HDL-C. In EX group, the level of P-TG was higher and M-PRO was lower in -B6 rats. There were no differences in M-GLY, L-TG, P-TC and P-HDL-C. These results suggest that a lowered intake of vitamin B6 may impair the adaptation of animals to fuel metabolism related to a decrease of fatty acid oxidation and attenuates the exercise-traning effect on blood lipid profile.

남성 사무직 근로자의 중성지방/고밀도 지단백 콜레스테롤 비와 대사증후군 간의 관계 (Relationship between Metabolic Syndrome and the Triglyceride/High-density Lipoprotein- Cholesterol ratio in Male Office Workers)

  • Purpose: The triglyceride-to-high-density lipoprotein-cholesterol (TG/HDL-C) ratio is one of the main predictive indices for cardiovascular disease. This study was examined the relationship between TG/HDL-C ratio and metabolic syndrome (MetS) in male office workers. Methods: Secondary analysis was conducted to determine the risk between the TG/HDL-C ratio and MetS in male office workers. A total of 765 people underwent the 'regular workplace health checkups in 2014'. Among the subjects who were male and responded to the questionnaire and health lifestyle survey, 470 (61.4%) excluding those with missing and/or abnormal values were analyzed. The association between MetS, MetS components, and the TG/HDL-C ratio was examined by a Chi-square test, One-way ANOVA, Turkey post-hoc test and Logistic regression analysis. Results: The number of males with MetS was 70 (14.9%) and the number of MetS components increased with increasing TG/HDL-C ratio (p<.001). Logistic regression analysis with an adjustment for potential confounders revealed a 31.8 times higher odds ratio of the Quartile4 group for MetS than that of the Quartile1 group (p<.001). Conclusion: These results show that the likelihood of MetS, particularly the risk of MetS in the Quartile4, increases with increasing TG/HDL-C ratio.

Association between drinking behaviors and components of metabolic syndrome in subjects in their 20s and 30s: data obtained from the Korea National Health and Nutrition Examination Survey (2016-2018)

  • BACKGROUND/OBJECTIVES: Numerous studies have examined the relationship between drinking behaviors and metabolic syndrome (MetS) for adults, but these include very few studies for young adults. This study therefore undertook to investigate the association between drinking behaviors and components of MetS among adult drinkers aged 20-30 years. SUBJECTS/METHODS: Using the 2016-2018 Korea National Health and Nutrition Examination Survey data, drinking behaviors of adults in the age group 20-30 years were divided into 4 groups: 1) group A, good drinking habits; 2) group B, frequent binge drinking but not frequent drinking; 3) group C, frequent drinking but not frequent binge drinking; 4) group D, frequent drinking and binge drinking. The association between MetS components and drinking behaviors was analyzed by applying multiple logistic regression analysis. RESULTS: We determined the prevalence risk compared to group A. In men, the prevalence risk of high triglyceride (TG) increased 2.051-fold in group C and 1.965-fold in group D. Moreover, in group D, the prevalence risk of low high density lipoprotein cholesterol (HDL-C) increased 0.668-fold, high blood pressure (BP) increased 2.147-fold, and MetS increased 1.567-fold. In women, there was an increased prevalence risk of low HDL-C (0.353-fold) and MetS (3.438-fold) in group C, whereas group D showed increased prevalence risk of abdominal obesity (2.959-fold), high TG (1.824-fold, and low HDL-C (0.424-fold). CONCLUSIONS: Our study indicates that frequent drinking increases the risk of high TG, whereas frequent and binge drinking increases the risk of high TG, low HDL-C, high BP, and prevalence of MetS in men. In women, frequent drinking without binge drinking increases the risk of low HDL-C and MetS, whereas frequent and binge drinking increases the risk of abdominal obesity, high TG, and low HDL-C. We propose that improvements in the drinking behaviors can reduce the prevalence of MetS.

내인성 및 외인성 Estrogen이 관상심장질환 위험인자에 미치는 영향 -제1보 : 내인성 및 외인성 Estrogen이 혈액 지질 농도에 미치는 영향- (The Effects of Endogenous and Exogenous Estrogen on Risk Factors of Coronary Heart Disease -Part I : On Serum Lipid Levels-)

  • To determine the effects of endogenous and exogenous strogen on serum lipid levels, twenty nonsmoking healthy Korean women were participated in this experiment for 12 weeks. They were assigned to three groups : (1) eight women aged 22 to 30(yr) for the premenopausal(Pre) group, (2) eight, aged 49 to 60(yr) for the postmenoparusal(Pst) group, (3) four, aged 23 to 30(yr) for the oral contraceptive(OC) group which used triphasic OC formulation. Fasting blood samples representing every phase of the hormonal levels were obtained from the subjects of the Pre and the OC group. From the subjects of the Pst group, fasting blood samples were obtained once per three weeks for 12 weeks. All the serum data were adjusted for dietary effects, exercise, personality type and body mass index(BMI) by using analysis of covariation(ANCOVA). Serum lipid levels of the three groups were significantly different. While serum levels of triglycerides(TG)(p<0.0001), low density lipoprotein-chloesterol(LDL-C)/high density liporotein-cholesterol(HDL-C) ratio (LDC-C/HDL-C)(P<0.01) and total cholesterol (TC)/HDL-C ratio (TC/HDL-C)(P<0.001) were significatnly high in the Pst group, serum HDL-C(P0.001) level was significantly high in the Pre group. The OC group showed significantly low serum TC(P<0.0001) and LDL-C(P<0.0001) levels. There was no signidicant difference in the fluictuation of serum lipid levels during the menstrual cycle of the Pre group. However, in the OC group, serum TG level was significantly increased at phase 2(P<0.05) where exogenous estrogen administration was highest. Even though other serum lipid levels of the OC group were not significantly fluctuated according to the exogenous estrogen administration, there was a trend of increased levels of serum TC, LDL-C, LDL-C/HDL-C and TC/HDL-C and decreased level of HDL-C during the menstruation period. Also, serum TC level was high(P<0.005) and serum TG level was low (P<0.005) at the baseline of the OC group compared with the periods of OC administration. When screening and counseling the female population at risk for coronary heart disease(CHD), the result of this study suggest that in may be desirable to divide the population into several groups according to their personal physiological characteristics, such as age, OC administration, menstrual cycle and menopause, as well as general risk factors for CHD.

고혈압군과 정상혈압군 환자에서 hs-CRP와 지질 측정값의 비교 (Comparison of hs-CRP and Lipid Values Between Hypertensive and Normotensive Groups)

  • Lipids such as total cholesterol (TC), high density lipoprotein-cholesterol (HDL-C) and high sensitivity C-reactive protein (hs-CRP) are known as good risk factors for coronary artery diseases caused by arthrosclerosis. We measured the serum concentrations of hs-CRP, TC, and HDL-C of 6,128 persons who underwent medical examinations in the Health Screening Center at Kangbuk Samsung Hospital. We classified the subjects into normotensive and hypertensive groups according to the blood pressure measured. We investigated whether there was a difference of hs-CRP and lipids between the normotensive and hypertensive groups. There was significant difference of TC, TC/HDL-C ratio, and hs-CRP concentration between the normotensive and hypertensive groups in both men and women, but there was no significant difference of HDL-C concentration between two groups. Our results indicated that the concentrations of hs-CRP and lipids were higher in patients with hypertension and confirmed the usefulness of lipids and hs-CRP in predicting the risk for coronary artery diseases.

  • Purpose : The lipid profile is a major predictive index for cardiovascular disease, but there have only been a few studies conducted on the relationship between lipid ratio and cardiovascular disease risk factors in the Korean population. To address this research gap, we investigated the association between three lipid ratios and cardiovascular disease risk factors among Korean adults. Methods : This study used data from the seventh Korea National Health and Nutrition Examination Survey conducted in 2016. Lipid ratios included triglyceride (TG)/high density lipoprotein cholesterol (HDL), total cholesterol (TC)/HDL, and low density lipoprotein cholesterol (LDL)/HDL. Cardiovascular risk factors included in this research were: systolic blood pressure $(SBP){\geq}140$, diastolic blood pressure $(DBP){\geq}90$, fasting blood sugar $(FBS){\geq}126$, $HbA1c{\geq}6.5$, body mass index $(BMI){\geq}25$, waist circumference (WC) $men{\geq}90$, $women{\geq}85$, and metabolic syndrome (MetS). A complex samples logistic regression test was performed to analyze the association between lipid ratios and cardiovascular disease risk factors. Results : 1) TG/HDL ratio had statistically significant relationships with DBP, FBS, HbA1c, BMI, WC and MetS. 2) TC/HDL ratio was correlated to SBP, DBP, FBS, BMI, WC, and MetS. 3) LDL/HDL ratio had association with BMI, WC, and MetS. Conclusion : We identified significant association between lipid ratios and cardiovascular disease risk factors. The three lipid ratios were particularly strongly associated with BMI, WC, and MetS.

건강한 성인에서 성별에 따른 혈중 콜레스테롤 농도와 우울증상의 연관성 : 후향적 의무기록 조사 (Gender Difference in Associations between Serum Cholesterol Levels and Depression Symptoms in Healthy General Population)

  • 연구목적 본 연구는 우울 증상과 혈중 콜레스테롤 농도의 연관성이 성별에 따라 차이가 있는지 살펴보고자 하였다. 방법 건강검진을 받은 일반인 중 27,452명(남자 15,044명, 여성 12,408명)을 대상으로 후향적 의무기록 조사를 통하여 이루어졌다. 대상자들의 벡우울척도(Beck Depression Inventory, BDI)점수를 구하고 총콜레스테롤(Total cholesterol, TC), 중성지방(Triglyceride, TG)), 고밀도 지단백 콜레스테롤(High density lipoprotein cholesterol, HDL-C), 저밀도 지단백 콜레스테롤(Low density lipoprotein cholesterol, LDL-C)을 분석하였다. 결과 남성은 총콜레스테롤(TC)과 벡우울척도(BDI) 점수 사이에는 통계적으로 유의한 상관성을 보이지 않았다. 중성지방(TG)은 벡우울척도(BDI) 점수와 양의 상관성을 보였으며(r=0.020, p<0.01), 고밀도 지단백 콜레스테롤(HDL-C)와 저밀도 지단백 콜레스테롤(LDL-C)은 벡우울척도(BDI) 점수와 음의 상관성을 보였다(r=-0.016, p<0.01 ; r=-0.015, p<0.05) 여성의 경우도 총콜레스테롤(TC)과 벡우울척도(BDI) 점수 사이에는 통계적으로 유의한 상관성이 없었으며 중성지방(TG)은 벡우울척도(BDI)점수와 양의 상관성을 보였고(r=0.020, p<0.01) 고밀도 지단백 콜레스테롤(HDL-C)과는 음의 상관성을 보였다(r=-0.019, p<0.01). 남성과 달리 저밀도 지단백 콜레스테롤(LDL-C)은 통계적으로 유의한 상관성을 보이지 않았다. 결론 남녀 모두 중성지방(TG)이 높을수록, 고밀도 지단백 콜레스테롤(HDL-C)이 낮을수록 우울 증상의 정도가 심한 것으로 나타났으며, 남성에서만 저밀도 지단백 콜레스테롤(LDL-C)이 낮을수록 우울 증상이 심한 것으로 나타났다.

60%HRmax 운동프로그램이 혈중 저밀도, 고밀도 콜레스테롤 수치 변화에 미치는 영향 (The effect of 60%HRmax exercise program in LDL-C, HDL-C)

  • The purpose of this study is to investigate the effect of 60%HRmax exercise program on LDL-C, HDL-C. The subject of this study, 16 female university students in Seoul, who are randomly sampled and divided into two groups : experimental group (N=8), and control group (N=8). The following results are obtained this study: First the LDL-C levels of two groups are not significantly different in the training period. The LDL-C level of experimental group decreased in after-exercise(4 week, 8 week) in comparison with before-exercise. Specially it decrease significant after 4 week exercise. And it is no significant difference in control group. Second, the HDL-C levels of two groups are not significantly different before exercise but are significant different after exercise(4 week, 8 week). Although the HDL-C level of the experimental group decrease in after-exercise(4 week, 8 week), but it is not significant decrease. And it is not significant difference in control group.

건강체력 수준과 심혈관질환 관련 위험인자와의 관련성 (The Link between Health-related Physical Fitness Level and Cardiovascular Disease-related Risk Factors)

  • Purpose: The purpose of this study was to identify the link between health-related physical fitness level and cardiovascular disease-related risk factors in adult male workers. Methods: We tested cardiovascular disease-related risk factors (waist circumference, SBP, DPB, fasting glucose, TC, HDL-C, TG, LDL-C) and health-related physical fitness ($VO_2max$, grip, Sit-up, Flexibility, Body fat) and divided health-related physical fitness level of the subjects into 3 groups - A (very good, n=56), B (good, n=59), and C (below-average, n=57) according to the criterion of the Health and Fitness counseling guidelines of KOSHA. The statistical techniques such as standard deviation, one-way ANOVA and multiple regression (p<.05) were used. Results: There were significant differences between group C and group B & A (p<.001) in waist circumference, DBP, Fasting glucose, HDL-C, TG, LDL-C. In TC/HDL-C, TG/HDL-C, LDL-C/HDL-C, Group C was higher than group B and A. Conclusion: On the basis of these results, we identified that improvement of health-related physical fitness level positively effects on the decrease of cardiovascular disease-related risk factors.
