Purpose: Perioperative chemotherapy improves survival outcomes in locally advanced (LA) gastric cancer. Materials and Methods: We retrospectively analyzed patients with LA gastric cancer who were offered perioperative chemotherapy consisting of epirubicin, oxaliplatin, and capecitabine (EOX) from May 2013 to December 2015 at Tata Memorial Hospital in Mumbai. Results: Among the 268 consecutive patients in our study, 260 patients (97.0%) completed neoadjuvant chemotherapy, 200 patients (74.6%) underwent D2 lymphadenectomy, and 178 patients (66.4%) completed adjuvant chemotherapy. The median follow-up period was 17 months. For the entire cohort, the median overall survival (OS), 3-year OS rate, median progression-free survival (PFS), and 3-year PFS rate were 37 months, 64.4%, 31 months, and 40%, respectively. PFS and OS were significantly inferior in patients who presented with features of obstruction than in those who did not (P=0.0001). There was no difference in survival with respect to tumor histology (well to moderately differentiated vs. poorly differentiated, signet ring vs. non-signet ring histology) or location (proximal vs. distal). Survival was prolonged in patients with an early pathological T stage and a pathological node-negative status. In a multivariate analysis, postoperative pathological nodal status and gastric outlet obstruction on presentation significantly correlated with survival. Conclusions: EOX chemotherapy with curative resection and D2 lymphadenectomy is a suggested alternative to the existing perioperative regimens. The acceptable postoperative complication rate and relatively high resections, chemotherapy completion, and survival rates obtained in this study require further evaluation and validation in a clinical trial.

At concentrations of 1 mM, the protective effects of carnosine and related compounds including anserine, homocarnosine, histidine, ${\beta}$-alanine were investigated against copper-catalyzed oxidative damage to deoxyribose, ascorbic acid, human serum albumin, liposome, and erythrocytes. Carnosine and anserine reduced Cu (II) to bathocuproine-reactive Cu (I) in a time- a and a dose-dependent manner while the others did not. Carnosine reduced 86% of $100\;{\mu}M$ Cu (II) in 60 min. Carnosine, homocarnosine, anserine, and histidine inhibited copper-catalyzed deoxyribose degradation by 75, 66, 65, and 45%, respectively. In the presence of $1\;{\mu}M$ Cu (II), carnosine and related compounds inhibited ascorbic acid oxidation by 55-85% after incubation for 20 min. In the presence of 0.15 mM ascorbic acid and 0.8 mM $H_2O_2$, carnosine, anserine, homocarnosine, and histidine inhibited copper-catalyzed oxidation of human serum albumin by 41, 21, 29, and 24%, respectively, as determined by carbonyl formation. These compounds also significantly inhibited copper-catalyzed liposomal lipid peroxidation as measured by malondialehyde and lipid hydroperoxides. Carnosine, anserine, homocarnosine, and histidine inhibited hemolysis of bovine erythrocytes induced by 0.1 mM Cu (II). These results suggest that histidine-containing dipeptides may play an important role in protecting against free radical-mediated tissue damage.
This study used Omija extract as a natural congelation to compare and analyze soybean curd's physicochemical and sensory quality characteristics in order to improve functional benefits and taste of soybean curd. When Omija extract concentration increased, protein content went up considerably while crude fat and yield significantly decreased. In the pH change, the group with Omija extract were lower than control and the change was not much noticeable but slightly checked as the storage period was extended. The turbidity tended to increase as the storage period was longer. In the acidity change, the group with 0.5% Omija extract showed rapid increase on the 4th day after starting storage, and it can be interpreted that decomposition started at the moment. As the storage period was extended, brightness and yellowness remarkably decreased and redness considerably enhanced: higher concentration Omija extract worked to decreased brightness and to increase yellowness and redness. In accordance with the storage period, hardness, brittleness and gumminess increased and springness decreased, but there was no considerable change in cohesiveness: in accordance with the concentration, hardness, brittleness and gumminess significantly increased, but there was no considerable change in cohesiveness. In terms of sensory quality, the group with 1% of Omija extract showed the best appearance, flavor, taste and after swallowing results. The group with 1% Omija extract was the most preferred, $4.89{\pm}0.32$ in the overall preference. In conclusion, adding Omija extract can improve soybean curd's physicochemical and sensory quality characteristics. Moreover, the extracts can be expected to play an important role in encouraging Omija's value and widening its appliances to various food.

Background A thin perforator flap is one of the best methods for covering defects. This study aimed to revisit and further test the rapidly advancing field of flap thinning techniques. Methods We performed two cadaveric studies to test the known flap thinning methods, and then applied these methods to a clinical series. In the first study, five cadavers were used to observe the anatomical relation of the perforator with the subdermal plexuses and the subcutaneous fat layer by injecting a colored latex solution. The second study was done on four cadavers independently from the first study. Last, a clinical series was performed on 15 patients. Results The areolar fat lobules of 10 anterolateral thigh perforator (ALT), seven deep inferior epigastric artery perforator (DIEAP), and six thoracodorsal artery perforator (TAP) flaps were dissected to reduce the flap thickness guided by the colored vascular pattern. On average, the ALT, DIEAP, and TAP flaps were reduced to $32.76%{\pm}9.76%$, $37.01%{\pm}9.21%$, and $35.42%{\pm}9.41%$, respectively. In the second study, the areolar fat lobules were directly dissected in six ALT, six TAP, and four MSAP flaps, and an average reduction in flap thickness of $53.41%{\pm}5.64%$, $52.30%{\pm}2.88%$, and $47.87%{\pm}6.41%$, respectively, was found. In the clinical series, 13 out of the 15 cases yielded satisfactory outcomes with an average thickness reduction of $37.91%{\pm}7.15%$. Conclusions These multiple studies showed that the deep fat layer could be safely removed to obtain a thin yet viable perforator flap. This evidence suggests that the macroscopic flap thinning technique can achieve thin flaps. Surgeons should consider this technique before embracing the latest technique of supermicrosurgery.
This study was intended to examine the pattern and level of experience of voice problems and risk factors in school teacher, to offer basic data to prevent voice problems. The subjects were 180 school teachers from primary, middle, and high school teachers in C City, Chonbuk. Data were collected by questionnaires survey from April 30, 2001 to May 10, 2001. 56.6% of the subjects experienced voice problems such as "H-notes difficult"($2.68{\pm}.86$), followed by "Tired voice"($2.32{\pm}.93$), "Effortful voice"($2.27{\pm}.90$), and "Drying of throat"($2.21{\pm}.91$). According to general characteristics, smoking subjects showed higher voice problem than no smoking subjects and this difference was statistically significant(t=2.76, p=.007). According to voice related characteristics, "Speaking loudly" subjects showed higher voice problem than those no "Speaking loudly"(t=-2.02, p=.045). "Speaking effortful" subjects measured higher voice problem than those of "Speaking effortful" and this difference was statistically significant(t=-3.34, p=.001). The inspection of the relation of cause affecting school teachers showed that it had an interacting effect of voice habit, smoking, experienced voice problem, treatment(due to voice problem) experience, by staged it accounted for 55.3% of the total voice problem. The total voice problem indicated a positive correlation(p=.000). Conclusion: over half of the school teachers have experienced voice problems, higher perceived related of voice symptom and physical symptom factors. Also, because there are significant differences in voice problem according to smoking, voice habits of "Speaking loudly", "Speaking effortful", experienced voice problem, treatment(due to voice problem) experience.
This study was conducted to investigate the antitoxic components in the water extract of the roots of Trichosanthes kirilowii (Cucurbitaceae). The results were as follows: Generally, detoxication effects by the water extract of T. kirilowii increased in proportion to the concentrations. Experimental animals were treated with cadmium and T. kirilowii water extract by oral administration. When 40 mg/kg dosage of T. kirilowii extract was administrated it showed the highest antitoxic effects in metallothionein induction. After the water extract treatment, body weights did not increase in proportion to the extract concentrations. These results suggest that T. kirilowii extract increased metallothionein concentration and decreased the toxicity of cadmium in rats. In vitro the antitoxic activity of water extract of T. kirilowii on NIH 3T3 fibroblasts was evaluated by the MTT (3-(4,5-dimethylthiazol-2-yl)-2,5-diphenyl-2H-tetrazolium bromide) and SRB (sulforhodamine B protein) assays. The light microscopic study was carried out to observe morphological changes of the treated cells. These results were obtained as follows; The concentration of $10^{-2}\;mg/ml$ of T. kirilowii extract was shown significant antitoxic activity. The number of NIH 3T3 fibroblasts were increased and tend to regenerate. These results suggest that T. kirilowii extract retains a potential antitoxic activity.
The expression, purification, and characterization of cold-adapted lipase from the psychrophile, Janthinobacterium sp. were investigated. The gene encoding lipase from Janthinobacterium sp. PAMC 25641 was cloned into a pET28a(+) vector and heterologously expressed in Escherichia coli BL21 (DE3). The amino acid sequence deduced from the nucleotide sequence (930 bp) corresponded to a protein having 309 amino acid residues with a molecular weight of 32.7 kDa and a pI of 5.55. Recombinant E. coli harboring the Janthinobacterium lipase gene were induced by addition of isopropyl-${\beta}$-D-thiogalactopyranoside. $Ni^{2+}$-NTA affinity chromatography was used to purify the lipase, which had a specific activity of 107.9 U/mg protein. The effect of temperature and pH on the activity of lipase was measured using p-nitrophenyl octanoate as a substrate. The stability of the lipase at low temperatures indicated it is a cold-adapted enzyme. The lipase activity was increased by $Na^{2+}$, $Mg^{2+}$, and $Mn^{2+}$, and decreased by $Zn^{2+}$ and $Co^{2+}$. Analysis of the lipase activity using various p-nitrophenyl esters showed a strong preference toward short acyl chains of the esters, indicating the ability of the cold-adapted lipase to hydrolyze short-chain esters.
Four local cattle were ($145{\pm}9.7kg$) used in a $4{\times}4$ Latin square design to study the effect of different levels of rumen degradable protein (RDP) in straw based ration on purine derivatives excretion and microbial N supply in cattle. The four rations were formulated at the same amount of energy but varying RDP approximately 50 (U0), 75 (U1), 100 (U2) and 150 (U3) percent levels of RDP requirement for maintenance. They were fed ranged from 101 to 304 g RDP/d. Apparent digestibility of all nutrients increased significantly (p < 0.01) in cattle fed ration U2 than other rations. Rumen $NH_3-N$ concentration increased from 43 to 130 mg/l in response of RDP intake. Purine derivatives excretion increased significantly (p < 0.01) with incremental level of 203 g RDP/d (U2) intake and positively correlated (r=0.69, p < 0.01, n=16) with amount of RDP intake. The rates of rumen microbial N supply were 16.8, 27.2, 39.1 and 32.9 g/d for rations U0, U1, U2 and U3 respectively. Efficiency of microbial N supply (EMNS) per kg of DOMR were 19.0, 25.3, 33.0, and 28.6 g and per MJ of ME. Intake were 0.62, 1.00, 1.44 and 1.21 g for U0, U1, U2 and U3 respectively and highest results were obtained in cattle fed U2 ration. Results of this study suggest that PD excretion and EMNS were increased as incremental level of RDP intake (U2) in local cattle.

Objectives : The purpose of this study was to investigate the change of posture balance and muscular strength before and after exercise by conducting a health gymnastics program for the elderly with chronic diseases of musculoskeletal system. Methods : The subjects of this study were 32 elderly people who had no experience participating in the musculoskeletal system linkage gymnastics program over 65 years old in a rural area in H city, Gyeonggi-do. The data were analyzed by computerized processing with SPSS 23.0. Results : The results of the study were as follows: First, the average of muscular strength before and after gymnastics according to general characteristics was significant in average according to age, presence of spouse, education level, and cohabitation type. Second, the subjects exercised for 2 days a week, and 25.59(±0.51) minutes on average. Third, the change of balance of the face (t=2.993, p=.011), shoulder (t=3.811, p=.002) and pelvic left and right (t=3.584, p=.004) was statistically significant in the posture balance. Fourth, muscular strength was statistically significant in motor function of AMS, SMS, and FMS (p<0.001). Conclusion : Therefore, after applying the health gymnastics program, the improvement of posture balance and muscular strength of the elderly became apparent, so it is necessary to disseminate this gymnastics program. The health gymnastics program is expected to positively improve the quality of life for the elderly.

As a series of studies on improving quality of seasoned-dried Pacific saury, storage stability of seasoned-dried saury treated with liquid smoke (T2 product) was examined during storage at ambient temperature $(19\pm5^{\circ}C)$, comparing with control (C, seasoning only) and T1 (treatment of $0.05\%$ Rosemary as a antioxidant). The histamine contents (15.33-32.21mg/100g) of 3 seasoned-dried products were much lower than tolerance limit of intake during 80 days of storage. The water activity of 3 seasoned-dried products was in range of 0.6920.735. The pH of T2 showed relatively lower than the others during storage. The POV of T2 was slightly higher until 30 days, but was lower after 45 days compared with Tl. The TBA value and viable cell count of T2 were the lowest among the samples during storage. The color values of 3 seasoned-dried products were not significantly different during storage. The sensory scores of T2 was the highest than those of T1 and C during storage. Therefore, liquid smoking method was a useful technique to extend shelf-life and retain good sensory quality of seasoned-dried Pacific saury.
