중환자실 간호사의 기관내 흡인 임상지침 수행 현황 (Adherence to Clinical Guideline for Endotracheal Suction in ICU Nurses)

  • Purpose: This study aimed to evaluate the adherence to the clinical guideline for endotracheal suction in nurses working at intensive care units (ICU) and to identify the characteristics of nurses with good adherence. Methods: This study was a descriptive study to evaluate the pattern of adherence and its related factors to endotracheal suction. One hundred fifty five nurses working at ICU participated in this study. We used a questionnaire developed based on American Association for Respiratory Care (AARC) guidelines and other associated factors from previous studies. Results: Around half of the participants reported that their adherence to the clinical guideline was poor. Items deviated from the recommended guideline were reasons for initiating a suctioning, applied suction pressure ranged from 20 to 200mmHg, and applied catheter size from 6 to 17 french. Other factors deviated were the depth of inserted catheter, and inappropriate use of normal saline instillation. The most significant factor was related to hospital; the misused or misled clinical protocol. Conclusions: The adherence to the clinical guidelines of the endotracheal suction in ICU nurses was not appropriate, which might contribute to the patient health outcomes. More enhanced continuing education as well as hospital regulation is warranted.

  • Purpose: To identify the differences in perception of and adherence to the COVID-19 social distancing behavior guidelines among health care workers, patients, and the general public and to use them to prevent the spread of COVID-19. Methods: From October 16 2020 to November 30, 2020, a survey was conducted among 85 health care workers, 85 patients, and 82 general public regarding the perception of and adherence to the COVID-19 social distancing behavior guidelines. Results: Patients scored significantly higher than the general public in the adherence to the COVID-19 social distancing behavior guideline, and there was no difference between health care workers and general public, and patients and health care workers. In the multivariate analysis, the factors that influence the adherence of the COVID-19 social distancing behavior guide were found in women and the perception of the COVID-19 social distancing behavior guideline. Conclusion: In order to promote the implementation of the COVID-19 social distancing behavior guideline, it is necessary to increase the perception of the COVID-19 social distancing behavior guideline and provide additional education in men. It is necessary to investigate the reason behind why men have low adherence to the COVID-19 social distancing behavior guideline.

노인의 식생활지침 실천 평가도구 개발 (Development of an evaluation tool for dietary guideline adherence in the elderly)

  • 한국인을 위한 식생활지침은 건강한 식생활을 위한 가이드로 활용되는데, 식생활지침에 대한 세부 항목이 제시되지 않아서 지침에 대한 실천도를 자세히 평가하기는 어렵다. 따라서 본 연구는 식생활지침 실천도를 평가할 수 있는 세부 항목들을 선별하여 지침 실천도를 평가하는 평가도구를 만들고자 수행되었다. 노인의 식생활지침 실천 평가도구 개발을 위하여 문헌검토와 전문가 자문을 거쳐 총 35개의 평가 후보 항목을 추출하고, 전국단위 5개 지역의 남녀 노인 800명 (남 400명, 여 400명)을 대상으로 설문조사를 수행하였다. 식생활지침의 세 가지 영역별로 후보 항목에 대한 탐색적 요인분석으로 평가도구에 사용될 항목을 선별하고, 확인적 요인분석을 수행하여 구성타당도를 검증하였다. 구조방정식 모형의 경로계수를 활용하여 항목별 가중치를 부여하여 식생활지침 실천 점수를 계산하고, 전국조사 결과를 바탕으로 평가도구의 등급 체계를 마련하였다. 본 연구에서 개발된 노인 식생활지침 실천 평가도구는 총 28개 항목으로 식품섭취 영역 13항목, 식생활습관 영역 7항목, 환경을 생각하는 식생활 문화 영역 8항목으로 구성되었다. 식품섭취 영역에는 신선 생채소, 황색채소, 생과일, 콩·견과류, 생선·해산물, 고기·달걀 섭취, 우유·유제품, 잡곡 섭취, 물, 다양한 식품군 섭취, 육류 가시지방 제거, 단 음료, 염장 식품 섭취 항목이 포함되었고, 식생활습관 영역에는 건강체중 유지, 활동량 증가 노력, 운동, 과식정도, 과음빈도, 아침식사 빈도, 식사의 규칙성 항목이 포함되었다. 환경을 생각하는 식생활문화 영역에는 조리 및 음식 먹기 전 손 씻기, 유통기한 (소비기한) 확인, 개인 접시에 덜어 먹기 실천, 냉장고에 보관한 음식 덜어 먹기, 제철음식 섭취, 지역 농산물 이용, 식품 원산지 확인, 음식물 쓰레기 줄이기가 포함되었다. 전국설문조사 노인 (n = 800)의 식생활지침 실천 평균 점수는 56.9점이었고, 식품섭취 영역 49.8점, 식생활습관 영역 63.2점 그리고 식생활문화 영역은 58.6점이었다. 식생활지침 실천 점수는 기 개발된 노인 영양지수 및 식품정보이해력 점수와도 유의한 상관성을 보여서 식생활지침 실천 평가 도구가 식품섭취와 식행동 뿐만 아니라 지속가능한 식생활 실천 부분도 잘 반영한다는 것을 확인할 수 있었다. 앞으로 노인 맞춤형 식생활지침 실천 평가 도구가 노인들의 식품섭취, 식생활습관 및 식생활문화 영역를 종합적으로 평가하는 쉽고 간편한 검색 평가 도구로 활발하게 활용될 것으로 기대된다.

신종인플루엔자 감염학생의 스트레스와 자가치료지침 수행 (Stress and Self Care Guideline Adherence in Students with Influenza A (H1N1))

  • Purpose: This study examined levels of stress and adherence to self care guidelines in elementary and middle school students who were infected with the influenza A (H1N1) virus in 2009. Method: A total of 649 students from J city participated in the survey. Stress was measured with the Impact of Event Scale-Revised which was developed by Weiss and Marmar (1997) and translated by Eun and colleagues (2005). Adherence to self care guidelines was measured with the scale developed by the authors based on the self care recommendations from Korea Centers for Disease Control and Prevention. Result: Levels of stress and adherence to self care guidelines were higher in elementary school students. However, the severity of participants' stress was less than moderate. The adherence level was significantly different depending on status of taking an antiviral agent, health education and self care education regarding the virus infection. Conclusion: Findings suggest that effects of health education under the national crisis situation due to influenza A (H1N1) virus pandemic were helpful. More in-depth study is needed to understand and to improve middle school students' self care behaviors.

한국 유방암 경험자들의 유방암 식사지침 수행 정도와 건강관련 삶의 질의 연관성 (The Association between the Adherence to Dietary Guidelines for Breast Cancer Survivors and Health-related Quality of Life among Korean Breast Cancer Survivors)

  • Objectives: We examined the association between the adherence to dietary guidelines for breast cancer survivors and health-related quality of life in a cross-sectional study of Korean breast cancer survivors. Methods: A total of 157 women aged 21 to 79 years who had been diagnosed with stage I to III breast cancers according to the American Joint Committee on Cancer (AJCC) and had breast cancer surgery at least 6 months before the baseline were included. We used a Korean version of the Core 30 (C30) and Breast cancer 23 (BR23) module of the European Organization for Research and Treatment Cancer Quality of Life Questionnaire (EORTC-QLQ), both of which have been validated for Koreans. Participants were asked about their adherence to dietary guidelines for breast cancer survivors, suggested by the Korean breast cancer society, using a 5-point Likert scale. We summed dietary guideline adherence scores for each participant and calculated the least squares means of health-related quality of life according to dietary guideline adherence scores using the generalized linear model. Results: Breast cancer survivors who had higher adherence to dietary guidelines for breast cancer survivors had lower constipation scores than those with lower adherence (p for trend=0.01). When we stratified by the stage at diagnosis, this association was limited to those who had been diagnosed with stage II or III breast cancers. Also, sexual functioning scores increased significantly with increasing adherence scores of dietary guidelines among those with stage II or III breast cancers (p for trend < 0.001). However, among those who had been diagnosed with stage I, higher scores of dietary guidelines were associated with higher scores of pain (p for trend=0.03) and breast symptoms (p for trend=0.05). Conclusions: Our study suggested that the health-related quality of life levels of breast cancer survivors are associated with the adherence to dietary guidelines and may differ by the stage of the breast cancer.

Large Variation in Clinical Practice amongst Pediatricians in Treating Children with Recurrent Abdominal Pain

  • Purpose: To evaluate intra- and inter-observer variability and guideline adherence amongst pediatricians in treating children aged between 4 and 18 years referred with recurrent abdominal pain (RAP) without red flags. Methods: The first part of the study is a retrospective single-center cohort study. The diagnostic work-ups of eight pediatricians were compared to the national guidelines. Intra- and inter-observer variability were examined by Cramer's V test. Intra-observer variability was defined as the amount of variation within a pediatrician and inter-observer variability as the amount of variation between pediatricians in the application of diagnostic work-up in children with RAP. Prospectively, the same pediatricians were requested to provide a report on their management strategy with a fictitious case to prove similarities in retrospective diagnostic work-up. Results: A total of 10 patients per pediatrician were analyzed. Retrospectively, a (very) weak association between pediatricians' diagnostic work-ups was found (0.22), which implies high inter-observer variability. The association between intra-observer diagnostic was moderate (range, 0.35-0.46). The Cramer's V of 0.60 in diagnostic work-up between pediatricians in the fictitious case implied the presence of a moderately strong association and lower inter-observer variability than in the retrospective study. Adherence to the guideline was 66.8%. Conclusion: We found a high intra- and inter-observer variability and moderate guideline adherence in daily clinical practice amongst pediatricians in treating children with RAP in a teaching hospital.

  • Purpose: This study was to examine the relationships between knowledge with same sub-areas of education needs and adherence to self-management guidelines, and to investigate the factors that influence adherence. Method: One hundred eleven patients with terminal stage of kidney failure were conveniently selected from a university hospital in Incheon. Results: The level of knowledge was high, with a mean score of 15.25. The level of adherence to self-management guidelines was low, with a mean score of 64.53. The patients' education needs were high. The significant influencing factors on adherence were gender, educational level, and two sub-areas of knowledge, 'exercise and daily life' and 'follow-up care and treatment'. Conclusion: Relationships between knowledge, education needs, and adherence should be further examined before developing and implementing intervention programs for adherence to treatment guideline in hemodialysis patients.

Inverse association of improved adherence to dietary guidelines with metabolic syndrome: the Seoul Metabolic Syndrome Management program

  • BACKGROUND/OBJECTIVES: The Seoul Metabolic Syndrome Management (SMESY) program is a 1-yr lifestyle modification program targeting metabolic syndrome (MetS) in Seoul residents. This study investigated the associations between adherence to dietary guidelines and MetS among the SMESY program participants. SUBJECTS/METHODS: Data of 54,385 participants aged 20-64 yrs who completed the SMESY program in 2015, had information on adherence to dietary guidelines, and were not medicated for diabetes, hypertension, or dyslipidemia were analyzed. Participants underwent MetS screening and completed a lifestyle questionnaire including adherence to 10 dietary guidelines before and after participation. Participants were classified according to the number of MetS risk factors at baseline (MetS group, ≥ 3; risk group, 1-2; healthy group, none). Adherence to dietary guidelines was determined from the number of "yes" responses regarding the fulfillment of each guideline on ≥ 5 days/week. Multiple logistic regression was used to evaluate associations between newly diagnosed MetS and changes in adherence to dietary guidelines. RESULTS: In the MetS group, MetS prevalence decreased after the SMESY program (men, -41.9%p; women, -48.7%p), and all risk factors were significantly improved (P < 0.0001). All groups exhibited improved adherence to all dietary guidelines after participation (P < 0.0001). In the MetS group with positively changed adherence scores, the MetS prevalence decreased by -44.1%p for men and -49.5%p for women, whereas the prevalence in those with negative changes decreased by -38.1%p for men and -48.6%p for women. In the risk group, those with positively changed adherence scores had significantly decreased odds ratios (ORs) for newly diagnosed MetS compared with those with negative changes (OR, 0.70; 95% confidence interval [CI], 0.61-0.80 for men; OR, 0.88; 95% CI, 0.79-0.99 for women). CONCLUSIONS: The SMESY program may effectively reduce the risk of MetS among adults with risk factors by improving adherence to dietary guidelines.

The prevalence of obesity and the level of adherence to the Korean Dietary Action Guides in Korean preschool children

  • The objective of this study was to investigate the associations between the prevalence of overweight and obesity and the degree of adherence to the Korean Dietary Action Guides for Children (KDAGC). In a cross-sectional study based on a child care center-based survey in Seoul, Korea, we collected parental-reported questionnaires (n = 2,038) on children's weight and height, frequency of fruit and vegetable consumption, and the quality of dietary and activity behaviors based on the 2009 KDAGC Adherence Index (KDAGCAI) which was developed as a composite measure of adherence to the KDAGC. Overweight and obesity were determined according to age- and sex-specific BMI percentile from the 2007 Korean national growth chart. Associations were assessed with generalized linear models and polytomous logistic regression models. Approximately 17.6% of Korean preschool children were classified as overweight or obese. Obese preschoolers had lower adherence to the KDAGCAI compared to those with lean/normal weight. Preschoolers with a high quality of dietary and activity behaviors had a 51% decreased odds ratio (OR) of being obese (highest vs. lowest tertile of KDAGCAI-score, 95% CI 0.31, 0.78; P = 0.001); the associations were more pronounced among those who were older (P = 0.048) and lived in lower income households (P = 0.014). A greater frequency of vegetable consumption, but not fruit, was associated with a borderline significant reduction in the prevalence of obesity. Our findings support the association between obesity prevention and high compliance with the Korean national dietary and activity guideline among preschool children.
