The Importance of Nutritional Assessment and Dietary Counseling in Infants and Young Children with Common Illnesses (소아의 흔한 질병 상황에서 식이, 영양의 중요성)

  • The influence of nutrition during early life on physical growth as well as mental development has been thoroughly discussed in the literature. The physical dimensions of the body are greatly influenced by nutrition, particularly during the period of rapid growth in early childhood. Nutritional status affects every pediatric patient's response toillness. Good nutrition is important for achieving normal growth and development. It is indicated that permanent impairment of the central nervous system may result from dietary restriction of imbalance during certain periods of life. If children under 3 years of age show a good nutritional status, it may be assumed that they are well nourished. Several common diseases of children such as iron deficiency, chronic constipation and atopic dermatitis are known food related diseases. Patients with chronic illness and those at risk of malnutrition should have detailed nutritional assessments done. Components of a complete nutritional assessment include a medical history, nutritional history including dietary intake, physical examination, anthropometrics (weight, length or stature, head circumference, midarm circumference, and triceps skinfold thickness), pubertal staging, skeletal maturity staging, and biochemical tests of nutritional status. The use of age, gender, and disease-specific growth charts is essential in assessing nutritional status and monitoring nutrition interventions. Nutrition assessment and dietary counseling is helpful for the cure of disease, and moreover, the prevention of illness.

  • Retinopathy of prematurity (ROP) is among the most common causes of childhood blindness. Three phases of ROP epidemics have been observed worldwide since ROP was first described in the 1940s. Despite advances in neonatal care, the occurrence of ROP and associated visual impairment has been increasing somewhere on Earth and remains difficult to control. Conventional treatment options for preventing ROP progression include retinal ablation using cryotherapy or laser therapy. With the emergence of anti-vascular endothelial growth factor (anti-VEGF) treatment for ocular diseases, the efficacy and safety of anti-VEGF therapy for ROP have recently been actively discussed. In the advanced stage of ROP with retinal detachment, surgical treatment including scleral buckling or vitrectomy is needed to maintain or induce retinal attachment. At this stage, the visual outcome is usually poor despite successful anatomical retinal attachment. Therefore, preventing ROP progression by timely screening examinations and treatment remains the most important part of ROP management.

Age and Gender Differences in the Relationship of Cognitive Impairment, Vascular Risk Factors, and Subclinical Carotid Atherosclerosis from Community-based Elderly (나이와 성별에 따른 지역사회 거주 노인에서 무증상 경동맥 죽상경화증에 대한 혈관위험인자 및 인지장애와의 관련성)

  • To evaluate age and gender differences in the relation of cardiovascular risk factors, cognitive impairment, and subclinical carotid atherosclerosis from aged people using by a cross sectional method. Sixty-nine healthy elders living in the community who had not previously undergone carotid ultrasonography were included. We conducted life style surveys, and cognitive function tests including Korean-mini-mental state examination (K-MMSE) and clinical dementia rating-Korean. Various biomarkers from blood were assessed; fasting insulin-like growth factor-1, lipid-profile, high sensitivity C-reactive protein, total homocysteine, glucose, insulin, Homeostasis model assessment (HOMA) for insulin resistance index, vitamin B12, and folate level. Carotid intima-media thickness (C-IMT), and plaques were measured using carotid ultrasonography and aortic ultrasonography, a valid index of atherosclerosis. For the elderly subjects (aged 65-82 years), cognition impairment was more prevalent in females while subclinical atherosclerosis was more prevalentin males. Increased C-IMT has been kept in males, and C-IMT shows increasing trend and the peak at about 80 year-old in females with increasing age. The significant correlations between C-IMT and many vascular risk factors including age, triglyceride, abnormal homocysteinein male, and K-MMSE, insulin, HOMA index and abnormal aortic ultrasonography in female were different in each gender, with the exception of homocysteine (p<0.05). This data suggests that there were differences of age and gender characteristics in terms of subclinical atherosclerosis, cognitive impairment and vascular risk factors in community-living elders. Further larger and longitudinal studies across entire age are required to better understand the effects of risk factors on subclinical atherosclerosis.

Performance of Fusarium oxysporum EKT01/02 isolate in cyanide biodegradation system

  • This study reports a cyanide resistant and/or tolerant fungus, isolated from the rhizosphere of Zea mays contaminated with cyanide-based pesticides. The isolate was characterised using molecular biology. The effect of free cyanide and heavy metals on the growth of isolate in a synthetic gold mine wastewater was examined. The molecular analyses identified the isolate as Fusarium oxysporum EKT01/02 (KU985430/KU985431). The isolate had a free cyanide degradation efficiency of 77.6%. The results indicated greater growth impairment in culture containing Arsenic (optical density 1.28 and 1.458) and cyanide (optical density 1.315 and 1.385). Higher growth was observed in all cultures supplemented with extracellular polymeric substance. This study showed that the isolate possesses wide substrate utilisation mechanism that could be deployed in environmental engineering applications.

A convergence study about Influences of hematopoietic stem cell transplantation on children growth (소아암 환아의 조혈모세포이식 후 성장에 관한 융합 연구)

  • This is a convergence study about influences of hematopoietic stem cell transplantation on children growth. For this explanatory survey research, data were collected with medical record of 112 children with malignant and hematological diseases received HSCT from February to March, 2009. To analyze the growth after HSCT, mixed-effects model was used. The mean SDS of height and weight were negative values in HSCT. The mean value of SDS were significantly lower in autologous HSCT group by height(p=0.0008) and weight(p= 0.0012). Significant factors on changes of SDS of height growth were age at HSCT(p=0.0251), autologous HSCT(p=0.0020) and total dose of steroid in allogeneic HSCT (p=0.0403) and age at HSCT(p=0.0042), autologous HSCT(p=0.0035), and duration of TPN(p=0.0159) for weight growth. According to the results, we must learn to recognize the predicting growth impairment after HSCT in children. regarding nursing interventions should be conducted in the care of these children.

  • The major urinary proteins(MUPs) of mice that bind hydrophobic molecules known as pheromones are regulated in part by the actions of growth hormone. The expression of the MUPs was therefore investigated in transgenic mice that express a human growth hormone-releasing factor gene from a metallothionein gene promoter(MT-GRF) and as a result have elevated growth hormone levels. MUPs were severely down-regulated in the urine of these animals compared to normal mice or to control transgenic mice expressing another gene(the inhibin a subunit) from the same metallothionein promoter(MT-Inh) and more MUPs disappeared in male mice than female ones. MUPs were also down-regulated in the urine of the UT-GRF-injected mice. In addition, it was observed that the urine of the MT-GRF mice included a high molecular weight protein that co-migrates with the major serum protein albumin, indicating an impairment in glomerular filtration within the kidney. The urinary loss of serum proteins was more severe in male MT-GRF mice than female ones. Thus the overexpression of human GRF mimics changes observed in MUP protein expression and glomerular function in other models of growth hormone hypersecretion with sex-dependent differential effects.

  • The GJB2 mutation is mostly recessive in non-syndromic hearing loss, but specific mutations display a dominant type and syndromic hearing impairment. Both U54K and R75Q mutations present a dominant type in pedigrees with associated skin disorders. The purpose of this study was to investigate whether two GJB2 mutations can exhibit a dominant-negative effect on the growth abrogation and the gap junctional intercellular communication capacity exerted by wild-type connexin 26. A specific mutant region of GJB2 showed a loss of gap junction activity and a dominant negative effect on wild-type GJB2. The two mutants exerted a dominant-negative effect on the GJIC capacity and have independently effected GJB2 regulated growth of Hela cells; however, they have no dominant-negative growth effect on wild-type GJB2. It is proposed that the different mechanisms of the dominant-negative effect on wild-type GJB2 involve cell growth and GJIC function. This study describes mutations found in Korean deaf patients and that are typical of other east Asian regions.

The Correction of Clinodactyly on Thumb by Bone Graft from Deformed Metacarpal Region-Previous Correction of Polydactyly: Case Report (무지에 합지증이 동반된 사지증의 치험례)

  • Clinodactyly is defined as an angulation of a digit in the radio-ulnar plane. This anomaly can be congenital, dominantly inherited, or acquired due to trauma or inflammation. Although the deformity usually causes little functional impairment, correction is made because of cosmetic problems. Male subject, with polydactyly on thumb(Wassel's type VII) received first surgery at the age of one. And at the age of six, abnormal growth on the radial side of the first metacarpal bone and ulnar deviation of the distal phalanx of the thumb at the interphalangeal joint had developed. The authors used the growing bony segment from the first metacarpal bone as a bone graft for the correction of clinodactyly on thumb. Z-plasty incision was made on the concave(ulnar) side of thumb and a wedge osteotomy was made on the distal phalanx. The bone graft was inserted into the gap of the distal phalanx of the thumb and fixed it with K-wires. Deformity of the metacarpal bone and clinodactyly on thumb was corrected effectively without donor site morbidity with noticible growth of the grafted metacarpal bone 12 months after surgery.

Endocrine dysfunction and growth in children with medulloblastoma (소아 수모세포종 환자에서 치료 후의 내분비적 장애와 성장변화)

  • Purpose : In medulloblastoma, craniospinal radiation therapy combined with chemotherapy improves the prognosis of tumors but results in significant endocrine morbidities. We studied the endocrine morbidity, especially growth pattern changes. Methods : The medical records of 37 patients with medulloblastoma were reviewed retrospectively for evaluation of endocrine function and growth. We performed the growth hormone stimulation test in 16 patients whose growth velocity was lower than 4 cm/yr. Results : The height loss was progressive in most patients. The height standard deviation score (SDS) decreased from $-0.1{\pm}1.3$ initially to $-0.6{\pm}1.0$ after 1 year(P<0.01). Growth hormone deficiency(GHD) developed in 14 patients. During the 2 years of growth hormone(GH) treatment, the improvements of height gain or progressions of height loss were not observed. Twelve patients(32.4 percent) revealed primary hypothyroidism. One of six patients diagnosed with compensated hypothyroidism progressed to primary hypothyroidism. Primary and hypergonadotropic hypogonadism were observed in two and one patients respectively. There was no proven case of central adrenal insufficiency. Conclusion : Growth impairment developed frequently, irrespective of the presence of GHD in childhood survivors of medulloblastoma. GH treatment may prevent further loss of height. The impairment of the hypothalamic-pituitary-gonadal and hypothalamic-pituitary-thyroidal axis is less common, while central adrenal insufficiency was not observed.

Effects of Kyungohkgo(Qióng yù gào) on Growth and Learning Ability in Growth Deficiency Rat with Insufficient Nutrition Diet (경옥고(瓊玉膏)가 영양소 결핍으로 유도한 성장장애 흰쥐의 성장 및 학습효과에 미치는 영향)

  • Objectives : Effects of Kyungohkgo($Qi{\acute{o}}ng\;y{\grave{u}}\;g{\grave{a}}o$) on intellectual development and learning ability were investigated growth and intellectual impairment rat with insufficient nutrition diet. Methods : We divided male Sprague-Dawley rats into 4 groups(A, B, C, D). They were normal group(A), growth deficiency rat with insufficient nutrition diet group(B), growth deficiency rat with 0.1% Kyungohkgo($Qi{\acute{o}}ng\;y{\grave{u}}\;g{\grave{a}}o$) group(C) and growth deficiency rat with 0.2% Kyungohkgo($Qi{\acute{o}}ng\;y{\grave{u}}\;g{\grave{a}}o$) group(D). They were administered for 5 weeks. We measured body weight, serum growth hormone, insulin-like growth factor and thyroid stimulating hormone, RBC, concentration of Hb and PCV ratio, total WBC and its composition, the values of plasma glutamic oxaloacetic transaminase(GOT) and glutamic pyruvic transaminase(GPT) activities and morris water maze test in escape distance, escape time and escape speed. Results : 1. Body weight showed a tendency to increase in the Kyungohkgo($Qi{\acute{o}}ng\;y{\grave{u}}\;g{\grave{a}}o$) groups and 0.2% Kyungohkgo($Qi{\acute{o}}ng\;y{\grave{u}}\;g{\grave{a}}o$) group showed significantly different than control groups. 2. Serum growth hormone, insulin-like growth factor and thyroid stimulating hormone showed a tendency to increase in Kyungohkgo($Qi{\acute{o}}ng\;y{\grave{u}}\;g{\grave{a}}o$) groups, however these values showed no significantly different. 3. About the counts of RBC, 0.1% and 0.2% Kyungohkgo($Qi{\acute{o}}ng\;y{\grave{u}}\;g{\grave{a}}o$) groups showed significantly different than control groups. Concentration of Hb was higher than control group in Kyungohkgo($Qi{\acute{o}}ng\;y{\grave{u}}\;g{\grave{a}}o$) groups. And 0.2% Kyungohkgo($Qi{\acute{o}}ng\;y{\grave{u}}\;g{\grave{a}}o$) group showed significantly different than control groups in PCV ratio. 4. The counts of total WBC and its composition showed no significantly different in all treatment groups. 5. The values of plasma glutamic oxaloacetic transaminase(GOT) and glutamic pyruvic transaminase(GPT) activities showed no significantly different in all treatment groups. 6. In the morris water maze test, in escape distance and escape speed, Kyungohkgo($Qi{\acute{o}}ng\;y{\grave{u}}\;g{\grave{a}}o$) groups showed no significantly different than control group. But Kyungohkgo($Qi{\acute{o}}ng\;y{\grave{u}}\;g{\grave{a}}o$) groups showed the increasing tendency. Conclusions : So Kyungohkgo($Qi{\acute{o}}ng\;y{\grave{u}}\;g{\grave{a}}o$) have an effect of promoting growth of rats and might be effect to treat various kinds of growth delay in children.
