A Public-Private Collaboration Model for Treatment Intervention to Improve Outcomes in Patients with Tuberculosis in the Private Sector

  • Background: The treatment success rates in patients with tuberculosis are known to be lower in the private sector compared to the public sector. To improve treatment outcomes in the private sector we developed a public-private collaboration model for strengthening health education and case holding activities with public health nursing in the private sector. Methods: We performed a prospective cohort study in new smear positive pulmonary tuberculosis patients treated at private hospitals, selected by non-randomization, with an intervention consisting of health education and case holding activities by specially trained public health nurses (intervention group) results were compared with cases treated without the intervention (conventional group). Physicians were asked to treat both groups routinely. The treatment outcomes of patients under treatment by the National Tuberculosis Programme were also analyzed for comparison. Results: There were 172 cases each in the intervention and conventional groups. The mean age was 48.9${\pm}$19.0 and 48.2${\pm}$19.7 in the respective groups (p=0.66). The PHN interacted with the cases in the intervention group by initial face to face interview and telephone calls an average of 7.1${\pm}$9.2 times during the initial six months. The intervention group showed a significantly higher treatment success rate, 91.6%, (Rate Ratio [RR]; 1.23, 95% Confidence Interval [CI]; 1.12-1.36), lower default, 3.6%, (RR; 0.31, 95% CI; 0.13-0.75) and transfer-out rate, 3.0%, (RR; 0.32, 95% CI; 0.12-0.86) than the conventional group where they were: 75.0%, 11.6%, 9.3%, respectively. The success rate was even higher than the rate (80.5%) of 1,027 cases treated in health centers (RR; 1.11, 95% CI; 1.05-1.17). Of the completed cases in the intervention group, 82.2% regarded the role of the public health nurse as very helpful. Conclusion: The treatment success rate, of tuberculosis patients in the private sector, was significantly improved by an intervention using a public-private collaboration model.

보건소 관리 결핵환자의 퇴록시 치료성공 요인 (Related Factors of Treatment Success of Patients with Tuberculosis Management in Public Health Centers)

  • Objectives: This retrospective study is to identify related factors of treatment success of patients with tuberculosis at community health centers. Methods: The subjects of this study were 1,417 patients with tuberculosis treated in 28 community health centers. The predictors of tuberculosis treatment success were analyzed in terms of 2 areas, which were characteristics of patients and health centers(TB control program). The characteristics of patients consist of 2 factors, such as demographic & diagnosis and treatment. The present conditions of health centers consist of 3 factors, location of centers, resources, and community activities. Data were analysed using X2- test and logistic regression methods. Results: The significant differences between success group and failure group were sex(p=0.003), age(p=0.013), job(p=0.000), type of patients(p=0.001), past history(p=0.029), BCG injection(p=0.009), sputum culture examination(p=0.017), period of treatment(p=0.000), location of center(p=0.001), population per staff(p=0.015), FTE(p=0.027), education days of staff(p=0.005), BCG injection rate(p=0.001), case detection rate (p=0.003), and health education provision rate(p=0.044). Then these variables were analysed using logistic regression analysis. Significant positive factors of treatment success were occupation(95% CI:1.3-6.1), periods of treatment(95% CI:1.5-2.2), center in large city(95% CI:1.2-16.7), center in middle city(95% CI:2.1-24.3), job education related TB(95% CI:1.02-1.3), and BCG injection rate(95% CI:1.1-303.4). Significant negative factors of treatment success were male(95% CI:0.1-0.5) and treatment after default(95% CI:0.005-0.5). Conclusions: Tuberculosis is still one of serious diseases in Korea, because it causes highest mortality rate among OECD countries. This study may provide information to improve treatment effectiveness of tuberculosis at community health centers.

Adherence to Varenicline and Abstinence Rates for Quitting Smoking in a Private Health Promotion Center-Based Smoking Cessation Clinic

  • Background: Varenicline is an effective smoking cessation aid. However, smokers prescribed with varenicline do not always receive varenicline for 12 weeks, as recommended. This study analyzed the subjects who received varenicline and investigated the effect of varenicline treatment duration on the success rate of 6-month smoking cessation. Methods: This study retrospectively analyzed 78 subjects, who received varenicline, out of the 105 smokers that had visited the smoking cessation clinic after medical examination from September 2007 to December 2009. Results: The subjects were all males. Twenty-two subjects (28.2%) had varenicline treatment for 12 weeks or longer; 18 subjects (23.1%) for 8~12 weeks; 22 subjects (28.2%) for 4~8 weeks; and 16 subjects (20.5%) for less than 4 weeks. The total success rate of the 6-month smoking cessation was 47.4%. The success rate of the 6-month smoking cessation was 63.6% in the group that received varenicline for 12 weeks or longer, which was higher than 41.1% of the group that early terminated the varenicline treatment (p=0.074). The period of varenicline treatment was extended for one more week, the odds ratio of the 6-month smoking cessation success increased to 1.172-folds (p=0.004; 95% confidence interval, 1.052~1.305). Adverse events occurred in 30.8% of the subjects who received varenicline, but no serious adverse events were found. Conclusion: If varenicline treatment period is extended, the odds ratio of the success rate for the 6-month smoking cessation increases. Therefore, an effort to improve drug compliance for varenicline in clinical practices could be helpful for the long-term success of smoking cessation.

초보자에서 마네킨에게 직접 후두경과 삽관용 후두마스크기도기를 이용한 기관내삽관의 비교 (Comparison of tracheal intubation using the Macintosh laryngoscope versus the intubating laryngeal mask airway in novice users - A manikin study -)

  • Purpose : This research was designed to provide basic data for advanced pre-hospital airway management by comparing the time to ventilation and success rate for tracheal intubation performed with Macintosh laryngoscope and intubating Laryngeal Mask Airway(ILMA) in a manikin. Methods : All participants were novice users among EMT-Paramedic students and were divided into two groups: (1) the group for Macintosh laryngoscope guided tracheal intubation(MLG-TI) (2) the group for ILMA guided tracheal intubation(ILMA-TI). After an introductory lecture and demonstration, each group made an attempt ten tracheal intubation to compare the ventilation time and success rate for tracheal intubation. Results : 1) There was significant difference in the time to ventilation through MLG-TI, the time to first and second ventilation through ILMA-TI of the 10 attempts. 2) The time to first ventilation through ILMA-TI was significantly shorter than that of ventilation through MLG-TI. 3) There was no significant difference between the time to ventilation through MLG-TI and the time to second ventilation through ILMA-TI. 4) The success rates of ILMA-TI were significantly higher than those of MLG-TI. Conclusion : ILMA-TI can be an alternative method for MLG-TI in advanced pre-hospital airway management.

A Comparison of Two Techniques for Ultrasound-guided Caudal Injection: The Influence of the Depth of the Inserted Needle on Caudal Block

  • Background: Caudal epidural injections have been commonly performed in patients with low back pain and radiculopathy. Although caudal injection has generally been accepted as a safe procedure, serious complications such as inadvertent intravascular injection and dural puncture can occur. The present prospective study was designed to investigate the influence of the depth of the inserted needle on the success rate of caudal epidural blocks. Methods: A total of 49 adults scheduled to receive caudal epidural injections were randomly divided into 2 groups: Group 1 to receive the caudal injection through a conventional method, i.e., caudal injection after advancement of the needle 1 cm into the sacral canal (n = 25), and Group 2 to receive the injection through a new method, i.e., injection right after penetrating the sacrococcygeal ligament (n = 24). Ultrasound was used to identify the sacral hiatus and to achieve accurate needle placement according to the allocated groups. Contrast dyed fluoroscopy was obtained to evaluate the epidural spread of injected materials and to monitor the possible complications. Results: The success rates of the caudal injections were 68.0% in Group 1 and 95.8% in Group 2 (P = 0.023). The incidences of intravascular injections were 24.0% in Group 1 and 0% in Group 2 (P = 0.022). No intrathecal injection was found in either of the two groups. Conclusions: The new caudal epidural injection technique tested in this study is a reliable alternative, with a higher success rate and lower risk of accidental intravascular injection than the conventional technique.

항만혁신클러스터의 성공을 위한 평가요소에 관한 연구 (A Study on the Evaluation Factor for Success of Port Innovative Cluster Using Kohonen Network)

  • 본 연구는 항만혁신클러스터의 성공을 위한 평가요소를 분석하기 위한 것이다. 이를 위해 본 연구에서는 항만혁신클러스터 성공을 위한 평가항목을 정책, 자원, 운영 등 3가지로 구분하였다. 그리고 3항목은 다시 12개의 요소로 세분화하였다. 뉴럴네트웍에 의한 훈련 및 검증을 위해 전체 30개의 설문자료를 훈련용과 검증용으로 각각 15부씩 구분하고, 구분된 자료를 이용하여 훈련 및 검증하였다. 코호넨 네트웍에 의해 수행한 결과 예측의 정확성은 $73.33\%$로 나타났다. 평가항목 및 세부요소의 중요도는 코호넨 네트웍에 의해 산출되었다. 그 결과 항만혁신 클러스터의 성공을 예측한 그룹의 경우에는 자원항목이 다른 항목에 비해 가장 중요한 것으로 나타났다.

Risk Factors of Treatment Failure in Diabetic Foot Ulcer Patients

  • Background Some diabetic feet heal without complication, but others undergo amputation due to progressive wounds. This study investigates the risk factors for amputation of diabetic feet. Methods A total of 55 patients who visited our institution from 2008 to 2012 were included in the study. The patients with abnormal fasting blood sugar levels, lower leg vascularity, and poor nutrition were excluded from the study group, and the wound states were unified. The patients were categorized into a treatment success group (n=47) and a treatment failure group (n=8), and their hemoglobin A1C (HgA1C), C-reactive protein (CRP), white blood cell count (WBC), and serum creatinine levels were analyzed. Results The initial CRP, WBC, and serum creatinine levels in the treatment failure group were significantly higher than that of the treatment success group, and the initial HgA1C level was significantly higher in the treatment success group. The CRP and WBC levels of both groups changed significantly as time passed, but their serum creatinine levels did not. Conclusions The initial CRP, WBC, and serum creatinine levels were considered to be risk factors for amputation. Among them, the serum creatinine level was found to be the most important predictive risk factor. Because serum creatinine represents the renal function, thorough care is needed for the feet of diabetic patients with renal impairment.

Computed Tomography Findings Associated with Treatment Failure after Antibiotic Therapy for Acute Appendicitis

  • Objective: To identify the CT findings associated with treatment failure after antibiotic therapy for acute appendicitis. Materials and Methods: Altogether, 198 patients who received antibiotic therapy for appendicitis were identified by searching the hospital's surgery database. Selection criteria for antibiotic therapy were uncomplicated appendicitis with an appendiceal diameter equal to or less than 11 mm. The 86 patients included in the study were divided into a treatment success group and a treatment failure group. Treatment failure was defined as a resistance to antibiotic therapy or recurrent appendicitis during a 1-year follow-up period. Two radiologists independently evaluated the following CT findings: appendix-location, involved extent, maximal diameter, thickness, wall enhancement, focal wall defect, periappendiceal fat infiltration, and so on. For the quantitative analysis, two readers independently measured the CT values at the least attenuated wall of the appendix by drawing a round region of interest on the enhanced CT (HUpost) and non-enhanced CT (HUpre). The degree of appendiceal wall enhancement (HUsub) was calculated as the subtracted value between HUpost and HUpre. A logistic regression analysis was used to identify the CT findings associated with treatment failure. Results: Sixty-four of 86 (74.4%) patients were successfully treated with antibiotic therapy, with treatment failure occurring in the remaining 22 (25.5%). The treatment failure group showed a higher frequency of hypoenhancement of the appendiceal wall than the success group (31.8% vs. 7.8%; p = 0.005). Upon quantitative analysis, both HUpost (46.7 ± 21.3 HU vs. 58.9 ± 22.0 HU; p = 0.027) and HUsub (26.9 ± 17.3 HU vs. 35.4 ± 16.6 HU; p = 0.042) values were significantly lower in the treatment failure group than in the success group. Conclusion: Hypoenhancement of the appendiceal wall was significantly associated with treatment failure after antibiotic therapy for acute appendicitis.

소방이 형성된 흉막질환에서 유로키나제 주입치료의 예후인자 (The Predictors of Effectiveness on Urokinase Instillation Therapy into Loculated Pleural Effusion.)

  • 연구배경 : 흉막의 염증성 변화가 생기면 흉막의 혈관 및 임파관의 투과성의 증가로 인한 혈장단백질의 유입으로 인해 흉막 비후와 격막의 형성으로 인하여 소방이 형성된다. 이러한 소방이 형성된 흉막질환에 있어 흉막강내 섬유소용해 치료의 하나로 유로키나제 치료가 효과적인 것으로 보고되고 있으나, 이에 대한 치료효과 예측을 위한 지표에 대한 연구는 미미한 실정이다. 이에 저자들은 소방이 형성된 흉막질환에서 일반적으로 시행되는 생화학적 지표들과 초음파검사상의 격막의 정도를 이용하여 유로카나제 치료효과에 대한 예측인자에 대해 알아보고자 하였다. 방 법 : 1993년 4월부터 1996년 4월까지 인하병원에서 소방이 형성된 흉막삼출액 소견을 보인 35명의 환자를 대상으로 흉부 초음파검사와 흉막삼출액에 대한 생화학적 검사를 시행하였고, 1개의 경피적 도관을 삽입하여 배액량이 100ml/day 이하로 감소되면 유로키나제를 주입하였고 배액량이 50ml/day 이하로 측정되면 유로키나제 주입을 중단하였다. 흉부단순촬영 추적검사에서 50% 이상의 폐실질의 확장을 보이면 치료효과군으로, 50% 미만의 확장을 보이면 치료실패군으로 분류하여, 각군들에 대한 초기 흉막 삼출액의 당, 단백질, LDH, ADA, pH, 백혈구 수와 초음파 상의 격막 정도를 무에코형, 선상 격막형, 벌집형으로 구분하여 비교하였다. 결 과 : 1) 대상환자 35명중 치료효과군은 27명 (77.1%)이었고, 치료실패군은 8명(22.9%)였으며, 성과 연령에 있어서의 차이는 없었다. 2) 치료효과군과 치료실패군 사이의 증상발현 기간에 있어서 치료실패군에서 유의하게 길었으며, 유로키나제 주입치료 기간중의 배액량도 적음을 알 수 있었다. 치료효과군에서 $89.7{\pm}35.9mg/dl$, 치료실패군에서 $41.2{\pm}47.1mg/dl$로 치료실패군에서 의미있게 낮은 결과를 보였으며, LDH는 치료효과군에서 $878.7{\pm}654.3IU/L$, 치료실패군에서 $2711.1{\pm}973.1IU/L$로 치료실패군에서 의미있게 높은 결과를 보였고(P<0.05), 단백질, pH, ADA, 백혈구수에 있어서는 의미있는 차이가 없었다. 결 론 : 이상의 결과로 소방이 형성된 흉막질환에서 경피적 도관 삽입을 통한 유로키나제 주입치료의 효과를 판정할 수 있는 지표로 증상발현 기간이 길수록, 흉막삼출액의 당이 낮을수록, LDH 의 수치가 높을수록, 초음파 검사상 격막의 정도가 심할수록 치료가 어려운 것을 나타내는 예측인자임을 추정할수 있었다.

확산모형과 군집분석을 이용한 게임제품의 흥행요소 분석 (Success Factors of Game Products by Using a Diffusion Model and Cluster Analysis)

  • As the global game market has been more competitive, it has been important to analyze success factors of game products. In this paper, we applied a Bass Diffusion Model and Clustering Analysis to identify the success factors of games based on data from Steam, an international game platform. By using a diffusion model, we first categorize game products into two groups : successful and unsuccessful games. Then, each group has been analyzed by using clustering analysis based on product features such as genres, price, and minimum system requirements. As a result, success factors of a game have been identified. The result shows that customers in game industry appreciate sophisticated contents. Unlike many other industries, price is not considered as a key success factor in the game industry. Expecially, advanced independent video games (commonly referred to as indie games) with killer contents show competitiveness in the market.
