Cox의 상호작용이론을 근거로 한 원예활동 프로그램이 경증치매노인의 일상생활수행능력 및 우울에 미치는 효과 (The Effects of a Horticultural Program based on Cox's Interaction Model on Ability for Daily Life and Depression in Older Patients with Mild Dementia)

  • Purpose: This study attempts to examine the effects of a horticultural program on activities of daily living (ADL) and depression among older patients with mild dementia. Methods: This study adopted a quasi-experimental design-based nonequivalent control group pretest and posttest design, enrolling 30 older patients with mild dementia. The Korean Form of Geriatric Depression Scale was utilized, while the data was collected from the experimental group for 60 minutes, twice weekly for 6 weeks in 12 sessions. ADL and depression were assessed for both the experimental and the control group. Overall functions were assessed only for the experimental group. Results: The experimental group showed improvement in physical function, cognitive function, as well as psychological, emotional function and social function following each session (p<.001). The horticulture program was effective in both ADL ($Z^2=5.65$, p<.001) and depression (t=-5.24, p<.001). Conclusion: In this study, the horticultural therapy based on the Cox's interaction model had positive effects for older patients with mild dementia. Therefore, horticultural therapy may be commendably applied to older patients with mild dementia as a nursing intervention.

개두술 여성환자에게 시행한 방광훈련이 유치도뇨관제거후 자가배뇨에 미치는 효과 (The Effectiveness of Bladder Training on Self Voiding after Removal of Catheter in Female Patients with Craniotomy)

  • The purpose of this study was to identify the effectiveness of bladder training on self voiding after removal of catheter in female patients with craniotomy, finally to develop a bladder rehabilitation program for cognitive impaired patients. Nonequivalent control group posttest design was used. The population of this study consisted of 34 hospitalized neurosurgical patients, all patients have been received craniotomy. 17 patients were assigned to the experimental group and another 17 patients to the control group. The homogeneity of general characteristics of the subjects was no significant difference. Bladder training program consisted of pre-training education, the bladder training, positive verbal reinforcement. The experimental group has been received bladder training and the control group has been received gravity drainage. The dependent variable, the frequency of voiding trial untill self voiding achieves, the frequency of urinary retention, the amount of residual urine, the occurrence, of urinary incontinence, were measured during 3 days after catheter removed. The data analyzed with SPSSWIN ; frequency, percentage, t-test and $X^2$-test were used to analyze homogeneity of general characteristics of subjects between the experimental and the control group. T-test, Mann-Whitney U test, and $X^2$-test were used to determine the effect of bladder training. The result of the study were as follows : There was significant difference in the frequency of voiding trial untill self voiding achieves between the experimental group and the control group. There was no significant difference in the frequency of urinary retention between the experimental group and the control group. There was no significant difference in the amount of residual urine between the experimental group and the control group. However, there was significant difference in the amount of residual urine in urinary retention patients. There was significant difference in the occurrence of urinary incontinence between the experimental group and the control group. In conclusion, bladder training program as a nursing intervention was effective in conclusion, bladder self voiding ability after removal of catheter for craniotomy patients. Therefore, it is recommended to use the bladder training program clinically for the bladder management of cognitive impaired patients.

Effects of a Single Session of Brain Yoga on Brain-Derived Neurotrophic Factor and Cognitive Short-Term Memory in Men Aged 20-29 Years

  • Purpose : This study aimed to evaluate the effects of a cognitive enhancement brain yoga program on short-term memory and serum brain-derived neurotrophic factor (BDNF) levels according to the cognitive state in men aged 20-29 years. Methods : Thirty healthy volunteers aged 20-29 years were divided into four groups: brain yoga group, yoga group, combined exercise group, and control group. Seven people were assigned randomly per group. A single-session intervention was conducted over 50 min and consisted of three parts: warm-up, main exercise (brain yoga, yoga, combined exercise, or non-exercise), and cool-down. Serum BDNF levels were measured using enzyme-linked immunosorbent assay, and short-term memory was evaluated using the forward number span test before and after the intervention. Results : BDNF levels significantly increased within the brain yoga group after the intervention (from 28874.37±5185.57 to 34074.80±7321.12, p=.003), whereas there were no significant differences pre-and post-intervention in the other groups. The inter-group comparison showed a significant interaction between the brain yoga group and the combined exercise group (p=.036) but no significant interaction between any of the other groups. Forward number span scores were significantly increased in the brain yoga group (from 9.43±9.83 to 23±7.92, p=.012) and theyoga group after the intervention (from 13.43±9.41 to 24.14±8.45, p=.011), whereas there were no significant changes after the intervention in any other groups. Conclusion : Our findings showed that a single-session, 50-minute brain yoga exercise improved short-term memory and increased serum BDNF levels in healthy men aged 20-29 years and that yoga improved only short-term memory in healthy men of this age group.

노화 및 인지 능력에 따른 인지반응시간 비교 (Comparison of Cognitive Response Time according to Ageing and Cognitive Ability)

  • 목적 : 반응시간은 인지 능력과 노화에 대한 연구에서 중요한 역할을 한다. 본 연구는 컴퓨터 프로그램을 이용한 인지반응시간(Cognitive response time: CRT) 검사를 실시하여 정보처리에 대한 인지 능력의 영향을 확인하는 것을 목적으로 한다. 연구방법 : 대구 및 경북 지역에 거주하고 있는 65~79세의 일반 노인(Normal elderly: NE) 30명과 기억성 경도인지장애(Amnestic mild cognitive impairment: aMCI) 노인 30명을 대상으로 하였다. 결과 분석은 통계 분석 프로그램 R 4.0.2(University of Auckland, Auckland, New Zealand)를 사용하였다. 결과 : CRT 검사의 세 개 하위 영역에서의 총 반응시간은 집단 및 연령에 따라 유의한 차이를 보였고, 오류율은 일부 하위 영역에서 연령 또는 집단에 따라 유의한 차이를 보였다. aMCI 집단의 CRT 검사 수행력은 전반적인 인지 및 기억 검사의 수행력과 유의한 상관성을 갖는 것으로 나타났다. 결론 : 인지반응시간을 측정하는 CRT 검사를 통해 노화 및 인지 능력에 따른 정보처리과정과 처리속도의 변별적 수행을 관찰할 수 있었다. 또한, 이 검사의 수행력이 전반적인 인지 및 기억 검사와 유의한 상관성을 갖고 있음을 확인하였다. 따라서 지역사회 일반 노인의 초기 인지장애를 예측하는 간편 도구로써 CRT 검사가 의미있게 사용될 수 있기를 기대한다.

한국형 전산화 인지재활프로그램(CoTras)이 외상성 뇌손상 환자의 전두엽-집행기능에 미치는 영향 (Effects of Korean Computer-Based Cognitive Rehabilitation Program(CoTras) on Frontal-Executive Functions in Patients with Traumatic Brain Injury)

  • 본 연구는 외상성 뇌손상 환자를 대상으로 한국형 전산화 인지재활프로그램(CoTras)이 전두엽-집행기능에 미치는 효과를 알아보고자 하였다. 2013년 8월부터 2014년 3월까지 경남 창원에 위치하는 C병원에 내원한 외상성 뇌손상 환자 10명을 두 그룹으로 무작위 할당한 후 실험군에게 한국형 전산화 인지재활프로그램을, 대조군에는 자가인지훈련을 1일 1회, 주 5회, 4주 동안 적용하고 그룹 간 비교를 실시하였다. 이때 중재의 효과를 검증하기 위하여 한국판 몬트리올 인지평가(Korean Montreal Cognitive Assessment; K-MoCA), 시계 그리기 검사(Executive Clock Drawing Test; ECDT), 선로잇기 검사(Trail Making Test; TMT), Rey-도형 검사(Rey Complex Figure Test; RCFT)를 사용하였다. 그 결과, 실험군은 K-MoCA, TMT, RCFT-delayed recall에서 유의하게 향상된 반면(p<.05), 대조군은 모든 평가에서 유의한 변화가 없었다. 또한, K-MoCA, TMT, RCFT-delayed recall에서 실험군과 대조군의 변화량이 서로 유의한 차이가 있음을 확인하였다(p<.05). 본 연구는 한국형 전산화 인지재활프로그램이 외상성 뇌손상 환자의 전두엽-집행기능을 향상시키기 위해 국내 임상 환경에서 사용하기에 적절한 중재 도구임을 지지한다.

만성 뇌졸중 환자의 트레드밀 훈련에서 인지적 이중과제훈련이 보행 능력에 미치는 영향 (The Effects of Cognitive Dual Task Training on Walking Ability in Treadmill Training with Chronic Stroke Patients)

  • Purpose : The purpose of this study was to compare the effect of treadmill training and cognitive task with in the course of treadmill training at the same time with chronic stroke patients. Methods : Fourteen chronic stroke patients participated. Participants were randomly assigned to the control and experimental group(7 experimental, 7 control). All of participants were in-patients at local hospital and had been receiving a traditional rehabilitation program, five days a week. The both groups have undergone 4weeks. The experimental group trained in treadmill and cognitive task at the same time, but control group trained only treadmill. 10m walking test, Timed Up & Go (TUG) test and 6 Minutes walking(6M walking) test to measure the walking speed, dynamic balance and waling endurance ability were carried out before and after the training. Results : The result of the study were as follow:10m walking test were significantly increased both groups(p<.01), but not significant between groups(p>.05). TUG test were significantly increased both groups(p<.001) and between groups(p<.01). 6M walking test were significantly increased both groups(p<.001), but not significant between groups(p>.05). Conclusion : Ahead of return to the community to patients with stroke, cognitive task with in the course of treadmill training at the same time was effective in improving the dynamic balance ability.

경도인지장애노인 대상 융복합 운동 프로그램의 효과 : 신체 인지 기반 복합 인지-운동 중심 (The Effect of Combined Cognitive-Motor Learning Program with Mild Cognitive Impairment Elderly Patients)

  • 본 연구는 경도인지 장애 노인들을 대상으로 신체 지각에 기반한 BF 활동과 PNF 활동을 결합한 인지-운동 프로그램의 효과를 살펴봄으로서 신체 지각 기반 인지-운동 활동이 향후 치매 예방 프로그램으로 적용이 가능할지 현장 활용성을 탐색해 보고자 하였다. 검증을 위해 치매 노인들을 대상으로 2014년 5월 16일부터 2014년 8월 1일까지 12주 동안 20명을 선정하여 인지-운동 학습군(이하 CC군)과 작업 치료 학습군(이하 OT군)을 대조군으로 각각 10명으로 나누어 프로그램을 검증하였다. 연구대상자는 CC군과 OT군으로 나누어 해당 프로그램을 60분씩 12주간 참여하였으며, 인지 기능 검사(MMSE-K), 신체 균형 능력 검사(Time up & go test(이하 TUG), Tandem gait test(이하 TA)), 노인 삶의 질 검사(GQOL-D)를 실험 전(0주), 실험 후(12주)에 측정하여 비교 및 분석하였다. 연구 결과, 인지 기능 검사(MMSE-K)에서는 두 집단 모두 유사한 학습 효과를 보여주었다. 그러나, TA & GQOL-D 검사에서는 CC군이 OT 군보다 향상된 학습 효과를 보여주었다. 이러한 결과는 복합 인지-운동 학습 유형이 작업 치료 학습 유형보다 균형능력 향상과 삶의 질 향상에 기여한다는 것을 보여주는 결과로서, 향후 기억 장애 개선 프로그램으로 복합 인지 운동 활동이 고려될 수 있음을 시사한다.

융합형 맞춤 작업치료 활동프로그램이 경도인지장애 노인의 심리 안정과 사회참여에 미치는 영향 (The Effect of Convergence Tailed Occupational Therapy Activities Program on Mental Stability and Social Participation in Elderly People with Mild Cognitive Impairment)

  • 본 연구는 경도인지장애 노인을 대상으로 융합형 맞춤 작업치료 활동프로그램을 실시하여, 경도인지장애 노인의 심리 안정 및 사회참여에 미치는 영향에 대해 알아보았다. 경도인지장애 노인 30명을 대상으로 무작위로 실험군과 대조군으로 각각 15명씩 나누어 6주간 주 2회 총 12회기를 3단계로 나누어 진행하였다. 중재 전과 후의 심리안정을 알아보기 위해, 한국판 노인우울척도(Korean Geriatric Depression Scale; KGDS)을 실시하였고, 사회참여를 비교하기 위해 Frenchay 일상생활활동 자기평가지수(Frenchay Activity Index; FAI)를 실시하였다. 중재 결과 실험군에서 중재 전과 후에 심리 안정, 사회참여는 증가를 보였으며, 대조군에서는 심리 안정에는 증가를 보였으나 사회참여에서는 증가를 보이지 않았다. 따라서 연구결과 융합형 맞춤 작업치료 활동프로그램은 경도인지장애를 가진 노인의 심리 안정 및 사회참여에 긍정적인 영향을 미친다는 사실을 알 수 있었다.

다차원적 프로그램이 시설노인의 인지, 신체기능 및 우울에 미치는 영향 (The Effects of Multidimensional Program on Cognition, Physical Function and Depression for Institutionalized Elderly)

  • Purpose: This study was conducted to examine the effect of multidimensional program on cognition, physical function and depression among institutionalized elderly and the relationship between study variables and resident's characteristics and health related variables. Method: This study involved a one group pre and post test, comparison of variables over a 12 month period. To investigate this research question, data of 114 residents of a nursing home were analyzed. Results: There was statistically significant difference in MMSE-K (t=-2.63, p=.010), ADL (t=-2.85, p=.005), and depression (t=4.66, p=.000) before and after program participation. Conclusion: These results indicate that, for a year their regular involvement in a broad spectrum of multidimensional program activities can improve in cognitive, physical and emotional perspectives, but the level of IADL decreased significantly (t=-6.72, p=.000). Further testing is required with the control group, to compare with community resident elders in order to explore the effects on social skill of elderly.

The effect of game-based dual-task training for executive function and repetitive behaviors in patients with autism

  • Exergames are playing an important role in healthcare/rehabilitation. It has also been used to improve motivation among patients with reduced cognition. The purpose of this pilot study was to evaluate the feasibility of using augmented reality (AR) with game-based cognitive-motor training programs for executive function, restricted and repetitive behaviors (RRBs) in children with autism spectrum disorder. Sixteen children aged 6 -16 years were randomly allocated to the experimental group and control group. Outcome measures were performed before and after the intervention and included executive function, restricted and repetitive behavior. A satisfactory survey was conducted post-intervention. A statistically significant improvement was observed in working memory and cognitive flexibility in the experimental group (P<0.05). However, despite no statistical improvements in cognitive inhibition and four subscales of RRBs, promising changes were observed in all the subscales of the executive function and the behavioral outcomes. Parents appreciated the program and children enjoyed the interaction with the AR game-based training. The findings of this preliminary feasibility study showed that AR using Kinect v2 motion with a cognitive-motor game content can be used for children with autism. However, there is a need for conducting a large-scale study to evaluate his effectiveness on executive function and restricted and repetitive behaviors.
