실행공동체로서의 과학교실이 가지는 구조적 요인 사이의 관계 탐색 -초등과학 실험수업의 모둠활동 사례를 중심으로- (An Exploration of the Relationships Among the Structural Elements of Science Classroom as Community of Practice: Focusing on the Case of Small-Group Activities in Practical Work of Elementary Science)

  • 본 연구는 '실행공동체로서의 과학교실(SCaCoP)'이 가지는 구조적 요인 간의 관계를 탐색하는 것을 목적으로 하였다. 이를 위해 초등과학 실험수업의 모둠활동을 5가지의 구조적 요인인, 학습책임감, 공동의 관심사, 개방적 참여, 호혜적 인간관계, 그리고 실행을 중심으로 살펴보았다. 자료 수집은 초등학교 과학교실의 5개의 모둠을 대상으로 5번의 수업관찰과 면담을 통해 이루어졌으며, 분석 결과는 다음과 같다. 첫째, 호혜적 인간관계는 개방적 참여의 필요조건으로 작용하고 있었다. 둘째, 학생들의 공동의 관심사 요인에는 '구성원들의 흥미'와 '수업의 주제'의 두 가지 차원이 나타났으며, 두 차원은 각각 개방적 참여 요인과 학습책임감 요인의 영향을 주고받았다. 셋째, 실행요인은 다른 요인들과 역동적인 관계를 맺으며 다른 요인들의 특징을 강화시켜주었다. 본 연구결과를 토대로 실행공동체로서의 과학교실이 가지는 구조적 요인 간의 관계를 제안하였으며, 이를 통해 학생들의 과학학습을 공동체에 참여로 보는 관점에서 가질 수 있는 시사점에 대해 논의하였다.

지역사회 민간사례관리자의 실천 성공 경험의 내용과 의미 (Content and Meaning of case Management Practice Success Experience of Regional Society Civil Case manager)

  • 이 연구의 목표는 사례관리자들의 사례관리 성공경험을 탐색하여 현장에서 활용 가능한 목표를 도출하는 것이다. 이 연구의 방법은 자료 수집을 위해 전문 사례관리 사회복지사 20명에 대한 각 2회의 초점집단 인터뷰(Focus group interview)와 자료 분석을 위해 Glaser의 근거이론 방법(Grounded theory method)중 사건코딩을 사용하였다. 분석결과 4개 범주, 10가지 하위범주, 25가지 요소가 도출되었다. 성공경험의 내용은 다음과 같다. [1범주] 클라이언트의 만족과 능력, 하위범주는 (1)역량강화의 시작, (2) 변화의 목표 찾기,(3) 변화의 달성, (4) 클라이언트 주도의 성공, [2범주] 사례관리자의 능력, 하위범주는 (5) 균형있는 인간관 형성, (6) 기술과 능력 향상, [3범주] 조직과 환경, 하위범주 (7) 실천 지원체계확립, [4범주] 신뢰관계 (8) 사례관리자와 클라이언트의 신뢰향상, (9) 지역사회와 클라이언트의 신뢰향상, (10) 사례관리자와 지역사회의 신뢰향상. 이 연구의 결과는 현장의 사례관리 실천 목표를 분명히 하도록 하였고, 향후 기관평가, 사례관리영역에 반영하는 대안으로 제시했다.

바이오휘드백을 통한 자기조절 훈련이 스트레스반응과 면역기능에 미치는 효과에 관한 연구 (A Study on the Effect of Self-Management and Relaxation Training through Biofeedback on Influencing the Stress Response and Immune Functions)

  • The purpose of this study was to test the effectiveness of self-management relaxation training through biofeedback and progressive muscle relaxation methods. The effectiveness of the experimental methods was tested by measuring the degree of symptoms of stress (SOS), the McNair's profile of Mood States (POMS), the levels of ephinephrine, norepinephrine, pulse rate, blood pressure and natural killer cells. The subjects of this study were sixty six nursing students divided into four groups : two groups were the biofeedback and progressive muscle relaxation groups, the other two groups served as control groups. One was a group of sophomores with no experience at all, the other a junior group without self -management or relaxation training. This study was conducted for eight weeks of clinical practice from April, 26th 1998 to June, 20th 1998. Biofeedback training was done with software developed by J&J company (1-410 form for abdominal respiration training). Progressive muscle relaxation training was done with u audiotape recorded according to Jacobson's Theory. The data were analyzed with frequencies, means, and analysis of covariance using the SPSS program and the significance level of statistics was 5%. The results of the study are : 1) The importance of clinical practice stress reduction is shown in that the level of symptoms of stress in the experimental groups in clinical practice was higher than in the group receiving only a lecture. 2) The relaxation training methods of biofeedback and progressive muscle relaxation were effective in reducing the symptoms of stress under the clinical practice stress conditions. 3) The effectiveness of the biofeedback training relaxation method to reduce symptoms of stress was higher than that of progressive muscle relaxation. 4) The relaxation training methods of biofeedback and progressive muscle relaxation were effective in reducing stressful mood states. 5) The relaxation training methods of biofeedback and progressive muscle relaxation were not effective in reducing epinephrine and norepinephrine levels. 6) The relaxation training methods of biofeedback and progressive muscle relaxation were effective in increaing the number of natural killer cells. 7) The relaxation training methods of biofeedback and progressive muscle relaxation were effective in decreasing high systolic and diastolic values of blood pressure and high pulse rates. In summary, the relaxation methods of biofeedback and progressive muscle relaxation in reducing clinical practice stress were effective in lowering the level of symptoms of stress and the profile of stressful mood states. They were also effective in lowering high blood pressure and pulse rates. The relaxation methods were effective in increasing the number of natural killer cells as part of the immune function. However, relaxation methods were not effective in reducing the catecholamine level. The biofeedback training method for reduction of symptoms of stress was more effective than the progressive muscle relaxation method.

PETTLEP 모델 기반 심상훈련 적용이 간호대학생의 핵심기본간호술 수행자신감 및 수행능력, 자율실습 참여도에 미치는 효과 (Effects of PETTLEP Model-based Image Training on Nursing Student' Confidence and Competency in Core Basic Nursing Skills,Participation in Self-Practice)

  • 본 연구는 PETTLEP 모델 기반 심상훈련을 기본간호학실습 교육에 적용하여 핵심기본간호술 수행자신감 및 수행능력, 자율실습 참여도에 미치는 효과를 규명하기 위한 비동등성 대조군 전후 유사실험연구이다. 연구대상은 K도 소재 U대학교 간호학과 2학년 재학생 중 연구목적을 이해하고 연구참여에 자발적으로 동의한 74명을 무작위로 실험군과 대조군에 배정하여 자료 수집하였다. 수집된 자료는 SPSS Statistics 23.0 프로그램을 이용하여 대상자의 일반적 특성은 빈도와 백분율, 정규성검정은 왜도와 첨도를 이용하였으며, 실험처치의 효과측정을 위한 종속변수 검정은 paired t-test로 분석하였다. 연구결과 PETTLEP 모델 기반 심상훈련은 핵심기본간호술 수행자신감(t=4.18, p<.001), 핵심기본간호술 수행능력(지식(t=2.241, p=.032), 간호술기(t=8.402, p<.001)), 자율실습 참여도(t=6.822, p<.001)에 통계적으로 유의한 차이가 있었다. 본 연구 결과를 토대로 PETTLEP 모델 기반 심상훈련은 기본간호학실습 교육에 적용 가능한 교수학습방법이 될 수 있음을 확인하였다. 따라서, 핵심기본간호술 중 난이도가 높은 항목의 수행능력 향상을 위해 PETTLEP 모델 기반 심상훈련을 활용할 수 있을 것으로 기대된다.

Alveolar ridge preservation with a collagen material: a randomized controlled trial

  • Purpose: Resorption of the alveolar bone is an unavoidable consequence of tooth extraction when appropriate alveolar ridge preservation (ARP) measures are not taken. The objective of this trial was to test the hypothesis that dimensional changes in the alveolar bone after tooth extraction would be reduced by inserting an equine collagen membrane and a collagen cone to fill and seal the alveolus (as ARP), in comparison to extraction with untreated alveoli. Methods: In this randomized clinical trial, 31 patients were directly treated with the collagen material after extraction of a tooth from the maxilla (the ARP group). Twenty-nine patients served as the control group. After extraction, no further treatment (i.e., no socket preservation measures) was performed in the control group. Changes in the alveolar process immediately after extraction and after an 8 (${\pm}1$)-week healing period were evaluated 3-dimensionally. Blinded analyses were performed after superimposing the data from the digitalized impressions and surfaces generated by cone-beam computed tomography. Results: Both the ARP and control groups showed a reduction of bone in the alveolar area after tooth extraction. However, significantly less bone resorption was detected in the clinically relevant buccal region in the ARP group. The median bone reduction was 1.18 mm in the ARP group and 5.06 mm in the control group (P=0.03). Conclusions: The proposed hypothesis that inserting a combination material comprising a collagen cone and membrane would lead to a difference in alveolar bone preservation can be accepted for the clinically relevant buccal distance. In this area, implantation of the collagen material led to significantly less alveolar bone resorption. German Clinical Trials Register at www.drks.de, DRKS00004769.

치과치료시 소아의 나이에 따른 행동 조절의 난이도 및 소요시간에 관한 연구 (DIFFICULTY IN MANAGING THE BEHAVIOR AND TIME REQUIRED FOR TREATING THE PEDODONTIC PATIENTS IN ASSOCIATION WITH THEIR AGE)

  • The present study was undertaken to examine the relationship between time required for each step in the treatment process for pedodontic patients. and age of the patients, and experience of special training for pedodontics and career of dental practitioners. Information of these matters was gained from 580 questionnaires collected from 69 practitioners (62 male and 7 female). The questionnaires included questions about the patients' sex(303 male and 207 female) and age. The patients were categorized into 5 different age groups : Group 1, age $1{\sim}3$-year ; Group 2, $4{\sim}6$-year ; Group 3, $7{\sim}9$-year ; Group 4, $10{\sim}12$-year ; Group 5, $13{\sim}15$-year. The questions about the dental practitioners were the years of experience in private dental practice(5 years or more than 5 years) and whether or not they took the special training for pedodontics in the authorized institutes. The practitioners were asked to answer the questions about Frankl scale of the patients' behavior during the treatment, and time spent for managing the patients to be subjected to local anesthetic injection (the first behavior control), time for the injection, and time for the subsequent treatment. The results obtained by analyzing the information collected from the questionnaires were as follows : 1. The younger the patients, the lower the Frankl scale was counted at the time of the first behavior control, injection, and the subsequent treatment(p<0.001). 2. The lowest Frankl scale was scored during the injection regardless of the age of the patients. 3. Time for management and treatment was decreased in the order of age Groups 1 and 2< Group 3 < Groups 4 and 5. 4. The patients showed a more positive frankl scale in response to the treatment performed by those who were more-experienced in dental practice as compared with those were less-experienced. 5. Pedodontic training experience of the practitioners did not appear to influence the patients with respect to the treatment time and Frankl scale.

동영상을 활용한 사전학습과 역할학습이 기본간호학 실습 교육에서 간호대학생의 자기조절학습에 미치는 효과에 대한 융합연구 (A Convergence Study about the Effects of Pre-learning and Role Learning Using Video on Self-regulated Learning of Nursing Students in Fundamental Nursing Practice Education)

  • 본 연구는 동영상을 활용한 사전학습과 역할학습이 간호대학생의 자기조절학습에 미치는 효과를 확인하고자 수행되었다. 연구설계는 비동등성 대조군 전후 설계에 의한 유사실험 연구이다. 연구대상은 G군 소재 간호학과 2학년 학생으로 실험군 84명, 대조군 76명으로 총 160명이었다. 자료수집 기간은 2016년 3월 2일부터 6월 20일까지였다. 실험군에게는 동영상을 활용한 사전학습과 역할학습을 진행하고 대조군에게는 교수시범의 전통적 방법을 진행한 후 자기조절학습의 변화를 측정하였다. 자료분석 방법은 ${\chi}^2-test$, independent t-test, ANCOVA를 사용하였다. 연구결과 자기조절학습의 인지적 구성 요소인 시연, 초인지에서, 동기적 구성요소인 자기효능감에서, 자원관리 구성요소인 도움구하기에서 유의한 차이가 나타났다. 본 연구는 간호학생들의 자기조절학습을 도모하고 효과적인 실습 교육을 운영하는데 기초자료를 제공하였다.

Effect of Action Observational Training on Gait in People with Stroke

  • Purpose: This study examined the effects of action observational training to improve the gait function for patients with stroke. Methods: The participants were divided into two groups: right hemiplegia group (n=12) and left hemiplegia group (n=12). All groups received conventional therapy for five sessions for 30 minutes, each for three weeks. Left and right hemiplegia group practiced additional action observational training for five sessions for 20 minutes each for three weeks. They participated in three weeks of action observational training coupled with immediate physical practice (intervention), followed by a final assessment. The duration of each action observation video sequence was 10 minutes, followed immediately by practice of the observed motor skill (10 minutes). The gait velocity, cadence, swing time, step length, and BOS (base of support) were examined using the GAITRite system. Results: The results of this study showed significant improvement in the gait function. The outcomes of the gait abilities from gait velocity, cadence, swing time, step length of the affected side, and BOS (base of support) were improved significantly in the right hemiplegia group (p<0.05). In the left hemiplegia group, there was no significant improvement in the gait velocity, cadence, and BOS except for the swing time and step length of the affected side. The left and right group comparisons between the groups were not significant (p<0.05). Conclusion: Action observation training improves the gait function. These results suggest that action observational training is feasible and suitable for stroke patients.

전화와 우편을 이용한 개별적 인플루엔자 예방접종 권고의 효과 - 한 삼차병원 가정의학과를 방문했던 노인환자를 대상으로 한 연구 - (Effectiveness of Telephone and Postcard Reminders for the Influenza Vaccination - A Study in the Elderly Who Have Visited a Family Practice Center in a Tertiary Care Hospital -)

  • Objectives : Routine vaccination against influenza is recommended for all people aged 65 years or more. Without active encouragement to receive the vaccine, the rate of compliance is generally low. A study was undertaken to assess and compare the effectiveness of two reminder systems in improving the influenza vaccination rate. Methods : A total of 2,017 patients aged 65 or over in a tertiary care hospital family practice center were randomly assigned to a control, a telephone reminder, or a postcard reminder group before the influenza season in 1998. The outcome measure, receipt of vaccination, was determined by telephone interview. Demographic and clinical characteristics were checked through the medical chart review. Results : Among the 1,312 patients who actually received reminders and interviewed, the vaccination rate was 46.7% in the control group, 56.3% in the postcard reminder group, and 63.3% in the telephone reminder group. Vaccination compliance was significantly higher in people reminded by telephone (Odds ratio [OR], 2.00; 95% confidence interval [CI], 1.52-2.64) and postcard (OR, 1.55; 95% CI, 1.18-2.02) compared to that in the control group. Of the characteristics investigated, number of high risk co-morbidity was positively associated with vaccination compliance while current smoking was negatively associated. Conclusions : This result suggests that telephone and postcard reminders can significantly improve compliance with influenza vaccination in this group of Korean elderly. However, additional strategies need to be developed to encourage vaccination among noncompliant.

Differences in mandibular condyle and glenoid fossa morphology in relation to vertical and sagittal skeletal patterns: A cone-beam computed tomography study

  • Objective: This study aimed to evaluate the following null hypothesis: there are no differences in the morphology of the temporomandibular joint (TMJ) structures in relation to vertical and sagittal cephalometric patterns. Methods: This retrospective study was performed with 131 participants showing no TMJ symptoms. The participants were divided into Class I, II, and III groups on the basis of their sagittal cephalometric relationships and into hyperdivergent, normodivergent, and hypodivergent groups on the basis of their vertical cephalometric relationships. The following measurements were performed using cone-beam computed tomography images and compared among the groups: condylar volume, condylar size (width, length, and height), fossa size (length and height), and condyle-to-fossa joint spaces at the anterior, superior, and posterior condylar poles. Results: The null hypothesis was rejected. The Class III group showed larger values for condylar width, condylar height, and fossa height than the Class II group (p < 0.05). Condylar volume and superior joint space in the hyperdivergent group were significantly smaller than those in the other two vertical groups (p < 0.001), whereas fossa length and height were significantly larger in the hyperdivergent group than in the other groups (p < 0.01). The hypodivergent group showed a greater condylar width than the hyperdivergent group (p < 0.01). The sagittal and vertical cephalometric patterns showed statistically significant interactions for fossa length and height. Conclusions: TMJ morphology differed across diverse skeletal cephalometric patterns. The fossa length and height were affected by the interactions of the vertical and sagittal skeletal patterns.
