A preliminary simulation for the development of an implantable pulsatile blood pump

  • A preliminary study of a new pulsatile pump that will work to a frequency greater than 1 Hz, is presented. The fluid-structure interaction between a Newtonian blood flow and a piston drive that moves with periodic speed is simulated. The mechanism is of double effect and has four valves, two at the input flow and two at the output flow; the valves are simulated with specified velocity of closing and reopening. The simulation is made with finite elements software named COMSOL Multiphysics 3.3 to resolve the flow in a preliminary planar configuration. The geometry is 2D to determine areas of high speeds and high shear stresses that can cause hemolysis and platelet aggregation. The opening and closing valves are modelled by solid structure interacting with flow, the rhythmic opening and closing are synchronized with the piston harmonic movement. The boundary conditions at the input and output areas are only normal traction with reference pressure. On the other hand, the fluid structure interactions are manifested due to the non-slip boundary conditions over the piston moving surfaces, moving valve contours and fix pump walls. The non-physiologic frequency pulsatile pump, from the viewpoint of fluid flow analysis, is predicted feasible and with characteristic of low hemolysis and low thrombogenesis, because the stress tension and resident time are smaller than the limit and the vortices are destroyed for the periodic flow.

Effects of blood flow restriction on gluteus muscles thickness, density, and WAI for children with cerebral palsy (혈류제한이 뇌성마비 아동의 큰볼기근 두께와 밀도 및 백색영역지수에 미치는 영향)

  • Background: This study was to identify the effects of blood flow restriction on gluteus muscles thickness, density, and white area index for children with cerebral palsy. Design: Randomized controlled trial. Methods: Twenty children with cerebral palsy were assigned randomly to an experimental (n=10) or control (n=10) group. The experimental group performed a bicycle exercise after blood flow restriction. The control group performed a bicycle exercise. The study used an ultrasonic instrument to measure gluteus muscles thickness, density, and WAI. Wilcoxon signed ranks test was used for determination of differences before and after treatment, and a Mannn-whitney U test was used for determination of differences between treatment groups. Results: As a result of comparison within groups, the experimental and control group showed significant difference for gluteus muscles thickness, density, and WAI after the experiment (p<0.05). In comparison between two groups, the experimental group showed more significant difference in gluteus muscles thickness, density, and WAI than the control group (p<0.05). Conclusion: Based on these results, a bicycle exercise after blood flow restriction effectively improved the gluteus muscles thickness, density, and WAI of children with cerebral palsy.

Effect of Strength Training Combined with Blood Flow Restriction Exercise on Leg Muscle Thickness in Children with Cerebral Palsy

  • Purpose: The purpose of this study was to investigate the effect of strength training combined with blood flow restriction on leg muscle thickness in children with cerebral palsy. Methods: Nineteen children with cerebral palsy, aged between five and 10 years of age, living in area N, were recruited. Ten participants were classified into a blood flow restriction group and nine into a strength exercise group. The experimental group performed strength training using a blood flow restriction cuff on the leg, and the control group performed strength training without blood flow restriction. A paired t-test was performed to confirm intragroup changes before and after five weeks of the experiment, and an independent t-test was performed to confirm intergroup changes, and the significance level was α=0.05. Results: The rectus femoris, gastrocnemius and gluteus medius muscles showed significant differences in the groups after five weeks (p<0.05). There was a significant difference between the groups in the rectus femoris and gastrocnemius after five weeks (p<0.05). Conclusion: As a result of this study, it was found that strength training combined with blood flow restriction had a positive effect on the changes in leg muscle thickness in children with cerebral palsy. This suggests the possibility of using it in the future as basic data for strength training methods and blood flow restriction exercises for children with cerebral palsy.

Blood Flow and Skin Temperature Increases by Monochromatic Infrared Energy Irradiation

  • Purpose: The purpose of this study was to determine the effect of monochromatic infrared energy (MIRE) on the blood flow of the superficial radial artery and local skin temperature in healthy subjects. Methods: Forty healthy volunteers were recruited and randomly assigned to MIRE group (n=20) and placebo group (n=20). The MIRE group received a 890 nm MIRE irradiation on the forearm using two therapy pads for 30 minutes. The therapy pad was composed of an array of 60 diodes. MIRE unit was set at bar 8, that corresponds to a diode power of 10 mW and a power density of $63mW/cm^2$. The placebo group received sham MIRE. Peak blood flow velocity (PBFV), mean blood flow velocity (MBFV), and skin temperature (ST) were measured pre- and post-MIRE irradiation. Results: There was a significant difference in PBFV (p<0.001), MBFV (p<0.001), and ST (p<0.001) between the pre- and post-treated values in the MIRE group. In contrast, no significant difference was found between the pre- and post-treated values in the placebo group. There was significant difference in mean change values from baseline of PBFV (p<0.001), MBFV (p<0.001), and ST (p<0.001) between the MIRE group and the placebo group. There was a significant increase in PBFV (p<0.001), MBFV (p<0.001), and ST (p<0.001) following MIRE irradiation. Conclusion: The arterial blood flow and local skin temperature of the forearm in the healthy subjects were significantly increased following MIRE irradiation.

Cardioprotective Potency of the Antioxidante Amifostine in the Ischemic and Reperfused Isolated Rat Heart (항산화제 Amifostine의 허혈 및 재관류시 흰쥐 적출심장의 심근 보호기능)

  • Background: S-2-(3 aminoprophlamino) ethylphosphorothioic acid(WR-2721) is one of the radical scavenging thiols. We tested its protective effects in the reperfused heart. Material and Method: The experimental setup was the constant pressure Langendorffs perfusion system. We investigated the radical scavenging properties of this compound in isolated rat hearts which were exposed to 20 minutes ischemia and 20 minutes reperfusion. Four experimental groups were used:group I, control, Amifostine 50 mg(1 mL) peritoneal injection 30 minutes before ischemia(group II), Amifostine 10 mg(0.2 mL) injection during ischemia through coronary artery(group III),and Amifostine 50 mg(1 mL) peritoneal injection 2 hrs before ischemia(group IV). The experimental parameters were the levels of latate, CK-MB, and adenosine deaminase(ADA) in frozen myocardium, the quantity of coronary flow,and left ventricular developed pressure, and it's dp/dt. Statistical analysis was performed using repeated measured analysis of variance and student t-test. Result: The coronary flow of group II and IV were less than group I and III at equilibrium state but recovery of coronary flow at reperfusion state of group II, III, and IV were more increased compared with group I. The change of systolic left ventricular devoloping pressure of group II and IV were less than control group at equilibrium state, which seemed to be the influence of the pharmacological hypotensive effect of amifostine. But it was higher compared with group I at reperfusion state. The lactic acid contents of group II were less than control group in frozen myocardium.(Group I was 0.20 0.29 mM/g vs Group II, which was 0.10 0.11 mM/g). The quantity of CK-MB in myocardial tissue was highest in group IV (P=0.026 I: 120.0 97.8 U/L vs IV: 242.2 79.15 U/L). The adenosine deaminase contents in the coronary flow and frozen myocardium were not significantly different among each group. Conclusion: Amifostine seemed to have significant cardioprotective effect during ischemia and reperfusion injuries of myocardium.

Visualization of Flow inside a Regenerative Turbomachinery

  • In this study, we visualized the internal flow of a regenerative turbomachinery using the direct injection tracer method. For visualization, the working fluid was water and the tracer was oil colors (marbling colors). Droplets were injected at the inlet of the machinery and the streak were recorded using a high-speed camera with high-power light sources. While circulating inside the groove, the droplets were translated by the rotational motion of the impeller. When the droplets flow out of the impeller groove, relative to the impeller, they moved more slowly. And the droplets repeatedly reentered into the groove and circulated again. Then the droplets either flowed to the outlet or to the stripper. As a result, this experiment has confirmed the internal circulating flow of a regenerative turbomachinery.

Contralateral Heating Effects of Contrast Bath and Warm Bath (대조욕과 온열욕의 교차성 열효과)

  • The purposes of this study were to measure skin temperature and blood flow on the contralateral upper extremity when heat is applied to one upper extremity, were to compare the effect of contrast bath. The subjects were 38 healthy adults with no history of peripheral vascular disease. The subjects of contrast bath were 18 persons and the subjects of warm bath were 20 persons. The subjects of one group were seated with their right arm in water($42^{\circ}C$) up to the mid-forearm. The subjects of the other group were seated with their right arm up to the mid-forearm in water which was changed from warm to cold using the contrast bath technique. The continually changing temperatures and blood flow were measured by an independent observer at intervals of 10, 15, 20, 25 and 30 minutes respectively after the start of the procedure. The results were as follows. The temperature of the warm bath group rose 4.28% over the pre-experimental temperatures and the temperature of the contrast bath group rose 3.41%. There was no statistically significant difference between the two groups. The blood flow of the warm bath group rose 8.31% over the pre-experimental blood flow and the blood flow of the contrast bath group rose 17.24%. There was a statistically significant between the two groups 20 minutes after the start of the procedure. Thus the contrast bath is a more effective method than the warm bath to increase blood flow.

Estimation of Pollution Contribution TMDL Unit Watershed in Han-River according to hydrological characteristic using Flow Duration Curve (유량지속곡선을 이용한 수문특성별 한강수계 총량관리 단위유역의 오염기여도 추정)

  • After the Total Maximum Daily Loads(TMDLs) was applied, it became beyond the limit of concentration management. However, it does not adequately reflect the characteristics of various watersheds, and causes problems with local governments because of the standard flow set. Thus, in this study, the Han River system is organized into four groups in estimating the Pollution Contribution by applying the Flow Duration Curve(FDC) created by the daily flow of data from the HSPF. And the method of this study is expected to be valuable as basic data for the TMDLs. As a result, Group I contains the main watersheds with no large hydraulic structures and tributary watersheds. There is no specificity in the FDC and the Pollution Contribution is estimated as rainfall runoff. Group II contains watersheds near the city where the FDC is maintained above a certain level during the Low Flow Conditions and the Pollution Contribution is estimated as the discharge flow of large scale point pollution facilities. Group III contains the main watersheds in which the large hydraulic structures are installed and FDC is curved in the Low Flow Conditions. So the Pollution Contribution is estimated as the water quality of the large hydraulic structures. Group IV contains the upstream in mainstream watersheds in which the large hydraulic structures are installed and the FDC is disabled before the Low Flow Conditions. As the flow is concentrated in the High Flow Conditions, the non-point pollution sources are estimated as the Pollution Contribution.

Clinical Comparison Between Inside Blood Flow Type and Outside Blood Flow Type in the Hollow Fiber Oxygenator (Hollow Fiber Oxygenator에서 Inside Blood Flow Type과 Outside Blood Flow Type의 임상적 비교)

  • The hollow fiber oxygenator is the most advanced one for the cardiopulmoanry bypass. They have two different types of the hollow fiber systems according to the way how the blood go through the fibers. One is inside blood flow type and the other outside type. In order to find out which is better to prevent blood cell destruction, we selected 40 valve replacing patients and divided them into 2 groups prospectively. In group I [n=20], inside blood flow type[BCM-7a], CO2 excretion is more effective than group II, that is partly because of the relative large surface area of the BCM-7. In group II [n=20], outside blood flow type [MAXIMAa], they have better quality to preserve platelet count. We also studied about several other items such as SaO2, Hemoglobin and RBC, WBC, fibrinogen, LDH, plasma hemoglobin, haptoglobulin and so on. But we cannot find any differences between two groups with any statistical meanings [p<0.05]. We conclude that both of two oxygenators are excellent in the aspects of gas exchange and blood cell preservation.

The Effect of the Project Learning Method on the Learning Flow and AI Efficacy in the Contactless Artificial Intelligence Based Liberal Arts Class

  • In this study, the educational effect were sought to be identified after developing and applying project learning for the artificial intelligence based liberal arts education for the non-computer majors. A paired-sample t-test was performed within each group to determine the extent of improvement in the learning flow and artificial intelligence efficacy in the experimental and control groups. After class, an independent sample t-test was performed to examine the statistical effects of pre-test and post-test on the learning flow and artificial intelligence efficacy in the experimental and control groups. The experimental group and control group demonstrated significant improvements in the learning flow and artificial intelligence efficacy before and after class, each respectively. There was no statistically significant difference in the learning flow between the experimental group for which the project learning method was applied and the control group for which only theory and practice were conducted in the artificial intelligence class. It was also confirmed that the experimental group for which the project learning method was applied improved the efficacy of artificial intelligence to a significant level compared to the control group which only proceeded with theory and practice.
