Isolated aortic valvular heart disease : analysis of etiology and surgical experience (대동맥판막질환의 임상적 고찰 -판막병리 및 임상성적에 대하여-)

  • Clinical and pathologic data were reviewed in 20 patients who had have surgery for isolated aortic valvular heart disease between April 1978 and April, 1987. Hospital mortality was 10%, with no additional late mortality during a mean follow-up period of 24.1 months. Prosthetic valve failure developed in 3 patients and two had reoperation. Niety four percent of the survivors who were in NYHA Funtional class III or IV before operation are now in class I or II. Ninety percent of all patients are still alive at a maximum follow up of 9 years. The clinical histories, gross and histologic examination of valves estabilished the causes for isolated aortic valve disease: 3 rheumatic, 2 congenital bicuspid, 2 hypertention, 2 aortitis and each one case of floppy valve, medial cystic necrosis of aorta, bacterial endocarditis. But etiology was unknown in 8 cases. Sixteen patietns had myxoid degeneration, defined as significant disruption of the valve fibrosa and its replacement by acid mucosaccharides and cystic changes. Myxoid degeneration was also the primary pathologic abnormality in the patients with 2 hypertention, 2 rheumatic, 1 aortitis, 1 bacterial endocarditis, 1 floppy valve, 1 congenital bicuspid. The patients with myxoid degeneration of uncertain origin were 8. Histologic finding of all of them revealed nonspecific patients with myxoid degeneration of uncertain orgin were 8. Histologic finding of all of them revealed nonspecific chronic valvulitis with myxoid degeneration. This finding may indicate that the etiology w uld be infectious.

Dose range finding study of KOB03, a new polyherbal medicine for the treatment of allergic rhinitis, by oral administration for four weeks in Sprague-Dawley rats (한약제제 KOB03의 랫드에서의 4주 반복 경구투여에 의한 독성 연구)

  • Objectives : To evaluate the toxicity of KOB03, polyherbal medicine for the treatment of allergic rhinitis, we performed the dose-range finding test of KOB03 by oral supplementation for 4 weeks in Sprague-Dawley rats. Methods : The water extract of KOB03 consisting of five different herbs was supplied from GLP company. KOB03 was supplemented by gavage at 0, 500, 2,500 and 5,000 mg/kg/day for 4-week consecutive days. We recorded the clinical signs of toxicity, body weight, organ weights, hematology, gross and histological changes in target organs of rats, and clinical chemistry analysis. Results : KOB03 at all doses was shown no mortality or abnormal clinical signs in rats during at the observation period. Furthermore, there was no difference in body weight and food-take consumption, organ weight, gross pathological findings, and urine analysis among the groups of rats treated with different doses of KOB03. The hematological analysis and clinical blood chemistry data were revealed no toxic effects from KOB03-supplemented rats. Conclusions : The results suggest that KOB03 is a wide margin of safety on dose-range toxicity in rats. The no observable adverse effect level (NOAEL) of the test, KOB03 in rats is no less than 5,000 mg/kg/day.

A Single Oral Dose Toxicity Study of Bamboo Leaf Water Extract in Sprague-Dawley Rats

  • The present study was carried out to investigate the potential acute toxicity of bamboo leaf water extract by a single oral dose in Sprague-Dawley rats. Twenty male and female rats aged 5 weeks were randomly assigned to four groups of 5 rats each and were administered singly by gavage at dose levels of 0, 1250, 2500, or 5000 mg/kg body weight. Mortalities, clinical findings, and body weight changes were monitored for the l4-day period following the administration. At the end of 14-day observation period, all animals were sacrificed and complete gross postmortem examinations were performed. Throughout the study period, no treatment-related deaths were observed. There were no adverse effects on clinical signs, body weight, and gross finding at any dose tested. The results showed that the single oral administration of bamboo leaf water extract did not induce any toxic effect at a dose level of below 5000 mg/kg in rats and that the minimal lethal dose were considered to be over 5000 mg/kg body weight for both sexes.

  • Background: Samul-tang (Si-Wu-Tang, SMT) is a traditional herbal formula, which has been widely used to treat various diseases such as menstrual irregularity, bleeding and leucorrhea. Although many studies have investigated the pharmacological properties of SMT, its toxicity information has not yet been fully elucidated. Methods: Five Sprague Dawley (SD) rats of each sex were given a single dose (5000 mg/kg) of SMT by gavage; control rats received the vehicle only. After the single administration, mortality, clinical signs, body weight changes and gross findings were monitored for 15 days in accordance with Good Laboratory Practice (GLP) principles. Results: In a single oral dose toxicity study, there was no adverse effect on mortality, clinical sign, body weight change or gross finding in any treatment group. Conclusions: The results indicate that SMT did not induce toxic effects at a dose level up to 5000 mg/kg in rats and its median lethal dose ($LD_{50}$) was considered to be over 5000 mg/kg/day body weight for both genders.

  • In speech signal processing, it is very important to detect the pitch exactly. Although various methods for detecting the pitch of speech signals have been developed, it is difficult to exactly extract the pitch for wide range of speakers and various utterances. Thus we propose a new pitch detection algorithm which takes advantage of the G-peak extraction. It is a method to detect the pitch period of the voiced signals by finding MZCI (maximum zero-crossing interval) of the G-peak which is defined as cut-off bandwidth rate of LPF (low pass filter). This algorithm performs robustly with a gross error rate of 3.63% even in 0 dB SNR environement. The gross error rate for clean speech is only 0.18%. Also it is able to process all courses with high speed.

  • Though the bamboo salt, called as "JUKYUM" has been widely used in Korea as panacea, it's toxicity were not screened completely. To investigate the toxicity of bamboo salt, we compared with the toxicity of crude salt and reagent-grade NaCl by performing single dose oral toxicity test in SD rats. Crude salt, natural sun-dried salt (crude salt) production, was purchased from the western seashore of Korean peninsular, and reagent-grade NaCl was purchased from Sigma company. Results of the single dose oral toxicity tests on bamboo salt, crude salt and reagent-grade NaCl to SD rats are as follows, $LD_{50}$ of bamboo salt was 4174mg/kg (male) and 4074mg/kg (female), that of crude salt was 4871mg/kg (male) and 4898mg/kg (female) and that of reagent-grade NaCl was 4247mg/kg (male) and 4025mg/kg (female), respectively. There were little differences in clinical signs and gross legions among groups. Finding of gross autopsy and necropsy of bamboo salt treated group were similar to other groups.er groups.

Accuracy of Preoperative Urinary Symptoms, Urinalysis, Computed Tomography and Cystoscopic Findings for the Diagnosis of Urinary Bladder Invasion in Patients with Colorectal Cancer

  • Background: To determine the accuracy of preoperative urinary symptoms, urinalysis, computed tomography (CT) and cystoscopic findings for the diagnosis of urinary bladder invasion in patients with colorectal cancer. Materials and Methods: Records of patients with colorectal cancer and a suspicion of bladder invasion, who underwent tumor resection with partial or total cystectomy between 2002 and 2013 at the Faculty of Medicine Siriraj Hospital, were reviewed. Correlations between preoperative urinary symptoms, urinalysis, cystoscopic finding, CT imaging and final pathological reports were analyzed. Results: This study included 90 eligible cases (71% male). The most common site of primary colorectal cancer was the sigmoid colon (44%), followed by the rectum (33%). Final pathological reports showed definite bladder invasion in 53 cases (59%). Significant features for predicting definite tumor invasion were gross hematuria (OR 13.6, sensitivity 39%, specificity 73%), and visible tumor during cystoscopy (OR 5.33, sensitivity 50%, specificity 84%). Predictive signs in CT imaging were gross tumor invasion (OR 7.07, sensitivity 89%, specificity 46%), abnormal enhancing mass at bladder wall (OR 4.09, sensitivity 68%, specificity 66%), irregular bladder mucosa (OR 3.53, sensitivity 70%, specificity 60% ), and loss of perivesical fat plane (OR 3.17, sensitivity 81%, specificity 43%). However, urinary analysis and other urinary tract symptoms were poor predictors of bladder involvement. Conclusions: The present study demonstrated that the most relevant preoperative predictors of definite bladder invasion in patients with colorectal cancer are gross hematuria, a visible tumor during cystoscopy, and abnormal CT findings.

Preventive Effects on Porcine Epidemic Diarrhea(PED) Using by PEDV Antiserum II. Clinical Sign, Histopathological Lesion and Immunohistochemical Finding (항혈청 투여에 따른 돼지 유행성 설사병 예방효과 II. 임상증상, 병리조직학적 검사, 면역조직학적 검사)

  • The purpose of this study was to investigate to protective effects against porcine epidemic diarrhea virus (PEDV) infection in piglets by administration of the PEDV antiserum orally at 2 hrs, 24hrs and 36hrs after birth. six piglets administered the antiserum were experimentally infected with PEDV at five-day-old. Control group were four piglets infected with PEDV only. Clinical signs and gross, histopathological lesion and immunohistochemical findings were examined. The results obtained were as follows; 1. In clinical signs, piglets of the control group appeared the typical signs of severe watery diarrhea, depression and anorexia but piglets of the PEDV antiserum treated group recovered progressively. In mortality, control group showed 75%, but PEDV antiserum treated group showed 16.7%, respectively. 2. In gross findings, piglets of the control group appeared the typical findings of congestion, distension of lumen, containing curdes of undigested milk in stomach. But piglets of the PEDV antiserum treated group appeared milder than those of control group. 3. In histopathological findings, piglets of the control group appeared the typical findings of villous atrophy and fusion, congesion, exfoliation, vacuolation, squamation, loss of cilia and proliferation of crypt. But piglets of the PEDV antiserum treated group appeared milder than those of control group. 4. In immunohistochemical findings, piglets of the PEDV antiserum treated group showed more intensive in reaction for IgG and IgG than those of control group. The recation for IgA was stronger than that of IgG. It was concluded that oral administration of PEDV antiserum to piglets was effective in preventing PEDV infection and reduced their mortality.

  • In digital age export expansion is the key fact of promoting Korea's economic growth. The policy that encourages exportation is being more emphasized to develop of the nation's economy. From the 90's, regionalism has been widespread throughout the world with globalization and internationalization led by WTO and FTA. The world is now becoming one economic bloc. It is necessary to understand how enterprises contribute to the exportation to establish and manage the policy that encourage and support the exportation. This study estimates SME's level of contribution on gross exports and understands the problem, while looking for better statistical alternatives and finding SME's export energizing policies. It is estimated that domestic SMEs contribute about 40-50% of gross exports. SME bureau and national statistical office should select optimal size of population of each industry, design and extract sample to conduct the export related survey extensively. SMEs should analyze the product and skills of the industry that is dominated by major companies to contribute to the exportation more efficiently. This effort will increase the level of contribution to gross exports in general.
