60세 이상 환자에서 발생한 외상성 견관절 전방 탈구 (Traumatic Anterior Shoulder Dislocation in Patients Older than 60 Years of Age)

  • Purpose: This study evaluated clinical results, and recommended treatment protocol of traumatic anterior shoulder dislocation in the patients older than 60 years of age. Materials and Methods: Thirty-eight patients with first traumatic anterior shoulder dislocation aged over 60 years were included. The average age was 69.4 (range, 60 to 87 years). There were 8 men (average age of 71.6) and 30 women (average age of 69). Most common cause of injuries was a fall on the outstretched hand. The additional injuries were evaluated using MRI or CT arthrogram in the patients with significant pain and weakness while movements after 2 weeks sling immobilization. Results: Fifteen patients (39%) had rotator cuff tears and 5 patients (14%) had greater tuberosity fractures. The sizes of rotator cuff tears were diverse; 2 partial tears, 1 small tear, 4 medium tears, 3 large tears and 5 massive tears. Among 5 massive cuff tears, 3 patients revealed cuff arthropathy after reduction. 4 patients (11%) had recurrent dislocation more than one time during 1 month after the first dislocation. Bankart lesions revealed in 5 patients and three of them had associated rotator cuff tears. 3 out of 5 patients with Bankart lesions, 13 out of 15 patients with rotator cuff tears and 3 patients with displaced greater tuberosity fracture had operations. Conclusion: The injury mechanism of shoulder dislocation in patients older than 60 years of age seems to have either anterior or posterior mechanism. The diagnosis and treatment should be approached 2 weeks after dislocation.

  • The purpose of this study was to analyze the stress distribution and the displacement in the maxillary complex after the application of the reverse headgear. The direction of force was parallel to the occlusal plane. Orthopedic force,300gm, was applied to the maxilla of the dry human skull in a forward direction. The stress distribution and the displacement within the maxillary Complex was analyzed by a 3-dimensional finite element method. The results were as follows: 1. The stress distribution at the molar region was greater than that at the anterior. 2. The stress distribution at the lateral side of the premaxilla was greater than that at the middle aide, especially high stress was noted at the canine eminence. 9. Compressive stress was noted only at the frontozygomatic suture of the zygomatic arch. 4. A forward, upward, and sideward displacement was noted at the entire nodal points of the zygomaticomaxillary suture portion. A displacement with a slight rotation was observed on the transverse palatine suture. 5. The maximum stress was observed at the lateral side of the maxillary tuberosity area, and generally the forward and downward displacement was noted at all this area.

한방치료로 호전된 견관절의 석회화 건염 4례 보고 (Calcific Tendinitis of the Shoulder by Oriental Medical Treatments : 4 Cases Report)

  • This study was performed to report the effect of oriental medical treatments on the 4 patients with calcific tendinitis of the shoulder. The 4 patients were treated by cupping, acupuncture, pharmacopuncture on tender points of the greater tuberosity of humerus. The state of patients was measured by visual analog scale(VAS) and shoulder range of motion(ROM). After mean 3.25 times of treatments, the significant recovery of shoulder ROM was measured and the significant pain relief was seen as the average of VAS changed from 8.5 to 1. These results suggest that oriental medical treatments including cupping, acupuncture, pharmacopuncture are effective to improve the calcific tendinitis of the shoulder. However more rigorous study should be followed.

상완골 근위부 골절의 치료 실패 후 견관절 치환술 (Shoulder Replacement Arthroplasty after Failed Proximal Humerus Fracture)

  • 상완골 근위부 골절은 상완골의 외과적 경부 또는 근위부에 발생하는 골절로 정의할 수 있으며, 적절한 치료에도 불구하고 다양한 합병증 및 후유증이 발생할 수 있어 그 치료가 매우 까다롭고 견관절 치환술 등의 수술적 치료를 요하는 경우가 많다. 상완골 근위부 골절 후 후유증은 Boileau가 제시한 분류법이 가장 많이 사용되며, 2개의 카테고리 및 4가지의 타입으로 분류할 수가 있다. 카테고리 I은 관절 내 감입 골절로 결절들과 상완골 두 사이에 저명한 해부학적 변형이 동반되지 않아 결절 절골술을 시행하지 않고도 해부학적 치환물을 사용할 수 있는 경우로서 결절의 불유합이 거의 없이 두경부의 붕괴(cephalic collapse)나 괴사가 있는 타입 1과 잠김 탈구(locked dislocation) 또는 골절-탈구와 관련이 있는 타입 2로 나눌 수 있다. 카테고리 II는 관절 외 골절이며 결절들과 상완골 두 사이에 육안적인 변형이 있는 경우로서 해부학적 치환물을 사용하기 위해서는 결절 절골술을 시행해야 하며, 외과적 경부의 불유합이 동반된 타입 3와 심각한 결절의 부정유합이 동반된 타입 4로 분류할 수 있다. 각 타입별 치료를 위해서 타입 1의 경우에는 결절 절골술을 시행하지 않고 비구속형 치환술을 우선적으로 고려해야 하지만, 타입 1C, 1D와 같이 외반이나 내반 변형이 동반되거나 회전근 개의 지방 변성이 심할 경우에는 역행성 견관절 전치환술 또한 고려해야 한다. 타입 2는 일반적으로 비구속형 치환술로 좋은 결과를 기대할 수 있으나 관절와 골 결손이 없으며 회전근 개의 결손이 동반된 경우 역행성 견관절 전치환술 또한 하나의 치료 방법으로 고려할 수 있다. 타입 3는 견관절 치환술보다는 골쐐기 이식 등을 함께 시행하여 내고정을 시행하는 것이 효과적일 것으로 보이며, 최근 역행성 견관절 전치환술의 결과에 대한 보고 또한 점차 증가하고 있는 추세이다. 마지막으로 타입 4는 역행성 견관절 전치환술이 우선적으로 고려되어야 할 것으로 보인다.

  • 석회성 건염은 건의 부착부 주변에 발생하는 병변으로 견관절에서는 주로 극상근의 대결절 부착 부위의 근위부에 발생하는 경우가 대부분이다. 저자들은 류마티스 관절염을 가진 환자에서 발생한 상완 이두근 단두건과 오구상완근의 연합건의 석회성 건염을 초음파 검사를 통해 진단한 증례를 경험하여 보고하는 바이다.

상완골 근위부 골절에 시행한 긴장 대 봉합을 동반한 관혈적 골수강내 고정술 (Open Intramedullary Nail with Tension Band Sutures on Proximal Humeral Fracture)

  • Purpose: to determine the results after open intramedullary nailing and tension band suture technique in proximal humerus fracture for improving the stability and decreasing the complications. Materials and Method: Authors reviewed 27 patients treated by open intramedullary nailing and tension band suture technique. Mean follow-up period was 39 months (24-59months). Surgical neck fracture were 6 cases, surgical neck fracture with shaft fracture were 3 cases, three part fracture with greater tuberosity fracture were 17 cases, four part fracture was 1 case and fracture and dislocation were 2 cases Results: We got the bony union in 26 cases. Average pain scale was 1 point (0-6), Neer score was 86 point(45-99) and ASES score was 85 point(40-100). We separate all cases in two groups based on age (65 years), L-spine t-score (-2.5) and Neer classification (2 and 3 part). There is no significance in pain scale, Neer score and ASES score between each group. Conclusion: As a method of surgical treatment on severe proximal humeral fractures, we recommend intramedullary nailing and tension band suture technique and it may have particular advantages in early exercise and satisfactory functional outcome.

褥瘡의 治療에 관한 문헌적 고찰 (The literary review on the Treatment of Pressure Sore)

  • Pressure sore is an area of ulceration and necrosis of the skin and underlying tissues usually occuring over the bony prominences of the body after prolonged or often repeated pressure. We reviewed and summarized the published articles and treatise on the treatment of pressure sore. The results were as follows : 1. Pressure sore occur due to prolonged or often repeated pressure. So it is better than decubitus ulcer that is called pressure sore. 2. The most common lesions of pressure sore are sacrum, ischial tuberosity, greater trochanter. 3. The cause of pressure sore are change of comprehension. urine, moisture, change of the ability of activity and exercise, shearing force. 4. The elements to influence on wound healing are collagen accumulation velocity, nutrition condition, Vitamine C, copper, iron. oxygen pressure, steroids, cell-toxic drug, radiation. 5. Non-operative treatments are managements of skin such as avoiding consistant pressure, dressing, preventing moisture, understanding patient and protecter, preventing spasm, improvement of systemic nutrition condition. 6. Operative treatements are debridement, suture, skin transplantation, muscle flap and musculaocutaneous flap surgery. Recently V-${\Gammer}$ advancement surgery in use of muscle and musculocutaneous flap is generally maded. 7. Complications of post-operation are wound rupture, infection, disappearance of transmitted skin, necrosis of flaps.

Unrecognized bony Bankart lesion accompanying a dislocated four-part proximal humerus fracture before surgery: a case report

  • Proximal humerus fractures are the third most common fractures, totaling 4% to 5% of all fractures. Here, we present the case of a 39-year-old man with a dislocated four-part fracture of the proximal humerus with a huge bony Bankart lesion. Preoperatively, the bony Bankart lesion of the glenoid was not visualized on computed tomography scans or magnetic resonance imaging because the fracture of the proximal humerus was comminuted, displaced, and complex. It was planned for only the humerus fracture to be treated by open reduction and internal fixation using a locking plate. However, a fractured fragment remained under the scapula after reduction of the dislocated humeral head. This was mistaken for a dislocated bone fragment of the greater tuberosity and repositioning was attempted. After failure, visual confirmation showed that the bone fragment was a piece of the glenoid. After reduction and fixation of this glenoid part with suture anchors, we acquired a well-reduced fluoroscopic image. Given this case of complex proximal humerus fracture, a glenoid fracture such as a bony Bankart lesion should be considered preoperatively and intraoperatively in such cases.

Minimally Invasive Percutaneous Plate Osteosynthesis via a Deltoid-splitting Approach with Strut Allograft for the Treatment of Displaced 3- or 4-part Proximal Humeral Fractures

  • Background: This study introduces a surgical technique with good clinical outcome useful in the treatment of osteoporotic displaced 3- or 4-part proximal humeral fractures. Methods: From May 2014 to February 2016, 16 patients with displaced 3- or 4-part proximal humeral fractures were treated by application of a locking plate with an endosteal strut allograft via a deltoid splitting approach with a minimum follow-up of 12 months. The allograft was inserted through a fractured gap of the greater tuberosity to support the humeral head and then fixed by a locking plate with meticulous soft tissue dissection to protect the axillary nerve. Surgical outcomes were evaluated by the American Shoulder and Elbow Surgeons (ASES) and visual analogue scale (VAS) scores, radiological imaging, and clinical examination. Fixation failure on radiographs was defined as a >$5^{\circ}$ loss of neck shaft angle (NSA) compared to that on an immediate postoperative radiograph. Avascular necrosis (AVN) of the humeral head was also evaluated. Results: In all cases, complete union was achieved. The ASES and VAS scores were improved to $85.4{\pm}2.1$ and $3.2{\pm}1.3$, respectively. Twelve patients (75.0%) had greater than a $5^{\circ}$ change in NSA; the average NSA change was $3.8^{\circ}$. Five patients (31.3%) had unsatisfactory ranges of motion exhibiting a <$100^{\circ}$ active forward flexion. No axillary nerve injuries or AVN were observed at the last follow-up. One patient was converted to reverse total arthroplasty due to severe pain and functional deficit. Conclusions: Minimally invasive fixation via a locking compression plate and an endosteal fibula strut allograft in Neer classification 3-or 4-part fractures with severe osteoporosis in elderly patients can achieve good clinical results.
