($CO_2$)레이저를 이용한 백서의 치은절제술시 출력에 따른 초기 치유과정의 비교 (A Comparative Study of Initial Healing Process in White Rats after Gingivectomy using $CO_2$ Laser of different watts)

  • 레이저를 이용한 연조직 수술은 출혈이 없어 시야를 좋게 하고 시술시간이 단축되며, 술후 종창이 최소화 되고, 통증이 감소화 최소화된 반흔 형성, 그리고 레이저가 조사되는 일부 부위의 멸균효과등이 장점이 있어 최근에 의학분야 및 치의학분야에서 많이 사용되는 추세에 있다. 이에 본 연구에서는 $CO_2$레이저를 이용한 백서의 치은절제술시의 치유과정과 레이저 출력을 달리 하였을때의 치유과정을 관찰하기 위해, 백서의 상악전치의 치은조직에서 치은절제술 효과를 얻을 수 있는 최소 출력인 4watts를 이용한 부위를 대조군으로, 6watts를 이용한 부위를 실험군으로 하여, 술후 2일, 3일, 1주, 3주후에 각각 실험동물을 희생시켜 치유결과를 조직학적으로 비교 관찰하여 다음과 같은 결과를 얻었다. 대조군과 실험군 모두에서 2일째에서만 작은 크기의 혈병이 관찰되었고, 그 이후에서는 관찰되지 않았다. 2. 염증세포 침윤지역 크기는 대조군과 실험군 모두에게 2일째 가장 컸으며, 그 크기는 시간이 경과될수록 줄어들어 2주째는 거의 소실되었으며, 실험군의 경우 대조군에 비해 2,3째까지 크기가 더 컸으나, 1주째부터는 크기의 차이가 관찰되지 않았다. 3. 육아조직은 대조군, 실험군 모두 시간의 경과에 따라 점점 성숙되어, 2주째부터는 거의 정상 치은 결합조직으로 대체되는 소견을 보였고, 3주째에서는 완전한 치유양상을 보였다. 실험군의 경우 대조군보다 3일째까지는 그 크기가 더 컸으나 1주째부터는 크기의 차이가 없다. 4. 대조군과 실험군 모두에게 치은의 상피화하는 2일째에서 시작되는 소견을 보였고, 1주부터 상피돌기와 부분적인 접합상피의 재생이 관찰되었으며, 2주째부터는 구강열구상피의 각화가 시작되어 3주째에는 각화의 완성이 관찰되었다.

치근이개부 수평결손시 조직재생에 관여하는 전구세포의 면역세포화학적 연구 (THE IMMUNOCYTOCHEMICAL STUDY OF THE PRECURSOR CELLS IN THE PERIODONTAL REGENERATION OF HORIZONTAL FURCATION DEFECT.)

  • The origin of fibroblasts, their proliferative activity and roles in the early stages of periodontal regeneration were investigated in order to better understand the periodontal healing process in furcation defects of the beagle dog after guided tissue regeneration. Newly divided cells were identified and quantitated by immunolocalization of bromodeoxyuridine (BrdU) injected 1 hour prior to sacrificing the animals. The results were as follows :1. During periodontal healing in horizontal furcation defect, three different stages, namely the granulation tissue, connective tissue, and bone formation stages, were identified on the basis of major types of cells and tissue. 2. In the early stages of periodontal regeneration, both the remaining periodontal ligament and alveolar bone compartment were the major sources. 3. The majority of BrdU-labeled fibroblasts were located at the following areas ; 1) the coronal zone of the defect in case of the connective tissue fanned on the root surface. 2) the area within an 400 ${\mu}m$ distance from the remaining bone level in case of the periodontal ligament. 3) the area within an 100 ${\mu}m$ distance from the bone surface in case of areas of active bone formation.4. The highly proliferative fibroblasts adjacent to bone surface played a major role in the formation of osteoblast precursor cells, whereas both paravascular and endosteal cells played a minor role in new bone formation, In conclusion, it was suggested that the fibroblasts in the remaining periodontal ligament and bone will play a major role in periodontal regeneration, whereas both paravascular and endosteal cells will play a minor role in new bone formation.

지방흡입술 후 연부 조직 괴사에 대한 술중 조직확장법을 이용한 치료 (Intraoperative Expansion Technique for Soft Tissue Necrosis after Liposuction)

  • Purpose: The liposuction has been generalized & undergone in the field of plastic surgery and it has had a great influence on fat graft. But despite of liposuction and fat injection being performed widely, we did not focus on complication associated with donor site. So we reported satisfactory result with the intraoperative sustained limited expansion & direct closure to donor site tissue necrosis after liposuction and analyzed the cause & measures of donor site necrosis with the literature investigation Methods: From November 2007 to June 2008, we treated four cases of tissue necrosis and infection surrounding the thigh and forearm. Necrosis after liposuction was 1 case, donor site necrosis after fat injection were 3 cases and MRSA was detected in 2 cases. first, we debrided the necrotic tissue and treated with potadine gauze soaking dressing & susceptible antibiotics. After confirmation of healthy granulation tissue, we used intraoperative sustained limited expansion and closed directly of defect & observed the results. Results: The patient was displeased and worried with the unexpected damage concerning the donor site and the procedure concerning time and financial exhaustion, but after confirming no contracture of the scar tissues and only a thin or slightly widened line of scar, showed satisfaction. Conclusion: Because it is fastidious to avoid donor site complication after liposuction & fat injection, enough explanation and understanding on possible complication to donor site are necessary and careful surgery procedure & materials are required. But if donor site necrosis were occurred although close consideration of possible causes of necrosis, we should think about not only wound healing process but also the sequela like scar contracture & cosmetic effect and treat the most adequate method to satisfying both concerns.

  • The aim of this study was to achieve healing of Peri-implantitis defects and hard tissue augmentation using a bovine-derived bone mineral on the defect site. Two patients were treated with the surgical approach. With a full muco-periosteal flap elevation, the implant surfaces were exposed and granulation tissue removed around the implant and between the threads. Each surface of the contaminated implant was prepared with the air-abrasive device(PerioFlow$^{(R)}$) for decontamination. Bovine-derived bone mineral(Bio-Oss collagen$^{(R)}$) was then used to fill the defects and muco-periosteal flaps sutured to achieve transmucosal healing. Radiographs and clinical photographs were taken before and after 6 months of healing and an estimate of bone fill was assessed. Within the limits of the present case report, a surgical approach in treatment of peri-implantitis defects using a collagen form of bovine bone mineral was visited. Although limited, the two cases showed the stability and biocompatibility of a bovine-derived bone mineral and effectiveness of air-abrasive device(PerioFlow$^{(R)}$) as a decontamination method.

Immunomodulatory properties of medicinal maggots Lucilia sericata in wound healing process

  • The healing properties of medicinal maggots (larval stage of Lucilia sericata) are widely used in the chirurgical debridement of non-healing wounds including diabetic foot ulcers, venous and pressure ulcers, where classical approaches have failed. Several kinds of wounds are prone to complications coming out of a specific wound bed environment. There are multi-resistant bacterial species present, their pathogenic impact is multiplied by their ability to form a biofilm. Moreover, immunological events in chronic wounds differ from those in acute wounds. Non-healing wounds are cycled in the early inflammation phase with increased levels of inflammation attributes like inflammation cytokines and matrix metalloproteinases produced by inflammation phase cells. Application of larval therapy promotes progress in the healing process to the next stages involving tissue granulation and re-epithelisation. Larval debridement is an effective method of cleaning the wound of cell debris, necrotic tissue and bacterial load. This happens in a mechanical and biological manner, but the whole complex mechanism of the maggot healing activity is still not fully elucidated. Centuries of clinical practice brings noticeable proof of the maggots' beneficial effect in wound healing management. This long history led to the investigation of the bioactive components of the larval body and its extracts in vitro. We introduce a review which describes the immunomodulation impact of maggot body components on the cellular and molecular levels of the wound healing process.

임플란트 주위염의 치료: 증례보고 (Treatment of Peri-implantitis: Cases Report)

  • This study aims to investigate the clinical outcome following treatment of peri-implantitis lesions. Five subjects with 7 implants were treated with surgical approach. Four subjects with 6 implants were initially treated with non-surgical approach or hygiene control. However, inflammation was not resolved and more bone loss was found. Therefore, surgical treatment was performed. After surgical exposure of the defect, granulation tissue was removed and implant surface was treated using tetracycline and chlorhexidine. Then, the flaps were sutured. The wound healing was performed in a non-submerged mode. The present finding demonstrates stable results without progression of bone loss. In one subject, deep V shaped bone defect was filled with bone substitute (ICB, CanCellous Bone, Rockey Mountain Tissue Bank, USA), and resorbable membrane (Lyoplant$^{(R)}$, B.Braun Aesculap AG, Germany) was placed over the grafted defect and healing abutment was connected. However, the inflammation was not resolved and more bone loss was found. At one month after regenerative surgery, the implant was removed.

Acute Myelopathy Caused by a Cervical Synovial Cyst

  • Synovial cysts of the cervical spine, although they occur infrequently, may cause acute radiculopathy or myelopathy. Here, we report a case of a cervical synovial cyst presenting as acute myelopathy after manual stretching. A 68-year-old man presented with gait disturbance, decreased touch senses, and increased sensitivity to pain below T12 level. These symptoms developed after manual stretching 3 days prior. Computed tomography scanning and magnetic resonance imaging revealed a 1-cm, small multilocular cystic lesion in the spinal canal with cord compression at the C7-T1 level. We performed a left partial laminectomy of C7 and T1 using a posterior approach and completely removed the cystic mass. Histological examination of the resected mass revealed fibrous tissue fragments with amorphous materials and granulation tissue compatible with a synovial cyst. The patient's symptoms resolved after surgery. We describe a case of acute myelopathy caused by a cervical synovial cyst that was treated by surgical excision. Although cervical synovial cysts are often associated with degenerative facet joints, clinicians should be aware of the possibility that these cysts can cause acute neurologic symptoms.

Construction of Chimeric Human Epidermal Growth Factor Containing Short Collagen-Binding Domain Moieties for Use as a Wound Tissue Healing Agent

  • Among the various human growth factors, epidermal growth factor (hEGF, consisting of 53 amino acids) has various effects on cell regeneration, stimulation of proliferation, migration of keratinocytes, formation of granulation tissues, and stimulation of fibroblast motility, which are important for wound healing. Owing to their multiple activities, EGFs are used as pharmaceutical and cosmetic agents. However, their low productivity, limited target specificity, and short half-life inhibit their application as therapeutic agents. To overcome these obstacles, we fused the collagen-binding domain (CBD) of Vibrio mimicus metalloprotease to EGF protein. About 18 or 12 amino acids (aa) (of the 33 total amino acids), which were essential for collagen-binding activity, were combined with the N- and C-termini of EGF. We constructed, expressed, and purified EGF (53 aa)-CBD (18 aa), EGF (53 aa)-CBD (12 aa), CBD (18 aa)-EGF (53 aa), and CBD (12 aa)-EGF (53 aa). These purified recombinant proteins increased the numbers of cells in treated specimens compared with non-treated specimens and control hEGF samples. The collagen-binding activities were also evaluated. Furthermore, CBD-hybridized hEGF induced phosphorylation of the EGF receptor. These results suggested that these fusion proteins could be applicable as small therapeutic agents in wound tissue healing.

Algin-Impregnated Vascular Graft II. Preliminary Animal Study

  • Microvel $^{\textregistered}$ double velour graft impregnated with a biodegradable algin was studied as a new vatscular graft. It is impervious to blood but still retains high porosity. This graft does not require preclotting during implantation and has good tissue ingrowth and biological healing properties. Two vascular grafts impregnated with algin (6mm in diameter) were implanted in the aorta of mongrel dogs without preclotting. Two identical grafts were preclctted and served as controls. The grafts were harvested 2 and 4 months postoperatively, and the healing pattern was examined by a light microscope after hemRtoxylineosin staining. It was observed that endothelial cells were incompletely covered on both algin-impregnated and control grafts after 2 month Implantation, while they were fully covered on both grafts after 4 month. There were no significant differences in subendothelial granulation tissue organization and fibrinoid material absorption between the algin-impregnated and control grafts. The algin-impregnated graft did not show any harmful effect on the healing and thus can be a new promising graft which is not necessary preclotting during the implantation.

  • NPWT는 90년대 이후 많은 연구가 이루어지며 발전해왔다. 이 기술은 사용하기에 간단하고 다양한 상처에 적용할 수 있고 효용성과 비용-효과가 뛰어나며 부작용이 적다. 따라서 NPWT를 일반적인 드레싱 대신에 사용하면 육아조직의 빠른 성장과 상처 수축을 통해 피부 이식이나 피판이 필요한 부위를 줄일 수 있고 봉합이 불가했던 부위를 가능하게 하므로 창상 환자에 대한 의료 비용을 감소시키고 만성 창상 환자가 입원하지 않고 움직이면서 창상 관리가 가능하도록 할 수 있는 유용한 방법이다.
