The Use of Nylon Tube as Aortic Prostheses: 2 Cases (Nylon tube를 이용한 대동맥 Prostheses (2례))

  • This is a report on two cases of aortic prostheses using Nylon tube. (Edwards-Tapp A-G Tube, Chemically treated braided Nylon arterial grafts). Especially, the complications after infection of synthetic graft are discussed with reviewing literature. First case was the patient who came to our hospital with rupture of the right femoral artery at the femoral fossa due to pyogenic necrotic process. After femoral arterial prostheses, good pulsation of dorsal artery of foot was obtained. However, the tube was obstructed after 8 weeks postoperatively due tll the complication of infection. In spite of the tube was removed because of obstruction and foreign body reaction of synthetic graft, an amputation of the leg was not necessary for formation of good collateral circulation. Second case was a case of aortic aneurysmal rupture in thoraco-abdominal junction which developed by the trauma of rib resection for osteomyelitis of the left 12th. rib An implantation of aortic graft was performed at the lowest tho13cic aorta by the way of thoraco-abdominal bypass without arterial pump. However, infection produced pyothorax in the left pleural cavity, exposing the tube within the pyothorax. The rupture of the anastomosed upper line occurred in 8 weeks postoperatively and the patient expired.

Pretibial Cyst after Anterior Cruciate Ligament Reconstruction Using bio-absorbable interference screw fixation - A Case Report - (생체분해성간섭나사를 이용한 전방십자인대 재건술 후 발생한 경골 근위부 낭종-증례 보고-)

  • Pretibial cyst formation is a rare complication after anterior cruciate ligament (ACL) reconstruction and there are only few cases reported. Pretibial cyst can be caused by variable reasons. Foreign body reaction due to breakdown of bio-absorbable screw for fixation, graft necrosis at tibial site, joint fluid leakage to tunnel and incomplete incorporation of graft to bony tunnel. The authors experienced one case of massive pretibial cyst after arthroscopic ACL reconstruction using bio-absorbable interference screw in 38-year-old male patient. Thus, authors report this rare case with literature view.

  • Maxillary implants are inserted in the upward direction, meaning that they oppose gravity, and achieving stable support is difficult if the alveolar bone facing the maxillary sinus is thin. Correspondingly, several sinus-lifting procedures conducted with or without bone graft materials have been used to place implants in the posterior area of the maxilla. Even with these procedures available, it has been reported that in about 5% of cases, complications occurred after implantation, including acute and chronic sinusitis, penetration of the sinus by the implant, implant dislocation, oroantral fistula formation, infection, bone graft dislocation, foreign-body reaction, Schneiderian membrane perforation, and ostium plugging by a dislodged bone graft. This review summarizes common maxillary sinus pathologies related to implants and suggests an appropriate management plan for patients requiring dental implantation.

Brain Death and Kidney Transplantation in Dogs (개의 뇌사와 신장이식)

  • Brain dead (BD) patients remain the largest source of solid organs for transplantation. BD has shown to decrease graft function and survival in rodent models. The aim of this study was to evaluate how brain death affects graft viability in the donor and kidney tolerance to cold preservation as assessed by survival in a canine transplantation. 13 Beagle dogs were used for the study. Brain death was induced by the sudden inflation of a subdural balloon catheter with continuous monitoring of arterial blood pressure and eletroencephalographic activity (n=3). Sixteen hours after conformation of brain death, kidney graft were retrieved (n=6). Non-BD donors served as controls (n=4). All kidneys were flushed with University of Wisconsin (UW) solution and preserved for 24 hours at 4$^{\circ}C$ before transplantation. Recipient survival rates, serum creatinine level were analyzed. Brain death induced the well-known Cushing reaction with a severe increase in blood pressure and tachycardia. Thereafter, cardiac function returned progressively to baseline within 8 hours and remained stable until the end of the experiment. All of dogs in both group transplanted were survived until 7 days (100%), and the kidneys showed functional early rejection at 8.3$\pm$0.5 days and 8.5$\pm$0.5 days after transplantation, in BD and allograft group, respectively. BD kidneys were functionally similar to control kidneys for 7 days after transplantated. Brain death has no deleterious effect on preservation injury and survival of dog kidney transplantation, although it induces changes in hemodynamic parameters. This study reveals that kidneys from BD donors do not exhibit more ischemia reperfusion injury, and support good early function and survival.

  • The grafting of methacrylonitrile(MAN) onto Kevlar 49 filament surface was carried out by anionic polymerization using sodium methylsulfinylcarbanion formed from sodium hydride and dimethyl sulfoxide(DMSO). The effects of reaction conditions on the grafting percentage(GP) and on the tensile strength of the fiber were investgated. GP marktedly increased with increasing metalation time, and NaH concentration, polymerization temperature and time. The tensile strength of fiber decrased with increasing metalation time, and NaH concentration, polymerization temperature and time. The optimum conditions to increase over 40% of GP with below 10% reduction rate of tensile strength of fiber : NaH concentration ; 30.6 mmol/l/0.5g Kevlar, metalation time : 10min, polymerization tempera- ture : 5$0^{\circ}C$, polymerization time: 20 sec, monomer concentration : 1.12mol/l/0.5g Kevlar.

  • To improve the mechanical properties of glass-fiber-reinforced polypropylene (PP) composites through interfacial adhesion control between the PP matrix and glass fiber, the surface of the glass fiber was modified with PP-graft-maleic anhydride (MAPP). Surface modification of the glass fiber was carried out through the well-known hydrolysis-condensation reaction using 3-aminopropyltriethoxy silane, and then subsequently treated with MAPP to produce the desired MAPP-anchored glass fiber (MAPP-a-GF). The glass-fiber-reinforced PP composites were prepared by typical melt-mixing technique. The effect of chemical modification of the glass fiber surface on the mechanical properties of composites was investigated. The resulting mechanical and morphological properties showed improved interfacial adhesion between the MAPP-a-GF and PP matrix in the composites.

  • The complication caused by a bioabsorbable interference screw is rare after anterior cruciate ligament reconstruction. We report a case of delayed inflammatory reaction at the tibial tunnel and femoral tunnel where the graft tendon had been fixed with a bioabsorbable interference screw ($Bioscrew^{(R)}$) for anterior cruciate ligament reconstruction using bone-patellar tendon-bone autograft.

  • The treatments of methacrylamide on silk fibers were studied in aqueous solution using potassium persulfate as an initiator, and suitable conditions of reaction were determined for weighing of silk fibers. The results obtained were summarized as follows ; The weight of MAA-treated silk fibers increased with monomer concentration. The adequate concentration of potassium persulfate was found to be 1.7%. Maximum weight increase was shown at initial pH 3.8 of reaction liquor controlled by buffer solution.
