  • Central cord syndrome (CCS) is the most common incomplete spinal cord injury (SCI) syndrome, occured by cervical hyperextension injury. It is characterized by disproportionately greater motor impairment in the upper extremities compared to the lower extremities, bladder dysfunction, and variable degrees of sensory loss below the level of injury. Recently 36 year-old male was admitted with upper motor neuron weakness, sensory loss in the upper extremities and stiff neck. We report the improvement of his symptoms through both western medical treatment and oriental medical treatment.

A Case Report of Genital Ulcer Treated by Korean Traditional Medicine (한방치료를 통해 호전된 외음부궤양 1례 증례보고)

  • Objectives: The purpose of this study is to report the improvement of vulvodynia and genital pruritus caused by unidentified genital ulcer after Korean medicine treatment. Methods: The patient who complained vulvodynia and genital pruritus caused by unidentified genital ulcers was treated by acupuncture, moxibustion, fumigation, and herbal medicine as Eunhwasagan-tang, Guibi-tang-gami-bang for 5 days. The effect of treatment was evaluated by Numeral Rating Scale (NRS), The 5 level of EuroQol 5 Dimension scale (EQ-5D-5L), EuroQol-Visual Analog Scale (EQ-VAS). Results: After the treatment, the chief complaint and general conditions were improved. NRS was decreased from 8 to 3 and scores of EQ-5D-5L and EQ-VAS were increased from 0.416, 10 to 0.904, 80 each. Conclusion: This study shows that the skin lesion, vulvodynia, and pruitus caused by genital ulcer was improved after the Korean traditional treatment and it can be effective medical alternatives or options for genital ulcer patients.

  • Objectives: This study was designed to investigate the clinical effectiveness of Ikgibohyeol-tang(Yìqìbǔxuè-tāng) granules and Deer antler granules on fatigue symptoms. Methods: This is a 39-patient case series from a retrospective chart review of outpatients who were treated with Ikgibohyeol-tang granules and Deer antler granules at a Kyung Hee University Korean Medicine Hospital (Seoul) between 1st, January 2015 to 1st, August 2021. We reviewed the patients who complained of fatigue and analyzed changes in the degree of discomfort with fatigue and accompanying symptoms and HRV (Heart Rate Variability) score before and after treatment. Results: After taking Ikgibohyeol-tang granules and Deer antler granules, the degree of discomfort with fatigue and accompanying symptoms decreased significantly compared to before taking them. After treatment, Autonomic nervous system activity, Stress resistance, Heart Stability increased, and Stress index, Degree of fatigue decreased significantly. Conclusions: This study showed subjective symptoms and objective indicator (HRV score) were improved by taking Ikgibohyeol-tang granules and Deer antler granules.

A Case Report of Korean Medicine Treatment for a Patient with Left Facial Palsy Diagnosed with Wegener's Granulomatosis (베게너 육아종으로 진단된 좌안면신경마비 환자에 대한 한의복합치료 1례)

  • Background: The study reports the effect of Korean medicine treatment on a patient with left facial palsy caused by Wegener's granulomatosis. Case information: The patient was receiving steroid treatment for facial palsy and was hospitalized for 21 days. He received acupuncture, electroacupuncture, bee venom pharmacopuncture, and moxibustion treatment along with Ssanghwa-tang and Cheongansohabwon herbal medicine treatment. During hospitalization, the Sunnybrook facial grading scale, House-Brackmann scale and length of time for the eye to completely close were evaluated at 11/16, 11/21, 11/27, 12/1, and 12/6. Post treatment, the degree of the movement in the left eyelid was improved, it took less time for the eye to close completely, the movement of the forehead wrinkle muscles improved, but that of the facial muscles under the eyes and left orbicularis oculi muscle showed little improvement. Conclusion: Korean medicine treatment for facial palsy caused by Wegener's Granulomatosis helps restore mobility of the forehead and orbicularis oculi muscles.

  • The National Bio Resource Project-Rat (NBRP-Rat) comprises the largest bank of laboratory rat (Rattus norvegicus) strains in the world. Its main focus is to develop infrastructure that will facilitate the systematic collection, preservation, and provision of rat strains. To breed effectively more than 180 rat strains in living stock, we establish the genetic control system in which a systematic set of genetic diagnoses and genetic monitoring are included. Genetic monitoring is performed by using 20 polymorphic markers. Monitoring is carried out when a living animal stock is re-established by using cryopreserved embryos or sperm or when a rat strain is first introduced to the NBRP-Rat by a depositor. Additional monitoring is then carried out on each strain every two years. Genetic diagnosis is performed largely by employing the Amp-FTA method. Protocols which detail how to perform a genetic diagnosis of 11 transgenes and 24 mutations have been made. Among the mutations, nine can be detected by simple gel electrophoresis of the PCR products, 11 by restriction enzyme treatment of the PCR products, and four by direct PCR product sequencing. Using this genetic control system, the NBRP-Rat can guarantee the genetic quality of its rat strains.

Enhanced antidiabetic efficacy and safety of compound K/β-cyclodextrin inclusion complex in zebrafish

  • Background: 20(S)-Protopanaxadiol 20-O-D-glucopyranoside, also called compound K (CK), exerts antidiabetic effects that are mediated by insulin secretion through adenosine triphosphate (ATP)-sensitive potassium ($K_{ATP}$) channels in pancreatic ${\beta}$-cells. However, the antidiabetic effects of CK may be limited because of its low bioavailability. Methods: In this study, we aimed to enhance the antidiabetic activity and lower the toxicity of CK by including it with ${\beta}$-cyclodextrin (CD) (CD-CK), and to determine whether the CD-CK compound enhanced pancreatic islet recovery, compared to CK alone, in an alloxan-induced diabetic zebrafish model. Furthermore, we confirmed the toxicity of CD-CK relative to CK alone by morphological changes, mitochondrial damage, and TdT-UTP nick end labeling (TUNEL) assays, and determined the ratio between the toxic and therapeutic dose for both compounds to verify the relative safety of CK and CD-CK. Results: The CD-CK conjugate ($EC_{50}=2.158{\mu}M$) enhanced the recovery of pancreatic islets, compared to CK alone ($EC_{50}=7.221{\mu}M$), as assessed in alloxan-induced diabetic zebrafish larvae. In addition, CD-CK ($LC_{50} =20.68{\mu}M$) was less toxic than CK alone ($LC_{50}=14.24{\mu}M$). The therapeutic index of CK and CD-CK was 1.98 and 9.58, respectively. Conclusion: The CD-CK inclusion complex enhanced the recovery of damaged pancreatic islets in diabetic zebrafish. The CD-CK inclusion complex has potential as an effective antidiabetic efficacy with lower toxicity.

  • Objectives This study was perfomed to assess the adherance to CARE (CAse REport) guideline of case reports in the Journal of Korean Medicine Rehabilitation Methods We searched the case reports published in the Journal of Korean Medicine Rehabilitation from January 2016 to April 2019 in the database of oriental medicine advanced searching integrated system (OASIS). Then we evaluated the quality of the searched case reports based on the CARE guideline. Results Totally 31 papers were selected after the screening the case reports by the inclusion and exclusion criteria. The report rate of the sub-items of the CARE guideline was 78.26% at the maximum, 60.87% at the maximum, and 70.97% on the average. The following items were reported only in less than 50% of them; 'Timeline', 'Diagnostic challenges', 'Diagnostic reasoning including other diagnoses considered' 'Prognostic characteristics', 'Follow-up and Outcomes', 'Patient Perspective', 'Informed Consent' Conclusions This study is expected to contribute to the overall improvement of the level of case reports in the Journal of Korean Medicine Rehabilitation.

Clinical Impact of Different Reconstruction Methods on Remnant Gastric Cancer at the Anastomotic Site after Distal Gastrectomy

  • Background/Aims: The anastomotic site after distal gastrectomy is the area most affected by duodenogastric reflux. Different reconstruction methods may affect the lesion characteristics and treatment outcomes of remnant gastric cancers at the anastomotic site. We retrospectively investigated the clinicopathologic and endoscopic submucosal dissection outcomes of remnant gastric cancers at the anastomotic site. Methods: We recruited 34 consecutive patients who underwent endoscopic submucosal dissection for remnant gastric cancer at the anastomotic site after distal gastrectomy. Clinicopathology and treatment outcomes were compared between the Billroth II and nonBillroth II groups. Results: The tumor size in the Billroth II group was significantly larger than that in the non-Billroth II group (22 vs. 19 mm; p=0.048). More severe gastritis was detected endoscopically in the Billroth II group (2 vs. 1.33; p=0.0075). Moreover, operation time was longer (238 vs. 121 min; p=0.004) and the frequency of bleeding episodes was higher (7.5 vs. 3.1; p=0.014) in the Billroth II group. Conclusions: Compared to remnant gastric cancers in non-Billroth II patients, those in the Billroth II group had larger lesions with a background of severe remnant gastritis. Endoscopic submucosal dissection for remnant gastric cancers in Billroth II patients involved longer operative times and more frequent bleeding episodes than that in patients without Billroth II.

Application of Vision Sensor to Communication Device for Person with Serious Disability

  • We developed a communication device for person with serious disability to use slight movement. This device is developed mainly for patients suffering from ALS or a cerebral infarction. They often have communication difficulty because of deterioration of muscle functions. A feature of this device is that slight movements of user's finger, eyes or lips can be detected by using a vision sensor. Due to the features of the vision sensor, it is quite easy even for person with serious disability to use a communication device. By the field test it is confirmed that the vision sensors have superior performances as an input device for communication device. Experiments to use an EMG (electromyography) sensor and a rotary sensor are also tested to compare the performances.
