Preoperative Identification of Facial Nerve in Vestibular Schwannomas Surgery Using Diffusion Tensor Tractography

  • Objective : Facial nerve palsy is a common complication of treatment for vestibular schwannoma (VS), so preserving facial nerve function is important. The preoperative visualization of the course of facial nerve in relation to VS could help prevent injury to the nerve during the surgery. In this study, we evaluate the accuracy of diffusion tensor tractography (DTT) for preoperative identification of facial nerve. Methods : We prospectively collected data from 11 patients with VS, who underwent preoperative DTT for facial nerve. Imaging results were correlated with intraoperative findings. Postoperative DTT was performed at postoperative 3 month. Facial nerve function was clinically evaluated according to the House-Brackmann (HB) facial nerve grading system. Results : Facial nerve courses on preoperative tractography were entirely correlated with intraoperative findings in all patients. Facial nerve was located on the anterior of the tumor surface in 5 cases, on anteroinferior in 3 cases, on anterosuperior in 2 cases, and on posteroinferior in 1 case. In postoperative facial nerve tractography, preservation of facial nerve was confirmed in all patients. No patient had severe facial paralysis at postoperative one year. Conclusion : This study shows that DTT for preoperative identification of facial nerve in VS surgery could be a very accurate and useful radiological method and could help to improve facial nerve preservation.

Association of Polymorphisms in the Bovine Leptin Gene with Ultrasound Measurements for Improving in Korean Cattle

  • The identification method that inflects real time ultrasound (RUT) and the potential application of marker assisted selection (MAS) for improvement of a cow population of Hanwoo (Korean Native cattle) was studied. The averages of RUT longissimus muscle area, RUT fat thickness, and RUT marbling score scanned at the 13th rib were 55.78 $cm^2$, 3.70 mm and 3.83 scores, respectively. We investigated the effects of the two SNPs (Kpn2 I and Msp I) in the leptin gene on carcass traits for Hanwoo cows by using ultrasound measurements. Genotype CC of the Kpn2 I had a significantly higher effect on back fat thickness (4.23 mm) and longissimus muscle area (57.57 $cm^2$) than genotype TT (3.14 mm, 53.93 $cm^2$, respectively, p<0.05). Genotype AA of the Msp I had a significantly higher effect only on marbling score (5.37) than genotype AB (3.57, p<0.05) and BB (3.37, p<0.05). Significant effects of SNPs in the leptin gene were found for the ultrasound measures of body composition in live cattle.

A Study on the Distribution of the Elementary Girls' Size Dimensions according to Ages and Body Shapes (학령기 여아 연령별, 체형별 치수분포특성)

  • The purpose of this study was to analyse the body sizes of $7{\sim}12$ years elementary school girls and also to categorize KS size dimensions by the detailed information of ages and body shapes. For the study, the data of SizeKorea(2004) was analysed. Height, bust, waist, hip, the ratio of waist to height and hip to height were significant between age groups, but the ratio of bust to height was not. Therefore, the increase of bust size was resulted in growth of bust circumference, instead of bust volume. In the same height group, over 11 year girls had smaller waist, while over 12 year girls had bigger hip. For Grouping girls by ages and body shapes, the ages were divided into 2 groups, under 10 years old and 11 to 12 years old. The body shapes classified into 3 groups 'Stout-shape', 'Middle-shape', and 'slim-shape', by the ratios of bust to height and hip to height. 'Stout-shape' was significantly big at the almost sizes, but 'Middle-shape' was significantly big at only circumferences, not lengths. In addition, drop(the difference between bust and hip) and lower-drop(the difference between waist and hip) were in inverse proportion to the ratios of bust to height and hip to height. It meant the increases of bust-ratio and hip-ratio of 'Stout-shape' were resulted in overweight, rather than female matureness. The distribution of sizes over 0.5% were grouped for grading system and the subtotal percentiles of each group were calculated for industrial data. The groups which covered more than 10% of consumers were 2 to 6 and the 1 or 2 groups for 'Stout-shape' were also observed, so that children's ready-to wear companies could use them efficiently for their own consumer target.

  • An X-ray diffractometer which has various X-ray optics can give qualitative and quantitative information for a sample using a nondestructive analysis method. A parallel beam optic passes the parallel beam and removes divergent beam generated from an X-ray tube. The parallel beam optic used in the X-ray diffractometer was fabricated by wire cut and grading of stainless steel plates and was evaluated its performance using an X-ray imaging system. The measured parallelization of 6.6 mrad for the fabricated the parallel beam optic was a very close to the expected value of 6 mrad. An X-ray imaging technique for evaluating the parallel beam optics can estimate parallelization for each plate and can be used to other X-ray optics.

  • Natural disasters caused by climate change are increasing globally. There are few studies on the quantitative analysis methods for predicting damages in the island area due to sea level rise. Therefore, it is necessary to study the damage prediction analysis method using the GIS which can quantitatively analyze. In this paper, we analyze the cause and status of sea level rise, quantify the vulnerability index, establish an integrated terrestrial modeling method of the ocean and land, and establish a method of analyzing the damage area and damage scale due to sea level rise using GIS and the method of making the damage prediction figure was studied. In order to extract the other affected areas to sea level rise are apart of the terrain model is generated by one requires a terrain modeling of target areas are offshore and vertical reference system differences in land, found the need for correction by a tidal observations and geoid model there was. Grading of terrain, coastline erosion rate, coastal slope, sea level rise rate, and even average by vulnerable factors due to sea level rise indicates that quantitative damage prediction is possible due to sea level rise in the island area. In the case of vulnerable areas extracted by GIS, residential areas and living areas are concentrated on the coastal area due to the nature of the book area, and field survey shows that coastal changes and erosion are caused by sea level rise or tsunami.

  • Quality evaluation of red ginsengs is determined by outer shape and inner qualities. Especially, the inner qualities are main grading criteria. Currently, red ginsengs are classified into 3-grades; heaven, earth and good. The best heaven grade must not include inner holes and sponge tissues. This study was conducted to develop a red ginseng sorting system using x-ray image processing technique. Because of lens characteristic, gray values of the central region in the x-ray image are higher and gradually decreased towards the edge regions. This difference of gray values gives trouble in segmentation and detection of inner holes in red ginseng image, so preprocessing technique is necessary. The preprocessing was done by subtracting source image from an empty background image. But, simple subtraction was not quite appropriate because of too small contrast between inner holes and sound part. Scaled subtraction images were obtained by multiplying all gray values by some numbers. However this method could not help to set threshold value because the gray values of root part are generally lower than body part when red ginseng is exposed to the x-ray. To determine threshold value for detecting inner holes, an algorithm was developed by increasing overall gray values of less clear images.

CT-guided Celiac Plexus Block Using Anterior Approach (전산화 단층촬영 유도하 복강신경총 차단이 암성통증관리에 미치는 영향)

  • Backgroud: We have performed the CT-guided celiac plexus block (CPB) using anterior approach to evaluate the safety and efficacy of the procedure and to determine the role of CT. Methods: CPB were done in 10 patients (5 men and 5 women: mean age, 58.1 years) with intractable upper abdominal pain due to terminal malignancy of the stomach (n=3), pancreas (n=4), gallbladder (n=2), and liver (n=1). To permit an anterior approach, patients lay supine on the CT scan table during the procedure. One 21-guage Chiba needle was placed just anterior to the diaphragmatic crus between the celiac and superior mesenteric arteries and 10~12 ml of dehydrated alcohol was injected. Degree of pain relief following the procedure was assessed and pain was graded on a numeric rating scale (NRS) from 0 to 10. Results: The results suggest a direct relation between the degree of celiac invasion and the response to the CPB. With CT guidance, it is possible for us to direct the needle into more accurate region, allowing alcohol to be deposited in specific ganglion area. Conclusions: CT-guided CPB using an anterior approach was an easy and effective way of reducing intractable upper abdominal pain due to terminal malignancies. CT-guidance allowed precise needle placement and safe procedure. Careful classification of cases is important to predict the degree of pain relief using the grading system based on the degree of involvement of the celiac plexus.

Effects of Decompression Therapy for 6 Cases with Lumbar Herniated Disc (감압치료가 요추간판탈출 6(증)례에 미치는 영향)

  • The Purpose of this study was to report magnetic resonance imaging(MRI) changes of six cases of Lumbar herniated disc which was treated with spinal decompression therapy, oriental medical therapy. 'Disc heights' were measured on sagittal view of $T_2$-weighted MRI. Size of the herniated disc was measured by MRI and 'disc herniations index'. The grading system and algorithm for 'disc degeneration' were based on MRI signal intensity, disc structure, distinction between nucleus and anulus, and disc height. Data for disc height, disc herniations index and disc degeneration grade were collected before and after the treatment together with calculation from the MRI. Case studies were showed that decompression therapy for the herniated disc has an positive effect on disc herniations index, disc regeneration, not disc heights. The clinical evidence for the use of decompression in herniated disc remains inconclusive because of limited researches. Further trials, which give attention to these areas, are needed before any firm conclusions may be made.

Application of portable digital radiography for dental investigations of ancient Egyptian mummies during archaeological excavations: Evaluation and discussion of the advantages and limitations of different approaches and projections

  • Purpose: In the age of X-ray computed tomography (CT) and digital volume tomography (DVT), with their outstanding post-processing capabilities, indications for planar radiography for the study of the dentition of ancient Egyptian mummies may easily be overlooked. In this article, the advantages and limitations of different approaches and projections are discussed for planar oral and maxillofacial radiography using portable digital X-ray equipment during archaeological excavations. Furthermore, recommendations are provided regarding projections and sample positioning in this context. Materials and Methods: A total of 55 specimens, including 19 skeletonized mandibles, 14 skeletonized skulls, 18 separate mummified heads, and 4 partially preserved mummies were imaged using portable digital X-ray equipment in the course of archaeological excavations led by the University of Basel in the Valley of the Kings between 2009 and 2012. Images were evaluated by 2 authors with regard to the visibility of diagnostically relevant dental structures using a 4-point grading system(Likert scale). Results: Overall, the visibility of diagnostically relevant dental structures was rated highest by both authors on X-ray images acquired using a dental detector. The tube-shift technique in the lateral projections of mandibular dentition achieved the second-best rating, and lateral projections achieved the third-best rating. Conclusion: Conventional planar digital X-ray imaging, due to its ubiquity, remains an excellent method-and often the only practicable one-for examining the skulls and teeth of ancient Egyptian mummies under field conditions. Radiographic images of excellent diagnostic quality can be obtained, if an appropriate methodology regarding the selected projections and sample placement is followed.

Parallel-excision infrabrow blepharoplasty with extensive excision of the orbicularis oculi muscle in an Asian population

  • Background Infrabrow blepharoplasty has become a common surgical method used to rejuvenate aged upper eyelids in Asians. In this paper, we describe the parallel excision method for infrabrow blepharoplasty as a useful alternative to the conventional elliptical excision method. The authors' experience over a 3-year period is presented and reviewed. Methods A retrospective review of parallel excision infrabrow blepharoplasty cases at our hospital between 2014 and 2017 was performed. Three oculoplastic surgeons compared preoperative and postoperative photographs using the Strasser grading system. Results From the medical records of 123 patients, a total of 93 patients with moderate-to-severe bilateral dermatochalasis were selected as subjects. The exclusion criterion was levator function less than 8 mm. The total mean follow-up period was 2 years (range, 0.5-3.5 years). The mean skin excision height and width were 9.75 mm (range, 5-16 mm) and 58.51 mm (range, 42-75 mm), respectively. All patients who underwent surgery recovered without major complications, and all patients had high levels of satisfaction and improvements in their visual field. In the Strasser evaluation performed by the oculoplastic surgeons, most patients were found to have excellent results. Conclusions The parallel excision method for infrabrow blepharoplasty is a safe and effective technique that yields more natural- and youthful-looking eyelids than the conventional elliptical excision method. In our method, more effective manipulation of the orbicularis oculi muscle led to a reduction in frontalis compensation, resolution of sunken eyelids, and correction of lateral hooding.
