TGF-β-activated Kinase-1: A Potential Prognostic Marker for Clear Cell Renal Cell Carcinoma

  • Background: TGF-${\beta}$-activated kinase-1 (TAK1) has been found to be over-expressed in a variety of solid malignancies and related to tumor growth. The aim of this study was to evaluate the expression level of TAK1 in clear cell renal cell carcinoma (ccRCC) and assess its value as a novel prognostic marker. Methods: TAK1 mRNA was assessed in 51 paired ccRCC tissues and adjacent normal tissues (ADTs) by real-time PCR. Tissue TAK1 protein was also assessed in 91 ADTs and 177 samples of ccRCC immunohistochemically for evaluation of relationships with clinical characteristics. Results: RT-PCR showed that TAK1 RNA level was significantly higher in ccRCC tissues than in the paired ADTs and immunohistochemistry confirmed higher expression of TAK1 protein in ccRCC samples compared with ADTs. TAK1 protein expression in 177 ccRCC samples was significantly correlated with T stage, N classification, metastasis, recurrence and Fuhrman grade, but not age and gender. Patients with low TAK1 levels had a better survival outcome. TAK1 expression and N stage were independent prognosis factors for the overall survival of ccRCC patients. Conclusions: Overexpression of TAK1 predicts a poor prognosis in patients with ccRCC, so that TAK1 may serve as a novel prognostic marker.

THE EFFECT OF HYPERBARIC OXYGEN THERAPY IN TREATMENT OF DIABETIC FOOT ULCER (당뇨병성 족부 병변에 있어 고압 산소 치료의 효과)

  • Purpose: The purpose of this study was to evaluate the effect of hyperbaric oxygen therapy(HBOT) and the resultant amputation rate in the treatment of diabetic foot ulcer. Material and methods: From 2000 Jan. to 2002 April, thirty two diabetic foot patients were admitted for treatment of foot ulcers and infection. Of the thirty two cases, thirteen patients received HBOT and nineteen did not. The results were analyzed by amputation rate and healing time. All were classified according to the Wagner classification. Results: Of the HBOT treated group, three(23%) patients underwent amputation: two below knee and one metatarsophalangeal disarticulations. Of the non-treated group, eleven(58%) patients underwent amputation, : four below knee and seven metatarsophalangeal disarticulations. The healing times, based on hospital days were average 16, 38, 43 days in the HBOT treated group and average 20, 50, 35 days in the non treated group respectively in Wagner grade II, III, IV. Conclusions: HBOT might be effective in decreasing amputation rate and hospital stay in diabetic patients with severe foot ulcers.

  • The purpose of the study is to identify the effect of length and right angle indication on the understanding of the concept of the figure when presenting the task of classifying the plane figures. we recorded thirty three 4th grade students' performance with eye-tracking technologies and analyzed the correct answer rate and gaze duration. The findings from the study were as follows. First, correctness rate increased and Gaze duration decreased by marking length in isosceles triangle and equilateral triangle. Second, correctness rate increased and Gaze duration decreased by marking right angle in acute angle triangle and obtuse triangle. Based on these results, it is necessary to focus on measuring the understanding of the concept of the figure rather than measuring the students' ability to measure by expressing the length and angle when presenting the task of classifying the plane figures.

Descriptive Epidemiology of Thyroid Cancers in Togo

  • Background: The purpose of this study was to provide epidemiological and histological data of thyroid cancers in Togo. Materials and Methods: This was a retrospective cross-sectional study of cases of thyroid cancers diagnosed from 2000 to 2014 (15 years) at the pathology laboratory of the Sylvanus Olympio Teaching Hospital of $Lom{\acute{e}}$. All cases of review of a thyroid sample (biopsies, surgical specimens) were collected from the data records of that laboratory. Results: Thyroid cancers represented 1.1% (7930cases) of all cancers registered during the study period. Mean age was $45.4{\pm}0.3$ years and the proportion of females was 78.3%. We identified 92.4% carcinomas and 7.6% lymphomas. Carcinomas were well differentiated in 80 cases and were dominated by the papillary type (47 cases). Metastasis was observed in 13% of patients. The pTNM classification evaluated in 18 cases showed a predominance of grade I (13 cases). Lymphomas were dominated by lymphoma diffuse large B-cell (5 cases). Conclusions: This study is the first global standard for thyroid cancer pathology in Togo. The high frequency of follicular form suggests an unrecognized iodine deficiency. The improvement of the technical platform of the LAP (immunohistochemistry) will increase the diagnosis of rare forms of thyroid cancer.

Deep Learning Based Tree Recognition rate improving Method for Elementary and Middle School Learning

  • The goal of this study is to propose an efficient model for recognizing and classifying tree images to measure the accuracy that can be applied to smart devices during class. From the 2009 revised textbook to the 2015 revised textbook, the learning objective to the fourth-grade science textbook of elementary schools was added to the plant recognition utilizing smart devices. In this study, we compared the recognition rates of trees before and after retraining using a pre-trained inception V3 model, which is the support of the Google Inception V3. In terms of tree recognition, it can distinguish several features, including shapes, bark, leaves, flowers, and fruits that may lead to the recognition rate. Furthermore, if all the leaves of trees may fall during winter, it may challenge to identify the type of tree, as only the bark of the tree will remain some leaves. Therefore, the effective tree classification model is presented through the combination of the images by tree type and the method of combining the model for the accuracy of each tree type. I hope that this model will apply to smart devices used in educational settings.

Study on The Contact Metamorphism of Weolagsan Granite (월악산화강암(月岳山花崗岩)의 접촉변성(接觸變成)에 관(關)하여)

  • The Weolagsan area consists of four units; (1) Low grade meta-sediments of the upper members of Ogcheon age unknown group such as Changri (mainly black slate and phyllitic rock), Majeonri (mainly alternation of slate, limestone and chert) and Hwanggangri Formation (pebble bearing phyllitic sediments); (2) Samtaesan Formation of Chosun System of Ordovician; (3) So called meta-volcanics and (4) Weolagsan Granite and its associations which intruded above mentioned meta-sediments and meta-volcanics. This study was focused to know the Woelagsan granite and its metasomatic effects to the country rocks petrographically and petrochemically. According to the field survey, microscopic work and some chemical analysis, the granite is a "normal granite" based on the Streckeisen's classification and belongs to a mass of the Central-zone younger group in Ogcheon geosynclinal belt. The granite metasomatized the country rocks along its northern contact zone. Zone of calcareous and cherty rocks (Majeonri formation) was silicified partly and skarned locally at the contact with the granite. The chemical analysis of the zone show no difinite variations in contents of $SiO_2$ and CaO with the distance from the granite. It seems to be indicated that the silicification of this part was not so metasomatized by the granite body, but thermally affected as much as to be partially remelted in the specific parts of the formations. Meta-volcanic rock zone was slightly chloritized near contact with the granite. Limestone of Samtaesan Formation was silicified and skarned along the contact zone by the granite body. The chemical analysis of the zone show some noticiable changes in compositions of $SiO_2$ and CaO with distance from the granite boundary. It can be imagined that the silicification of this zone was metasomatically originated by Woelagsan Granite. According to chemical analysis on several trace elements, the ratio of Zn/Cr and Ni/Cr are relatively higher than that of Cu/Cr in the above mentioned silicified zones. Generally the variation of these metal elements in the zones tend to be regular with distance from the granite body.

A Study on Students' Questioning Activity in Science Classes (II) - Analysis of the Patterns of Students' Questions - (과학 수업에서의 학생 질문에 대한 연구(II) - 학생 질문의 유형별 분석 -)

  • This study was conducted with two science classes of the 8th grade students in Seoul during 4 weeks. The numbers of students in the classes were 37 and 38, and they were taught for 12 class hours. Questions obtained for 12 class hours from 75 students were analyzed and grouped into patterns. All together 1.108 questions from the students were classified into six categories: 'No Connection' (7%). 'Contradiction' (3%), 'Recall' (23%), 'Reframe' (40%), 'Application' (18%), and 'Extension' (9%). Irrelevant questions to learning and questions of false proposition were classified into 'No Connection' and 'Contradiction', respectively. Questions repeating what were already explained were grouped into 'Recall'. Those requiring other examples and/or additional explanations were grouped into 'Reframe'. Those requiring practical applications and/or explanations for other concrete facts were grouped into 'Application'. Finally. Questions for higher and/or other concepts were grouped into 'Extension'. We also discussed educational implications of the above categorized questions in this study.

Association of Immunohistochemically Defined Molecular Subtypes with Clinical Response to Presurgical Chemotherapy in Patients with Advanced Breast Cancer

  • Gene expression profiling (GEP) has identified several molecular subtypes of breast cancer, with different clinico-pathologic features and exhibiting different responses to chemotherapy. However, GEP is expensive and not available in the developing countries where the majority of patients present at advanced stage. The St Gallen Consensus in 2011 proposed use of a simplified, four immunohistochemical (IHC) biomarker panel (ER, PR, HER2, Ki67/Tumor Grade) for molecular classification. The present study was conducted in 75 newly diagnosed patients of breast cancer with large (>5cm) tumors to evaluate the association of IHC surrogate molecular subtype with the clinical response to presurgical chemotherapy, evaluated by the WHO criteria, 3 weeks after the third cycle of 5 flourouracil, adriamycin, cyclophosphamide (FAC regimen). The subtypes of luminal, basal-like and HER2 enriched were found to account for 36.0 % (27/75), 34.7 % (26/75) and 29.3% (22/75) of patients respectively. Ten were luminal A and 14 luminal B (8 HER2 negative and 6HER2 positive). The triple negative breast cancer (TNBC) was most sensitive to chemotherapy with 19% achieving clinical-complete-response (cCR) followed by HER2 enriched (2/22 (9%) cCR), luminal B (1/6 (7%) cCR) and luminal A (0/10 (0%) cCR). Heterogeneity was observed within each subgroup, being most marked in the TNBC although the most responding tumors, 8% developing clinical-progressive-disease. The study supports association of molecular subtypes with response to chemotherapy in patients with advanced breast cancer and the existence of further heterogeneity within subtypes.

  • This study examined an instruction method for the improvement of information processing abilities in elementary school students. Current elementary students are required to develop information processing abilities to create new knowledge from this information flooded age; however, there is a shortage of instruction strategies for these information processing abilities. This research proposes a method for information processing ability based Barrow's problem-based learning model, and was applied to real elementary students. Students developed an improved ability to create new knowledge and to present relationships with information through the process of problem solving. This study performed experimental research by comparing pre- and post-tests for twenty-three fifth grade elementary students over the course of eight months. This study produced a remarkable improvement in information selection, information reliability, information classification, information analysis, information comparison, and information internalization.

Surgical Results of Growth Hormone-Secreting Pituitary Adenoma

  • Objective: We retrospectively analyzed the surgical outcomes of 42 patients with growth hormone (GH)-secreting pituitary adenoma to evaluate the clinical manifestations and to determine which preoperative factors that significantly influence the remission. Methods: Forty-two patients with GH-secreting pituitary adenoma underwent transsphenoidal surgery (TSS) between 1995 and 2007. The patient group included 23 women and 19 men, with a mean age of 40.2 (range 13-61) years, and a mean follow-up duration of 49.4 (range 3-178) months after the operation. For comparable radiological criteria, we classified parasellar growth into five grades according to the Knosp classification. We analyzed the surgical results of the patients according to the most recent stringent criteria for cure. Results: The overall rate of endocrinological remission in the group of 42 patients after primary TSS was 64% (26 of 42). The remission rate was 67% (8 of 12) for microadenoma and 60% (18 of 30) for macroadenoma. The remission rate was 30%(3 of 10) for the group with cavernous sinus invasion and 72% (23 of 32) for the group with intact cavernous sinus. Cavernous sinus invasion in Knosp grade III and IV was significantly correlated with the remission rate. There was a significant relationship between preoperative mean GH concentration and early postoperative outcome, with most patients in remission having a lower preoperative GH concentration. Conclusion: TSS is thought to be an effective primary treatment for GH-secreting pituitary adenomas according to the most recent criteria of cure. Because the remission rate in cases with cavernous sinus invasion is very low, early detection of the tumor before it extends into the cavernous sinus and a long-term endocrinological and radiological follow-up are necessary in order to improve the remission rate of acromegaly.
