Between July, 1990 and July, 1992, 6 male patients of truncus arteriosus, whose age ranged from 2 months to 18 months, underwent total surgical correction without a extracardiac conduit. Their anatomic types were type I in 3, type II in 2 and III, in one by the Collett-Edwards classification. Surgical techniques were similar to the first description by Lecompte except for the fact that distal pulmonary arterial stumps were approximated to ventriculotomy site without Lecompte maneuver in all cases. Also in all cases, mon-ocusps were placed using glutaraldehyde fixed autologous pericardial patch directly in right ventricular outflow tract. Three patients died postoperatively and the causes of death were myocardial failure, pulmonary hypertensive crisis and pulmonary complication due to progressive pulmonary vascualr obstructive disease respectively. The three survivors have been followed up for 6~10 months with good functional results.
37 years old female was admitted with chief complaints of dyspnea on exertion and hemoptysis. Past history and family history were non-contributory. Physical examination showed Grade III systolic murmur at the apex, which transmitted to the back. E. K. G. and X-ray findings were compatible with the mitral insufficiency. With small size of Beall mitral valve, mitral valve replacement was done under the cardia-pulmonary bypass using hemodilution technic. Patient was tracheotomized after operation and assisted respiration was done for four weeks. Postoperatively, all signs were fine and patient walked around the ward without any difficulty, but she was in psychotic state. On postoperative 60th day, she complained of sudden dyspnea and on chest film, tracheal stenosis was found and recannulation of the tracheal tube was made. Thereafter, she was quite fine until postoperative 110th day when she, by berself, removed the tracheal cannula and died of asphyxia. Autopsy findings of the valve showed no thrombosis, no variance of the valve, and good endothelization of the valve cuffs. Asphyxia, due to removal of the tracheal connula by herself under psychotic state, was considered to be the cause of death in this patient who had tracheal stenosis after tracheostomy.
Purpose: This study was explored the meaning of hospice volunteer's experience in caring for cancer patients and tried to understand their experiences. Method: The data was collected from 7 participants living in Seoul and Gyeonggi province from Jan. to Apr. 2005. Collection of data was by means of in-depth interviews. The analysis of the data was made the phenomenological analytic method suggested by Colaizzi(1978). Result: The result is consisted of nine theme-cluster; experienced a fear, limitation of activity, experienced social care, experienced physical care, good death, experienced necessity of recharging, experienced bereavement care. Conclusion: The result above indicated that health professionals must develop the management and education of volunteer of hospice care for various hospice care. Also, We should to encouraged the continuous education and efficient management. And support system should be developed.
The congenital cystic adenomatoid malformation [C.C.A.M.] of the lung is a rare lesion consisted of multiple cysts in the pulmonary parenchyma lined cuboidal or columnar epithelium. The C.C.A.M. presents clinically in three ways:[1] stillborn or perinatal death, [2] progressive respiratory distress in the newborn, and [3] acute and chronic pulmonary nfections in the older infant and child. We recently experienced two cases of C.C.A.M. of the lung, and have good surgical results. The first case was 29 days old male and the second case was 16 month old female who have been suffered from severe respiratory difficulty and pulmonary infection repeatedly each other. The lobectomies were performed, and the postoperative course was uneventful.
In the past several years the popularity of the motor cycle has produced an increasing incidence of the injuries to the larynx and trachea. Most of all on accidents come to death and survivors to the hospital are rare. Early diagnosis and to keep air way are necessary to initiate proper treatment in injury of upper air way. Meticulous apposition of mucous membrane and reconstitution of laryngeal skeleton are important. We experienced a rare case of 26 year old men with cricothyroidal transection after trauma. On Oct. 17, 1985, the patient struck his neck on baggage frame of truck when dropping from his motor cycle on sudden stop. Emergency tracheal intubation on distal segment of trachea was accomplished by otolaryngologist in a local clinic. He was transferred to our hospital. Exploration 2 hours later revealed complete separation of cricoid cartilage from thyroid cartilage. The recurrent laryngeal nerve could not be identified. Anastomosis of thyroid and cricoid was accomplished and Portex endotracheal tube was inserted as splint for 10 days. No stenosis developed. The air way appeared adequate for moderate physical activity though paramedian fixation of vocal cord paralysis. Postoperative follow-up course has been good after he discharged on POD 14 days.
Patient with aortic valvular disease have increased left ventricular work and greater myocardial oxygen demand, which may aggravate the effect of concomitant coronary artery disease. Thus in patient who repair aortic valve replacement, concomitant aortocoronary bypass surgery is often performed when angiographically significant coronary artery disease is present. This approach is supported by reports that revascularization does not increase operative risk when associated coronary artery disease is present and significantly reduce the occurrence of late sudden death. Recently we have experienced one case of aortic valve replacement concomitant with aorta-coronary bypass surgery. The patient was 56 year-old male and admitted with complaint of anterior chest pain especially during his exercise. He was diagnosed as aortic valve stenosis and regurgitation [GIII] with proximal right main coronary artery occlusion We performed aortic valve replacement with aorta coronary bypass surgery by use of saphenous vein. Post operative course was uneventful and chest pain was relieved. Post operative coronary angiogram disclosed good patency of grafted vessel.
A bronchopleural ~stula[BPF] is a major therapeutic challange to the thoracic surgeons due to difficulties in its management. We have experienced 9 cases of postpneumonectomy BPF, and all of them occured after the right side pneumonectomy. Seven patients underwent pneumonectomy due to lung cancer, and one patient each received pneumonectomy due to bronchiectasis and pulmonary tuberculosis. The most frequently used method of treatment was Eloesser operation which was done in 7 patients, and one had open thoracostomy and one had betadine irrigation until he died suddenly due to pulmonary vein rupture. The result of treatment was poor with five mortalities including one operative death. Two patients are in good condition in 28 months and 4 years of follow-up, but two are lost in follow-up. Tissue sealant such as Tisseel was used in three patients but seems to have no effect at all in the treatment of postoperative BPF.

Idiopathic pulmonary arterial hypertension eventually leads to right-sided heart failure and sudden death. Its mortality rate in children is still high, despite improvements in pharmacological therapy, and therefore novel treatments are necessary. The Potts shunt, which creates an anastomosis between the left pulmonary artery and the descending aorta, has been proposed as a theoretically promising palliative surgical technique to decompress the right ventricle. We report the case of a 12-year-old girl with suprasystemic idiopathic pulmonary hypertension and right ventricular failure who underwent a Potts shunt for palliation with good short-term results.

Ovarian cancer is one of the leading causes of cancer-related deaths in gynecological malignancies. Over 70% of ovarian cancer cases are high-grade serous ovarian cancers and have high death rates due to their resistance to chemotherapy. Despite advances in surgical and pharmaceutical therapies, overall survival rates are not good, and making an accurate prediction of the prognosis is not easy because of the highly heterogeneous nature of ovarian cancer. To improve the patient's prognosis through proper treatment, we present a prognostic prediction model by integrating high-dimensional RNA sequencing data with their clinical data through the following steps: gene filtration, pre-screening, gene marker selection, integrated study of selected gene markers and prediction model building. These steps of the prognostic prediction model can be applied to other types of cancer besides ovarian cancer.
