막걸리 박 열수추출물의 혈당지수 및 제2형 당뇨 모델 동물에서 경구혈당내성에 미치는 영향에 관한 연구 (Glycemic Index and Oral Glucose Tolerance Test of Takju (Korean turbid rice wine) Lees Extract)

  • Takju lees extract is expected to be a promising material for functional food. This study was conducted in order to assess the glycemic index (GI) of Takju lees extract and its effects in an oral glucose tolerance test (OGTT). The GI for Takju lees extract was evaluated with 10 healthy young adults (Male 5, Female 5). OGTT was conducted in 20 male db/db mice, fed on a diet containing 2% Takju lees extract for 4 weeks. Those animals were subjected to OGTT after one oral administration of Takju lees extract at 2 g/kg BW. The GI of the Takju lees extract was measured at 97.97. The effects of the Takju lees extract on the oral glucose tolerance test in db/db mice evidenced no differences as compared to the control group. In conclusion, Takju lees extract is a high GI material, and it has no effect on blood glucose levels in a type II diabetic animal model. Further studies will be required to confirm its anti-diabetic effects.

당뇨병 환자의 치료순응도에 따른 단기간 혈당조절정도와 관련 요인 (Short-term Glycemic Control and the Related Factors in Association with Compliance in Diabetic Patients)

  • Objectives : Generally, it seemed that the therapeutic result in diabetic patients was changed by compliance. This study was conducted on the basis of assumption that the therapeutic result id diabetic patients could control according to compliance. This study was conducted to analyze the related factors in association with compliance to drug, diet and exercise therapy. Methods : 224 diabetic patients in Kyungpook National University Hospital were selected through the interviews and HbA1c values from 1 Jan. to 28 Feb.1997. The drug compliance was tested by regularity of drug administration, the diet compliance was tested by restriction of food, exactly allocation, balance of nutrient, measuring food and the exercise compliance was tested by regularity of exercise per day. We assessed compliance by percentage, $x^2-test$ and generalized logit regression model(method:enter). Results : The significant variable was the satisfaction to medical personnels in drug, the knowledge to disease in diet, the participation of the diabetic education in exercise therapy and the satisfaction to medical personnels in HbA1c. Using the generalized logit model(method : enter) in compliance change, the significant variables were the satisfaction to medical personnels and the complication in drug; the significant variables were the age at the first diagnosis, the family history, the concern of health, the knowledge of disease, the self-exertion for therapy and the complication in diet: the only significant variable was the gender in exercise therapy. Conclusions : The degree of glycemic control in diabetic patients was influenced by compliance. In order to improve patient's compliance, we must foster the knowledge on the diseases, lead participation for diabetic education. Because the satisfaction to medical personnels was the important variables, we must build up good relationship between doctors and patients.

인슐린 비의존형 당뇨병 환자에서 영양음료 섭취가 혈청 당질에 미치는 영향 (Effect of Enteral Nutrition Supplementation on Glucose Metabolism in patients with NIDDM)

  • This study investigated the effect of enteral nutrition supplementation on glucose and lipid metabolism in non-insulin dependent diabetes mellitus(NIDDM) patients(n=29). Nutrition formula(400kcal/day) were supplied daily for eight weeks as a substitute for a snack or a meal. Subjects were divided into three groups based on changes of fasting blood glucose(FBG), glucose response area(GRA) on oral glucose tolerance test(OGTT), before and after intake of nutrition formula : group 1(the group of a decrease in FBG and GRA, n=20), group 2(the group of a decrease in FBG and an increase in GRA, n=4), and group 3(the group of an increase in FBF and GRA, n=5). Before nutrition supplementation, group 3 showed a longer tendency of DM duration and a lower tendency of insulin and C-peptide response are than those of group 1 and 2. At 8 weeks after nutrition supplementation, group 1 showed a significant increase in insulin and C-peptide response areas but group 2 and 3 showed no change in those areas. After nutrition supplementation, all three groups showed a tendency of decrease in glycated hemoglobin and no significant changes in the levels of serum triglycerides, HDL, LDL, total cholesterol, albumin, transferrin, creatinine, GOT and GPT. The results suggest that using an enteral nutrition formula in NIDDM patients is a good substitute for a meal or snack and could improve blood glucose control without any changes in lipid levels, and liver and kidney functions. The beneficial effect of nutrition supplementation on glycemic control resulted from components of nutrition formula had such as additional fiber and high monounsaturated fatty acid as the source of fat to be helpful 세 glycemic control in diabetics.

  • 당뇨병 유병율이 증가함에 따라 당뇨병은 사회경제적 및 보건학적 이슈이다. 당뇨병 조절은 질병의 치료라기보다 평생에 걸쳐 관리해야 하는 것으로 자기간호가 수행되어야 한다. 본 연구는 성인 당뇨병을 혈당 조절군과 비조절군으로 구분하여 당뇨병 유병 위험 요인의 차이에 대해 입증하고자 한다. 나아가 당뇨병 환자들이 지각하는 감정요인과 당뇨병 유병과의 관련성을 밝히고자 한다. 이를 위해 5년치 국민건강영양조사 자료의 당뇨병 유병 성인들의 혈당조절 수준 영향요인을 분석하였다. 결과, 당뇨병 유병자인 혈당비조절군과 혈당조절군에 대한 유병률의 요인에 대해 입증하였으며, 특히 우울증이나 스트레스와 같은 부정감정이 당뇨병 유병 및 혈당조절에 유의한 연관이 있음을 확인하였다. 따라서 당뇨병에 대한 진단은 우울증과 스트레스 같은 부정감정을 고려하여 병행치료가 필요하다고 여겨진다.

The art of diabetes care: guidelines for a holistic approach to human and social factors

  • A holistic approach to diabetes considers patient preferences, emotional health, living conditions, and other contextual factors, in addition to medication selection. Human and social factors influence treatment adherence and clinical outcomes. Social issues, cost of care, out-of-pocket expenses, pill burden (number and frequency), and injectable drugs such as insulin, can affect adherence. Clinicians can ask about these contextual factors when discussing treatment options with patients. Patients' emotional health can also affect diabetes self-care. Social stressors such as family issues may impair self-care behaviors. Diabetes can also lead to emotional stress. Diabetes distress correlates with worse glycemic control and lower overall well-being. Patient-centered communication can build the foundation of a trusting relationship with the clinician. Respect for patient preferences and fears can build trust. Relevant communication skills include asking open-ended questions, expressing empathy, active listening, and exploring the patient's perspective. Glycemic goals must be personalized based on frailty, the risk of hypoglycemia, and healthy life expectancy. Lifestyle counseling requires a nonjudgmental approach and tactfulness. The art of diabetes care rests on clinicians perceiving a patient's emotional state. Tailoring the level of advice and diabetes targets based on a patient's personal and contextual factors requires mindfulness by clinicians.

반응표면분석을 이용해 개발한 황금과 대두콩/서리태 분말 혼합 쿠키가 정상인의 혈당반응에 미치는 영향 (Effect of cookies made with soybean/seoritae and Hwanggum using response surface methodology on the blood glucose response in healthy adults)

  • 본 연구에서는 반응표면분석을 이용하여 개발한 대두/서리태 분말, 황금, 아이소말토 올리고당 첨가 쿠키가 정상인들의 혈당반응에 미치는 영향을 살펴보고자 하였다. 반응표면분석의 중심합성 계획에 따라 독립변수 대두/서리태 분말, 황금, 아이소말토 올리고당 3가지에 대한 반응변수로는 전반적인 기호도로 설정하였다. 반응모형의 결정계수는 대두 쿠키가 0.923, 서리태 쿠키가 0.927로 1에 근접하였으며, 전체적인 모형이 0.05보다 낮아 적합한 반응모형으로 나타났다. 대두쿠키는 안장점을 나타내고 있어 능선분석을 실시한 결과 최고점을 만족하는 각 변수의 조건은 대두분말 68.7%, 황금 분말 2.5%, 이소말토 올리고당 75.2%로 결정되었다. 서리태 분말 쿠키 또한 능선분석 실시 결과 서리태 분말 56.5%, 황금 분말 3.8%, 이소말토 올리고당 56.2%에서 최대 값을 보여주었다. 최적배합비에 따라 제조한 대두/서리태 쿠키의 조직감 측정 결과 단단한 정도와 바삭거리는 정도는 유의적으로 낮아졌고, 전반적인 기호도는 대조군보다 대두/서리태 쿠키의 전반적인 기호도가 유의적으로 향상되었다. 그리고 최적배합비로 개발된 쿠키가 정상인의 혈당반응에 미치는 영향은 대조군보다 대두쿠키 섭취군에서 혈당반응면적과 당지수가 유의적으로 낮아졌으며, 당부하지수 계산 결과 대두쿠키 19.9, 서리태쿠키 21.9로 나타났다. 당부하지수 20이하는 저당지수 식품에 해당이 되므로 대두쿠키가 저당지수 식품에 포함되는 것을 확인할 수 있었으며, 본 연구를 통해 혈당상승을 지연시킬 수 있는 쿠키 개발 가능성을 제안할 수 있었다.

우리나라 다빈도 섭취 과일의 당 함량 및 혈당지수에 관한 연구 (Sugar composition and glycemic indices of frequently consumed fruits in Korea)

  • Fruits are generally recommended for a balanced meal, as they are good sources of vitamins, minerals, and fiber, which may improve blood glucose control. However, fruits have simple sugars with a wide glycemic index (GI) range. The purpose of this study was to analyze the sugar content and composition and to determine the glycemic indices of the most frequently consumed fruits in Korea, including apple, tangerine, pear, water melon, persimmon, grape, oriental melon, and peach. The sugar content and composition of the fruits were analyzed by high performance anion-exchange chromatography (Dinonex model DX-600). The GI of the fruits was measured in 13 healthy subjects (seven females and six males) after permission was received from the University Hospital institutional review board (KHU-IRB 1114-06). The subjects consumed 50 g of glucose as a reference and carbohydrate portions of eight fruits. Blood samples were collected at 0, 30, 60, 90, and 120 min after consuming the fruits. The GI values for the fruits were calculated by expressing the increase in the area under the blood glucose response curve for each subject. As a result, the total sugar contents of 100 g fruits were: grape (13.9 g), apple (12.3 g), persimmon (11.9 g), oriental melon (11.2 g), watermelon (9.3 g), tangerine (8.9 g), peach (8.6 g), and pear (8.3 g). The GI values of the fruits were as follows: GI value of peach ($56.5{\pm}14.17$), watermelon ($53.5{\pm}18.07$), oriental melon ($51.2{\pm}18.14$), tangerine ($50.4{\pm}15.16$), grape ($48.1{\pm}14.05$), persimmon ($42.9{\pm}18.92$), pear ($35.7{\pm}14.38$), and apple ($33.5{\pm}11.92$). These findings will help individuals choose fruit for controlling blood sugar.

Effect of diabetes-specific oral nutritional supplements with allulose on weight and glycemic profiles in overweight or obese type 2 diabetic patients

  • BACKGROUND/OBJECTIVES: Diabetes-specific oral nutritional supplements (ONS) have anti-hyperglycemic effects, while D-allulose exerts anti-diabetic and anti-obesity effects. In this study, we investigated the efficacy and safety of diabetes-specific ONS, including allulose, on glycemic and weight changes in overweight or obese patients with type 2 diabetes mellitus (T2DM). SUBJECTS/METHODS: A single-arm, historical-control pilot clinical trial was conducted on 26 overweight or obese patients with T2DM (age range: 30-70 yrs). The participants were administered 2 packs of diabetes-specific ONS, including allulose (200 kcal/200 mL), every morning for 8 weeks. The glycemic profiles, obesity-related parameters, and lipid profiles were assessed to evaluate the efficacy of ONS. RESULTS: After 8 weeks, fasting blood glucose (FBG) level significantly decreased from 139.00 ± 29.66 mg/dL to 126.08 ± 32.00 mg/dL (P = 0.007) and glycosylated hemoglobin (HbA1c) improved (7.23 ± 0.82% vs. 7.03 ± 0.69%, P = 0.041). Moreover, the fasting insulin (δ: -1.81 ± 3.61 μU/mL, P = 0.017) and homeostasis model assessment for insulin resistance (HOMA-IR) (δ: -0.87 ± 1.57, P = 0.009) levels decreased at 8 weeks, and body weight significantly decreased from 67.20 ± 8.29 kg to 66.43 ± 8.12 kg (P = 0.008). Body mass index (BMI) also decreased in accordance with this (from 25.59 ± 1.82 kg/m2 to 25.30 ± 1.86 kg/m2, P = 0.009), as did waist circumference (δ: -1.31 ± 2.04 cm, P = 0.003). CONCLUSIONS: The consumption of diabetes-specific ONS with allulose in overweight or obese patients with T2DM improved glycemic profiles, such as FBG, HbA1c, and HOMA-IR, and reduced body weight and BMI.

제2형 당뇨 모델 db/db 마우스에서 4개월의 자화수 섭취 후 혈당, 항산화 상태 및 세포 DNA 손상 개선 효과 (Four months of magnetized water supplementation improves glycemic control, antioxidant status, and cellualr DNA damage in db/db mice)

  • 자화수는 가격이 비싸지 않고 쉽게 만들 수 있으며 환경 친화적인 음용수임에도 불구하고 자화수 섭취의 건강상의 유익에 관해서는 과학적인 문헌이 매우 제한적이다. 본 연구는 유전적 소인에 의해 제2형 당뇨병이 발현되는 모델인 db/db 마우스 6주령 14마리를 당뇨군과 자화수군으로 나누어 각 7마리씩 배치하고, 이형접합체 마우스 10마리를 대조군으로 하여 총 3군으로 나누어 16주 동안 자화수를 투여하였다. 혈당 강하 효과를 보기 위해 공복혈당, 인슐린 농도, 내당능 검사, 당화 헤모글로빈을 측정 하였고, DNA 손상 감소 효과, 항산화 상태를 알아보았다. 당뇨군과 자화수군의 혈당치는 대조군에 비해 유의적으로 높았으며, 당뇨군의 혈당은 자화수 섭취 16주까지 높게 지속되었으나 자화수군의 혈당은 섭취 10주 후부터 유의적으로 감소하였다. 혈중 당화헤모글로빈 함량도 마찬가지로 대조군보다 당뇨군에서 증가하였고 자화수군에서는 유의적으로 감소하였으나 경구 당부하 검사 결과, 혈당 곡선하면적 (area under the curve, AUC)은 당뇨군과 자화수 투여군 사이에 유의적인 차이를 보이지 않았다. 당뇨쥐의 DNA 손상 정도는 세 가지 지표 (DNA in tail, tail length, tail moment) 모두 자화수군이 당뇨군에 비해 유의적으로 감소하였으며 이러한 효과는 혈액과 간에서 모두 나타났다. 적혈구 항산화 효소인 SOD, GSH-Px glutathione peroxidase 활성도는 대조군, 당뇨군, 자화수군 간에 차이를 보이지 않았고, 혈장 총항산화능인 TRAP 수준도 자화수 섭취로 인한 차이를 볼 수 없었다. 결론적으로 자화수 투여가 db/db mouse를 사용한 제2형 당뇨의 혈당 강하를 포함한 glycemic control에 유리한 개선효과를 보일 뿐 아니라 혈액과 간의 DNA 손상 감소효과까지 보임을 확인하였다. 그러나 그 기전에 대해서는 확실하게 규명하지 못하였으므로 앞으로 자화 수음용 효과를 뒷받침할 더욱 깊고 다양한 기전 연구가 수행되어야 할 것이다.

중환자의 혈당 조절: 최신 업데이트 (Glucose Control in Intensive Care Unit Patients: Recent Updates)

  • Proper glucose management in hospitalized patients can improve clinical outcomes. In particular, intensive care unit (ICU) patients are known to have significantly higher rates of mortality from changes in blood glucose due to severe comorbidities. Improving glucose control in ICU patients, therefore, can improve mortality and prognosis. Several studies related to the management of blood glucose in the ICU patients have been conducted. Intensive glucose management of surgical ICU patients has been successful. However, studies on medical ICU patients did not demonstrate positive effects of strict glycemic control. There is no independent glucose management goal for neurological ICU patients. However, maintenance of the usual glucose control target of 140-180 mg/dL is recommended for ICU patients. Intravenous insulin infusion is essential for glucose control in ICU patients not consuming a regular diet, and caution should be exercised to prevent hypoglycemia.
