국민건강보험공단 사례관리 사업이 당뇨병 환자의 혈당 조절에 미치는 효과 (Effects of Case Management Program on Glycemic Control among Patients with Diabetes)

  • 본 연구는 국민건강보험공단의 당뇨병 사례관리 프로그램 참여자들을 대상으로 사례관리 프로그램의 중재 효과 및 효과의 지속성을 확인하고자 하였다. 기간은 2007년 5월 1일부터 2007년 6월 30일까지 국민건강보험공단의 당뇨병 사례관리 프로그램 참여자 115명(남성 51명, 여성 64명)을 조사대상으로 하였다. 조사대상은 사례관리 1회 실시군(Group I)과 사례관리 2회 실시군(Group I)으로 구분하여 사례관리 실시 직전, 실시 직후, 6개월 및 12개월 후의 사례관리 프로그램의 중재효과 및 효과의 지속성을 평가하였다. 연구결과 당뇨병 사례관리 프로그램 참여자들의 혈당치는 사례관리 실시 직전에 비해 실시 직후에 유의하게 개선되었으나, 6개월과 12개월 후에는 다시 증가하는 경향을 보였다. 또한 사례관리 1회 실시군(Group I)에 비해 사례관리 2회 실시군(Group II)에서 사례관리 프로그램의 중재 효과가 지속됨을 확인할 수 있었다.

중소형 의료 환경에서 U-헬스케어 서비스 향상을 위한 전문가 시스템 기반 IT/의료 융합 솔루션 (An IT/Medical Converged Solution based on the Expert System for Enhancing U-Healthcare Services in Middle-sized Medical Environment)

  • 최근들어, U-헬스케어는 진료에 있어 인력 및 시간 등 많은 비용을 절감할 수 있는 연구로 주목받고 있다. 그러나, 센싱, 측정 등 헬스케어에 대한 기반 기술들은 충분히 연구된 반면, IT 기술이 의료 기술과 접목된 IT/의료 융합 기술은 아직 초기 단계에 있다. 이에 본 논문에서는 이미 검증된 기술들과 전문가 시스템을 기반으로 개인 병원이나 가정 등의 중소형 의료 환경에 적용 가능한 U-헬스 케어 솔루션으로서의 헬스케어 시스템을 제안한다. 그 동안 IT 분야의 눈부신 발전으로 네트워크 및 데이터베이스 등 타 분야에 융합 가능한 IT 요소 기술들이 충분히 발전해 있고, 헬스케어의 경우 시간보다는 임무 수행이 중요한(Mission-Critical) 환경이므로, 새로운 기술 개발도 중요하지만 이미 검증된 기술들을 활용하는 것이 바람직하다. 제안된 솔루션은 전문가 시스템을 기반으로 체지방, 혈압, 그리고 혈당 등의 측정된 데이터를 이용해 자동화된 의료 서비스를 제공한다. 또한 의사와 간호사, 관계자등에게 편리한 진료 환경을 제공한다. 또한, 필요시 전문 지식이 없는 사람들도 자가 진단이 가능하므로 향 후 다양한 방면에서 의료 서비스 비용 절감이 기대된다. 특히, 각 장비들이 표준화된 블루투스 기술을 통해 통신하므로, 다양한 의료기기로 확장 가능한 솔루션이다. 이로부터, 우리는 전문가 시스템을 통한 비용 절감에 의해 U-헬스케어의 자가 측정 및 진단 서비스가 향상되었다고 말할 수 있다.

Morphological, Molecular, and Biochemical Characterization of Astaxanthin-Producing Green Microalga Haematococcus sp. KORDI03 (Haematococcaceae, Chlorophyta) Isolated from Korea

  • A unicellular red microalga was isolated from environmental freshwater in Korea, and its morphological, molecular, and biochemical properties were characterized. Morphological analysis revealed that the isolate was a unicellular biflagellated green microalga that formed a non-motile, thick-walled palmelloid or red aplanospore. To determine the taxonomical position of the isolate, its 18S rRNA and rbcL genes were sequenced and phylogenetic analysis was performed. We found that the isolate was clustered together with other related Haematococcus strains showing differences in the rbcL gene. Therefore, the isolated microalga was classified into the genus Haematococcus, and finally designated Haematococcus sp. KORDI03. The microalga could be cultivated in various culture media under a broad range of pH and temperature conditions. Compositions of the microalgal cellular components were analyzed, and its protein, carbohydrate, and lipid compositions were estimated to be 21.1 ± 0.2%, 48.8 ± 1.8%, and 22.2 ± 0.9%, respectively. In addition, D-glucose and D-mannose were the dominant monosaccharides in the isolate, and its amino acids were composed mainly of aspartic acid, glutamic acid, alanine, and leucine. Moreover, several polyunsaturated fatty acids accounted for about 80% of the total fatty acids in Haematococcus sp. KORDI03, and the astaxanthin content in the red aplanospores was estimated to be 1.8% of the dry cell weight. To the best of our knowledge, this is the first report of an Haematococcus sp. isolated from Korea, which may be used for bioresource production in the microalgal industry.

혐기성 조건에서 에탄올의 주입에 따른 프로피온산의 저감에 관한 연구 (Effect of Ethanol on the Reduction of Propionate under Anaerobic Condition)

  • 혐기성 소화공정 중 생성되는 주요 중간대사산물인 프로피온산의 분해대사에 대한 연구중 에탄올과의 산화 환원 반응인 coupling 반응으로써 혐기성 소화공정에서 프로피온산 축적을 저감시킬 수 있는 연구를 수행하였다. 따라서, 본 연구는 혐기성 공정에서 프로피온산의 전환에 따른 동력학적 반응과 에탄올과의 상호 반응에 따른 특정기질 선호영향을 모델에 적용하여 살펴보았다. 본 연구는 4단계의 실험으로 수행되었다. 1, 2, 3단계는 각기 다른 기질에 순화된 미생물들을 이용하여 프로피온산 1 g COD/L와 에탄올의 농도를 각각 0, 100, 200, 400과 1,000 mg/L로 주입하여 프로피온산과 에탄올의 혐기성 분해과정을 비교 연구하였으며, 4단계에서는 Glu-MCR과 HPr-MCR의 순화미생물의 혼합비를 조절하여 프로피온산 1 g COD/L를 주입하였을 때의 혐기성 분해를 연구하였다. 본 연구에서는 수정된 경쟁적 모델을 이용하여 특정기질 선호현상을 규명하였고, 에탄올 농도의 증가에 따라 아세트산 형성반응의 $K_{s2}$값의 증가와 메탄화 과정에서의 아세트산 생성 및 분해과정에 해당되는 $K_3$값이 일부 증가하는 결과를 얻을 수 있었다. 또한 순화미생물들에 따라 프로피온산과 에탄올의 분해에 미치는 영향이 다른 결과를 얻을 수 있었다.

갱년기 모델 유도 흰 쥐에서 갈근과 지황 복합물 및 유산소 운동이 대사이상 개선에 미치는 영향 (Effects of a Pueraria lobata-root based combination supplement containing Rehmannia glutinosa and aerobic exercise on improvement of metabolic dysfunctions in ovariectomized rats)

  • 본 연구는 갈근과 지황 복합물과 유산소 운동이 난소절제로 갱년기를 유도한 흰 쥐에서 나타나는 지질 및 당대사 이상에 미치는 영향을 확인하였다. 난소절제 후 8주간 AIN-76A 식이를 급여한 결과 체중증가량, 식이섭취량, 체 지방, 혈장 LDL-C, 공복혈당 증가를 통해 갱년기 동물 모델이 유도되었음을 확인할 수 있었다. 갈근과 지황 복합물과 유산소 운동은 서로 다른 기전으로 에스트로겐 분비 저하로 나타나는 비만 및 대사이상을 정상화하는 유익한 결과를 나타냈다. 지방 조직에서 확인된 관련 기전으로는 갈근과 지황 복합물로 인한 아디포넥틴 유전자 발현 증가 및 렙틴저항 억제, 유산소 운동으로 인한 아디포넥틴 및 인슐린 수용체 유전자 발현 증가, 갈근과 지황 복합물 및 유산소 운동의 병행으로 인한 아디포넥틴 유전자 발현 증가 상승효과가 관찰되었다. 이상의 결과로 갈근과 지황 복합물과 유산소 운동의 병행은 폐경기 에스트로겐 결핍으로 인한 대사이상을 개선하는 효과가 있을 것으로 기대한다. 향후에는 폐경기 유도 동물모델의 개선과 함께 단백질 및 신호전달분자 수준에서 추가적인 기전연구가 필요할 것으로 사료된다.

고지방 식이로 유도된 비만 쥐에서 HPJ 추출물의 항비만 효과 (Anti-Obese Activity of HPJ Extract on High Fat Diet-Induced Obese Mice)

  • In this study, we investigated the anti-obese activity of HPJ extract in C57BL/6J mice. The C57BL/6J mice were randomly divided into five groups: normal control group (Con), high fat diet control group (HFD), treatment groups with HPJ at 125 mg/kg (HPJ125), 250 mg/kg (HPJ250), or 500 mg/kg (HPJ500). To induce an obesity, mice were fed by a high fat diet for 6 weeks, and mice were administered with HPJ extract once a day for 8 weeks. At the end of treatment, we examined the effect of HPJ extract on body weight, plasma lipid, and lipogenic enzymes. HPJ extract was found to lower whole body and epididymal adipose tissue weights and lowered plasma levels of glucose, insulin, triglyceride (TG), total cholesterol (TC), non-esterified fatty acid (NEFA) and leptin, compared to those in HFD group. Histological analyses of the liver and fat tissues of mice treated with HPJ extract revealed significantly decreased number of lipid droplets and decreased size of adipocytes compared to the HFD group. In addition, HPJ extract preserved the morphological integrity of pancreatic islets. To elucidate an action mechanism of HPJ extract, Western blot and RT-PCR were performed using epididymal adipose tissues. HPJ extract up-regulated the levels of phosphorylated adenosine monophosphate-activated protein kinase (AMPK) and its substrate, acetyl-CoA carboxylasse (ACC). HPJ extract also attenuated lipogenic gene expressions of sterol regulatory element-binding protein $1{\alpha}$ (SREBP$1{\alpha}$), fatty acid synthase (FAS), sterol-CoA desaturase 1 (SCD1) and glycerol-3-phosphate acyltransferase (GPAT) in dose-dependent manners. In contrast, expressions of lipolytic genes such as peroxisome proliferator-activated receptor-$\alpha$ (PPAR-${\alpha}$) and CD36, and fatty acid $\beta$-oxidation gene, carnitine palmitoyltransferase-1 (CPT-1) were increased. These results suggest that HPJ extract ameliorates obesity through inhibiting synthesis of lipogenic enzymes as well as stimulating fatty acid oxidation resulting from activation of AMPK, and HPJ extract could be developed as a potential therapeutic agent for obese patients.

  • Effects of antibioties on micro-organism have been reported by many scientists, such as Krampitz and Werkman, Fisher, Gale and Rodwell, Klimick Cavalito and Bailey, Umbreit, etc. On the mechanism by which penicillin act, Fisher(1947), Platt(1947), and Cavallito, considered that penicillin might act on bacteria by inhibiting with the normal function of SH-group of glutathione in the metabolism of the cell. Resenbrance of penicillin to gultathione in structure and the inactivation of penicillin by cysteine make us approve of the above inhibiting theory of SH-group. Galland (1947) and Schmidt (1947) reported that penicillin inhibited the activity of ribonuclease, Phosphatase, and mononucleotidase. Gale (1948) discovered that the gram positive bacteria had lost the power to uptake glutamic acid by ribonucleic acid in the medium contained penicillin: growth of gram positive organism was inhibited by the results that penicillin inhibited the uptake of amino acid byribonucleic acid, acting on ribonucleic acid of gram positive bacteria. Hotchkiss (1950) cultured S. aureus in the medium contained glucose and amino acids, and studied the effect of penicillin on protein synthesis. Peptide formation in living cells was inhibited by penicillin, while amono acid was utilized as before the addition of penicillin. On the otherhand, Binkley (1951) found penicillin interfered hydrolase of glutath one, and Hans (1950) reported penicillin inhibited the transpeptidation. On the machanism by which streptomycin acts. Cohen (1947) reported steptomycin made a irreversible complex with desoxyribonucleic acid, by the fact that desoxyribonucleic acid formed the precipitates with diguanide group of steptomycin. Zeller (1951) reported, on the other hand, streptomycin inhibited diamine oxidease. Geiger (1947) and Umbreit (1949) reported that steptomycin inhibited condensation of oxaloacetate and pyruvate in E. Coli and Oginsky et al (1949) reported steptomycin inhibited oxaloacetate-pyruvate reaction in Kreb's cycle. On the mechanism by which terramycin acts, Hahn & Wisseman (1951) reported that the formation of adaptive enzyme was inhibited by terramycin in E. Coli cultivated in the medium contained loctose, and that the protein synthesis was inhibited by terramycin. However, effects of antibiotics on amino acid metabolism have not been discussed much in spite of its important role in living cells. Especislly, effects of anitibiotics on higher plants have scarcely been reported. Here, to prove the effect of antibiotics on higher plants, and the mechanism by which, through amino acid metabolism, they promote or inhibit growth of plants, amino acids in bean plants treated with penicillin, streptomycin, and terramycin were analyzed by paper chromatography. And to clarify the antagonis of cysteine (as SH-group) against penicillin, through amino acid metabolism, amino acids in bean plants treated with cystene and penicillin, at the same time, were also analyzed.

CGA 제조기법을 응용한 PVA 하이드로젤의 담체 특성 (Media Characteristics of PVA-derivative Hydrogels Using a CGA Technique)

  • We manufactured PVA-derived hydrogels using a foam generation technique that has been widely used to prepare colloidal gas aphrons(CGA). These gels were differentiated to the conventional gels such as for medical or pharmaceutical applications, which have tiny pores and some crystalline structure. Rather these should be used in de-pollution devices or adhesion of cells or biomolecules. The crosslinkers used in this work were amino acid, organic acid, sugars and lipids(vitamins). The structures of the gels were observed in a scanned electron microscope. Amino acids gels showed remarkably higher swelling ratios probably because their typical functional groups help constructing a highly crosslinked network along with hydrogen bonds. Boric acid and starch would catalyze dehydration while structuring to result in much lower water content and accordingly high gel content, leading to less elastic, hard gels. Bulky materials such as ascorbic acid or starch produced, in general, large pores in the matrices and also nicotinamide, having large hydrophobic patches was likely to enlarge pore size of its gels as well since the hydrophobicity would expel water molecules, thus leading to reduced swelling. Hydrophilicity(or hydrophobicity), functional groups which are involved in the reaction or physical linkage, and bulkiness of crosslinkers were found to be more critical to gel's cross linking structure and its density than molecular weights that seemed to be closely related to pore sizes. Microscopic observation revealed that pores were more or less homogeneous and their average sizes were $20{\mu}m$ for methionine, $10-15{\mu}m$ for citric acid, $50-70{\mu}m$ for L-ascorbic acid, $30-40{\mu}m$ for nicotinamide, and $70-80{\mu}m$ for starch. Also a sensory test showed that amino acid and glucose gels were more elastic meanwhile acid and nicotinamide gels turned out to be brittle or non-elastic at their high concentrations. The elasticity of a gel was reasonably correlated with its water content or swelling ratio. In addition, the PVA gel including 20% ascorbic acid showed fair ability of cell adherence as 0.257mg/g-hydrogel and completely degraded phenanthrene(10 mM) in 240 h.

2단의 슬러지-고정상 반응기에서 페놀 함유 폐수의 혐시성 처리에 관한 연구 (A Study on the Anaerobic Treatment of the Phenol-bearing Wastewater with two Sludge Blanket-Packed Bed Reactors in Series)

  • This study was carried to investigate the biodegradability of phenol in the wastewater with the two sludge blanket-packed bed reactor in series. Each reactor had a dimension of 0.09 m i.d. and 1.5 m height and consisted of two regions. The lower region was a sludge blanket of 0.5 m height and the upper region was a packed-bed of 1 m height. The packed bed region was charged with ceramic raschig rings of 10 mm i.d., 15 mm o.d. and 20 mm length. The reactors were operated at 35$\circ$C and the hydraulic retention time(HRT) was maintained 24 hours. The synthetic wastewater composed of glucose and phenol as major components was fed into the reactor in a continuous mode with incereasing phenol concentration. In addition, the nutrient trace metals($Na^+, Mg^{2+}, Ca^{2+}, PO_4^{3-}, NH_4^+, Co^{2+}, Fe^{2+}$ etc.) were added for growing anaerobes. The phenol concentration of the effluent, the overall gas production, the composition of product gas, the efficiency of COD reduction and the duration of acclimation period were measured to determine the performance of the anaerobic wastewater treatment system as the phenol concentration of the influent was increased from 600 to 2400 mg//l. Successfully stable biodegradation of phenol could be achieved with the anaerobic treatment system from 600 to 1, 800 mg/l of the influent phenol concentration. The upper level of influent phenol loading was high enough to meet most of the practical requirement. The duration of acclimation increased with the phenol loading. At steady state of the influent phenol concentration of 1800 mg/l, the treatment performance indicated the phenol reduction efficiency of 99%, the COD reduction efficiency of 99% and the gas production rate of 37 l/day. At the influent phenol concentration of 2400 mg/l, however, the operation of the treatment system was noted unstable. While the concentration of methane in biogas decreased with increasing the influent phenol loading, the carbon dioxide was increased. However, the concentration of hydrogen was varied negligibly. The concentration of methane was high enough to be used as a fuel. As a result, it is suggested that anaerobic phenol wastewater treament was economical in the sense of energy recovery and wastewater treatment.

3T3-L1 지방전구세포에서 용아초 에틸아세테이트 추출물의 항비만 효과 (Anti-Obesity Effect of Ethyl Acetate Extracts from Agrimonia pilosa Ledeb. in 3T3-L1 Preadipocytes)

  • 본 연구에서는 3T3-L1 지방전구세포를 이용하여 용아초 에틸아세테이트 추출물의 항비만 활성을 확인하고자 하였다. 용아초 에틸아세테이트 추출물에 의한 지방세포 분화 및 adipogenesis 저해 활성을 확인하기 위해 추출물을 3T3-L1 지방전구세포에 분화를 유도하면서 농도별(50, 100 ${\mu}g/mL$)로 처리하였고, 그 결과 용아초 에틸아세테이트 추출물은 지방세포의 분화를 억제시켰다. 이 같은 활성에 대한 기전을 확인하기 위해 PPAR${\gamma}$ 전사활성과 지방세포 분화에 관여하는 유전자들의 활성을 확인해 보았다. 실험 결과 용아초 에틸아세테이트 추출물은 PPAR${\gamma}$ 전사 활성을 억제시켰고 PPAR${\gamma}$ 및 C/EBP${\alpha}$의 mRNA 발현을 농도 의존적으로 감소시켰으며 지방세포 분화에 관여하는 adipokine들의 발현을 조절하는 것으로 나타났다. 따라서 용아초 에틸아세테이트 추출물의 항비만 효과는 지방 생성의 주요 전사인자인 PPAR${\gamma}$와 C/EBP${\alpha}$의 유전자 발현조절을 통해 지방 분화 억제 및 지방 축적을 효과적으로 감소시키는 것으로 보이며, 효과가 있는 농도가 100 ${\mu}g/mL$로 천연물질로써 비교적 낮은 농도에서 효과가 나타나는 것으로 보아 항비만 소재로의 개발 가능성이 있을 것으로 사료된다.
