Purpose: Understanding exactly detailed anatomy and morphology of scapula is very important for further surgical procedures. This study was intended to provide accurate anatomic and morphologic information of scapula by exactly measuring scapular thickness using computed tomography and reconstructing its geometric model. Materials and Methods: Eight average lengths and two angles of 102 scapular structures obtained from 51 cadavers were generally measured by computed tomography. Also, to measure the scapular thickness, sagittal planes of each scapula were divided almost equally and the thicknesses of each sagittal plane was measured by computed tomography. After measuring every thickness, average results were calculated and the gender difference was compared by Student t-test. Results: Average results of the thickness of glenoid fossa, lateral border, medial border, and the middle 1/3 portion of the scapular body were 22.4 mm, 13.83 mm, 4.44 mm and 2.06 mm, respectively. Also, male scapulars were found to be significantly thicker than female. Based on these measured thicknesses, we reconstructed the 3-dimensional geometric model of scapula. Conclusion: From these results, glenoid fossa and lateral border were the thickest part of scapula, while the middle 1/3 portion of the scapular was the thinnest.

The purpose of this study was to investigate the early effects of irradiation on the temporomandibular joint in rats. Male rats were singly irradiated with the dose of 5 Gy or 10 Gy to their head and neck region by /sup 60/Co X ray. Experimental animals were sacrificed at each of the following time intervals -1, 2, 3, 5, 7 and 14 days. The specimens were examined with a light microscope, and treated with H & E staining and immuno-histochemical staining. The results were as follows, 1. By light microscopic findings, proliferative and hypertrophic zone were narrowed and hematopoietic cells were few in number at 5 days after irradiation. Repair signs were seen at 7 days after irradiation when decrease in osteoclast, increase in hematopoietic cells and increase of proliferative zone were noted. The 10 Gy irradiated group showed more severe histopathologic change than the 5 Gy group, and their repair was more slow. 2. In the S -100 antibody, positive cells were examined in the glenoid fossa. Positive cells of irradiated group showed more slight decrease in number than the control group. Low radiosensitivity and slow repair was noted in the glenoid foosa. 3. The interarticular disc was high radioresistant, and any histopathologic changes were not seen in disc. 4. Repair was examined clearly with the response to the antibodies. Especially by 5 days after irradiation 5 Gy group showed S-l00 positive cells in hypertrophic zone next to proliferative zone, chondroitin-4-sulfate positive cell in erosive zone next to hypertrophic zone, type-1 collagen positive cell in subchondral bone.
Many different appliances have been used to move teeth or to attempt to inhibit or stimulate growth of the jaws. Especially, Orthopedic force was used to apply a constant force on the mandible. Various orthopedic forces bring the changes of surrounding tissue, growth and development on the mandibular joint. Author carried out this study to find the histological changes of the mandibular joint in rats when the mandible applied posterior displacement. For the purpose of this study, $009\times036inch$ closed coil spring and 030inch orthopedic wire were used between the post occipital region and the chin region in order to render the orthopedic forces, posteriorly. The experimental animals vu. used 1 month old (GroupI), 3 months old (Group II), and 12 months old (Group III) rats. Following results were obtained; 1. At the beginning of the experimental Group I, there were predominant increment of Cartilage layer in the posterior parts of condylar head. Especially, the mature chondrocyte zone was increased. 2. In the experimental Group II, the chondroblastic zone was decreased while it showed slightly increased mature cartilage zone. 3. In the experimental Group III, the mature chondrocyte zone was disappeared gradually and there was a tendency to reduce the endochondral bone formation in condylar head. 4. After 30 days of experimental Group I and in experimental Group II, there was a tendency to decrease the immature cellular zone in the glenoid fossa by pressure. 5. There were no histological changes in the articular disc by pressure.

The superior shoulder suspensory complex is composed of glenoid fossa, coracoid process, coracoclavicular ligament, distal clavicle, acromioclavicular ligament, acromion. Traumatic double disruptions of this complex lose its suspensory action on the shoulder joint and result in functional loss and deformity. Careful radiologic evaluation and appropriate management are required for injuries to this complex. Ipsilateral fractures of clavicle and scapula create unstable anatomic situation on shoulder joint. Conservative treatment usually fails to achieve good functional recovery due to rotator cuff weakness, nonunion, delayed union, malunion and neurovascular injury. Authors studied the result of operative treatment of ipsilateral clavicle and scapular fractures to prevent such complications. Seven cases were treated with open reduction and internal fixations of clavicle alone or clavicle and scapula simultaneously and followed up for nineteen months(twelve months - thirty-eight months). All but one patient showed good or excellent functional result according to the scoring system of Rowe. Poor result was developed in the case which had brain injury. Rigid fixations of clavicle alone or clavicle and scapular fractures both can achieve stable reduction of the fractures and prevent sequelae. We concluded that operative treatment of ipsilateral fractures of clavicle and scapula is safe and yields predictable good results.

Objective. Disc displacement may affect the joint space narrowing between condyle head and glenoid fossa. This study was designed to evaluate the correlation between the joint space change and the directions of disc displacement. Study Design. Two hundreds temporomandibular joints MR images of TMD patients (170 joints) and asymptomatic volunteers (30 joints) were evaluated for this purpose. Anterior disc displacement was divided into 3 stages (normal, little to mild, and moderate to severe displacement) based on sagittal images. And sideways displacement was classified as 3 categories (center, medial and lateral displacement) based on coronal images, then joint spaces were measured at medial, central and lateral parts of condyle head on coronal MR images, respectively. The joint spaces of 7 groups divided according to the severity and the direction of disc displacement were compared. Results. The reduction of the joint space was affected by sideways disc displacement at the opposite side of the condyle head, except the cases accompanied with severe anteriorly and laterally displaced disc. Conclusion. The sideways disc displacement affected on the opposite side temporomandibular joint space width.
Background: Condylar dislocation can arise as a complication in patients who required mandibular and/or condylar reconstruction and were operated on with fibula free flap (FFF) using surgical guides designed using simulation surgery. Surgeons should be aware of the complications in these present cases when planning and performing reconstructions as well as predicting prognoses. Cases presentation: Two cases showed condylar dislocation in mandibular reconstruction using a FFF fixed with a reconstruction plate. Three cases showed condylar dislocation in mandibular reconstruction using a fibula free flap fixed with a mini-plate. Conclusion: Despite the lack of clinical symptoms in these cases following mandibular reconstruction using an FFF, the mandibular condyle was severely displaced away from the glenoid fossa. A surgeon must have sufficient time to consider the use of a long flap with thickness similar to that of the mandible, ways to minimize span and bending, and methods of fixation. The patient, moreover, should be educated on condylar dislocation. Customized CAD/CAM-prototyped temporomandibular condyle-connected plates may be a good alternative even if virtual simulation surgery is to be performed before surgery. These considerations may help reduce the incidence of complications after mandibular reconstruction.

The techniques of joint mobilization and traction are used to improve joint mobility or to decrease pain by restoring accessory movements to the shoulder joints and thus allowing full, nonrestriced, pain-free range of motion. In the glenohumeral joint, the humeral head would be the convex surface, while the glenoid fossa would be the concave surface. The medial end of the clavicle is concave anterioposteriorly and convex superioinferiorly, the articular surface of the sternum is reciprocally curved. The acromioclavicular joint is a plane synovial joint between a small convex facet on lateral end of the clavicle and a small concave facet on the acromion of the scapula. The relationship between the shape of articulating joint surface and the direction of gliding is defined by the Convex-Concave Rule. If the concave joint surface is moving on a stationary convex surface, gliding occur in the same direction as the rolling motion. If the convex surface is moving on a stationary concave surface, gliding will occur in an opposite direction to rolling. Hypomobile shoulder joints are treated be using a gliding technique.

There are various modalities in the treatment of facial asymmetry, but in severe case with TMD by actively growing deformed condyle, the treatment choice is removing the condyle growth center and TMD symptom such as click or muscular discomfort. In our one case, the patient was complain of facial asymmetry. There are severe deformed condyle head with bird-head fashion and enlarged mandibular ramus and body vertically about 18 mm, overgrowthed Rt. mandible body horizontally about 20 mm. She had intermittent Lt. TMJ clicking and muscular discomfort. The author diagnosed it as Lt. hemimandibular hyper-plasia & R, hemimandibular elongation, a combination form with TMD. the condyle was in active growing state in scintigraphic analysis. So we extirpated the deformed condyle by intrasoral sagittal split ramus osteotomy and reshaped the condyle and mandibular distal fragment extraorally. The distal fragment was readapted in glenoid fossa and fixated. In Rt. mandibular body area, autogenous onlay bone graft on the inferior border of mandible was performed to correct the asymmetry. The clicking and facial asymmetry was corrected and we report this results with other literature findings.
The purpose of this study was to investigate changes of the mandible of a growing rat when they are subjected to a retractive force and those after removal of the retractive force. The experimental animals were Sprague-Dawley male rats of four weeks of age. A mandible was retracted with 50 grams of force on each side in the posterior and superior direction for 8 hours per day. The animals were sacrificed after 1 week, 2-week and 4-week force application, and after 4-week force application-4-week force removal period. The changes of rat mandibular growth following retractive force on the growing rat mandible were observed histologically and biometrically. The findings were as follows ; 1. Histologically, the thickness of the condylar cartilage was slightly reduced in the anterosuperior region with the retractive force. However, in the group of 4-week force application-4-week force removal, there was no significant difference in the thickness of the condylar cartilage. 2. There were no significant histological changes in the articular disk and glenoid fossa through the experimental period. 3. The length and anterior height of the mandible subjected to the retractive force were significantly smaller and greater than those of the control group. 4. There were no significant differences in the mandibular length between 4-week force application - 4-week force removal and the control group. 5. It was concluded that a mandibular retractive force produced inhibitory effects in the growth of the mandible, but that these effects were not sustained during mandibular growth in this experimental model.
