Factors Influencing Desired Postnatal Weight Loss in Women after Birth (출산 후 여성의 체중 감소 기대치에 대한 영향 요인)

  • Purpose: The purpose of this study was to identify factors influencing desired postnatal weight loss in women after birth. Methods: With correlational survey design, 191women of the study participants completed questionnaires on their desired postnatal weight loss and related factors, and body and health concerns during their hospitalization after birth. Data were collected from February to May, 2011. Data were analyzed using t-test, ANOVA, Pearson correlation coefficients, and multiple linear regression. Results: The women's gestational weight gain, pre-pregnant BMI, body concerns, expected time to reach the desired weight after birth and health concerns were identified as factors influencing desired postnatal weight loss in women after birth. The model explained 68% of the variance. Conclusion: Results suggest that nurses in women's health care should consider women's gestational weight, pre-pregnant BMI and their body concerns, health concerns and expected time to reach the desired weight after birth when developing weight management program for women after birth.

Assessment of Gestational Age using New Ballard Examination in High-Risk Infants (고위험신생아에 있어 New Ballard Examination(NBE)을 이용한 재태기간 사정)

  • Knowing the accurate GA is critical in nursing care of high-risk newborns. A descriptive study was performed to examine the reliability and clinical applicability of the new Ballard examination (NBE) in high-risk infants. Method: A NBE was performed to measure GA by assessing the neuromuscular and physical maturity in the course of physical examination of a convenient sample of 50 high-risk infants. Results: 1) There was a highly correlation between both the GA by LMP (GA-LMP) and GA by NBE (GA-NBE) (r = .894, p = .000) 2) There was a greater positive relationship in neuromuscular maturity than physical maturity in the GA-NBE of the high-risk newborn (r = .657 versus r = .915, p <. 05). 3) The high-risk infants were thoes with congenital anomalies, prematurity, and RDS(Respiratory Distress Syndrome). Male infants showed a higher neuromuscular maturity, compared to female infants. 4) There was a positive correlation between neuromuscular, physical, total maturity, GA-LMP and GA-NBE in the birth weight, 1 minute Apgar score. Conclusion: The study supports the reliability an clinical relevance of NBE in assessment of the accurate GA in high-risk infants.

Folate Levels of Umbilical Cord Blood and Pregnancy Outcomes (한국인 임신 여성의 제대혈 엽산 농도와 임신의 결과)

  • The maintenance of adequate folate levels in the umbilical cord blood is esential for supplying tissue requirements of fetal growth. However, there is data on folate levels in the cord blood of Korean infant. The present investigation was undertaken to determine folate levels in cord blood and aassess relationships between folate levels and pregnancy outcomes. Dietary and supplementary folate intake was obtained from thirty subjects who were in the third trimester fo pregancy . The umbilical cord blood was drawn at delivery and pregnancy outcomes for the subjects were collected from their medical records. Erythrocyte and plasma folate levels in the cord blood were analyzed. The subjects were divided into two groups ; high folate (HF, $\geq$654ng/ml) and low folate (LF, <654ng/ml) groups according to erythrocyte folate levels in cord blood. Dietary folate intake and the amount of supplemental folates were not significantly different between the two experimental groups. However, infant birth weight (3540$\pm$295g) and placental weight(910$\pm$85g) for the HF group were significantly higher(p=0.0041 and p=0.109, respectively) than those for the LF group, which were 3127 $\pm$419g and 823$\pm$80g , respectively. Although it was not significant, the gestational weight gain for the HF group was 2.8kg higher than that for the LF group. Thus, the erythrocyte folate level in the cord blood was significantly related to infant birth weight and placental weight. These results confirm that a high erythrocyte folate level in the umbilical cord blood promotes both fetal and placental growth and improves gestational weight gain as well.

The effects of the parents' social class on the low birthweight among the births, 1995-2001 (부모의 사회계급이 1995-2001년도 출생아의 저체중에 미치는 영향)

  • To investigate the effect of parents' social class on birthweight in Korea during the period of transition to a market economy, 1995-2001. Methods: All births resulting from pregnacies reported in 1995-22001(n=4,298,374) were studied with respect to social variation in birthweight. The results were adjusted for maternal age, parity, parents' education, gestational age, total births, the dead births among total births, the number of births in one delivery. Results: Between 1995 and 2001, mean birth weight was 3271g and low birth rate was 3.41%. Maternal education, faternal education, parents' occupation, the number of birth in one delivery, total births, gestational age, and the number of deaths among. total births were all independently related to the birthweight. Parents with lower education showed higher low-birthweight rates compared with parents with university level of education(OR: 2.16 for mother and 1.68 for father). Especially, mother's education showed stronger relationship with birthweight than fathers' education. The differences in birth weight by parents' social class, especially parents' educational level became stronger between 1995 and 2001. Discussion: This study suggests that the social differences of birth weight were increasing in order 1995 to 2001. Especially, this increasing tendency in variation in birth weight by social class was shown after economic crisis, 1998.

Assessment of Gestational Age based on Newborn Maturity Rating ; Ballard Examination (신생아의 성숙도 평정에 의한 재태기간 사정)

  • Newborn period is a transitional stage for independent adaptation from intrauterine to extrauterine life by maintaining respiration, temperature and nutrition. In general, the adaptability of the newborn is proportional to the gestational age(GA), so knowing the accurate GA is critical to develop nursing process in the newborn nursery. A newborn ma turity rating, a Ballard examination, has been used to measure GA by assessing the maturity of new-born. It consists a total of 12 items, which is the 6 items for the neuromuscular maturity and the 6 items for the physical maturity A total of 75 new-born were assessed for the maturity and GA using the Ballard examination. The results are follows : 1) The score of each item of Ballard examination is propotional to GA using the Ballard examination as well as LMP. 2) There was a greater positive relationship between neuromuscular, physical and total maturity, and the GA measured by Ballard examination, than the GA measured by LMP. 3) Any stressful environment to the newborn could influence to the maturity of newborn. In summary, the study showed the Ballard examination Is more reliable and clincially feasible method to measure the accurate G4 compared to the GA by LMP. Therefore, it suggests the application of Ballard examination to measure the new born maturity and GA is beneficial in developing nursing process. The expansion of the study with the variety of the subject characteristics nil on hance the clinical applicability of the examination.

Adiponectin Level in Non-Pregnant Women, Pregnant Women without Diabetes and Pregnant Women with Diabetes

  • Gestational diabetes mellitus (GDM) is a common complication during pregnancy and one of the main causes of adverse fetal-maternal outcomes. However, the pathogenesis of GDM has not been clearly stated. Adiponectin, an adipose tissue-derived plasma protein, is involved in regulation of insulin resistance and glucose hemostasis, and thus is a key modulator of insulin action and glucose metabolism. In this study, we investigated to compare serum adiponectin levels in pregnant women with diabetes, pregnant women who are without diabetes, and non-pregnant women, and to evaluate relationship between serum adiponectin. levels and metabolic parameters. Forty-one pregnant women with diabetes, fifty-nine pregnant women without diabetes and forty non-pregnancy women were recruited. Adiponectin levels were significantly lower in pregnant women with diabetes when compared to non-pregnant women and pregnant women without diabetes. Pregnant women without diabetes at second trimester had lower adiponectin levels compared to non-pregnant women. Adiponectin was negatively correlated with BMI, fasting insulin, HOMA-IR, total cholesterol, and triglyceride. In conclusion, this study confirmed that the decreased level of adiponectin precedes the onset of abnormal glucose level during pregnancy and also normal pregnant women had lower adiponectin levels compared to non-pregnant women. This knowledge may help to identify strategies for lowering the occurrence of GDM in women who are at high risk of developing the disorder.

Endovascular Treatment of a Ruptured Posterior Inferior Cerebellar Artery Aneurysm during Pregnancy

  • Aneurysmal subarachnoid hemorrhage (SAH) during pregnancy is quite rare, however it has a high maternal mortality rate. A pregnant woman in the 16th gestational week was admitted to our hospital with a drowsy level of consciousness. A brain magnetic resonance (MR) image showed hemorrhage on the prepontine cistern, and both sylvian fissures, and MR angiography and cerebral digital subtraction angiography demonstrated an aneurysm at the left posterior inferior cerebellar artery (PICA). We performed endovascular coil embolization attempting to minimize radiation exposure. She was discharged with no neurologic deficit and delivered a healthy baby by cesarean section at the 38th week of gestation. This case study reported the shortest gestational period and this is the first report on an aneurysmal rupture arising from PICA which was treated using an endovascular method. Using an appropriate technique for reduced radiation exposure to the fetus and limited alterations in maternal-fetal physiology, endovascular coil embolization could guarantee good results in treatment of aneurysmal SAH in pregnant women.

Stress and Coping Style of High-risk Pregnant Women's Spouses (고위험 임부 배우자의 스트레스와 대처양상)

  • Purpose: This study determined the level of stress and type of coping style of spouses with high-risk pregnant women. Methods: Subjects were 102 spouses with high-risk pregnant women at 6 hospitals in Seoul and Gyeonggi province from January to August, 2009. The tools for this study were stress scale and coping scale. The data were analyzed by t-test, ANOVA, Scheffe's test and Pearson's correlation coefficient. Results: The average score of subjects stress was 2.18 and coping score was 2.46 point out of 4.00. The highest score of stress categories was 2.44 point in emotional problems and the lowest score was 1.72 in communication and support resources. The stress level was significant differences according to length of marriage, number of hospital visits, satisfaction with marriage relationship, hospitalization days and gestational age respectively. Spouses tended to use an active coping style (2.60) rather than a passive coping style (2.31). There were significant differences according to mother's age and gestational age in active coping and educational level in passive coping. Conclusion: Considering the level of stress coping style and characteristics of high-risk pregnant women's spouses, a nursing intervention should be provided to help them alleviate stress and to encourage active coping.
