Responses and adverse effects of carboplatin-based chemotherapy for pediatric intracranial germ cell tumors

  • Purpose: Cisplatin-based chemotherapy has been commonly used for the treatment of intracranial germ cell tumors (IC-GCTs). However, this treatment exhibits some adverse effects such as renal problems and hearing difficulty. Carboplatin-based chemotherapy was administered to pediatric patients with IC-GCTs from August 2004 at the Samsung Medical Center. In this study, we assessed the responses and adverse effects of carboplatin-based chemotherapy in pediatric IC-GCTs patients according to the risk group, and compared the results with those of the previous cisplatin-based chemotherapy. Methods: We examined 35 patients (27 men and 8 women) diagnosed with IC-GCTs between August 2004 and April 2008 and received risk-adapted carboplatin-based chemotherapy at the Samsung Medical Center. Patients were divided into either low-risk (LR) or high-risk (HR) groups and a retrospective analysis was performed using information from the medical records. Results: Although hematological complications were common, hearing difficulties or grade 3 or 4 creatinine level elevation were not observed in patients who underwent carboplatin-based chemotherapy. The frequency of febrile neutropenia did not differ between the risk groups. The overall survival was 100% and event-free survival (EFS) was 95.7%. The EFS rate was 100% in the LR group and 90% in the HR group, respectively. Conclusion: Despite their common occurrence in high-risk patients, no lethal hematological complications were associated with carboplatin-based treatment. The current carboplatin-based chemotherapy protocol is safe and effective for the treatment of pediatric patients with IC-GCTs.

Clinical Implication of Surgically treated Abdominoperineal Soild Tumor in the Newborn : A Single-Center Experience

  • Purpose: Abdominoperineal solid tumors presenting in neonates often require surgical intervention during the neonatal period. Although we report our single-center experience, this study would be meaningful to understand the clinical implications of these neoplasms. Methods: We retrospectively reviewed and analyzed the clinical data and characteristics of 22 patients (${\leq}28$ days old) diagnosed with histopathologically confirmed abdominoperineal solid neoplasms (benign or malignant) after surgical resection. Results: The mean gestational age and postnatal age at the time of operation were $38.3{\pm}1.8weeks$ and $13.5{\pm}8.3days$, respectively. Most patients (18/22, 81.8%) were diagnosed during antenatal care visits; however, 4 (18.2%) were identified after birth. The mean tumor size was $6.4{\times}5.3cm$ (3.5-17.0 cm), and tumors occurred most frequently within the sacrococcygeal region (8/22, 36.4%). Histopathologically, 14 patients (63.6%) demonstrated benign tumors and 8 (36.4%) demonstrated malignant tumors. Germ cell tumors and hepatoblastomas were the most commonly observed tumors. Fortunately, all patients showed a localized pattern of tumor involvement without distant metastasis. No recurrence or mortality was observed during the follow-up period (mean $66.4{\pm}44.2months$). Conclusion: Abdominoperineal solid tumors occurring in neonates show variable clinical patterns during the antenatal and postnatal monitoring/screening periods. We conclude that aggressive and multidisciplinary approaches could achieve good clinical results in these patients.

흉선종의 경피 세침흡인 세포학적 검색 (Percutaneous Fine Needle Aspiration Cytology of Thymoma)

  • The fine needle aspiration (FNA) cytologic findings in 16 cases of histologically confirmed thymoma are reported. The aspirates were obtained under fluoroscopic guidance. The cytologic diagnoses were inadequate sample in one case, thymoma in 12(75%), small cell carcinoma or thymoma in 1, benign mesenchymal tumor in 1, and germ cell tumor in one. The cytologic features were detailed according to the constituent epithelial cell type, and into 4 small of epithelial cells and lymphocytes. Fifteen cases were classified into 4 small epithelial cell type, 6 intermediate epithelial cell type, 1 large epithelial cell type, 1 large pleomorphic epithelial cell type, and 3 spindle-shaped epithelial ceil type. Cytologic differential diagnosis was discussed, and the important criteria for the cytologic diagnosis of thymoma were reviewed. This review leads us to think that nonoperative cytologic approaches in the diagnosis of the thymoma are possible, and that correct cytologic diagnosis of thymoma with FNAs can easily be made, if adequate samples are obtained However the invasiveness and histologic type could not be predicted by cytological features only. Knowing various cytologic and histologic features of thymoma will be helpful for the diagnosis of thymoma and the differential diagnosis of modiastinal tumors.

Diffusion Weighted Imaging Can Distinguish Benign from Malignant Mediastinal Tumors and Mass Lesions: Comparison with Positron Emission Tomography

  • Background: Diffusion-weighted magnetic resonance imaging (DWI) makes it possible to detect malignant tumors based on the diffusion of water molecules. It is uncertain whether DWI is more useful than positron emission tomography-computed tomography (PET-CT) for distinguishing benign from malignant mediastinal tumors and mass lesions. Materials and Methods: Sixteen malignant mediastinal tumors (thymomas 7, thymic cancers 3, malignant lymphomas 3, malignant germ cell tumors 2, and thymic carcinoid 1) and 12 benign mediastinal tumors or mass lesions were assessed in this study. DWI and PET-CT were performed before biopsy or surgery. Results: The apparent diffusion coefficient (ADC) value ($1.51{\pm}0.46{\times}10^{-3}mm^2/sec$) of malignant mediastinal tumors was significantly lower than that ($2.96{\pm}0.86{\times}10^{-3}mm^2/sec$) of benign mediastinal tumors and mass lesions (P<0.0001). Maximum standardized uptake value (SUVmax) ($11.30{\pm}11.22$) of malignant mediastinal tumors was significantly higher than that ($2.53{\pm}3.92$) of benign mediastinal tumors and mass lesions (P=0.0159). Using the optimal cutoff value (OCV) $2.21{\times}10^{-3}mm^2/sec$ for ADC and 2.93 for SUVmax, the sensitivity (100%) by DWI was not significantly higher than that (93.8%) by PET-CT for malignant mediastinal tumors. The specificity (83.3%) by DWI was not significantly higher than that (66.7%) for benign mediastinal tumors and mass lesions. The accuracy (92.9%) by DWI was not significantly higher than that (82.1%) by PET-CT for mediastinal tumors and mass lesions. Conclusions: There was no significant difference between diagnostic capability of DWI and that of PET-CT for distinguishing mediastinal tumors and mass lesions. DWI is useful in distinguishing benign from malignant mediastinal tumors and mass lesions.

원발성 종격동 종양의 임상적 고찰 (Clinical Analysis of Primary Mediastinal Tumors)

  • 본 인제대학교 의과대학 서을 백병원 흉부외과학교실에서는 1987년 9월 부터 1995년 12월까지 원발성 종격동 종양의 진단하에 수술을 시행한 40례를 대상으로 하여 임상적 고찰을 하였다. 관찰 대상은 남자 18례와 여자 22례로 연령은 Vll에서 68세까지 였으며 평균 )4.1세였다. 종양의 진단시 증상은 흉통(12.5%), 기침(12.5%), 호흡곤란(7.5%) 경부종괴(7.5%), 흉부불쾌감(5.0%) 등이었다. 진단은 모든 예에서 단순 흉부엑스선 사진과 흉부 전산화단층촬영을 실시하였고 5례 에서는 자기공명영상을 시행하였다. 경피적 침생검은 22례에서 시행하여 16례에서 조직학적 진단을 얻었다(민감도 72.7%). 종양의 위치별 분포는 전상부 종격동 24례(60.0%), 후부 종격동 14례(35.0%), 중부 종격동 2례(5.0%)였다. 종양의 종류는 흉선종 11례(27.5%), 신경성종양 10례(25.0%), 배아세포종 7례(17.5%), 낭종 8례(20.0%), 거대 림프 절비대(Castleman's disease) 2례(5.0%), 방추세포육종 1례(2.5%) 그리고 림프종 1례(2.5%), 였다. 악성 종양은 5례로 침습성 흉선종 3례, 방추세포육종 1례, 림프종 1례였다. 치료는 양성의 모든 예와 악성종양 3례에서 완전절제하였고, 2례는 수술이 불가능하였다. 수술후 사망은 없었고 합병증은 창상피열, 성대마비, 어깨강직이 각각 1례씩 있었다.

종격동 종양과 낭종의 임상 경험 (Clinical Experience of Mediastinal Tumors and Cysts)

  • 경상대학교 의과대학 흉부외과학 교실에서는 1988년 4월부터 19%년 3월까지 47명의 원발성 종격동 종양 및 낭종 환자에 대하여 수술적 치험을 하였다. 환자들의 연령분포는 5세에서 67세로 평균연령은 36.4세였곡 남녀의 성비는 1:1.5였다. 임상증상은 홉통이(27.7%) 가장 많았고, 안검하수(14,9%), 전신쇠약(12.8%), 기침 (10.6%) 복시(10.6%) 등의 순이먼으며 증상이 없는 경우도 8례(17.0%) 있었다 종양의 위치는 전상부 종격동 이 63.8%로 가장 많았고· 술후 병리조직학적 진 단상으로는 흉선종(32.9%)이 가장 많았으며 낭종(21.3%), 신경 절 종잉(17.0%) 배아세포종(12.8%), 간엽종양(6.4%) 순의 빈도를 보였다. 수술시 양성종양 41례와, 악성종양 6 례중 3례는 완전절제가 가능하였으나 나머지 3례는 부분절제만이 가능했고 술후 6례 모두에서 화학요법 또 는 방사선요법을 시행하였다. 술후 합병증으로는 Homor 증후군, 유미흉, 상대정맥 파열, 그리고 횡격막 신경 마비 등이 각각 1례씩 발생하였으며 술후 사망례는 없었다.

종격동 종양의 임상적 고찰 (Clinical Evaluation of the Mediastinal Tumors)

  • 순천향대학 천안병 원은 1984년 2월부터 1994년 2월까지 종격동 종양 51례를 수술하였다. 연령은 11개월에서 75세까지고 평균연령 39.4$\pm$18.8세였고 성비는 1:1.4였다. 임상증상은 무증상이 21.6%로 가장 많았고 종격동 종양의 60.6%가 전상부에 위치해 있었다. 조직학적 병명의 빈도는흉선종 (3).3%), 신경성종양(25.5%), 배아세포종(19.6%), 종격동낭종(9.8%) 및 간염종양(5.9%) 순이었다. 양성 종양 46례는 완전절제였고 악성종양 5례중 2례는 완전절제, 1례는 불완전절제, 2례는 조직생검만 시행한 후 방사선치료하였다. 술후 합병증은 5례(9.8%)에서 창상파열 2례, 기흉 1례, 성대마비 1례 및 지속적 인공호흡요구 1례 등이었다.

Screening for Del 185 AG and 4627C>A BRCA1 Mutations in Breast Cancer Patients from Lahore, Pakistan

  • Breast cancer contributes to approximately 23% of the cancer cases identified and 14% of cancer related deaths worldwide. Including a strong association between genetic and environmental factors, breast cancer is a complex and multi factorial disorder. Two high penetration breast cancer susceptibility genes (BRCA1 and BRCA2) have been identified, and germ line mutations in these are thought to account for between 5% and 10% of all breast cancer cases. The human BRCA1 gene, located on 17q, is involved in the regulation of cell proliferation by aiding in DNA repair, transcriptional responses to DNA damage and cell cycle check points. Mutations in this gene enhance cell proliferation and facilitate formation of tumors. Two mutations, the 185 deletion of AG and the 4627 substitution from C to A, are founder mutations in the BRCA1 gene for breast cancer in Asian populations. Allele specific PCR was performed to detect these selected mutations in 120 samples. No mutation of 4627 C to A was detected in the samples and only one of the patients had the 185 del AG mutation in the heterozygous condition. Our collected samples had lower consanguinity and family history indicating the greater involvement of environmental as compared to genetic factors.

Pineal and Suprasellar Germinoma Cooccurence with Vertebra Plana: A Case Report

  • Germinoma is the most common type of intracranial germ cell tumors (GCTs). Pineal gland and suprasellar region are the most frequent sites of central nervous system (CNS) involvement. Intracranial masses caused by Langerhans cell histiocytosis (LCH) mimics features of CNS GCTs. LCH frequently involve spine and is the most common cause of vertebra plana in children. A 15-year-old boy presented with progressing symptoms of polydipsia, polyuria, general headache, nausea and severe back pain. Brain MRI showed brain tumor with simultaneous involvement of suprasellar region and pineal gland. An excisional biopsy of suprasellar mass was done. The pathologic assessment confirmed the diagnosis of germinoma. Patient's treatment continued accordingly. A spine MRI, done due to persistent backache, showed a vertebra plana. We reevaluated the primary diagnosis suspecting LCH. Germinoma of CNS was confirmed and a biopsy of vertebral lesion resulted in hemangioma. Thus we report a case of CNS germinoma with co-occurrence of vertebra plana. We emphasized the importance of histopathologic diagnosis of pineal/suprasellar masses and primary investigation of other CNS regions including spine for possible metastasis or comorbidities.

  • 원발성 전 종격동 종양은 종격동 종양의 반이상을 차지하고 그 종류도 다양하다. 흉선 상피 종양이 가장 흔하고 악성 흉선종은 드물다. 생식세포종은 두 번째로 흔한 전 종격동 종양으로 보다 어린나이에 발생하며 대부분 양성이다. 임파종은 대부분 전신성 질환의 흉부 침습 형태로 나타나나 원발성 흉부 임파종은 Hodgkin씨 병이 많다. 저자는 대표적인 전종격동 종양의 임상증상과 영상의학적 소견을 소개한다.
