The Necessity of Establishing a Specialist Program for Geriatric Dentistry for the Elderly of Korea Based on their General Health Status (노인치과 전문과목 신설 필요성 : 우리나라 노인의 건강실태를 중심으로)

  • Korea became an aged society as of 2018, and will have plunged into a super-aged society by 2025. For the dental treatment of elderly patients, their general conditions are to be considered to make appropriate treatment plans and strategies, based on advanced knowledge of geriatric dentistry and clinical experience. In this study, the social demand for geriatric dental specialists for the treatment of the elderly of Korea is investigated by looking at their general health conditions, ADL (Activities of Daily Living), and IADL (Instrumental Activities of Daily Living), along with the review of the advanced training programs of geriatric dentistry in other countries.

A CLINICAL STUDY ON THE CARE OF ODONTOGENIC INFECTIONS IN THE ADMISSION PATIENTS WITH AGE-RELELATED GERIATRIC DISEASES (노인성 전신질환 입원환자에서 치성감염 관리에 관한 임상적 연구)

  • This is a reprospective study on the care of odontogenic infections in admission patients with geriatric diseases. The study was based on a series of 480 patients at Dong San Medical Center, Wonju Christian Hospital and Il San Health Insurance Hospital, From Jan. 1, 2000, to Dec. 31, 2002. The Obtained results were as follows: 1. The systemic malignant tumor was the most frequent cause of the geriatric diseases with odontogenic infectious diseases, and refractory lung disease, systemic heart disease, type II diabetes mellitus, cerebrovascular disease, bone & joint disease, senile psychologic disease were next in order of frequency. 2. Male prediction(57.5%) was existed in the odontogenic infectious patients with geriatric diseases. But, there were female prediction in senile psychologic disease, systemic heart disease and cerebrovascular disease. 3. The most common age group of the odontogenic infectious patient with geriatric disease was the sixty decade(47.9%), followed by the seventy & eighty decade in order. 4. In the contents of chief complaints on the odontogenic infectious patients with geriatric disease, peak incidence was occurred as toothache(52.7%), followed by extraction wish, tooth mobility, oral bleeding, oral ulcer, fracture of restoration, gingival swelling in order. 5. In the diagnosis group of odontogenic infectious diseases, periodontitis, pulpitis & periapical abscess were more common. 6. In the treatment group of odontogenic infectious diseases, the most frequent incidence(34.2%) was showed in primary endodontic treatment (pulp extirpation, occlusal reduction and canal opening drainage) and followed by scaling, incision & drainage, only drugs, pulp capping, restoration in order.

Current status of dental geriatric education in Korea (한국의 노인치과학 교육 현황)

  • Purpose: The aim of this study was to evaluate the current status of dental geriatric education in Korea. Materials and methods: One of the faculty members related in geriatric education was selected in each Korean dental school and the questionnaire on geriatric dental education was sent to them by e-mail. The questionnaire consists of the topics about undergraduate geriatric education and administration of the geriatric education. The information obtained from the questionnaire was compared with that of other countries in the aspects of curriculum, teaching methods, subjects, and existence of specific clinics, etc. Results: Seven schools have geriatric dentistry in undergraduate education curriculum. Among those, only two schools had it taught by theoretical lecture as well as clinical lecture. Two dental schools had specific geriatric clinic among seven dental schools. Compared with the USA and western European countries, the geriatric dental education in Korea is at a developing stage and was perfunctory without diverse clinical experience. Conclusion: In Korean dental schools, geriatric education was mostly conducted by lectures, and clinical teaching programs were not well organized compared with developed countries. It seems that the status of geriatric dental education in Korea has not been well established academically or administratively yet.

Quantitative analysis of periodontal pathogens present in the saliva of geriatric subjects

  • Purpose: At present, information regarding periodontal disease in geriatric patients is scarce. The purpose of this study was to quantify the periodontal pathogens present in the saliva of Korean geriatric patients and assess the relationship between the bacterial levels and the periodontal condition. Methods: Six putative periodontal pathogens were quantified by using a real-time polymerase chain reaction assay in geriatric patient groups (>60 years) with mild chronic periodontitis (MCP), moderate chronic periodontitis (MoCP), and severe chronic periodontitis (SCP). The copy numbers of Porphyromonas gingivalis, Tannerella forsythia, Treponema denticola, Aggregatibacter actinomycetemcomitans, Fusobacterium nucleatum, and Prevotella intermedia were measured. Results: It was found that the bacterial copy numbers increased as the severity of the disease increased from MCP to SCP, except for P. intermedia. For P. intermedia, it was found that samples in the MCP group yielded the largest amount. It was also found that the quantities of P. gingivalis, T. forsythia, and T. denticola, the so-called "red complex" bacteria, were lower than those of F. nucleatum, A. actinomycetemcomitans, and P. intermedia in all of the samples. Conclusions: Collectively, the results of this study suggest that the levels of P. gingivalis, T. forsythia, F. nucleatum, and T. denticola present in saliva are associated with the severity of periodontal disease in geriatric patients.

ORAL STATUS OF GERIATRIC PATIENTS, AND THEIR DEMAND AND SATISFACTION IN PROSTHODONTIC TREATMENT (노인환자의 구강상태, 치료요구도와 만족도)

  • In this study 240 geriatric patients, aged 60 year and over were evaluated using a questionnaire and oral examination for their oral status, demand and satisfaction of prosthodontic treatment. The followings are the findings : 1. The single main complaint was regarding from the existing removable prosthesis, When the complaints were categorized into larger groups, complaints regarding existing prosthesis were 32% and followed by 30% in caries and endodontic problems. 2. Approximately half of the geriatric patients had 21 or more remaining dentition. This number however diminished with increase in age of the surveyed patient. 3. According to the result from questionnaire, 48% of patients wearing removable prosthesis and 43% with fixed prosthesis were not satisfied with existing prosthesis. Thirty-seven percentages of patients were not satisfied with aesthetics and unable to chew food properly. 4. Seventy-seven percentages of patients requested for new prosthesis and the majority of these patients had complaints of difficulties in chewing and discomfort. 5. Thirty-nine percentages of patients were wearing removable prosthesis. More complete dentures were found on maxilla and partial dentures on mandible.

Relationship between the number of functional teeth and Geriatric Oral Health Assessment Index(GOHAI) in elderly (노인의 기능 치아 수와 구강건강평가(GOHAI)와의 관련성)

  • Objectives: The purpose of the study was to investigate the association between remaining functional teeth and oral health-related quality of life using Geriatric Oral Health Assessment Index(GOHAI) in the elderly. Methods: A self-reported questionnaire was completed by 237 female elderly in Daegu from May to September, 2015. The questionnaire consisted of general characteristics of the subjects and Geriatric Oral Health Assessment Index(GOHAI). A total of 216 female elderly data were analyzed for oral health status and their GOHAI score. Data were analyzed using SPSS 23.0 version. Logistic regression analysis showed the association of functional teeth and GOHAI score. Results: Number of functional teeth can affect oral health related quality of life in the elderly. When functional teeth is small, the odds ratio(OR) was 2.119(95% CI: 1.215-3.696). After adjusting age, residence type and education, OR was 1.900(95% CI: 1.060-3.399). Policies to preserve natural tooth should be recommended for increasing quality of life in general. Conclusions: This study showed significant association between number of functional teeth and the GOHAI score of the elderly women.

The Present State and Prospect of Geriatric Denture Prosthetic Dentistry Affairs among Community Dental Hygienists (전국보건소 치과위생사의 노인의치보철사업 현황 및 전망)

  • The purpose of This study was to survey the present state and prospect of geriatric denture prosthetic dentistry among community dental hygienists in Korea. Questionnaires about the present state and prospect of geriatric denture prosthetic dentistry were distributed to the community dental hygienists in Korea, July 2006. Collected questionnaires from 89 cases were analyzed. All collected data were analyzed using the SPSS program. The significance of differences among groups were assessed using chi-square test or Fisher's exact test. The results of this study were summarized as follows: The public relations of the geriatric denture prosthetic dentistry were enforced good, mainly by village offices. Appointed dental clinics made a production of full dentures connecting in Korean Dental Association. Above findings suggest that further study about the present state and prospect of geriatric denture prosthetic dentistry is necessary in improving the oral health related quality of life in elders.

  • The apicoectomy is an operation in which the root apex is removed and the adjacent periapical pathologic tissue is curetted. In this operation, there are several factors that may lead to the surgical wound infection, such as, local, systemic, environmental, endogenous and surgical factor. The systemic medical and surgical factor that may compromise patients defense are more important cause of the wound infection. The postoperative infection is likely to occur owing to poor systemic condition(especially geriatric cancer patient with operation, radiotherapy and chemotherapy), postoperative accumulation of hematoma & seroma and other contaminated factors. So, the authors established the immediate rubber drainage into the sutured wound of dental apicoectomy for the prevention of postoperative infection. The results are more favorable without the wound infection in total 31 cases of the dentistry of Dong San Medical Center & Wonju Christian Hospital.
