요양병원 간호사와 간호조무사의 직무만족과 자존감 및 간호업무성과에 관한 연구 (Job Satisfaction, Self-Esteem, and Nursing Task Performance among Registered Nurses and Nurse Assistants in Long-Term Care Hospitals)

  • Purpose: This study was conducted to compare job satisfaction, self-esteem, and nursing task performance between registered nurses (RN) and nurse assistants (NA) in long-term care hospitals. Method: The sample consisted of 203 nursing personnel (RN 99 and AN 104), who were working in one of 5 long-term care hospitals in 5 different areas. Data were collected by structured questionnaires from March 15 to April 30, 2010, and analyzed using the descriptive statistics, independent t-test, One-way ANOVA, and Pearson correlation coefficients with SPSS/Win 17.0 program. Result: There was no significant difference between nurses and nurse assistants in job satisfaction and self-esteem. But nurses had higher levels of nursing task performance than nurse assistants. Conclusions: These findings demonstrate the necessity of developing programs that will help to improve job satisfaction and self-esteem in nurses and nursing task performance in nurse assistants. Strategies need to be developed to maximize the value of the existing workforce without impacting on the quality of care delivered.

전국 의료기관의 냉 요법 실무지침 및 임상간호사의 냉 요법 실무 현황 (Nursing Protocols of Cold Application in Different Medical Institutions in Korea)

  • Purpose: The purpose of this study was to evaluate nursing protocols for superficial cryotherapy in different medical institutions. Methods: The study was conducted with a cross-sectional descriptive design. The medical institutions including general hospitals, hospitals, and geriatric hospitals were randomly selected. A total of 435 nurses from 126 institutions completed a questionnaire. Data were collected from December 2014 to June 2015. Results: Forty-two institutions (39.5%) had nursing protocol for cryotherapy. The nurses reported that durations of cold application were 2 minutes to 5 hours. Frequently used cold therapy devices in order of frequency were frozen gel packs, ice packs, and frozen IV fluid bags. There were variances in the duration of cold application according to the types of institution ($x^2=7.78$, p=.020) and nursing units ($x^2=26.42$, p<.001). In addition, intervals of cold application were different according to the nursing units (x=12.23, p=.032). There were differences in cold application instruments by regional groups (x=70.38, p<.001). Most of the nurses (95.6%) responded that national nursing protocol for superficial cryotherapy were needed. Conclusion: There were difficulties in providing consistent nursing interventions because of the practical differences and absence of evidence-based guidelines for cryotherapy. The researchers recommend that basic studies with various instruments be conducted and proper nursing protocols be developed for cryotherapy.

의료기관 근무 간호조무사의 직무만족 영향요인 (Factors Influencing Job Satisfaction among Nursing Assistants Working in Medical Institutions)

  • Purpose: The purpose of this study was to identify the influential factors on job satisfaction among nursingassistants working in medical institutions. Methods: Small and medium-sized hospitals and general hospitals (S City, U City, N City), 11 Geriatric hospitals(S City, N City, U City, T City, I City), and 29 clinics (S City, I City, N City). Data was collected from 194employed nursing assistants from February 15 to April 15, 2021. The obtained data were analyzed usingdescriptive statistics, ANOVA, Scheffe's test, Pearson's correlation analysis, and hierarchical regression analysis. Results: We confirmed a positive correlation between job satisfaction and organizational commitment, andconducted a hierarchical multiple regression analysis using monthly income, work-life balance, organizational commitment, and work confidence as influencing factors for job satisfaction, and found that organizational commitment and work confidence were significant influencing factors for job satisfaction. Conclusion: To improve the job satisfaction of nursing assistants, it is necessary to improve their understanding of the organizational culture in the workplace and expand education and training opportunities to enhance their work confidence to induce long-term retention. In addition, it was suggested that clear job scope and adequatecompensation are necessary for efficient human resource management.

B광역시 4개 요양병원 입원노인의 우울 관련요인 (The Factors Related to Depression of the Elderly patients in Four Geriatric Hospital Located in B Metropolitan City)

  • Purpose: The purpose of this study was to identify the factors related to depression of elderly patients in geriatric hospitals. Methods: The subjects were 195 elderly patients who met the inclusion criteria of scores more than 18 on the K-MMSE score and no reported mental disease. The data were collected from February 20 to March 20, 2009. The research instruments utilized in this study were depression (GDSSF-K), activities of daily living and self esteem (RSES), social support, life satisfaction. Data were analyzed Pearson correlation and Multiple Stepwise Regression using SPSS 15.0. Results: Depression score were negatively correlation with ADL, social support, life satisfaction and self-esteem. Among the factors studied related to depression, life satisfaction had highest explanatory power of 36.5% and it was followed by physical health status and activity of daily living. These explained 43.7% of the depression. Conclusion: The mean GDSSF-K 8.94, which indicates the higher than middle levels of depression. The findings suggest that it is important to develop educational programs to increase life satisfaction, physical health status and activity of daily living. Nursing interventions, including volunteer activities, health promotion program, and sports program could be useful in enhancing these factors.

요양병원 간호사를 위한 전문직 자아개념 (Nurses' Self-Concept Instrument, NSCI) 도구의 타당도와 신뢰도 (Validity and Reliability of the Nurses' Self-Concept Instrument (NSCI) for Registered Nurses in Long-term Care Hospitals)

  • Purpose: The purpose of this study was to evaluate the validity and reliability of the Korean Version of the Nurses' Self-Concept Instrument (NSCI) geriatric hospital nurses in Korea. Methods: Bilingual nursing professionals performed translation and back-translation. Reliability and validity of the content and construction of the instrument were confirmed. Internal consistency reliability was determined. Construction and concurrent validity were verified using factor analysis and correlation coefficients. Results: The total 14 items for the Korean version of the Nurses' Self-Concept Instrument (NSCI) were retained through item analysis. In explanatory factor analysis, four subcategories were proposed with their names of each factor: 'Leadership', 'Staff relation', 'Knowledge', and 'Care'. The four factors accounted for 78.81% of the variances. The Cronbach's ${\alpha}$ regarding internal consistency were .77~.91 for the NSCI subscales. Correlation among four subcategories ranged .62~.84. Conclusion: The findings show that the Korean version of the Nurses' Self-Concept Instrument is reliable and valid for measuring professional Self-Concept of geriatric hospital nurses in Korea.

요양병원 간호사의 공감능력과 치매 환자 통증에 대한 태도 및 통증관리 수행도 (Geriatric Hospital Nurses' Empathy, Attitude and Pain Management for Patients with Dementia)

  • Purpose: The purposes of this study were to identify nurses' empathy, attitude and pain management for patients with dementia and the factors associated with their performance of pain management. Methods: The study design was a cross-sectional survey. The participants in the study were 114 nurses working at 12 geriatric hospitals. This study utilized the Interpersonal Reactivity Index (IRI), which measures four empathy factors. Nurses' attitudes toward pain and performance of pain management, and general and pain related characteristics were measured by self-administered questionnaires. Results: Barriers to pain management for patients with dementia were absence of guidelines, prejudice of pain in dementia, and lack of time and knowledge deficit. There was a significant positive correlation between empathic concern and attitudes, perspective-taking of empathy and pain management. There was also a correlation between empathy and pain management. Stepwise linear regression analysis indicated that the significant factors affecting the performance of pain management included perspective-taking of empathy, use of pain management in dementia guideline and attitudes toward pain. These factors explained 24.0% of variance. Conclusion: The findings from this study suggest that empathy and positive attitude are the important provider attributes which needs to be enhanced by educational programs. It is also necessary to develop and disseminate guidelines for a dementia specific pain management.

노인요양병원 간호사의 병원감염관리에 대한 인지도 및 실천도: 융복합적 접근 방안 모색 (Perception and practice of hospital infection control in nurses of geriatric hospital : for convergent approach)

  • 본 연구는 융복합적 접근을 위해 노인요양병원 간호사의 병원감염관리에 대한 인지도와 실천도를 파악하고, 제변수간의 관계를 조사하는 연구이다. Lee(2011)가 사용한 병원감염관리 설문지를 이용하여 2012년 10월 29일부터 11월 3일 까지 자료를 수집하였고, 수집된 자료는 t-test, ANOVA 및 Correlation 으로 분석하였다. 병원감염관리 실천도의 평균 점수는 인지도 평균 점수 보다 통계적으로 유의하게 낮았다. 영역별 인지도와 실천도는 손씻기 영역, 호흡기감염관리 영역, 소독 오염물품관리 영역, 감염성 폐기물관리 영역에서 통계적으로 유의한 차이가 있었으며 실천도가 인지도보다 낮았다. 인지도는 연령, 학력, 감염관리지침서 유무에 따라 차이가 있었으며, 실천도는 감염관리지침서 유무에서만 차이가 있었다. 병원감염관리 인지도와 실천도는 통계적으로 유의한 정적상관관계가 있었다. 따라서 본 연구결과는 노인요양병원 감염관리에 대한 인지도와 실천도를 높이기 위한 제도적 지원과 더불어, 지속적이고 다양한 교육방법을 통해 체계적이고 전문적이며 실제적인 교육을 제공하는 것이 필요하다는 것과 병원감염관리 능력을 증진시키기 위한 프로그램을 개발할 필요를 제기한다.

노인요양병원 요양보호사의 피부감염에 대한 인식 및 감염관리 수행 (Recognition of Skin Infection and Infection Management Practice on Caregivers in Geriatric Hospital)

  • 목적: 본 연구는 노인요양병원 요양보호사의 피부감염 인식과 감염관리 수행 정도를 파악하기 위한 서술적 조사연구이다. 방법: 본 연구는 G시와 J도에 소재한 7개 노인요양병원에서 근무하고 있는 요양보호사 209명을 대상으로 실시하였다. 수집된 자료는 SPSS 18.0 program을 이용하여 ${\chi}^2$-test, t-test와 ANOVA로 통계 분석하였다. 결과: 노인요양병원 근무 시 새로 생긴 피부감염은 76.6%가 '예'라고 응답하였으며, 피부감염의 진단은 접촉 피부염 42.5%, 옴 26.9% 그리고 습진 25.0% 였다. 피부감염 인식은 평균 3.81점이었고 감염관리 수행 정도는 412점 이었다. 대상자의 일반적 특성에 따른 감염관리 수행에서는 병상수(F=4.63, p=.011)와 하루 동안 돌보는 환자 수(F=2.67, p=.049)에서 통계적으로 유의한 차이가 나타났다. 결론: 본 연구결과는 요양보호사를 위한 보수교육 및 의료감염관리 표준지침 개발을 위한 기초자료로 활용할 수 있을 것이다.

요양병원 간호제공자의 좋은 죽음인식, 영적안녕이 노인간호수행에 미치는 영향 (Effects of Good Death awareness and Spiritual Well-being on Elderly Nursing Performance of Geriatric Hospital Nursing providers)

  • 본 연구의 목적은 요양병원 간호제공자의 좋은 죽음인식, 영적안녕, 노인간호수행 정도를 파악하고 노인간호수행에 미치는 영향요인을 파악하기 위함이다. 연구대상은 D광역시, S시, C도에 위치한 요양병원 5곳의 간호제공자 176명을 대상으로 구조화된 설문지를 이용하여 자료 수집하였다. 수집된 자료는 SPSS 22.0을 이용하여 평균, 표준편차, t-test, ANOVA, Pearson correlation, 다중회귀분석을 통해 분석하였다. 연구결과 좋은 죽음인식은 평균점수 4점 만점에 3.15점, 영적안녕은 6점 만점에 4.11점, 노인간호수 행 정도는 5점 만점에 4.15점으로 나타났다. 노인간호수행과 좋은 죽음인식(r=0.19, p=.011), 영적안녕(r=0.23, p=.002)과의 관계는 정적 상관관계가 있었다. 노인간호수행에 영향을 미치는 요인으로 좋은 죽음인식(${\beta}=.18$, p=.015) 영적안녕(${\beta}=.18$, p=.013)순 이었다. 본 연구결과를 바탕으로 요양병원 간호제공자의 노인간호수행 향상을 위하여 좋은 죽음인식, 영적안녕을 고려한 중재프로그램 개발이 필요하다.

일회성 웃음치료가 노인요양병원 업무종사자의 직무스트레스와 직무몰입에 미치는 효과 (Effect of one time laughter therapy on job stress and job involvement of geriatric hospital workers)

  • 본 연구는 일회성 웃음치료가 노인요양병원 업무종사자의 직무스트레스와 직무몰입에 미치는 효과를 확인하기 위한 비동등성 대조군 전후 설계의 유사실험연구이다. 연구대상자는 J도 I시 소재 노인요양병원 두 곳의 업무종사자로 실험군과 대조군 각 40명씩이었다. 대상자에게 일회성 웃음치료 적용후 전후 혈압, 맥박, 혈중 코티졸 농도를 측정하였고, 설문지를 통하여 직무스트레스와 직무몰입을 측정하였다. 수집된 자료는 SPSS Win 19.0을 이용하여 빈도, 백분율, 평균, 표준편차 X2 test, Fisher's exact test, t-test를 실시하였다. 연구결과 대상자의 수축기혈압, 혈중 코티졸 농도는 실험군이 대조군에 비해 유의하게 감소하였고 직무스트레스와 직무몰입은 실험군과 대조군에서 유의한 차이가 없었다. 따라서 일회성 웃음치료는 노인요양병원 업무종사자의 스트레스의 생리적 반응에 영향을 주어 스트레스 감소에 효과가 있는 간호중재임이 확인되었다.
