Spinal cord stimulation in chronic pain: technical advances

  • Chronic severe pain results in a detrimental effect on the patient's quality of life. Such patients have to take a large number of medications, including opioids, often without satisfactory effect, sometimes leading to medication abuse and the pain worsening. Spinal cord stimulation (SCS) is one of the most effective technologies that, unlike other interventional pain treatment methods, achieves long-term results in patients suffering from chronic neuropathic pain. The first described mode of SCS was a conventional tonic stimulation, but now the novel modalities (high-frequency and burst), techniques (dorsal root ganglia stimulations), and technical development (wireless and implantable pulse generator-free systems) of SCS are becoming more popular. The improvement of SCS systems, their miniaturization, and the appearance of new mechanisms for anchoring electrodes results in a significant reduction in the rate of complications and revision surgeries, and the appearance of new waves of stimulation allows not only to avoid the phenomenon of addiction, but also to improve the long-term results of chronic SCS. The purpose of this review is to describe the current condition of SCS and up-to-date technical advances.

Effect of Relative Humidity on the Breakthrough of Charcoal Tubes during Mixed Organic Vapor Sampling (혼합 유기용제 포집시 습도가 활성탄관의 파과에 미치는 영향)

  • This study was designed to investigate effects of relative humidity on the breakthrough of charcoal tubes at a fixed vapor concentration and sampling time during mixed organic vapor sampling. A vapor generator was used to generate three different concentrations of mixed organic vapor and a stainless steel chamber was fabricated and utilized to maintain three different percentages of relative humidity while maintaining a constant temperature. The results were as follows; 1. At high relative humidity, breakthrough of mixed organic vapor occurred quickly at low vapor concentration than at high vapor concentration because of the reduced adsorption volume of charcoal tube due to humidity. 2. Breakthrough by competitive adsorption of vapors onto charcoal tube was observed at first from n-hexane having the lowest boiling point and highest vapor pressure among the three organic vapors investigated, followed by TCE. No breakthrough was observed from toluene under all experimental conditions. 3. For n-hexane, breakthrough was observed after 2 hours of sampling and breakthrough rates were increased as relative humidity increased. For TCE, breakthrough was found after 3 hours of sampling and breakthrough rates by sampling time were increased as vapor concentration increased. 4. The adsorbed amount of mixed organic vapor at breakthrough was shown to have statistically significant correlations with sampling time, relative humidity, and vapor concentration in descending order of correlation. Relative humidity and sampling time for n-hexane and sampling time and concentration for TCE were both statistically significantly correlated. 5. Relative humidity was found to affect the amount of breakthrough of mixed organic vapor and n-hexane. Among three percentages of relative humidity investigated, the amount of breakthrough at 85 % relative humidity was significantly larger than those of at lower percentages of relative humidity. No statistically significant difference was found between 25 % and 55 % relative humidity. 6. The results of multiple regression analysis between breakthrough and relative humidity, vapor concentrations showed that the coefficient of determination of mixed organic vapor was 0.263 and those of n-hexane and TCE were 0.275 and 0.189, respectively. 7. Flow rates of sampling pumps used were found to be affected by relative humidity present. At 25 %, 55 %, and 85 % relative humidity, the relative errors of sampling pump were 1.4 %, 13.4 %, and 18.6 %, respectively. In conclusion, the results of this study showed that high relative humidity could reduce the adsorption volume of charcoal tubes and subsequently increase breakthrough rates. Therefore, to prevent breakthrough when sampling mixed organic vapors, it is suggested that either sampling volume be reduced on the flow rate be lowered so as to minimize breakthrough of the most volatile organic vapor in the mixture. In addition, since the flow rates of a sampling pump can be adversely affected by high relative humidity, it is recommended to use a constant flow mode pump when sampling in the highly humid environment.

Dispersion of Particle Pulse in Human Lung Airway (인체기관지내의 입자펄스 확산 실험)

  • In order to develop the aerosol bolus technique which is thought to be a potential tool for probing geometries or abnormalities of small airways, an experimental system of measuring fast time variations of particle concentration in the inhaled and exhaled breathing air was developed. The system generates monodisperse sebacic acrid particles of 1 micron size and 1.2 of geometric standard deviation in high concentration of $10^8$ particles/cc, delivers a short pulse of particles at the controlled instant during inhalation using a solenoid valve, and measures the fast change of particle concentration in using the laser light scattering. Successful operation of the generator and the measuring system was confirmed by smooth concentration profiles in inhalation. It was also confirmed that maintaining a constant breathing rate is essential to stable outputs and any disturbance in flow rate near the mode (maximum concentration) induces a large number of spurious peaks in the exhalation. Experimental data without strict control of breathing flow rate showed a substantial amount of scatter. The measured results showed an improvement in scatter over the existing results. When compared with theoretical predictions from 1-D convective diffusion equation and other experiments, general characteristics of dispersion for several penetration depths showed a good agreement, but there exists some difference in absolute values, which is attributed to the difference in body conditions. Improvements are needed in the theory, especially in relation to correcting for the effect of breathing flow rate.

Low-Power Operation Method of Thermal-Energy Harvesting Sensor Circuit (Thermal Energy Harvesting용 센서회로의 저전력 구동 방법)

  • In this paper, we propose low-power operational methods for thermal-energy-harvesting sensor circuits. Here, the amount of harvested current has been measured as low as 8uA. However the DC power consumption of the sensor circuit is known to consume much larger than 8uA. Thus, We propose the hardware-based power gating and software-based active/sleep timing control schemes, respectively, for controlling the power consumption of sensor circuit. In the hardware-based power gating scheme, if the ratio of Toff/Ton is larger than 22, the sensor can consume less than 8uA. For the software-based active/sleep control scheme, if the ratio of Tslp/Tact is larger than 3, we can suppress the current consumption below 8uA. The hardware-based and software-based schemes proposed in this paper would be helpful in various applications of energy-harvesting sensor circuits, where the power consumption is limited by an amount of harvested energy.

  • The new spectrometer for X-ray Induced Electron Emission Spectroscopy (XIEES) .has been recently developed in KRISS in collaboration with PTI (Russia). The spectrometer allows to perform research using the XAFS, SXAFS, XANES techniques (D.C.Koningsberger and R.Prins, 1988) as well as the number of techniques from XIEES field(L.A.Bakaleinikov et all, 1992). The experiments may be carried out with registration of transmitted through the sample x-rays (to investigate bulk samples) or/and total electron yield (TEY) from the sample surface that gives the high (down to several atomic mono-layers in soft x-ray region) near surface sensitivity. The combination of these methods together give the possibility to obtain a quantitative information on elemental composition, chemical state, atomic structure for powder samples and solids, including non-crystalline materials (the long range order is not required). The optical design of spectrometer is made according to Johannesson true focusing schematics and presented on the Fig.1. Five stepping motors are used to maintain the focusing condition during the photon energy scan (crystal angle, crystal position along rail, sample goniometer rail angle, sample goniometer position along rail and sample goniometer angle relatively of rail). All movements can be done independently and simultaneously that speeds up the setting of photon energy and allows the using of crystals with different Rowland radil. At present six curved crystals with different d-values and one flat synthetic multilayer are installed on revolver-type monochromator. This arrangement allows the wide range of x-rays from 100 eV up to 25 keV to be obtained. Another 4 stepping motors set exit slit width, sample angle, channeltron position and x-ray detector position. The differential pumping allows to unite vacuum chambers of spectrometer and x-ray generator avoiding the absorption of soft x-rays on Be foil of a window and in atmosphere. Another feature of vacuum system is separation of walls of vacuum chamber (which are deformed by the atmospheric pressure) from optical elements of spectrometer. This warrantees that the optical elements are precisely positioned. The detecting system of the spectrometer consists of two proportional counters, one scintillating detector and one channeltron detector. First proportional counter can be used as I/sub 0/-detector in transmission mode or by measuring the fluorescence from exit slit edge. The last installation can be used to measure the reference data (that is necessary in XANES measurements), in this case the reference sample is installed on slit knife edge. The second proportional counter measures the intensity of x-rays transmitted through the sample. The scintillating detector is used in the same way but on the air for the hard x-rays and for alignment purposes. Total electron yield from the sample is measured by channeltron. The spectrometer is fully controlled by special software that gives the high flexibility and reliability in carrying out of the experiments. Fig.2 and fig.3 present the typical XAFS spectra measured with spectrometer.

An Experimental Study on the Stability of Breakwater Head by the Wave Directional Effects (입사파의 방향성효과에 의한 방파제 제두부의 안정성에 관한 실험적 연구)

  • The aim of this study is to check the application criteria of the conventional techniques and clarify the effects of breaker depth, seabed conditions on the stability in relation to the effects of uncertainty of storm duration and directional irregular waves. The typical damage modes were divided by the direct wave force on the armor unit and by the local scouring around the toe of a breakwater head by the model experiments. The destruction modes are defined, and some criteria on the damage modes and scouring/deposition at the toe of a breakwater head in relating the wave-bottom-structural conditions can be checked using the multi-directonal irregular wave generator system. According to the results, it is emphasized that the 3-D effects on the stability should be analyzed in the design of multi-purpose/function coastal structures in consideration of the evaluation of spatial variation of damage modes and hydraulic characteristics as well as the wave distribution along the structures.

  • In this paper, a single-bit 2nd-order delta-sigma modulator with the architecture of cascaded-of-integrator feedforward (CIFF) is proposed for precision measurement of current flowing through a secondary cell battery in a battery management system (BMS). The proposed modulator implements two switched capacitor integrators and a single-bit comparator with peripheral circuits such as a non-overlapping clock generator and a bias circuit. The proposed structure is designed to be applied to low-side current sensing method with low common mode input voltage. Using the low-side current measurement method has the advantage of reducing the burden on the circuit design. In addition, the ±30mV input voltage is resolved by the ADC with 15-bit resolution, eliminating the need for an additional programmable gain amplifier (PGA). The proposed a single-bit 2nd-order delta-sigma modulator has been implemented in a 350-nm CMOS process. It achieves 95.46-dB signal-to-noise-and-distortion ratio (SNDR), 96.01-dB spurious-free dynamic range (SFDR), and 15.56-bit effective-number-of-bits (ENOB) with an oversampling ratio (OSR) of 400 for 5-kHz bandwidth. The area and power consumption of the delta-sigma modulator are 670×490 ㎛2 and 414 ㎼, respectively.

  • XML is a meta-language as SGML and also can be xonsructed as an Internet versionof simplified SGML being used in confunction with XLL. Xpointer and XSL. Also W3C established DTDless Well-Formed XML document to use XML document on the Web. But it isnt offered system that consists of browsing, link and DTD generating facihty, and efficiently processes DTDless Well-Formed XML document. This paper studies on an implementation and design of system to process DTDless Well-Formed XML document on the client-side. This system consists of Well-Formed XML viewer displaying Well-Formed XML documet, XLL Processor processing Xll and Auto DTD generator constructing automatically DTDs based on multiple documents of the same class. This study focuses on automatic DTD generation during hyperlink navigation and an implementation of extended links based on XLL and Xpointer. ID and Xpointer location address are used as the address mode in the links. As a result of implement of this system, it conforms to validationof extended link facihties, extracts DTD from Well-Fromed XML Documents including same root element at the same class and constructs generalized DTD.

  • In this paper, a low-power and small-area asynchronous 1 kilobit EEPROM for passive UHF RFID tag chips is designed with $0.18{\mu}m$ EEPROM cells. As small area solutions, command and address buffers are removed since we design asynchronous I/O interface and data output buffer is also removed by using separate I/O. To supply stably high voltages VPP and VPPL used in the cell array from low voltage VDD, Dickson charge pump is designed with schottky diodes instead of a PN junction diodes. On that account, we can decrease the number of stages of the charge pump, which can decrease layout area of charge pump. As a low-power solution, we can reduce write current by using the proposed VPPL power switching circuit which selects each needed voltage at either program or write mode. A test chip of asynchronous 1 kilobit EEPROM is fabricated, and its layout area is $554.8{\times}306.9{\mu}m2$., 11% smaller than its synchronous counterpart.

Techno-Economic Analysis of Reversible Solid Oxide Fuel Cell System Couple with Waste Steam (폐스팀을 이용한 가역 고체산화물 연료전지의 기술적 경제적 해석)

  • Reversible solid oxide fuel cell (ReSOC) system was integrated with waste steam for electrical energy storage in distributed energy storage application. Waste steam was utilized as external heat in SOEC mode for higher hydrogen production efficiency. Three system configurations were analyzed to evaluate techno-economic performance. The first system is a simple configuration to minimize the cost of balance of plant. The second system is the more complicated configuration with heat recovery steam generator (HRSG). The third system is featured with HRSG and fuel recirculation by blower. Lumped models were used for system performance analyses. The ReSOC stack was characterized by applying area specific resistance value at fixed operating pressure and temperature. In economical assessment, the levelized costs of energy storage (LCOS) were calculated for three system configurations based on capital investment. The system lifetime was assumed 20 years with ReSOC stack replaced every 5 years, inflation rate of 2%, and capacity factor of 80%. The results showed that the exergy round-trip efficiency of system 1, 2, 3 were 47.9%, 48.8%, and 52.8% respectively. The high round-trip efficiency of third system compared to others is attributed to the remarkable reduction in steam requirement and hydrogen compression power owning to fuel recirculation. The result from economic calculation showed that the LCOS values of system 1, 2, 3 were 3.46 ¢/kWh, 3.43 ¢/kWh, and 3.14 ¢/kWh, respectively. Even though the systems 2 and 3 have expensive HRSG, they showed higher round-trip efficiencies and significant reduction in boiler and hydrogen compressor cost.
