촬영자세별 척추측만각과 척추전만각의 비교 분석에 따른 개선 방안 (Technical Improvement for Spine Radiography by Comparing Scoliotic and Lordotic Angle with Different Positioning Methods)

  • 척추촬영법은 의료기관별로 상이하고 각종 대학 및 국가시험을 위한 교재 등에 상이하게 기술되어 혼선을 빚고 있는 실정이다. 이에 동일한 환자의 선자세와 누운자세에서 촬영한 영상을 바탕으로 각 자세에서 척추측만각과 척추전만각의 차이를 비교함으로써 임상 및 관련기관, 참고서적에 가장 적절한 촬영 자세를 정립하여 제시하고자 하였다. 2009년 3월부터 2011년 9월까지 척추질환 환자 중 수술적 치료를 받지 않은 85명을 대상으로 Cobb's method를 이용하여 척추전만각(lordortic angle) 및 척추측만각(scoliotic angle)을 측정하였다. 각각의 측정자료는 t-test(SPSS 18)를 이용하여 유의성을 분석하였으며, 임상의 실태 파악을 위해 서울소재 10곳의 대학병원을 대상으로 척추촬영의 자세와 시행근거를 설문으로 조사하였다. 척추측만각의 측정 결과, 선자세가 바로누운자세보다 평균 20.98% 증가하였으며, 통계적으로 매우 유의한 차이를 보였다(p<.01). 척추전만각은 지지대잡은자세가 측와위자세보다 29.3% 증가하였으며, 통계적으로 매우 유의하였다(p<.01). 의료기관의 실태 결과, 누운자세촬영이 70.0%로 선자세촬영의 30.0%보다 월등하게 나타났다. 결론적으로 선자세촬영은 누운자세촬영보다 척추측만각 및 척추전만각이 증가하여 임상적 진단과 치료에서 중력이 가해진 선자세 촬영의 중요성과 일치하였으므로 임상의 검사자는 그 차이를 인식하여 필히 정확한 선자세에서 촬영이 요구된다.

ROC평가 방법을 이용한 CR과 DDR 흉부 영상의 비교 (The Evaluation of CR and DDR chest image using ROC analysis)

  • 본 연구의 목적은 ROC를 이용하여 일반 촬영기기에 따른 영상의 질을 평가해보고자 함이다. 본 연구의 수행을 위하여 CR(Computed Radiography)과 DDR(Direct Digital Radiography)을 사용하였으며 피사체로 흉부 팬텀을 사용하였다. 각 기기에서 영상을 획득한 후 ROC평가를 이용하여 영상의 질 및 기기의 특성을 평가하였다. 조사 조건으로 관전압 120kVp와 관전류량 3.2 mAs를 이용하였고 SID(Source to Image Distance)는 180cm로 설정하였다. 팬텀의 심장, 폐야, 흉추부위에 병소를 표현하였으며 각 장비에서 획득한 영상의 질 및 기기의 특성을 파악하기 위하여 방사선학 전공자 29명을 대상으로 ROC평가를 실시하였다. ROC 평가 결과 DDR의 TPF(true positive fraction)는 0.552, FPF(false positive fraction)는 0.474, CR의 TPF는 0.629, FPF는 0.405로 나타났다. 본 연구 결과 CR의 영상이 DDR의 영상보다 더 나은 영상의 질을 나타내는 것을 확인하였다. 영상의 질의 확연한 차이의 원인은 DDR의 경우 enhance board의 미 삽입으로 인한 영상 후처리의 미수행이라고 사료된다. 추후 DDR의 enhance board의 삽입 후 영상의 후처리가 가미된 DDR영상의 질에 대한 연구가 필요하며 본 연구의 결과로 인하여 영상의 후처리가 임상의 판독에 있어서 매우 중요한 요소임을 확인할 수 있었다.

3D 프린터를 이용한 발목관절 검사 보조기구의 유용성연구 (A Study on the Usefulness of an Ankle Joint Examination Assistive Device using a 3D Printing)

  • Mortise view 검사는 발목관절 검사로써 발목관절의 외상이나 염좌, 탈구가 의심되는 병변의 유무를 관찰한다. mortise view 검사 시 사용되는 보조기구는 영상에 인공물을 발생하거나, 종류가 다양하지 않아 환자 맞춤형으로 제작되지 않고 가격이 비싸다. 이에 본 연구는 3D 프린팅 기술을 이용하여 mortise view 검사보조기구(ShinHan Device; SHD)를 제작하였다. SHD를 사용했을 때와 시제품인 hologic tool을 사용했을 때의 목말종아리관절, mortise 관절, 목말정강관절의 길이를 5명의 연구자가 측정하여 평가하였다. SHD를 사용했을 때 평균값의 범위는 TTJ에서 39.42~39.47 mm, TCJ 31.41~31.57 mm, MJ 21.21~21.23 mm의 범위로 나타났다. hologic tool를 사용했을 때는 TTJ 39.73~39.79 mm, TCJ 31.46~31.50 mm, MJ 21.31~21.35 mm의 범위로 측정되었다. 본 연구결과, 모든 위치에서의 오차범위가 hologic tool에 비해 SHD를 사용 때 TTJ는 24%, TCJ는 17%, MJ는 36% 감소하였으며, SHD를 사용하였을 때 재현성 높은 검사자세를 구현할 수 있었으며(ICC=0.99), hologic tool에서 발생하던 인공물을 제거한 영상을 획득할 수 있었다.

치과의료기관별 의료폐기물 관리 현황 파악 (Identifying Medical Waste Management Status by Different Types of Dental Institutions)

  • Objectives: We aimed to examine whether dental waste was being managed adequately at different types of dental institutions in City D in South Korea. Methods: The staff responsible for disinfection at 101 dental offices and clinics (six dentistry departments of general hospitals, 12 dental hospitals, and 83 dental clinics) was interviewed. Results: Solid suction pump waste was handled appropriately at four of the general hospital dentistry departments (66.7%), six dental hospitals (50.0%), and 15 dental clinics (18.1%). Solid spittoon waste was handled appropriately at four general hospital dentistry departments (66.7%), seven dental hospitals (58.3%), and 14 dental clinics (16.9%). Developer and fixer were handled appropriately by a subcontractor at two general hospital dentistry departments (100.0%), five dental hospitals (100.0%), and 24 dental clinics (75.0%). Impression materials were handled appropriately at four general hospital dentistry departments (66.7%), six dental hospitals (50.0%), and 11 dental clinics (13.3%). The plastic covers of intra-oral radiography films were handled appropriately at five general hospital dentistry departments (100.0%), eight dental hospitals (72.7%), and 22 dental clinics (30.1%). Conclusion: South Korea must implement detailed and specialized guidelines for the disposal of solid and general medical waste from dental institutions. Moreover, waste disposal training should be provided annually, and not only once every three years.

Anatomical Characteristics and Surgical Treatments of Pincer Nail Deformity

  • Background Pincer nail deformity is a transverse overcurvature of the nail. This study aimed to define the anatomical characteristics of pincer nail deformity and to evaluate the surgical outcomes. Methods A retrospective review was conducted on 20 cases of pincer nail deformity of the great toe. Thirty subjects without pincer nail deformity or history of trauma of the feet were selected as the control group. Width and height indices were calculated, and interphalangeal angles and base widths of the distal phalanx were measured with radiography. We chose the surgical treatment methods considering perfusion-related factors such as age, diabetes mellitus, kidney disease, and peripheral vascular disease. The zigzag nail bed flap method (n=9) and the inverted T incision method (n=11) were used to repair deformities. The outcomes were evaluated 6 months after surgery. Results The interphalangeal angle was significantly greater in the preoperative patient group ($14.0^{\circ}{\pm}3.6^{\circ}$) than in the control group ($7.9^{\circ}{\pm}3.0^{\circ}$) (P<0.05). The postoperative width and height indices were very close to the measurements in the control group, and most patients were satisfied with the outcomes. Conclusions We believe that the width and height indices are useful for evaluating the deformity and outcomes of surgical treatments. We used two different surgical methods for the two patient groups with respect to the perfusion-related factors and found that the outcomes were all satisfactory. Consequently, we recommend taking into consideration the circulatory condition of the foot when deciding upon the surgical method for pincer nail deformity.

Bacterial Contamination of Digital Panoramic Dental X-Ray Equipment

  • Background: Digital panoramic dental X-ray equipment (PDX) is frequently used by patients and dental workers for diagnosis and examination in dental institutions; however, infection control has not been properly implemented. Therefore, in this study, we aimed to systematically review the potential risk of cross-infection in the dental environment by investigating the contamination level of general aerobic bacteria and Staphylococcus aureus, which are important in hospital infections, in PDX areas that people mainly contact. Methods: This survey was conducted from March to May 2023 and covered one general hospital, three dental hospitals, and nine dental clinics equipped with PDX. Bacteria samples were collected from the left-handle, right-handle, forehead support, and head side support as the patient's contact areas, as well as the X-ray exposure switch and left-click mouse button as the dental hygienist's contact areas of the PDX. The collected bacteria were spread on Petrifilm, and colonies formed after 48 hours of culture were counted. Results: General aerobic bacteria and S. aureus were detected in all areas investigated. Significant differences in bacterial counts between different regions of the PDX were observed in both groups (p<0.001). The detection rates of general aerobic bacteria (p<0.001) and S. aureus (p<0.001) were significantly higher in the contact areas of patients than those of dental hygienists. A positive correlation was observed between the forehead and the temple region in terms of general aerobic bacteria and S. aureus detection (r=1) (p<0.01). Conclusion: Taken together, the presence of many bacteria, including S. aureus, detected in PDX indicates that PDX has a potential cross-infection risk. Our results therefore highlight the need for the development of appropriate disinfection protocols for reusable medical devices such as PDX and periodic infection prevention training for hospital-related workers, including dental hygienists.

미만성 침윤성 폐질환의 진단: HRCT와 단순흉부X선사진의 비교 (Diffuse Infiltrative Lung Disease : Comparison of Diagnostic Accuracies of High-Resolution CT and Radiography)

  • 연구배경 : 미만성 침윤성 폐질환의 진단에 있어 HRCT와 단순흉부X선사진의 역할을 알아보고 진단의 정확성을 비교하고자 하였다. 방법 : 단순흉부X선사진과 HRCT를 시행 하였고, 임상적으로 그리고 병리조직학적으로 확진된 99명의 DILD환자와 12명의 정상인을 대상으로 하였다. 99명의 DILD환자들은 20종류의 급성 및 만성질환을 포함하였다. 단순흉부X선사진과 HRCT는 독립적으로 3명의 흉부 방사선과 의사가 진단명을 모르는 상태에서 임상적 정보 없이 각각 평가하였고, 3개의 가능성이 많은 진단을 선택하였으며 그중 첫번째로 가능한 진단명은 그 진단의 확신도를 3등급으로 나누어 기록하였다. 결과 : HRCT와 단순흉부Xtjs사진은 DILD의 검출에 있어서 민감도가 각각 98.9%와 97.9%이었다. 첫번째 가능한 진단명 중 그답이 옳은 경우는 단순흉부X선사진에서 48%, HRCT에서 60%에서 이루어 졌고, 3개의 감별진단을 모두 포함하였을 때는 단순흉부X선사진의 64%, HRCT의 75%에서 정확한 진단을 하였다. 확신도가 높은 첫번째 진단은 HRCT에서 55%로 단순흉부X선사진의 26% 많았다. 특히 확신도가 높은 첫 번째 HRCT진단이 옳은 경우는 UIP, 속립성 폐결핵, 미만성 세기관지염, 림프행성 폐전이에서 83-93%이었다. 결론 : HRCT와 단순흉부X선사진 모두 DILD의 검출에 있어서 매우 좋은 검사법이다. HRCT는 단순흉부X선사진에 비해 DILD 각 질환의 진단에 보다 정확하다. 특히 HRCT는 임상에서 자주 접하는 UIP, 속립성 폐결핵, 미만성 세기관지염, 림프행성 폐전이의 진단에 매우 유용하며, 드문 질환이지만 BOOP, 폐 조직구 증식증, 폐포성 단백증, LAM 등을 정확하게 진단할 수 있다. 또한 이러한 HRCT 진단들은 단순흉부X선사진 진단에 비해 신뢰도가 높고 정확하다.

Incidental findings in a consecutive series of digital panoramic radiographs

  • Purpose: The aim of this study was to determine the prevalence of incidental findings(IFs) on digital dental panoramic radiographs(DPRs) of asymptomatic patients attending a general dental practice. Materials and Methods: This was a retrospective study of 6,252 consecutive digital (photostimulatable phosphor) DPRs of patients who visited a Canadian general dental practice for a complete new patient examination. The IFs were grouped into dental-related anomalies, radiopacities and radiopacities in the jaws, changes in the shape of the condyles, and other findings in the jaws, such as tonsilloliths and mucosal antral pseudocysts. Their prevalence was determined. Results: Thirty-two percent of the DPRs showed at least 1 IF. The highest prevalence was found for dental-related anomalies(29% of all DPRs), of which impacted teeth were the most prevalent finding (24% of all DPRs), followed by idiopathic osteosclerosis(6% of all DPRs). A lower prevalence was noted for tonsilloliths(3%), and the prevalence of root tips, mucosal antral pseudocysts, and anomalies in condylar shape was approximately 1% each. Conclusion: The observed prevalence of 32.1% for IFs of any type underscores the need for a dental practitioner to review the entire DPR when a patient presents for an initial dental examination (or check-up) or for dental hygiene. Only a single IF (a central giant cell granuloma) provoked alarm, as it was initially considered malignant. Similarly, impacted teeth and suspected cysts need careful evaluation upon discovery to determine how they may be optimally managed.

Brazilian young dental practitioners' use and acceptance of digital radiographic examinations

  • Purpose: The aim of this study was to investigate the use and acceptance of digital radiographic examinations by Brazilian dental practitioners in daily practice and to evaluate the advances that have occurred over the past 5 years. Materials and Methods: Dental practitioners enrolled in extension courses at the Piracicaba Dental School, University of Campinas, Brazil, responded to a self­administered questionnaire in the years 2011 and 2015. They were asked about sociodemographic factors and their knowledge and use of digital radiographic examinations. Descriptive analysis was performed, as well as the chi-square and Fisher exact tests, with a significance level of 5% (${\alpha}=0.05$). results: A total of 181 participants responded to the questionnaire in the years 2011 and 2015. Most of the respondents worked in private practice, had graduated within the last 5 years, and were between 20 and 30 years old. In 2011, 55.6% of respondents reported having ever used digital radiographic examinations, while in 2015 this number increased significantly to 85.4% (p<.0001), out of which 71.4% preferred it to conventional images. Moreover, 21.4% of respondents reported having used digital radiographic examinations for more than 3 years. A significant increase in use of intraoral digital radiography (p = 0.0316) was observed in 2015. In both years, image quality and high cost were indicated, respectively, as the main advantage and disadvantage of digital radiographic examinations. conclusion: This study showed that digital radiology has become more common in Brazil over the past 5 years. Most of the Brazilian dental practitioners evaluated in 2015 used digital radiographic examinations.

Dental imaging characterization of micropigs

  • Recently the micropig has been developed as human disease model. The dental and orofacial region of micropig is similar to that of humans, so it has been used for testing implant materials and techniques. The purpose of this study is on dental image at each age using radiography and computed tomography. Total twenty-two male micropigs, two or three animals of each 1, 3, 5, 7, 9, 12, 18 and 24 months old, were given radiographic examinations. After general anesthesia, extra- and intra-oral radiographic technique and computed tomographic scans were performed to assess the dental characterization of micropigs. The total deciduous dental formula comprised 28 teeth and was depicted as Di 3/3, Dc 1/1, Dp 3/3. The total permanent dental formula comprised 44 teeth and was depicted as I 3/3, C 1/1, P 4/4, M 3/3. Hypodontia of the first premolars was common in the micropig. The permanent teeth erupted from 3 to 24 month after birth. The sequence of eruption of the permanent teeth was M1, P1, I3, C, M2, I1 + P3 + P4, P2, I2, M3. Dental imaging enables visualization of the unerupted teeth and gives more information about the development of the teeth. The growth pattern of the teeth obtained through radiographic and computed tomographic examination provides basic data in the micropig as animal model for dental research.
