The Clinical Effect with the Use of Gel Anesthesia within Gingival Sulcus during Scaling

  • Although scaling is the primary method for improving oral health, it is also associated with dental fear. The objective of this study was to empirically verify whether the use of gel anesthetic within the gingival sulcus during scaling relieves pain and improves other factors. A total of 128 patients scheduled to undergo scaling at a dental clinic of a general hospital located in the Gyeonggi Province, between July 2014 and July 2015, were enrolled in the study. The participants underwent scaling following the application of 20% benzocaine gel or placebo gel anesthetic within the gingival sulcus, and the data was collected using a questionnaire. There was a significant difference in the severity of pain, participant satisfaction, perceived sensitivity, overall discomfort, and fear of scaling between the two groups. The two groups were compared in terms of perceived need for gel anesthesia, willingness to pay for anesthesia costs, and willingness to receive scaling in the future. There were significant differences in all the three parameters depending on whether gel anesthesia was used or not. There were significant differences between the two groups in perceived sensitivity immediately after scaling and one day after scaling, with no difference seen one week after scaling. With regards to overall discomfort over time, there were significant differences between the two groups immediately after scaling. Based on these findings, we expect that application of gel anesthetic within the gingival sulcus during scaling will reduce pain, perceived sensitivity, overall discomfort, and fear of scaling with increased satisfaction.

임부의 사회심리적 적응과 분만경험 지각에 대한 연구 (Psychological Adaptation in Pregnancy and Perception of Birth Experience)

  • The purposes of study were to investigate the prenatal psychological adaptation and the perception of birth experience, and to identify the relationship between them. The subjects consisted of 162 women who visited the obstetrical outpatient clinic for prenatal examinations and who delivered the in babies at SNUH during the period from June 20 to August 10, 1990. The tools used for measurement were Lederman's Prenatal Self Evaluation Questionnaire and Marut & Mercer's scale of the Perception of Birth. The results are summarized as follows : 1. The orders and item means of psychosocial adaptation in pregnancy were the Acceptance of pregnancy(1.58). Identification of motherhood role(1.63). Relationship with husband(1.65) and Relationship with mother(1.67). The preparation for labor, concern for wellbeing of self and baby, and fear of pain, helplessness and loss of control were found to be less adaptive. 2. The level of the perception of the birth experience was mid-range(item mean : 3.22). The score of the perception of birth experience for primiparas was higher than for multiparas. However there was not a significant difference the groups. There were significant differences in the perception of the birth experience between certain general characteristics, namely, sex of the baby(p<0.05), type of delivery(p<0.005), and type of anesthesia(p<0.005). 3. There were significant differences in the perception of the birth experience between the groups below the mean and above the mean of concerti for wellbeing of self and baby, Fear of pain, Helplessness and loss of control, Relationship with husband and Identification of motherhood role (p<0.05). The perception of the birth experience was predicted by Fear of pain, Helplessness and loss of control (11%), Type of Delivery(6%), Concern for wellbeing of self and baby(3%), Preparation for labor(1%), sex of baby(1%), Relationship with mother(1%), Parity(1%) and Identification of motherhood role(1%). The Childbirth education should be revised to improve the psychosocial adaptation in pregnancy.

호스피스 자원봉사자 교육이 죽음에 대한 성향에 미치는 효과

  • The purpose of this study was to promote the popularization of hospice services by providing the information about the influences of hospice training on participants. We compared differences of attitudes toward death between those of pre-training and post-training by use of questionnaire. This study involved 137 volunteers participating in the hospice training held by one hospice center located in K-city. The questionnaire was composed of 46 items, 21 items of general information and 25 items of information about attitudes toward death. We applied Park's translated version(1992), originally distributed by Thorson and Powell(1988), in the assessment of participants' changed attitudes toward death. Participants were asked to fill out the questionnaire before and after the hospice training. The data were analged by frequency. ANOVA t-test, paired t-test. The results were as follows, 1.Pre-training items holding a high rank were 'The subject about the future life after death is distressing'(F=5.20), 'No worry about any occurrences around the body after a funeral'(F=4.75), 'Having little interest in being laid in a coffin after death'(F=4.56); items holding a low rank were 'Having little interest in any occurrences around the body after death'(F=2.22), 'Willing to deal with the aftermath of the death in detail'(F=1.94), 'No fear to have an idea to be cancerous'(F=1.72). 2.Post-training items holding a high rank were 'No fear to be dead as a result of sufferings from long-lasting illness'(F=3.18), 'No fear to have an idea to be cancerous'(F=3.16)', Having little interest in the future life after death'(F=3.09); items holding a low rank were 'Unpleasant to be not able to move after death(F=1.74), 'Distressing not to know about the future life after death'(F=1.61), 'The subject about the future life after death is distressing'(F=1.60). 3.There were significant difference in participants' attitudes toward death. Compared with pretraining(F=3.97), more positive were developed in post training(F=2.30). The results of this study indicate that hospice training renders positive regards toward death to the participants. Therefore every human being, who is able to speculate about the meaning of death, has to be exposed to the hospice training. In conclusion, hospice training should be popularized in the near future.

시군구 수준의 사회자본 추이와 사회자본과 COVID-19 관련 행위와 인식 간의 관계 (Social Capital Trends and the Relationship between Social Capital and COVID-19-Related Behaviors & Perceptions)

  • Background: The influence of social capital on the spread of coronavirus disease 2019 (COVID-19) and related behaviors and perceptions has been recognized during the COVID-19 pandemic. This study aims to analyze the trends in social capital using primary data from the Korean Community Health Survey, which is the only available source in Korea for local-level social capital analysis. It also investigates the relationship between various variables, including social capital, as factors influencing COVID-19-related behaviors and perceptions. Methods: The study analyzed the temporal trends of social capital using raw data from four community health surveys conducted in 2017, 2019, 2020, and 2021. A multilevel analysis was conducted to examine the relationship between social capital and COVID-19-related behaviors and perceptions following the onset of the COVID-19 pandemic in 2020. Results: Social capital consists of trust, bonding social capital, and bridging social capital. Within the trust sub-factor, trust in neighbors (Trust-1) declined after the COVID-19 pandemic, whereas trust in safety and general environment (Trust-2) and trust in medical services and public transportation (Trust-3) increased. Additionally, the gap between municipalities narrowed. COVID-19-related behaviors and perceptions, such as adherence to COVID-19 prevention measures, return to normal activities, and fear of COVID-19, showed improvement in 2021 compared to the previous year. Individual-level trust in neighbors was associated with reduced fear of COVID-19, while community-level trust in neighbors was associated with increased fear of COVID-19. Conclusion: Social capital plays a role in mitigating public health crises, and it is necessary to implement active policies that address the gap in social capital between metropolitan and rural areas. Strengthening risk communication regarding emerging infectious diseases such as COVID-19 is crucial.

간접 경험 및 롤 모델이 창업의지에 미치는 영향 (Influence of Indirect Experiences and Role Models on Entrepreneurial Intentions)

  • 본 연구는 창업의지에 미치는 영향을 분석하고 어떤 요인이 가장 큰 영향을 미치는지에 대해 실증적으로 분석하기 위해 이루어졌다. 이를 위해 창업의지에 미치는 영향요인으로 자아성취동기, 간접경험, 롤모델, 사업가에 대한 인식, 실패부담감을 독립변수로 설정하였다. 이를 실증적으로 분석하기 위하여 일반인을 대상으로 유효한 데이터 399개를 분석 대상으로 하였다. 통계분석에는 SPSS 25.0를 활용하였다. 본 연구의 실증 분석 결과, 자아성취동기, 간접경험, 롤모델, 사업가에 대한 인식, 실패부담감은 창업의지에 유의한 정(+)의 영향을 미치는 것으로 나타났다. 본 연구 결과는 창업의지에 영향을 미치는 요인 중에서도 자아성취동기가 가장 큰 영향을 미치는 것을 확인하였으며, 이론형 창업강좌, 창업특강, 부모나 지인들의 창업 유무 등 간접경험과 롤모델이 창업의지에 영향을 미치는 것을 실증적으로 분석하였다는 측면에서 의의가 있다.

암환자의 대처양상에 관한 연구 (A Study to the Coping Patterns of Cancer Patients)

  • Purpose: This was to describe the emotions that patients face when diagnosed with cancer to know the problems and coping styles that cancer patients experience during the treatment. Method: The qualitative method was used for this study. The participants were 90 cancer patients at five general hospital in Pusan, Korea. Data were collected by interviews with a semi-structured questionnaire from November 2000 to June 2001, and were analyzed using the modified constant comparative method. Result: The most common emotions on the diagnosis of cancer were shock(36.7%), followed by despair(25.6%), acceptance(24.4%), denial(l8.9%), complaint(16.7%), and fear(8.9%). The problems identified were the unpleasantness and physical discomfort related with the treatment(50.0%), the feelings of burden(41.1%), finance/occupation(38.8%), and fear of the future(26.6%). Coping styles to problems that the subjects have faced on the course of the treatment were compliance(36.6%), health care(31.l %), positive thinking(22.2%), despair/avoidance( 15.5%), seeking social support( 6.6%), information seeking(3.3%) and self-control(2.2%). Conclusion: It is very important to develop nursing interventions which can mitigate shock that patients experience, can help cancer patients to have hope for the future and to positively cope with cancer.

노인의 죽음 준비교육이 죽음의 불안도에 미치는 요인분석 연구 (A study on the influence of the preparative education on the Elderly's attitude for death)

  • This study attempt to analyze the influence of the preliminary education on the Elderly's attitude for death and to present basic data for the death-preliminary education. The data were collected by administerial the Questionnaire interview with 169 the elderly who was 200 the elderly over 60 year old in silver colleges. The Questions was consists the awareness recognition of death, character, attitudes toward for death. The statistical methods used for the analysis were t-test, factor analysis. The results were the follows. There was no statistically significant relations between the fear of death and the general characteristics the elderly, but the old women felt more anxiety than old men. Especially, more aged, unhealthy the elderly felt it more and the lower educated or the single felt it more severely. The change in the attitude for death: They attitude for death was considerably changed after the preliminary education. Fears and anxiety about death were more reduced and the inevitability of death was accepted positively. This result showed the influence of the preliminary death education had positive affliction of the elderly's attitude for death. Accordingly, with the practice of the preliminary education we can release the elderly from the fear for death and guide them to live meaningly.

조혈모세포이식 병동에 관한 문화기술지: 환자를 중심으로 (Ethnography on Isolation Unit for Hematopoietic Stem Cell Transplantation: Focusing on Patients)

  • Purpose: The purpose of the study was to understand how patients experience everyday life in an isolation unit for hematopoietic stem cell transplantation (HSCT). Method: The data were collected from 25 patients with HSCT at the isolation unit from January to March in 2008 in one general hospital in Korea. The data were collected by participant observations and ethnographic interviews and were analyzed using ethnographic method. Results: Four themes regarding environmental area emerged: 'barrier pulling up the drawbridge', 'very strange world', 'small and restricted space tied by IV and other treatment lines', and 'loud noise in a silent space.' Three themes regarding patients emerged: 'facing fear and anxiety', 'continuation of loneliness and lethargy', and 'compromising with a very long, dull, and boring time'. These themes describe how patients with HSCT suffer from continuous physical and psychosocial problems in a confined space, while endeavoring to control these problems and to search for hope for a new life. Conclusion: The results of the study provide an in-depth understanding of the experience and culture of patients in an isolation unit for HSCT. They would be used in developing practical programs to decrease patient's culture shock including fear and anxiety at isolation unit for HSCT.

소아암 환아와 가족에 대한 이해 (Understanding Pediatric Cancer Patients and Their Families)

  • In this thesis, Chapter I Introduction suggested the necessity of this research and defined related terms, and Chapter II defined hospice for children and examined the symptoms of pediatric cancers as well as the general characteristics of pediatric cancer patients. In particular, we surveyed the physical condition, psychological and emotional condition, financial condition, environmental aspect, educational aspect and spiritual aspect of pediatric cancer patients’ families, investigated pediatric cancer patients’ parents and siblings with regard to their understanding of the pediatric cancer patients’ death, and lastly considered spiritual care. Chapter III presented summaries and conclusions. In their developmental stage, pediatric cancer patients lack abilities to express themselves and are highly dependent on their parents, so parents who take care of cancer children have to make hard decisions and cancer children’s families are heavily burdened by the situation of preparing their children’s death and sending them away while denying their death, and for this reason they need help from specialists. That is, for pediatric cancer patients, we need highly experienced pediatricians or nurses skilful in managing young terminal patients as well as hospice counseling and family counselors for consulting on family crises. In particular, there is a keen need of child life support specialists. In addition, clergymen’s help is critical for spiritual care to ease the fear and terror of the unknown world, fear of death, etc. Moreover, in order to prevent cancer children from failing to adjust themselves to school life or peer relation after recovery, hospice service should provide cancer children with opportunities to learn school curriculums and associate with friends.

Nursing Students' Experience of Sexual Harassment During Clinical Practicum: A Phenomenological Approach

  • Purpose: To describe nursing students' experience of sexual harassment during clinical practicum. Methods: An interpretive phenomenological qualitative approach was used to understand contextual experiences of participants. Individual in-depth interviews were conducted to collect data from thirteen nursing students who experienced sexual harassment during clinical practice in general hospitals at D metropolitan city. All interviews were recorded and transcribed into Korean and English. Transcripts were analyzed using the data analysis method described by Diekelmann, Allen, and Tanner. Results: The following 12 themes emerged from the data: 'unprepared to respond', 'lack of education', 'unsure about when behavior crosses the line', 'power differential for nursing students', 'balancing self-preservation with obligations to patients', 'shame', 'feeling responsible for not being able to prevent the harassment', 'impact on patient care', 'fear of what might have happened', 'fear of repercussions', 'long term impact', and 'peer support'. Conclusion: Participants in this study described feeling an obligation to care for their patients. However, they seemed to be unable to balance this while feeling vulnerable to sexual harassment with strong negative feelings. Helping students recognize and effectively deal with sexual harassment is a critical element to assure quality learning for participants and maintain quality of care during clinical practice.
