간호사의 이직결정과 관련요인에 관한 연구 (Factors influencing the decisions of nurses to resign)

  • This study was to investigated the factors influencing the decisions of nurses to resign. The purpose was to contribute information useful for the effective management of nurse staffing resources. The specific objectives were as follows : 1. To investigate the level of job satisfaction of nurses who resign. 2. To analyze the relationship of job expectations and the decisions to resign. 3. To investigate the major reason for nurses' resignations. 4. To identify the relationship of the general characteristics of the nurses and the factors influencing their decisions to resign. 5. To analyze the relationship of the intention to resign and the factors influencing the decisions to resign. The sample consisted of 90 nurses who worked at Y hospital in Seoul from march. 1987 to Feb. 1988 and who had resigned. The instruments used for this study were slavitt et al's Index of Work Satisfaction Scale and a job expectations scale developed by the researcher Analysis of data was done using frequency, t-test and ANOVA. The results of study were as follows : 1. There was no difference in the job satisfaction level of nurses who resigned compared with nurses in active service but the subcategory interaction score was lower and the subcategory salary score was higher than for nurses in active service. 2. Low job expectation scores influenced the decision to resign. 3. The major reason for resigning were personal problems such as marriage, childbirth, and relocation of family. 4. The length of employment and the number of different service areas of the nurses who resigned were associated with the job satisfaction level. 5. The nurses who had have an intention to resign while in active service had lower scores for job satisfaction and the job expectation score was a greater influence on the decision to resign. In conclusion, the factors influencing the decisions of nurse to resign were job satisfaction, job expectations and intention to resign while still in active service.

교대 여부에 따른 기혼간호사의 역할갈등과 직무만족도 (A Comparative Analysis on the Married Nurses' Role Conflict and Job Satisfaction in Accordance with Shift Work)

  • Purpose: The purpose of this study is to investigate effects of shift work on married nurses' role conflict and job satisfaction. Method: Study subjects consisted of married nurses employed at four general hospitals with over 500 beds located in Seoul and Kyunggi area. The data were collected from March 15th to April 7th, 2004 using the survey and the subjects were asked to reply through self-administrative method. Results: Mean value of role conflict of nurses who worked shift work was measured 3.12 out of 5, and that of nurses who did not was measured 2.98. Mean value of job satisfaction of nurses who worked shift work was measured 2.79 out of 5, and that of nurses who did not was measured 2.86. There was a significant reverse correlation between mean level of role conflict and of job satisfaction. Both groups, nurses who worked shift work and nurses who did not, showed significant reverse-correlation in two areas. In the case of nurses who worked shift work, role conflict not showed a significant correlation. Job satisfaction showed a significant correlation with the plan to work as a nurse and remuneration in the case of nurses who worked shift work. Duty of supporting family was the factor affecting role conflict and measured 4.2% in the case of nurses who worked shift work, and the plan to work as a nurse and the academic background in the case of nurses who did not work shift work. Conclusion: To reduce role conflict and to improve the degree of job satisfaction, there should be taken a measure to improve a financial compensating system for working shift work for nurses who worked shift work, and to help nurses not working shift work to design plans for their job as nurses with stability.

교대근무 간호사의 임신경험에 대한 융합적 연구 (A convergence study on the Pregnant experience of shift nurses)

  • 본 연구의 목적은 교대근무 간호사의 임신 경험에 대하여 현상학적 분석을 통해 그 의미의 본질을 심층적으로 이해하고자 함이다. 이에 구체적인 연구문제는 '교대근무 간호사가 경험하는 임신의 의미는 무엇인가?'이다. 본 연구를 위해 3년 이내에 임신한 상태에서 종합병원에서 근무한 경험이 있는 간호사 6명이 참여하였다. 자료 수집은 개별 인터뷰 방법을 사용하였으며, 분석은 van Mannen(1990)의 해석학적 현상학 연구 방법을 적용하였다. 연구 결과 총 8개의 주제와 4개의 본질적 주제가 도출되었다. 각각의 본질적 주제는 '마음 놓고 임신하기 어려움', '동료간호사의 눈치를 봄', '태아의 건강이 걱정됨', '실효성 있는 대책을 요구함'이었다. 본 연구 결과는 교대근무 간호사의 임신경험에에 대한 융합적 이해를 제공한 것에 그 의의가 있다.

간호·간병통합서비스병동 간호사의 업무 적응 경험 (Experience of Adaptation to Work In Comprehensive Nursing Service Units of Nurses)

  • 본 연구는 간호·간병통합서비스병동 간호사의 업무 적응 경험을 심층적으로 파악하기 위해 현상학적 방법으로 시행되었다. 본 연구의 자료수집 기간은 2023년 11월~2024년 1월까지였으며, 일반 병동과 간호·간병통합서비스병동에서 각각 6개월 이상의 업무 경험이 있는 간호사를 대상으로 심층 면담이 이루어졌다. 심층 면담 시 녹음한 파일은 모두 전사 후 Giorgi의 현상학적 방법으로 분석하였다. 분석결과, 5개의 구성요소와 14개의 하위 구성요소가 도출되었으며 구성요소는 '변화된 업무 환경을 마주침', '예상하지 못했던 업무의 이질감', '당면한 현실을 평가함', '전략을 찾아 균형을 도모함', '전문직 간호사로 성장함'으로 나타났다. 이러한 연구결과를 바탕으로 간호·간병통합서비스병동 간호사가 업무에 적응할 수 있는 시스템을 구축하고 역량 강화를 위한 프로그램을 지속적으로 개발할 필요가 있다.

임상간호사의 간호사 윤리강령 인식의 영향 요인; 생명의료 윤리의식, 도덕적 민감성, 비판적사고 성향을 중심으로 (Factors influencing the recognition of clinical nurses' code of ethics; Focusing on consciousness of biomedical ethics, moral sensitivity, and critical thinking deposition)

  • 본 연구는 임상간호사의 생명의료 윤리의식, 도덕적 민감성, 비판적 사고성향 및 간호윤리강령인식 정도를 확인하고, 간호사 윤리강령 인식에 영향을 미치는 요인을 확인하고자 시도된 서술적 조사연구이다. 대상자의 일반적·윤리적특성은 빈도, 백분율, 평균과 표준편차로 산출하였고, 일반적·윤리적 특성에 따른 각 변수별 차이는 SAS 9.4를 이용하여, independent t-test, one-way ANOVA로 분석하였고, 이후 유의한 차이가 발견된 변수는 Scheffé 사후 검정을 이용하여 다중 비교하였다. 각 변수의 점수는 평균과 표준편차, 최소 값과 최대 값을 이용하였고, 변수 별 상관관계는 Pearson correlation coefficient로 분석하였다. 또한 간호사 윤리강령 인식에 영향을 미치는 요인을 파악하기 위하여 multiple regression으로 분석하였다. 대상자의 일반적·윤리적 특성과 변수 간의 상관관계는 모두 유의한 양의 상관관계를 보였다. 마지막으로 간호사 윤리강령 인식에 가장 크게 영향을 미치는 변수는 비판적 사고성향(p<.001)이었고, 다음으로 생명의료 윤리의식(p<.001), 도덕적 민감성(p<.001) 순으로 영향하는 것으로 나타났으며, 이에 대한 설명력은 27.4%이다. 따라서 임상간호사들에게 비판적 사고성향, 생명의료 윤리의식, 도덕적 민감성 등을 향상시킬 수 있는 교육 프로그램을 개발하고, 이를 적용시킴으로써 임상간호사들이 확고한 윤리적 가치관을 가지고 실무에 임할 수 있도록 도움을 주어야 할 것이다.

간호사의 최상의 임종간호 및 웰다잉 관계 연구 (The Relationship between Optimal End-of-Life Care and Well Dying of Nurses)

  • 본 연구는 환자와 환자 가족에게 최상의 임종간호를 제공하기 위해 간호사가 인식하는 최상의 임종간호와 웰다잉 간의 관계를 조사하였다. 연구대상은 서울 소재 종합병원에 근무하는 간호사 121명을 대상으로 2021년 4월 28일부터 5월 12일까지 수집된 자료를 분석하였다. SPSS/WIN 27.0을 이용하여 t-test, ANOVA, Pearson correlation coefficients로 분석하였다. 본 연구의 결과 대상자의 최상의 임종간호에 대한 인식의 총 평균은 5점 척도상 4.04±0.82점으로 최상의 임종간호에 전반적으로 '동의한다' 수준을 나타냈다. 웰다잉에 대한 인식의 총 평균은 5점 척도상 3.63±1.14점으로 평화롭게 죽는 것을 가장 중요하게 인식하였다. 대상자의 일반적 특성 중 연령에 따른 최상의 임종간호에 유의한 차이를 나타내(t=-.97, p=.013) 30대 간호사군이 20대 간호사군에 비해 최상의 임종간호에 더 높은 동의수준을 나타냈다. 최상의 임종간호와 웰다잉의 상관관계를 분석한 결과 유의한 상관관계는 없는 것으로 나타났다. 본 연구의 결과는 경력이 낮은 젊은 간호사들의 임종간호 인식을 높이기 위한 교육이 제공되어야 함을 시사한다.

종합병원에 근무하는 간호인력의 감성리더십과 조직성과 간의 관계 (The relationship between emotional intelligence in leadership and organizational performance of nurses in general hospitals)

  • In order to effectively attain the objectives of an organization, it is important for the leader to provide an environment where members can co-exist and mutually advance, and also to have an emotional impact on them. This study examines the structural relationship among emotional intelligence in leadership, organizational commitment, job satisfaction, turnover intention, and organizational performance effects of emotional leadership in the nursing profession of a general hospital. A structured questionnaire was sent to 550 nurses in 11 general hospitals in Seoul Metropolitan City, among which 350 responses were used in the analysis. The independent variable, emotional intelligence in leadership, was measured by 18 items, including self-awareness, self-regulation, social awareness and relationship management. The dependent variable, organizational effectiveness, consisted of 46 items, including 15 items regarding organizational commitment, 20 items on job satisfaction, 3 items on turnover intention and 8 items on organizational performance. The structural equation modeling technique was used to identify the impact of emotional intelligence in leadership on organizational effectiveness. The results of the analysis show that the emotional intelligence in leadership of superior nurses has a significant impact on the nurses'organizational commitment and job satisfaction. While the direct effect on turnover intention and organizational performance was not significantly high, organizational commitment and job satisfaction were parameters in low turnover intention and high job performance. In conclusion, emotional intelligence in leadership is critical as nursing services in hospitals are based on interpersonal relationships. Therefore, by developing appropriate programs and training, hospitals can anticipate improved self-awareness, self-regulation, social awareness and relationship management of nurses, and further improve the effectiveness of the nursing profession.

입원 기간 중 관상동맥 우회술 환자를 위한 심장재활 교육의 효과 (Effect of Cardiac Rehabilitation Education for Coronary Artery Bypass Grafting Patients during Their Hospitalization)

  • The purpose of this study was to evaluate the effect of cardiac rehabilitation education for CABG patients on anxiety & depression, self-esteem, general health status, health related quality of life, body mass index, serum lipid concentration (high density lipoprotein, low density lipoprotein, total cholesterol), heart rate and blood pressure. The subjects of the study consisted of 36 patients who underwent CABG at A hospital in Seoul, from January 1, 2006 to June 30, 2007. The experimental group (20 patients) received the individual education during the hospitalization. The control group (16 patients) received routine discharge education by ward nurses. Psychological variables, general health status and health related quality of life (QOL) after cardiac rehabilitation education were determined by self-reported questionnaire at 1st week, 3rd week and 6th week after the discharge. Physiological variables were measured in out patient visiting at 4th week after discharge. Score of anxiety & depression and health related QOL in the experimental group showed a significant decrease at 1st week, 3rd week and 6th week after the discharge compared with the control group. Score of self-esteem, general health status and physiological variables in the experimental group showed no significant difference from the control group.

항경련제를 복용하고 있는 지적장애 환자의 외래전신마취 하 치과치료 후 합병증 발생 조사 연구 (Analysis of Complications of Ambulatory General Anesthesia after Discharge in Patients Who are Taking Anticonvulsant)

  • Background: About 60% of patients with epilepsy showed a variety of complications after returning home. The purpose of this study is to get further information and to help in the anesthetic management of the disable patients with epilepsy. Methods: After searching the outpatient anesthesia list of Seoul National University Dental Hospital clinic of the disabled from September 2010 to March 2012, we found total 83 patient cases who were taking antiepileptic drugs. A nurse of anethesiology department called to the patient's home after general anesthesia and got surveys with questionnaires form filled with complications at home. We enrolled total of 51 patients who agreed to participate this survey. And we also reviewed pre-anesthesia evaluation sheet, anesthesia record, and recovery and discharge record. Results: 11 of 51 (21.6%) patients had seizure convulsion at home within 1 weeks. 33.4% of patients who had at least once seizure attack per week had a seizure attack within 3 days. And 50% of once a month frequency patients had a seizure attack within 1 week. 33% of 1 - 3 seizure attacks per year patient group had a convulsion within 1 week. But there was no seizure attack within 1 week in whom had no seizure attack history during more than 1 year. Conclusions: After dental treatment, patient taking anticonvulsant have so many complications-especially seizure, more than we respected. We need more research about handle these problems.

임상간호사의 셀프리더십, 자기효능감, 조직몰입이 안전관리활동에 미치는 영향 (The Effects of self-leadership, self-efficacy, and organizational commitment on safety Management activities in clinical nurses')

  • 본 연구는 종합병원 간호사를 대상으로 셀프리더십, 자기효능감, 조직몰입이 안전관리활동에 미치는 영향을 파악하기 위한 서술적 조사연구이다. 연구대상은 G시에 위치한 3개의 종합병원 간호사로 코로나19 펜데믹 상황임을 고려하여 온라인 설문을 실시하였다. 자료수집은 2021년 8월 20일부터 8월 30일이었다. 연구결과, 셀프리더십(r=.618, p<.001), 자기효능감(r=306, p<.001), 조직몰입(r=0518, p<.001)은 안전관리활동과 양의 상관관계를 보였다. 또한 셀프리더십, 자기효능감, 조직몰입이 안전관리활동을 38.2%로 설명하였다 (F=20.478, p<.001). 그러므로, 종합병원간호사의 환자관리활동을 높이기 위해 정기적인 리더십 교육, 자기효능감 교육 등을 실시한다면 도움이 될 것이다. 또한 조직몰입 향상을 위해 긍정적인 업무환경 조성과 간호사 업무에 대한 법적 기준 마련, 적절한 보상 체계의 마련이 된다면 환자안전관리활동에 긍정적 효과가 있을 것이다.
