Effect of Drug Use during Pregnancy (약물이 임신에 미치는 영향)

  • In recent year, problems caused by the abuse of drugs hove been analyzed in many cases, especially women's pregnancy. The purchasing of drugs without prescription, the misunderstanding of symptoms of pregnancy(such os vomiting, headache) os other ilnnesses, taking medicine during the pregnancy because of a chronic disease has caused many unfortunate cases Apart from these cases, pregnant women may take several medicines such as anodyne, tranquilizers, hypnotics, and diuretics which also cause critical situations. According to Piper and his colleagues 1987 study, in overage, pregnant women in the United Slates intake 3.1 kinds of additional dregs other than prenatal vitamins and mineral supplements. In those cases, both pregnant women and physicians anticipate inborn deformity. Most drugs which have whole body effects get to the unborn child via the placenta, however, many of these drugs do not have adverse effects(Shepard 1986, 1989). In general, drugs present a stronger effect to unborn children than they do to pregnant women due to the baby's excretion and to the drug metabolism that occurs in the mother's body through the placenta. The effects of dregs on unborn children show different results. depending upon the type of drug, dosage, characteristics. gestational weeks, genetic characteristics of the mother an baby, and many other environmental factors.

Survey for the Recognition and Performance rate in the Hospital Pharmacists on the Safety Rules about Hazardous Drugs (병원 약사들의 위해약물 안전 수칙의 인지도 및 수행도에 대한 조사연구)

  • The objective of this study was to evaluate the recognition and performance rates about the safety rules on hazardous drugs among the selected pharmacists. The 160 pharmacists working in 4 general hospitals and in the other 4 institutions specialized in the oncology division were surveyed through mail. Among the 137 respondents to the survey (response rate 85.6%), 111 pharmacists (81%) had recognized the terms of 'hazardous drugs'. In categories of vaccines and hormones, the degrees of the recognition rate were much lower than the cytotoxic medications. It was surveyed that the degree of recognition and performance of safety rules on injectable drugs were higher than the disposal and noninjectable medications. The higher recognition rate of the safety rules made the higher degree of performance. These results were expected to provide the incentive for guidelines on handling hazardous drugs based on Korean healthcare system.

New drug classification system in accordance with global harmonization (글로벌 조화에 부합하는 국내 의약품 분류체계 개선방안)

  • The objective of this study was to investigate drug classification system in Korea and other developed countries. Laws and regulations of Korea regarding the system were retrieved from sources posted in Ministry of Government Legislation. We also reviewed previous research reports performed as part of government's effort to reform the system The system in the foreign countries was retrieved from the official homepage operated by each country's government. There have been two research funded by Korean government, which strongly suggested that the system should be reformed. However, we found that the system was never reformed and still effective. Drug classification system in US and most western countries consists of two categories, i.e., prescription drugs and non-prescription drugs except UK, which classifies into three categories: Prescription Only Medicines, Pharmacy Medicines, and General Sales List Medicines. Interestingly, in Japan, non-prescription drugs are further classified into three groups: Group 1, 2, and 3. Recently, Ministry of Health and Welfare (MOHW) in Korea proposed a plan to reclassify all the approved drugs according to purportedly rational and scientific criteria. However, the plan does not include reform of the existing laws and regulations, which appears that it is just one-time action rather than a sustainable administration backed up by law. Therefore, it is recommended that Korean MOHW take appropriate action on laws and regulations with regard to the system to meet global harmonization standard.

Trends on the Curtailment of Drug Expenditure Before and After the Seperation between Prescription and Dispensing in General Hospitals By Drug Types (의약분업 전후 일부 종합병원의 약제종류별 약제비 삭감추이)

  • Fiscal crisis in the medical insurance has put the pressure upon hospitals by increasing the rate of curtailment, since the implementation of the separation between prescription and dispensing of Drug. The purpose of this study is to analyze the curtailment for antibiotics, injected drug and other drugs expenditure before and after the system of separation between prescribing and dispensing. Data were gathered from 13 general hospitals and used for analysis of trends on antibiotics and injected drug expenditure, and curtailment in 2000-2001 at three months intervals. The results were as follows; The curtailment rate of antibiotics expenditure has been increased in outpatient and inpatient since 2000. The curtailed antibiotics cost and injected drug cost in outpatient under the prescription within the hospital and in inpatient increased. The ratios of curtailment versus expenditure had increased in antibiotics, injected drugs, anticancer drugs, antiulcer drugs, albumine, antiinflammatory drugs. These results suggest that claim review system in social health insurance were over-focused mainly to control the cost and it might to impede the validity of claim review function in health insurance system. Therefore, it's needed to develope the scientific and reasonable parameter & criteria for claim review of drug expenditure.

Analysis of Influencing Factors on the Outpatient Prescription of Antipsychotic Drugs in the Elderly Patients (노인환자의 항정신병 약물 원외처방 내역에 미친 영향 요인 분석)

  • Background: Most antipsychotic drugs studies have been mainly conducted on side effects, randomized clinical trials, utilization rates, and trends. But there have been few studies on the influencing factors in elderly patients. The purpose of this study was to analyze the influencing factors on the outpatient prescription of antipsychotic drugs in the elderly patients. Methods: Active ingredients of antipsychotic drugs in Korea were selected according to the Korean Pharmaceutical Information Center (KPIC)'s classification. Data source was Korean Health Insurance Review and Assessment Service (HIRA) claims data in 2020 and target patient group was the elderly patient group. We extracted patients who have been prescribed one or more antipsychotic drugs and visited only one medical institution. Data were analyzed using descriptive statistics, chi-square, t-test, negative binomial regression. Results: A number of outpatients were 245,197 and prescriptions were 1,379,092. Most characteristics of patients were 75-85 year's old, female, health insurance type, no disease (dementia, schizophrenia), atypical drugs, cci score (>2) and characteristics of medical institution were neurology in specialty, rural region, general hospitals. Results of regression showed that patient's characteristics and medical center characteristics had significant effect on the outpatient prescription of antipsychotic drugs in the elderly patients. Conclusion: This study suggests that national policy of antipsychotic drugs in the elderly patients, with the consideration of the patients' and medical institutions' characteristics, is needed.

Drug Interaction Review of Prescriptions for Outpatients at General Hospital (종합병원의 외래환자 처방전에 대한 약물상호작용 검토)

  • To investigate drug interaction, 23,536 prescriptions published for 1 year were investigated with 'Drug Inter­action Fact 2002'. Dispensing records and a database file written in a local general hospital in South Korea were used as a sample. The number of total cases of drug interaction was 3,238 ($13.76\%$) out of 23,536 prescriptions. The incidence of drug interaction in each prescription the children, the adults, and the elderly were $1.33\%,\;10.97\%,\;25.50\%$, respectively. The incidences of drug interaction per each prescription were $22.03\%,\;20.52\%,\;0.51\%,\;and\;0.36\%$ in neurosurgery, internal med­icine, pediatrics, and orthopedics, respectively. In neurosurgery and internal medicine, risk-high drugs of drug interaction such as antihypertensive drugs, diuretics, and cimetidine were used very often in elderly. In this paper, several suggestions to reduce drug interaction were postulated with regard to the usage of analgesics, non-steroidal antiinflammatory drugs, and antibiotics.

  • Day stay anesthesia should include: rapid smooth onset; rapid recovery without residual side effects; absence of adverse effects (N/V); and providing postoperative analgesia. General anesthesia with multi-modalities (inhalation, intravenous and local anesthesia) may be preferable in day stay surgery. Future studies on new drugs and techniques for day stay anesthesia need comparing the increased coat of newer treatments with the potential financial savings resulting from earlier hospital discharge, reduced supplemental drugs, and earlier return to work.

Stevens-Johnson syndrome and abuse of anabolic steroids

  • Stevens-Johnson syndrome (SJS) is characterized by mucocutaneous tenderness and typical hemorrhagic erosions, erythema and epidermal detachment presenting as blisters and areas of denuded skin. SJS is often observed after drug use as well as after bacterial or viral infections. Several drugs are at high risk of inducing SJS, but there are no cases in the English literature regarding anabolic steroid use triggering SJS. In our paper, we describe a case in which use of anabolic androgenic steroids (AAS) was associated with SJS. The patient participated in competitive body-building and regularly took variable doses of AAS. Initial symptoms (headache, weakness, pharyngodynia, and fever) were ignored. After a week he presented to the Emergency Department with a burning sensation on the mouth, lips, and eyes. Painful, erythematous, maculopapular, and vesicular lesions appeared all over the body, including on the genitals. During hospitalization, he also developed a cardiac complication. The patient had not taken any drugs except AAS.

Development of Rapid and Simple Drug Identification and Semi Quantitative Analytical Program by Gas Chromatography-Mass Spectrometry (가스크로마토그래피/질량분석기를 이용한 약물의 확인 및 간이 정량분석 프로그램 개발)

  • Systematic toxicological analysis (STA) means the process for general unknown screening of drugs and toxic compounds in biological fluids. In order to establish STA, in previous study we investigated pattern of drugs & poisons in autopsy cases during 2007~2009 in Korea, and finally selected 62 drugs as target drugs for STA. In this study, rapid and simple drug identification and quantitative analytical program by gas chromatography-mass spectrometry(GC-MS) was developed. The in-house program, "DrugMan", consisted of modified chemstation data analysis menu and newly developed macro modules. Total 55 drugs among 62 target drugs were applied to this program, they were 14 antidepressants, 8 anti-histamines, 5 sedatives/hypnotics, 5 narcotic analgesics, 3 antipsychotic drugs, and etc. For calibration curves, fifty five drugs were divided into four groups of range considering their therapeutic or toxic concentrations in blood specimen, i.e. 0.05~1 mg/l, 0.1~1 mg/l, 0.1~5 mg/l or 0.5~10 mg/l. Standards spiked bloods were extracted by solid-phase extraction (SPE) with trimipramine-D3 as internal standard. Parameters such as retention times, 3 mass fragment ions, and calibration curves for each drug were registered to DrugMan. A series of identification, semi quantitation of target drugs and reporting the results were performed automatically. Calibration curves for most drugs were linear with correlation coefficients exceeding 0.98. Sensitivity rate of DrugMan was 0.90 (90%) for 55 drugs at the level of 0.5 mg/l. For standard spiked bloods at the level of 0.5 mg/l for 29 drugs, semi quantitative concentrations were ranged 0.36~0.64 mg/l by DrugMan. If more drugs are registered to database in DrugMan in further study, it will be useful tools for STA in forensic toxicology.
