Effect of Butyric Acid on Performance, Gastrointestinal Tract Health and Carcass Characteristics in Broiler Chickens

  • An experiment was conducted to study the effect of graded levels of butyric acid (butyrate) on performance, gastrointestinal tract health and carcass characteristics in young broiler chickens. Control starter (0-3 wk) and finisher (4-5 wk) diets were formulated to contain 2,900 kcal ME/kg and 22% CP, and 3,000 kcal ME/kg and 20% CP, respectively. Subsequently, four other experimental diets were formulated to contain 0.05% antibiotic (furazolidone) or 0.2, 0.4 and 0.6% butyric acid. Each diet was fed at random to 8 replicates of 6 chicks each throughout the experimental period (0-5 wk). The results showed that 0.4% butyrate in the diet was similar to antibiotic in maintaining body weight gain and reducing E. coli numbers but superior for feed conversion ratio. No added advantage on these parameters was obtained by enhancing the concentration of butyrate from 0.4 to 0.6% in the diet. Feed intake and mortality were not influenced by the dietary treatments. A reduction in pH of the upper GI tract (crop, proventiculus and gizzard) was observed by inclusion of butyrate in the diets of broilers compared to either control or antibiotic-fed group. Butyrate at 0.4% was more effective in reducing the pH than 0.2% butyrate. Within the lower GI tract, 0.4 and 0.6% butyrate was effective in lowering pH in the duodenum, but no effect was found in either the jejunum or ileum. The villus length and crypt depth in the duodenum increased significantly in all the butyrate treated diets irrespective of the level tested. Carcass yield was higher and abdominal fat content was lower significantly in all the butyrate treatment groups compared to the control or antibiotic group. From these findings, it is concluded that 0.4% butyric acid supplementation maintained performance, intestinal tract health, and villi development and carcass quality in broiler chickens.

  • To research the trends of study and the guidelines of investigation and management about gastrointestinal (GI) disease in oriental medicine, we reviewed and analysed articles published in Korea from 2004 to 2009. The western medicine in medical treatments of GI tract is notorious for their side effects. But, in oriental medicine, we couldn't find any side effects. Although we treat or manage clinical practices of the GI disease every day, however, we do not have abundant evidences about scientific mechanisms of those treated with oriental medicine therapy. Therefore, we suggest that the upward of qualities of life and the improvement of health-care system through the development of oriental medicine therapies in Korea will be need.

Endoscopic treatment of upper gastrointestinal postsurgical leaks: a narrative review

  • Upper gastrointestinal postsurgical leaks are life-threatening conditions with high mortality rates and are one of the most feared complications of surgery. Leaks are challenging to manage and often require radiological, endoscopic, or surgical intervention. Steady advancements in interventional endoscopy in recent decades have allowed the development of new endoscopic devices and techniques that provide a more effective and minimally invasive therapeutic option compared to surgery. Since there is no consensus regarding the most appropriate therapeutic approach for managing postsurgical leaks, this review aimed to summarize the best available current data. Our discussion specifically focuses on leak diagnosis, treatment aims, comparative endoscopic technique outcomes, and combined multimodality approach efficacy.

초유내 방어 세포의 구성과 프로스타글라딘 함량 분석 (Compositions of Defensive Cells and Assay of Prostaglandins in Human Colostrum)

  • It is accepted that colostral macrophages have protective effects on gastrointestinal tract of the neonates. Macrophages act as a major defensive cells in colostrum and serve as a main source of colostral prostaglandins which are known to exert cytoprotection for gastrointestinal tract of neonates against infectious agents and drugs such as aspirin. This study was conducted to evaluate the total cell numbers and differential counts for macrophages, neutrophils and lymphocytes in colostrum of Korean mothers. To compare the level of PGE2, 6-keto-PGF1$\alpha$, and TXB2 between colostrum and serum of postpartum mothers, radioimmunoassay adopting eicosanoids-antibody complex method was applied instead of charcoal method. The results were as follows : 1) Total defensive cell count was 7.6$\pm$2,37$\times$106 cells/ml, differential counts of macrophages, neutrophils and lymphocytes were 57.49$\pm$4.14%, 37.98$\pm$4.43% and 4.29$\pm$0.73% respectively. 2) The order of prostaglandin level in colostrum which are known to enhance development and cytoprotection of gastrointestinal tract, was 6-keto-PGF1$\alpha$, TXB2 and PGE2. Colostral PGE2 level was 584.6$\pm$72.3 pg/ml, higher than that of serum(p<0.01). 6-keto-PGF1$\alpha$, the most abundant prostaglandin in colostrum was higher than in serum level, too (p<0.01). Serum TXB2 level of postpartum mothers(n=42) was higher nine times than that of colostrum(p<0.01), which seems to cause vasoconstriction of uterus in postpartum period. 3) In preterm mothers, serum level of TXB2 level in both groups.

Controlled Release and Stabilization of Cefaclor from Alginate-based Matrices for Oral Delivery Design

  • Alginate based polymeric matrices were designed for controlled release and stabilization of cefaclor in gastrointestinal fluid. Cefaclor is known to be acid stable and subjected to be degraded at neutral and alkaline pHs. In order to achieve an effective release profile of cefaclor in gastrointestinal tract, a particular strategy in dosage form design should be required from the view point of maintaining its activity. The amphiphilic nature of cefaclor allowed its controlled release using ionic polymers based on ionic interaction between the drug and polymers. The thrust of this study was to develop a technique that delivers cefaclor keeping effective release rate in the intestinal tract. Considering the fast degradation of cefaclor in the intestinal fluid, the matrices were designed to release surplus amount of cefaclor. The alginate based matrices demonstrated increase in release rate in the simulated intestinal fluid, which was favorable to compensate the degraded portion of cefaclor. In addition, stabilization of cefaclor in the intestinal fluid was obtained by employing citric acid that provides an local acidic environment. The matrices might be valuably used for the development of an oral cefaclor dosage form.

Milk-Derived Growth factors as Neutraceuticals

  • Colostrum has lots of bioactive components and many growth factors including insulin-like growth factors, transforming growth factors and epidermal growth factor. Colostrum and milk derived growth factors widely mediate the growth of overall development and could be used as treatment of gastrointestinal disorder, wound repair process, bioacrivity in the neonatal GI tract and induction of oral tolerance. It is possible that milk derived growth factors as potential neutraceuticals for the specific consumers may have a great role in future food industry.

기스트와 말토마의 보험의학적 악성도 판단 (Medical review of Insurance claims for GIST and MALToma)

  • Medical verification of cancer diagnosis in insurance claims is a very important procedure in insurance administrations. Claims staffs are in need of medical experts' opinions about claim administration. This procedure is called medical claim review (MCR) and is composed of verification and advice. MCR verification evaluates the insured’s physical condition by medical records and compares it with product coverage. It is divided into assessment of living assurance benefit, verification of cancer, and assessment of the cause of death. Actually cancer verification of MCR is applicable to coding because the risk ratio in product development is usually coded data. There are some confusing neoplastic diseases in assessing the verification of cancer. This article reviews gastrointestinal stromal tumors (GIST) and mucosa-associated lymphoid tissue tumors (MALToma) of the stomach. The second most common group of stromal or mesenchymal neoplasms affecting the gastrointestinal tract is GIST. Nowadays there are many articles about the pathophysiology of GIST. However there are few confirmative theories except molecular cell biology of KIT mutation and some tyrosine kinase. Therefore, coding the GIST, which has previously been classified as an intermediate risk group according to NIH2001 criteria, for cancer verification of MCR is suitable for D37.1; neoplasm of uncertain or unknown behavior of digestive organs and the stomach. The gastrointestinal tract is the predominant site of extranodal non-Hodgkin's lymphomas. B-cell lymphomas of the MALT type, now called extranodal marginal zone B-cell lymphoma of MALT type in the REAL/WHO classification, are the most common primary gastric lymphomas worldwide. Its characteristics are as follows. First, it is different from traditional stomach cancers such as gastric adenocarcinoma. Second, the primary therapy of MALToma is the eradication of H. pylori by antibiotics and the remission rate is over 80%. Third, it has a different clinical course compared to traditional malignant lymphoma. Someone insisted that cancer verification is not possible for the above reasons. However, there have been findings on pathologic mechanism, and according to WHO classification, MALToma is classified into malignant B-cell lymphoma and it must be verified as malignancy in MCR.

  • Secondary peptic ulceration and gastritis have been known to be associated with stress, exogenous agents, drugs or infection. Salicylate (aspirin) ingestion has been known to be associated with increased incidence of gastric ulcer and more frequently as the cause of hemorrhagic gastric erosions and gastritis. Some medications such as tetracyclines and iron preparations have been associated with ulceration of the gastrointestinal tract. Chemotherapeutic agents including cytoxan and methotrexate also have been implicated in the development of mucosal and gastrointestinal ulcers. We have experienced two cases of hemorrhagic gastric ulcers due to creosote ingestion in a 13 month and a 5 year old boys. Creosote is a main component of one of the popular digestives, Jungrohwan in Japan and Korea.

As how artificial intelligence is revolutionizing endoscopy

  • With incessant advances in information technology and its implications in all domains of our lives, artificial intelligence (AI) has emerged as a requirement for improved machine performance. This brings forth the query of how this can benefit endoscopists and improve both diagnostic and therapeutic endoscopy in each part of the gastrointestinal tract. Additionally, it also raises the question of the recent benefits and clinical usefulness of this new technology in daily endoscopic practice. There are two main categories of AI systems: computer-assisted detection (CADe) for lesion detection and computer-assisted diagnosis (CADx) for optical biopsy and lesion characterization. Quality assurance is the next step in the complete monitoring of high-quality colonoscopies. In all cases, computer-aided endoscopy is used, as the overall results rely on the physician. Video capsule endoscopy is a unique example in which a computer operates a device, stores multiple images, and performs an accurate diagnosis. While there are many expectations, we need to standardize and assess various software packages. It is important for healthcare providers to support this new development and make its use an obligation in daily clinical practice. In summary, AI represents a breakthrough in digestive endoscopy. Screening for gastric and colonic cancer detection should be improved, particularly outside expert centers. Prospective and multicenter trials are mandatory before introducing new software into clinical practice.
