Early Gastric Cancer with Neurofibroma Mimicking a Metastatic Node: A Case Report

  • Neurofibromas are benign tumors that originate from the peripheral nerves, including neurites and fibroblasts. Generally, a solitary neurofibroma is located in the skin and rarely in other places. A 72-year-old female suffered from epigastric discomfort for 2 months. Endoscopic findings showed an early gastric cancer type IIc at the antrum. Abdominal computed tomography revealed early gastric cancer with a 1.6 cm-sized metastatic node posterior to the duodenum. Laparoscopic assisted distal gastrectomy and retro-pancreatic dissection were performed uneventfully. Histological examination revealed gastric adenocarcinoma, invading the mucosa without nodal metastasis, and a neurofibroma. Herein, we present a case of a gastric cancer patient with a solitary retroperitoneal neurofibroma which mimicked a distant metastatic node.

위용종 - 어떻게 할 것인가? (Gastric Polyp - Is It Serious?)

  • Gastric polyps are morphological diagnoses that collectively refer to various types of lesions, and are commonly found by accident in gastroscopy. Gastric polyps in the broad sense are commonly referred to as abnormal structures protruding toward the gastric lumen, but are generally used only in lesions of the gastric mucosa. The incidence of gastric polyps ranged from 1 to 30% and varied by reporter, and differs by region. Fundic gland polyps were the most common in the West, while hyperplastic polyps were the most common in Korea. Gastric polyps are usually discovered by accident, but because some kinds of polyps have the potential to transform of malignant tumors, histological diagnosis is essential. There should be adequate treatment and management according to the histological results.

소아의 Helicobacter pylori 감염에 의한 결절성 위염의 유전자 발현 양상 분석 (Analysis of Gene Expression in Helicobacter pylori-associated Nodular Gastritis in Children Using Microarray)

  • 목 적: Helicobacter pylori (H. pylori)에 감염된 환자에서 나타나는 결절성 위염은 소아에 특징적인 소견이지만, 지금까지 이에 대한 연구가 이루어지지 못 하였다. 본 연구에서는 H. pylori 감염이 없는 소아 환자의 위 점막의 유전자 발현 양상과 H. pylori에 감염에 의한 결절성 위염이 있는 소아 및 결절성 위염이 없는 소아의 위 점막 유전자 발현 양상을 microarray를 이용하여 비교함으로써 H. pylori 감염 시에 소아의 위 점막에 나타나는 유전자 발현 양상의 변화를 규명하고자 하였다. 방 법: 분당서울대학교 소아청소년과에 내원하여 상 부위장관내시경검사에 의한 위 점막 조직검사를 시행받은 12명의 소아청소년을 대상으로 하였다. 이들을 H. pylori 양성이면서 육안적인 병변이 없는 환아 4명의 HP(+)NG(-)군과, 위 전정부에서 결절성 위염 소견이 관찰된 환아 4명의 HP(+)NG(+)군으로 나누고, 연령 및 성별이 일치하는 H. pylori 음성인 4명의 소아를 대조군인 HP(-)NG(-)군으로 분류하였다. $-70^{\circ}C$에 냉동보관한 위 점막 조직에서 RNA를 추출하여 microarray를 시행하고 분석된 결과에 의거하여 발현 정도에 변화가 있는 유전자를 분석하였다. 결 과: 대상 환자 12명(남자 6명, 여자 6명, 평균 연령 9.8세)의 위 점막 조직에서 cDNA microarray 시행하여 H. pylori 감염의 유무에 따라 유전자 발현 정도를 비교하였을때, H. pylori 감염된 위 점막은 감염되지 않은 소아에 비해 182개의 유전자가 과발현 되었으며, 29개의 유전자가 발현이 저하되었다. H. pylori 감염이 확인 된 8명을 결절성 위염의 유무에 따라 유전자 발현 정도를 비교분석하였을 때, 상부위장관 내시경검사에서 H. pylori 감염과 연관된 결절성 위염을 보인 환자의 위 점막은 결절성 위염이 없는 H. pylori 감염 환아의 위 점막조직에 비해 5개의 유전자가 발현이 증가하였으며 5개의 유전자가 발현이 저하되었다. 결 론: 소아의 위 점막은 H. pylori 감염 여부에 따라 뚜렷한 유전자 발현의 차이를 보이며, 결절성 위염이 동반된 H. pylori 감염 환자의 위 점막은 일부 유전자에서 발현의 차이를 보이는 것을 알 수 있었다. 향후 추가적인 연구를 통해 소아에서 H. pylori 감염에 의한 결절성 위염의 발병 기전을 밝히려는 시도가 필요할 것이다.

Downregulated MicroRNA-133a in Gastric Juice as a Clinicopathological Biomarker for Gastric Cancer Screening

  • Circulatory miR-133a is a marker shared by several types of cancer. In this study we evaluated the feasibility of using miR-133a levels in gastric juice to screen for gastric cancer. A total of 204 samples of gastric juice and mucosa from gastric cancer, atrophic gastritis, gastric ulcer, superficial gastritis and healthy cases were collected by gastroscopy. The results showed that miR-133a levels in gastric juice and carcinoma tissues of patients with gastric cancer were significantly downregulated and positively correlated. Moreover, miR-133a in gastric juice has high operability, high reliability, high sensitivity, high specificity and relative stability, fit for clinical diagnosis of gastric cancer.

Indomethacin으로 유발된 생쥐의 위점막 손상에 대한 소체환(消滯丸)의 효과 (Effect of Socheh-wan Extract on Indomethacin-induced Gastric Mucosal Lesions in Mice)

  • Objectives : This study was carried out to investigate the effects of Soche-hwan extract on indomethacin-induced gastric mucosal lesions of mice. Methods : Experimental mice were divided randomly into four groups. The normal group, the gastropathy group of gastro-inflammation elicited mice, the misoprostol group of mice administered misoprostol after gastro-inflammation elicitation and the Soche-hwan group of mice administered Soche-hwan after gastro-inflammation elicitation. Results : The hemorrhagic erosion of gastric mucosa, the damage of arrangement of mucous secreted cells and HSP70 increased in the gastro-inflammation elicited group, but decreased significantly in the misoprostol and Soche-hwan extract administered groups. Cell proliferation of gastric mucosa decreased in the gastro-inflammation elicited group, but increased significantly in the misoprostol and Soche-hwan extract administered groups. The distribution of mucosal neck cells and mucosal surface cells and PNA positive reactions of surface mucus cells decreased in the gastro-inflammation elicited group, but increased significantly in the misoprostol and Soche-hwan extract administered groups. COX-1 positive cells decreased in the gastro-inflammation elicited group, but increased significantly in the misoprostol and Soche-hwan extract administered groups. iNOS mRNA and COX-2 mRNA increased in the gastro-inflammation elicited group, but decreased significantly in the Soche-hwan extract administered group. NF-kB p65, iNOS and COX-2 increased in the gastro-inflammation elicited group, but decreased significantly in the misoprostol and Soche-hwan extract administered groups. Conclusion : Soche-hwan extract had excellent effects on indomethacin-induced gastric mucosal lesions in mice.

Disruption of the Tff1 gene in mice using CRISPR/Cas9 promotes body weight reduction and gastric tumorigenesis

  • Trefoil factor 1 (TFF1, also known as pS2) is strongly expressed in the gastrointestinal mucosa and plays a critical role in the differentiation of gastric glands. Since approximately 50% of all human gastric cancers are associated with decreased TFF1 expression, it is considered a tumor suppressor gene. Tff1 deficiency in mice results in histological changes in the antral and pyloric gastric mucosa, with severe hyperplasia and dysplasia of epithelial cells, resulting in the development of antropyloric adenoma. Here, we generated Tff1-knockout (KO) mice, without a neomycin resistant ($Neo^R$) cassette, using the clustered regularly interspaced short palindromic repeats/CRISPR-associated nuclease 9 (CRSIPR/Cas9) system. Though our Tff1-KO mice showed phenotypes very similar to the previous embryonic stem (ES)-cell-based KO mice, they differed from the previous reports in that a reduction in body weight was observed in males. These results demonstrate that these newly established Tff1-KO mice are useful tools for investigating genetic and environmental factors influencing gastric cancer, without the effects of artificial gene insertion. Furthermore, these findings suggest a novel hypothesis that Tff1 expression influences gender differences.

Long-term Outcomes and Prognostic Factors of Gastric MALT Lymphoma

  • Purpose: This study aimed to evaluate the long-term prognosis of patients with gastric mucosa-associated lymphoid tissue (MALT) lymphoma, including overall survival (OS), remission, and factors associated with an aggressive disease course. Materials and Methods: Medical records of 153 patients diagnosed with gastric MALT lymphoma between 2013 and 2020 were retrospectively reviewed. Patients experiencing relapse, progression, high-grade transformation, or residual diseasewere included in the aggressive group and were compared with those in the indolent group. Additionally, the endoscopic findings of Helicobacter pylori-negative patients were reviewed. Results: Patient characteristics were as follows: mean age (56.9±11.2 years), sex (male, 51.0%), H. pylori infection (positive, 79.7%), endoscopic location (distal, 89.5%), endoscopic feature (superficial, 89.5%), clinical stage (stage I, 92.8%), invasion depth by endoscopic ultrasound (mucosa, n=115, 75.7%), and bone marrow result (no involvement, n=77, 100.0%). The median follow-up period was 59 months (mean, 61; range, 36-124) and the continuous remission period (n=149) was 51 months (mean, 50; range, 3-112). The 5-year survival rate was 97.7% while the 5-year continuous remission was 88.3%. Factors associated with the patients in the aggressive group were old age, sex(male), and clinical stage II or higher. H. pylori-negative patients' endoscopy revealed a high incidence of atrophic gastritis in the antrum. Conclusions: The long-term prognosis of gastric MALT lymphoma appears indolent and is indicated by the 5-year OS and continuous remission rates. Aggressive disease courses are associated with old age, sex (male), and clinical stage II or higher, but are not related to OS.

Immunocytochemical Study on the Enteroendocrine Cells in the Gastrointestinal Tract of Lateolabrax japonicus, Epinephelus septemfasciatus and Mugil cephalus

  • The gastrointestinal tract of three Percida, Lateolabrax japonicus, Epinephelus septemfasciatus and Mugil cephalus, was investigated immunocytochemically for the occurrence of somatostatin-. seotonin-, gastrin-, pancreatic polypeptide(PP)-, cholecystokinin-8(CCK-8)- and glucagon-immunoreactive cells. In Lateolabrax japonicus and Epinephelus septemfasciatus, five endorcrine cell types, such as somatostatin-, serotonin-, gastrin-, PP- and CCk-8-immunoreactive cells were demonstrated. In Mugil cephalus, however, six endocrine cell types, such as somatostatin-, serotnin-gastrin-, PP-, CCK-8- and glucagon-immunoreactive cells were detected. Somatostatin- and serotonin-immunoreactive cells were detected in the gastric mucosa of all species. Glucagon-immunoreactive cells were found only in the gastric mucoas of Mugil cephalus. In the pyloric caeca, PP-and CCK-8-immnuoreactive cells fo all species. gastrin-immunoreactive cells of Epinephelus septemfasciatus and Mugil cephalus, and serotonin-immunoreactive cells of Epinephelus septemfasciatus were demonstrated. In the intestinal mucosa of all species, gastrin-, PP- and CCK-8-immunoreactive cells were detected, and in the intestinal mucosa of Epinephelus septemfasciatus serotonin-immunoreactive cells were also detected. The frequency of these immunoreactive cells differs from each portion of the gastrointestinal tract of all species.

위 선종 및 선암에서 Trefoil Factor Family 1 단백의 발현 양상 (Expression Pattern of the Trefoil Factor Family 1 in Gastric Adenoma and Carcinoma)

  • Purpose: The trefoil factor family 1 (TFF1) has a protective effect against gastric mucosal damage induced by nonsteroidal anti-inflammatory drugs or ethanol. In addition, a TFF1 knockout mouse model has exhibited circumferential adenomas with high-grade dysplasia, of which $30\%$ progressed into frankly invasive carcinomas. We tried to determine whether the expression pattern of the TFF1 could be involved in the development of sporadic gastric carcinomas. Materials and Methods: We examined TFF1 expression in a series of 43 sporadic gastric carcinomas and 18 gastric adenomas by immunohistochemistry. Results: Strong positive TFF1 staining was identified primarily in the normal gastric mucosa, mainly in the cytoplasm of the superficial and foveolar epithelium. We found TFF1 expression in $55.8\%$ (24 out of 43) of the gastric carcinomas and in $16.7\%$ (3 out of 18) of the gastric adenomas. Statistically, TFF1 immunoreactivity was significantly higher in diffuse-type ($82.4\%$) than in intestinal-type ($38.5\%$) carcinomas(p=0.0058, Fisher's exact test). Conclusion: Our findings provide sufficient evidence that the expression of TFF1 in gastric cancer may simply disclose gastric-type differentiation of neoplastic cells and provide further support for the existence of at least two pathways of malignant transformation of the gastric mucosa: one via intestinal metaplasia and adenomatous dysplasia, leading to glandular carcinomas with intestinal-type differentiation, and the other via hyperplastic changes or de novo changes, leading to diffuse carcinomas and to a subset of glandular carcinomas displaying gastric-type differentiation.
