청신경초종에 대한 감마나이프 방사선 수술 (Gamma-Knife Radiosurgery for Vestibular Schwannoma)

  • Object : The goals of radiosurgery include preservation of neurological function and prevention of tumor growth. We document the results of gamma-knife radio-surgery for vestibular schwannoma. Method & Object : Eighty-two patients underwent stereotactic radiosurgery for an vestibular schwannoma from October, 1994 to December, 2000. Sixty-five of these patients were followed up for radiological and clinical evaluation. As pregamma-knife modality, surgical resection were done in 23 patients,and V-P shunt in 2 patients. Initial symptoms were headache(n=45), dizziness(n=16), tinnitus(n=17). While normal facial function(House-Brackmann grade 1) was present in 48 patients(73.8%), other patients showed grade 2 function in 8, grade 3 function in 7,and grade 4 function in 2. The Gardner/Robertson scale was used to code hearing function. Male to female ratio was 1:3. Mean tumor volume was $7.98cm^3$. Mean dose delivered to the tumor margin was 14.2Gy,and mean maximal dose was 28.3Gy. Results : Mean follow-up duration of 19.9 months. Thirty-five showed decrease(53.8%) in size, 19 patients(29.2%) stationary, 3(4.6%) initial decrease follow up increase, 5(7.6%) initial increase follow up decrease,and 59 patients (90.8%) were well controlled. Two patients experienced transient facial neuropathy, one transient trigeminal neuropathy, and one transient hearing deterioration. After gamma-knife radiosurgery, ventriculoperitoneal shunt was done in 4 patients. Conclusions : Gamma-knife radiosurgery can be used to treat postoperative residual tumors as well as in patients with concomitant medical problems in patients with preserved hearing function. Gamma-knife radiosurgery is safe and effective method to treat small, medium sized(less than 3cm in extracanalicular diameter), intracanalicular vestibular schwannoma, associated with low rate of cranial neuropathy.

  • Hypothalamic hamartoma[HH] is an unusual nonneoplastic developmental lesion associated with gelastic epilepsy and precocious puberty, mostly found in children. Although open surgery has been attempted when antiepileptic medication failed to control seizures, its deep location and surrounding vital structures often rendered surgery unsuccessful. We describe the outcome of gamma knife radiosurgery in three children with a HH associated with gelastic epilepsy and reviewed the literature for a possible therapeutic mechanism.

EBT-3 필름을 사용한 감마나이프 퍼펙션TM의 치료 계획 시스템 및 선량 분포 비교 분석 (Comparative Analysis of Treatment Planning System and Dose Distribution of Gamma knife PerfexionTM using EBT-3 Film)

  • 본 연구는 EBT3 필름을 이용하여 감마나이프 퍼펙션 모델의 3차원적인 선량분포 측정하고 기준값과 비교 분석하여 표준화된 측정방법의 기초로 활용하고자 한다. 2개 종합병원에 설치된 감마나이프 퍼펙션 모델의 선량 분포를 EBT3 필름을 이용하여 정확도와 정밀도를 평가하였다. 정확도 평가를 위해 4 mm 콜리메터를 사용하여 기계적인 중심축과 선량중심축의 일치도를 측정하였다. A병원 0.098 mm, 0.195 mm 이며 B 병원 0.229 mm, 0.223 mm 로 허용 오차 0.3 mm 이하로 측정되었다. 정밀도 평가는 4, 8, 16 mm 콜리메터(collimater) 각각의 x, y, z 3차원면 에서의 반치폭(FWHM : Full Width at Half Maximum)을 이미지-제이 프로그램을 이용하여 평가하였다. A 병원은 -0.283~0.583 mm, B 병원은 -0.857~0.810 mm로 50%선 ${\pm}1mm$ 이하의 기준에 적합하였다. 이미지-제이 프로그램을 이용한 선량 분포 분석의 경우 측정자 간의 오차가 발생 가능함으로 측정점에 대한 명확한 기준을 확립할 필요가 있으며, 감마나이프 방사선 수술이 시행되어지는 고선량 영역에서 사용 가능한 선량영역이 높은 필름을 이용한 치료계획과 실제 치료 조사면의 비교가 필요하다고 생각된다.

Surgical Experience of Infratentorial Meningiomas : Clinical Series at a Single Institution during the 20-Year Period

  • Objective : Based on surgical outcomes of patients with infratentorial meningiomas surgically treated at our institution, we analyzed the predictors for surgical resection, recurrence, complication, and survival. Methods : Of surgically treated 782 patients with intracranial meningioma, 158 (20.2%) consecutive cases of infratentorial location operated on between April 1993 and May 2013 at out institute were reviewed retrospectively. The patients had a median age of 57.1 years (range, 16-77 years), a female predominance of 79.7%, and a mean follow-up duration of 48.4 months (range, 0.8-242.2 months). Results : Gross total resection (Simpson's grade I & II) was achieved in 81.6% (129/158) of patients. Non-skull base location was an independent factor for complete resection. The recurrence rate was 13.3% (21/158) and the 5-, 10-, and 15-year recurrence rates were 8.2%, 12.0%, and 13.3%, respectively. Benign pathology, postoperative KPS over than 90, low peritumoral edema, and complete resection were significantly associated with longer recurrence-free survival rate. The 5-, 10-, and 15-year survival rates were 96.2%, 94.9%, and 94.9%, respectively. Benign pathology, postoperative KPS over than 90 and complete resection were significantly associated with a longer survival rate. The permanent complication rate was 13% (21/158). Skull base location and postoperative KPS less than 90 were independent factors for the occurrence of permanent complication. Conclusion : Our experience shows that infratentorial meningiomas represent a continuing challenge for contemporary neurosurgeons. Various factors are related with resection degree, complications, recurrence and survival.

당일 감마나이프수술 환자의 표준진료지침 개발을 통한 질 향상 효과 측정 (Measuring Effects of Quality Improvement through the Development of Critical Pathway for Gamma Knife Radiosurgery)

  • Objectives : A protocol called "critical pathway" was developed to standardize the management of hospital patients the day after they underwent gamma knife radiosurgery. The quality of improvement in patient outcomes was evaluated. Methods : Critical pathway was developed, according to the regulations of the I hospital, by analyzing the medical records of 22 inpatients who underwent gamma knife surgery within the period from January to April 2011 on the day of the surgery. The study included a group of 22 patients admitted to the hospital the day after they underwent gamma knife radiosurgery, between July and September 2011. The control group included 22 patients who had surgery employing the same method within the period from May to June 2011. To measure the effects on quality improvement, the average length of stay, the execution rate of the hospital discharge notice system, daily hospital revenue, and the satisfaction of the patients and the medical team were assessed. The patient questionnaire employed a four-point Likert scale while the medical-staff questionnaire employed a five-point Likert scale. Result : The average length of stay was significantly shorter in the study group compared to the control group (2.3 days vs. 3.8 days, P<0.05). The execution rate of the hospital discharge notice system was higher in the study group (100% vs. 72%) than in the control group. Daily hospital revenues were higher by 264,178 Korean won in the study group when compared to the control group. The study group showed greater satisfaction of patients compared to the control group based on a four-point Likert scale (P<0.05). The study group showed greater satisfaction in medical team compared to the control group based on a five-point Likert scale (P<0.05). Conclusion : The development and implementation of a critical pathway protocol for hospital admission the day after gamma knife radiosurgery is an effective care process that improves the clinical quality.

감마나이프 방사선 수술시 2차 발암 확률에 관한 연구 (A Study on the Probability of Secondary Carcinogenesis during Gamma Knife Radiosurgery)

  • 본 연구에서는 감마나이프를 이용한 방사선 수술 시 주변 정상 장기들의 피폭선량을 측정하여 2차 발암확률을 분석하고자 한다. 인체 조직 등가 물질로 구성된 소아 팬텀(Model 706-G, CIRS, USA)에 종양 볼륨은 0.25 cm3, 0.51 cm3, 1.01 cm3, 2.03 cm3 총 4개로 설정하였으며, 평균 선량은 18.4 ± 3.4 Gy로 하였다. 감마나이프 수술 장비의 테이블위에 Rando phantom을 설치한 후에 OSLD nanoDot 선량계를 Right eye, Left eye, Thyroid, Thymus gland, Right lung, Left lung 에 위치시켜 각각의 피폭선량을 측정하였다. 청신경초종질환의 감마나이프 방사선 수술 시 주변 정상 장기들의 방사선 피폭으로 인한 암 발생확률은 종양 볼륨 2. 03 cm3에서 100,000명 당 4.08명의 암이 발생함을 알 수 있다. 본 연구는 정위적 방사선 수술 시 발생할 수 있는 2차 방사선 피폭선량의 위험성을 연구하여 향후 확률적 영향과 관련하여 유용한 자료로 활용될 것으로 사료된다.

형태학적 연산과 뇌종양 평균 크기를 이용한 감마나이프 치료 범위 자동 검출 알고리즘 (Automatic Detection Algorithm of Radiation Surgery Area using Morphological Operation and Average of Brain Tumor Size)

  • In this paper, we proposed automatic extraction of brain tumor using morphological operation and statistical tumors size in MR images. Neurosurgery have used gamma-knife therapy by MR images. However, the gamma-knife plan systems needs the brain tumor regions, because gamma-ray should intensively radiate to the brain tumor except for normal cells. Therefore, gamma-knife plan systems spend too much time on designating the tumor regions. In order to reduce the time of designation of tumors, we progress the automatical extraction of tumors using proposed method. The proposed method consist of two steps. First, the information of skull at MRI slices remove using statistical tumors size. Second, the ROI is extracted by tumor feature and average of tumors size. The detection of tumor is progressed using proposed and threshold method. Moreover, in order to compare the effeminacy of proposed method, we compared snap-shot and results of proposed method.

두개인두종에 대한 감마나이프 방사선수술 (Gamma Knife Radiosurgery for Craniopharyngioma)

  • Objective : The purpose of this study are to evaluate the effectiveness of Gamma Knife radiosurgery(GKS) as a treatment of craniopharyngioma and to investigate the proper dose planning technique in GKS for craniopharyngioma. Method : Between May 1992 and March 1999, seven Gamma Knife radiosurgical procedures were done for residual tumor mass of 6 patients with craniopharyngioma after microsurgical resection. Conventional radiation therapy was not performed. In this study, their clinical, radiological and radiosurgical data were analyzed and the radiation dosage to the optic pathway, hypothalamus, pituitary stalk, and cavernous sinus were calculated and correlation with clinical outcome was evaluated. The mean follow-up period was 33.5 months(12.3-55.2 months). Result : The mean tumor volume was 4.4cc(0.4-18.0cc) and the maximum radiation dose ranged from 14 to 32 Gy(mean 20.9Gy). The radiation was given with isodose curve, 50-90% and the marginal dose varied within 8-22.4Gy(mean 12.7Gy). The mean number of isocenter was 4.3(1-12). The tumor was well controlled in all cases. In 5 of 7 cases, the size of tumor decreased to 10-50% of pre-GKS volume and remaining two showed no volume change. The mean dose to optic pathway was 5.7Gy(5.1-11.2Gy) and there were no complications. Conclusion : GKS seems to be effective for control of craniopharyngioma as an adjuvant treatment after microsurgical resection and even suboptimal dose for tumor margin is considered to be enough for tumor control. It is safe with careful dose planning to protect surrounding important structures, especially optic pathway. We believe conventional radiation therapy should be avoided because it has limitation for dose planning of additional treatments such as radiosurgery or intracystic instillation of radioisotope in case of recurrence.

터어키안 주변종양에 대한 감마나이프 방사선 수술 (Gamma Knife Radiosurgery for Juxtasellar Tumors)

  • Objective : Around the sellar area, there are many important structures. But, the optimal radiation dosage for minimal toxicity to surrounding neural tissue has not been firmly established. The purpose of this study is to evaluate the radiosurgical outcome of juxtasellar tumors and to investigate the relationship between radiation dosage and toxicity to neural tissue. Method : Between May 1992 and June 2000, we treated 65 juxtasellar tumors by using the Leksell Gamma Knife. Among them, 52 patients who could be followed more than 1 year were included in this study. The radiosurgical dosage to the optic pathway, cavernous sinus, Meckel's cave, hypothalamus, pituitary gland and stalk, and brain stem was analyzed and correlated with clinical outcome. The mean follow-up period was 33.5 months(range 12.2- 99.0 months). Result : The clinical response rate was 69.2%. The volume response rate was 61.0% and the radiologic control rate was 92.7%. There were 4 complications(7.7%) of 2 trigeminal neuropathy, 1 abducens nerve palsy, and 1 trigeminal and transient abducens nerve palsy. The optic apparatus appeared to tolerate doses greater than 10Gy. The risk of cranial nerve complications in cavernous sinus seemed to be related to doses of more than 16Gy. In 3 of 4 patients who received more than 16Gy to cavernous sinus, the abducens or trigeminal neuropathy occurred. Also, one patient who received more than 15Gy to the Meckel's cave, trigeminal neuropathy developed. The hypothalamus, pituitary gland and stalk, and brain stem were relatively tolerable to radiation. Conclusion : Gamma Knife radiosurgery seems to be an effective method to control the growth of juxtasellar tumors. To avoid injury to surrounding important neural tissue, careful dose planning and further study for radiation toxicity to neural tissue were needed.
