안심온담탕(安心溫膽湯)과 가미온담탕(加味溫膽湯)이 한랭(寒冷).유영(遊泳) 스트레스 생쥐의 뇌부위별(腦部位別) Monoamines 함량(含量)에 미치는 영향(影響) (Effects of the Ansymondam-tang and Gamiondam-tang on the Regional Brain Monoamines Contents of Cold Swimming Stressed Mice)

  • Objectives : This study aimed to evaluate the anti-stress effects of Ansymondam-tang and Gamiondam-tang on the contents of monoamines in the regional brain of mice forced cold swimming stress in $4^{\circ}C$ water. Methods : The experimental animals were forced cold swimming stress for 3 minutes, and administered of Ansymondam-tang(2.2mg/10g) and Gamiondam-tang(0.83mg/10g) extract for 7 days before stress. The monoamines contents were measured by HPLC method in various part(frontal cortex, hypothalamus, corpus striatum and hippocampus) of mice brain. Results: 1. In frontal cortex, the contents of norepinephrine were increased with statistical significantly in all of the administered group compared to control group. The contents of dopamine and serotonin were increased in Gamiondam-tang administered group has a statistical significance. 2. In hypothalamus, the contents of dopamine were increased with statistical significance in all of the administered group compared to control group. 3. In corpus striatum the contents of norepinephrine were decreased and the contents of dopamine were increased with statistical significance in all of the administered group compared to control group. The contents of serotonin were increased in Gamiondam-tang administered group has a statistical significance. 4. In hippocampus the contents of norepinephrine were decreased with statistical significance in Gamiondam-tang administered group compared to control group. The contents of dopamine were increased in Ansymondam-tang administered group has a statistical significance. Conclusion : This study reaches a conclusion that Ansymondam-tang and Gamiondam-tang has significant effects .on reducing and preventing stress in mice. Especially Gamiondam-tang is more effective in the statistical significance than Ansymondam-tang on the monoamines change in the mice brain.

  • Objectives: This study aimed to describe the use of traditional Korean medicine to relieve chest pain, insomnia, and dyspnea in a patient with unstable angina pectoris Methods: To relieve the symptoms, the patient was treated with a range of traditional Korean medicine, including acupuncture, moxibustion, and an herbal medicine (Gamiondam-tang-gagam). The numerical rating scale (NRS) was used to measure the patient's status and improvements in the frequency of symptoms. Results: After the treatment, the NRS score for chest pain, insomnia, and dyspnea decreased from 10 to 0. In addition, chest pain and insomnia almost subsided. Conclusions: This report suggests that Gamiondam-tang-gagam can be effective in relieving chest pain, insomnia, and dyspnea.

허혈성 심질환 환자의 두통 및 흉통 한방치험 1례 (A Case Report of an Ischemic Heart Disease Patient with Chest Discomfort and a Headache)

  • Objective: This case study describes the use of traditional Korean medicine to relieve chest pain and headaches in a patient with ischemic heart disease. Methods: The patient was treated with a range of traditional Korean medicine, including acupuncture, moxibustion, and herbal medicines (Gamiondam-tang and Cheonmabanhwa-tang-gagambang). The Numerical Rating Scale (NRS) was used to measure the patient's status and improvements in the frequency of symptoms. Results: After the treatment, the NRS score for chest pain and headaches decreased from 6 to 0. In addition, chest discomfort and palpitations almost subsided. Conclusion: This clinical case study suggested that Gamiondam-tang and Cheonmabanhwa-tang-gagambang could be effective in relieving chest discomfort and headaches.

온담탕(溫膽湯) 가감방(加減方)으로 증상 호전된 갱년기 증후군 환자 치험 4례 (4 Cases Report of Menopause Syndrome with Ondam-tang Gagambang)

  • Objectives: The Purpose of this study is to report the effects of Ondam-tang gagambang on menopause syndrome. Methods: Four patients who had menopause syndrome as chief complaints were treated by Gamiondam-tang or Kuibiondam-tang for two weeks. We evaluated the results of treatment by change of symptoms, Kupperman's Index and Menopause Rating Scale (MRS). Results: After treatment, menopause syndrome cases were improved. Almost scores of Kupperman's Index & Menopause Rating Scale (MRS) were reduced. Conclusions: This study suggests that Ondam-tang gagambang is effective on overall Menopause syndrome cases.

메티마졸 투여 중인 갑상선기능항진증 환자에서 한약의 임상증상 완화효과에 대한 증례보고 (A Case Report on The Relieving Effect of Herbal Medicine on Clinical Symptoms in Patients with Hyperthyroidism Taking Methimazole)

  • 본 증례는 그레이브스병에 의한 갑상선기능항진증으로 진단된 환자에서 메티마졸 단독 투여에도 계속된 갑상선기능항진증의 증상치료를 위하여 한의 변증진단(辨證診斷) 과정을 통하여 심담허겁(心膽虛㥘)과 심화상염(心火上炎) 변증형(辨證形)을 도출하고 가미온담탕(加味溫膽湯)에 청심연자음(淸心蓮子飮)을 합방(合方)한 한약을 메티마졸과 병행투여 한 사례이다. 본 증례에서 한약이 메티마졸의 갑상선 호르몬 억제 효과는 계속 유지하면서 갑상선기능항진증 환자의 증상을 완화한 효과를 보여준 결과는 갑상선기능항진증 치료에 있어서 한약이 베타 차단제를 대체하여 메티마졸과 병행투여할 수 있는 약물로써 가능성이 있다는 것을 보여준다.

Effect of Gamiondam-tang (GMODT), a Polyherbal Formula on the Pharmacokinetics Profiles of Tamoxifen in Male SD Rats

  • Objectives: The effects of Gamiondam-tang (GMODT) co-administration within 5min on the pharmacokinetics (PK) of tamoxifen were observed as a process of the comprehensive and integrative medicine, combination therapy of tamoxifen with GMODT to achieve synergic pharmacodynamics and reduce toxicity on the breast cancer. Methods: After 50mg/kg of tamoxifen treatment, GMODT 100mg/kg was administered within 5min. The plasma were collected at 30 min before administration, 30 min, 1, 2, 3, 4, 6, 8 and 24 hrs after end of GMODT treatment, and plasma concentrations of tamoxifen were analyzed using LC-MS/MS methods. PK parameters of tamoxifen (Tmax, Cmax, AUC, $t_{1/2}$ and $MRT_{inf}$) were analysis as compared with tamoxifen single administered rats using noncompartmental pharmacokinetics data analyzer programs. Results: Co-administration with GMODT induced increased trends of plasma tamoxifen concentrations to 1hr after end of administration, and then showed decreased trends of plasma tamoxifen concentrations, and especially significant (p<0.05) increases of plasma tamoxifen concentrations were demonstrated at 0.5hr after end of co-administration with GMODT and also related significant (p<0.05) decreases of $AUC_{0-inf}$ and $MRT_{inf}$ as compared with tamoxifen single formula treated rats, at dosage levels of tamoxifen 10 mg/kg and GMODT 100 mg/kg within 5 min, in this experiment. Conclusion: Based on the results of the present study, it is considered that single co-administration GMODT within 5min significantly inhibited the oral bioavailability of tamoxifen through variable influences on the absorption and excretion of tamoxifen, can be influenced on the toxicity or pharmacodynamic of tamoxifen.

심인성 사경증 환자 치험 1례 (A Clinical report on psychogenic torticollis)

  • Objectives : The purpose of this study was to investigate the clinical application of Korean Traditional treatment to psychogenic torticollis patient who has panic disorder as underlying disease. Methods : Herb medicine used in this study are Heosu-san(Huishou-san) which is previously used for torticollis in several studies, Guibi-tang(Guipi-tang) and Gamiondam-tang(Jiaweiwendan-tang) which are frequently used for phychological disease. Also, physical therapy and aroma therapy were practiced which basically stretches and relaxes muscles around cervical muscles. Self exercise of cervical muscles was carried out during acupuncture. Results : After the comprehensive treatment, the clinical symptoms of the patient were improved. Conclusions : It is suggested that oriental medical treatment are effective in psychogenic torticollis.

  • The aim of this study was to show the clinical aspects in outpatients who visited Oriental Neuropsychiatry Clinic. Method : We studied 419 patients who visited Oriental Neuropsychiatry Clinic of Dong-Sea Medical Center from march, 2007 to August, 2007. Results & Conclusion : 1. The ratio of male to female patients was 1:2 and age distribution was most frequent in 50s. 2. From onset, 32% of patients visited Oriental Neuropsychiatry Clinic in a week, 58% of patients visited in a month. 3. Oriental Neuropsychiatry diseases were 80%, Non-Oriental Neuropsychiatry diseases were 20%. The most common Oriental Neuropsychiatry diseases were Somatoform Disorders. 4. The most common diagnosises of the Oriental Neuropsychiatry diseases were Psychosomatic diseases. 5. The most common cheif corn1ains were Headache. 6. The most commonly used herbal medicine was Gamiondam-tang and Guibi-tang

Effect of Gamiondam-tang (GMODT), a Polyherbal Formula on the Pharmacokinetics Profiles of Tamoxifen in Male SD Rats (2) - Single Oral Combination Treatment of Tamoxifen 50 mg/kg with GMODT 100 mg/kg with 2.5 hr-intervals -

  • Objectives : In our previous study, single co-administration GMODT within 5 min significantly inhibited the oral bioavailability of tamoxifen through variable influences on the absorption and excretion of tamoxifen. Therefore, the object of this study was to elucidate the possible effects on the pharmacokinetics of tamoxifen after single oral co-administration of GMODT with 2.5 hr-intervals. Methods : After 50 mg/kg of tamoxifen treatment, GMODT 100 mg/kg was administered with 2.5 hr-intervals. The plasma were collected at 30 min before administration, 30 min, 1, 2, 3, 4, 6, 8 and 24 hrs after end of GMODT treatment, and plasma concentrations of tamoxifen were analyzed using LC-MS/MS methods. PK parameters of tamoxifen (Tmax, Cmax, AUC, $t_{1/2}$ and $MRT_{inf}$) were analysis as compared with tamoxifen single administered rats. Results : Two-half hr-interval co-administration with GMODT induced variable changes on the plasma tamoxifen concentrations as compared with tamoxifen single treated rats, and especially significant (p<0.05) increases of plasma tamoxifen concentrations were demonstrated at 0.5 (199.61%) and 1 hr (101.06%) after end of co-administration with GMODT, and also related significant (p<0.05) decreases of $t_{1/2}$ (-39.54%) and $MRT_{inf}$ (-43.94%) as compared with tamoxifen single formula treated rats, at dosage levels of tamoxifen 50 mg/kg and GMODT 100 mg/kg with 2.5 hr-intervals, in this experiment. Conclusions : According to the results, GMODT critically decreased on the oral bioavailability of tamoxifen through variable influences on the absorption and excretion of tamoxifen. Hence, the co-administration of GMODT and tamoxifen should be avoided in the comprehensive and integrative medicine, combination therapy of tamoxifen with GMODT on the breast cancer.

Bambusae Caulis in Taeniam Applicable for Medical Indications Associated with Inflammation

  • Jukyeo (竹茹; Bambusae Caulis in Taeniam: BCT) is an herbal medicine made from the inner part of the bamboo stem of Phyllostachys nigra Munro var. henonis Stapf ex Rendle or Phyllostachys bambusoides Siebold et Zuccarini. Although medical literature published in China and Korea decades ago introduced BCT as a remedy for reducing vomiting, fever, and hematemesis, decoctions containing BCT as a main herb (君藥) in Korea have been approved only for treating neurologic symptoms. Here, we discuss the anti-inflammatory function of BCT. Combined with the clinical usage of a BCT-containing decoction in treating inflammatory diseases in Japan, we raise the possibility of repurposing the BCT-containing decoctions for treating inflammatory diseases. The anti-inflammatory activity of BCT was mainly assessed by using RAW 264.7 cells. The regulation of NF-κB, Nrf2 and A20 activities was determined by western blot and quantitative RT-PCR. The list of decoctions containing BCT currently approved in Korea was obtained from the Korean Ministry of Food and Drug Safety (KFDA). BCT suppressed the activity of pro-inflammatory factor NF-κB elicited by LPS, activated an anti-inflammatory factor Nrf2, and induced A20 that is known to block several inflammatory pathways simultaneously, suggesting that BCT can suppress inflammation via at least 3 different pathways. KFDA approved 11 decoctions containing BCT as a major herb, including Gamiondam-tang (加味溫膽湯; GOT), for treating neurologic disorders. Interestingly, Jukyeoondam-tang (竹茹溫膽湯; JOT), whose composition is almost identical to GOT except for one herb, has been used to treat inflammatory pulmonary disorders including Covid-19 pulmonary infection in Japan Given the anti-inflammatory function of BCT evidenced by medical literature and experimental results and the clinical usage of JOT in treating inflammatory pulmonary disorders, we suggest a repurposing and extension of the BCT-containing decoctions approved in Korea to treating inflammatory diseases.
