Clinical Efficacy and Safety of Yukgunja-tang for Gastroesophageal Reflux Disease: A Systematic Review and Meta-Analysis

  • Objectives: The purpose of this meta-analysis was to evaluate the effects of Yukgunja-tang(YGJT, Rikkunshito, Liujunzi) on Gastroesophageal Reflux Disease(GERD). Methods: Sixteen electronic databases were used to search for studies published through March 2019, and a randomized controlled study was conducted to study the effects of YGJT or modified YGJT on GERD. Study quality was assessed using the risk bias tool provided by Cochran, and data analysis was performed using Review Manager 5.3.0 software Results: Two-hundred and forty articles were initially searched, and 13 studies that satisfied the study criteria were evaluated qualitatively; 11 of the 13 were included in the meta-analysis. In the two studies, the effects of YGJT and a placebo were compared. Meta-analysis showed that YGJT significantly improved FSSG (Frequency Scale for the Symptoms of GERD) scores, but not GSRS (Gastrointestinal Symptom Rating Scale) scores (FSSG: MD -2.40, 95% CI [-5.31, 0.51], p=0.11, GSRS: MD 0.05, 95% CI [-0.12, 0.22], p=0.55). Meta-analysis of nine studies comparing the efficacies of YGJT and conventional medicine showed that YGJT had a significant clinical effect (OR 5.72; 95% CI [3.41, 9.61]; I2 p<0.00001). Conclusion: This study suggests that YGJT effectively relieves the symptoms of GERD. Unfortunately, owing to the small sample sizes, limitations of several methodological qualities, we believe large-scale clinical studies with less bias will provide evidence of qualitative improvement.

위장관 증상을 동반한 불안장애 환자의 한방 치험 2례 (Two Case Reports of Anxiety Disorder Patients with Gastrointestinal Symptoms Treated with Traditional Korean Medicine)

  • This study investigated the clinical effect of Korean medicine treatments on two patients who experienced gastrointestinal symptoms with anxiety disorder. The patients were treated with herbal medicine (Yukgunja-tang, Banhabakchulchunma-tang) and acupuncture for 11 days and 26 days, respectively. The Beck Anxiety Inventory (BAI) and Gastrointestinal Symptom Rating Scale (GSRS) scores on the day of hospital admission and after treatment were compared; both the BAI and GSRS scores decreased after treatment. Moreover, the grade of the gastrointestinal symptoms changed from severe to mild or none. Therefore, these cases are useful for describing Korean medicine treatments for anxiety disorders in patients who experience gastrointestinal symptoms. However, additional case reports and studies are be needed in the future to confirm these findings.

만성 오심 구토 증후군 환자의 한의치료 1례 (A Case Report on Korean Medical Treatment for a Patient with Chronic Nausea and Vomiting Syndrome)

  • This study investigated the case of a nineteen-year-old female patient with chronic nausea, vomiting, and abdominal distension, who was diagnosed with CNVS and Spleen-Qi deficiency. Subjective symptoms were recorded with the Numerical Rating Scale (NRS) every morning, and the Gastrointestinal Symptom Rating Scale (GSRS) was used on the days of admission and discharge. For eleven days following admission, she took Bojungikki-tang-gagam and received acupuncture and moxibustion therapy. After three days of treatment, her nausea and vomiting ceased, and abdominal distension improved from NRS 3 to 0 after seven days of treatment. The GSRS score for the specific symptoms improved from 3 to 2; however, the total score remained largely unchanged (from 20 to 21). This case suggests that Korean medical treatment may improve CNVS.

위전절제술 후 재건술식에 따른 환자의 영양상태와 삶의 질 비교 (Comparison of Reconstructive Techniques after Total Gastrectomy as Determined by Patient Quality of Life and Nutritional Status)

  • 목적: 위전절제술 후 이용될 수 있는 재건 술식에 대해서는 많은 방법들이 보고되어 왔지만 지금까지도 이상적인 술식에 대해서는 논쟁의 여지가 있고 장기간에 걸친 비교 연구는 더욱이 부족한 실정이다. 이에 저자는 본원에서 실시한 4가지 술식들인 Roux-en-Y 술식(R-Y), 공장간치술(JI), 공장낭간치술(JPI), Double-tract 재건술(DT)간의 경과 관찰을 통해 보다 나은 재건 술식을 찾고자 하였다. 대상 및 방법: 본원에서 2000년 8월부터 2004년 1월까지 위암으로 근치적 위전절제수술을 받고 재발이 없고 추적이 가능한 환자 중 80세 이상과 병기 IV기를 제외하고 재건 술식에 따라 RY군(n=15), JI군(n=8), JPI군(n=8), DT군(n=10)으로 분류한 총 41명의 환자를 대상으로 수술 후 증상, 수술 후 1년, 3년째의 혈액 및 생화학적 검사의 변화, 체중 변화를 포함한 영양학적 변화 및 삶의 질을 평가하기위한 GSRS score를 분석하였다. 또한 내시경 소견을 통해 위식도 역류 여부를 확인하였다. 결과: 검사실소견은 3년째 총단백과 알부민에서 DT군이 유의하게 낮은 결과를 보였고(P=0.006, P=0.033), 체중감소는 JI군이 1년, 3년째에서 수술 전 체중의 84.50%, 86.50%로 가장 많았다(P=0.013, P=0.011). GSRS score에서 JPI군이 수술 후 1년, 3년 모두에서 유의하게 더 좋은 결과를 보였다. (P=0.028, P=0.003). 결론: 공장낭간치술은 위전절제술 후 환자의 삶의 질을 높일 수 있는 유용한 재건 방식으로 생각된다.

건칠약침을 포함한 한방치료 후 호전된 소화불량 환자 5례에 대한 증례보고 (Case Report of Five Patients with Dyspepsia Improved after Korean Medicine Treatment, Including GeonChil Pharmacopuncture)

  • Objectives: This study reports the effects of GeonChil (Rhus verniciflua stokes) pharmacopuncture in patients with dyspepsia. Method: We treated all five patients with GeonChil pharmacopuncture, acupuncture, and moxibustion and evaluated them using the Korean Gastrointestinal Symptom Rating Scale (KGSRS), which is used to assess overall gastrointestinal symptoms. Result: After treatment, the KGSRS showed improvement in all five patients. Marked improvement was seen especially in abdominal pain, heartburn, eructation, and increased flatus symptoms. Conclusion: GeonChil pharmacopuncture may represent a new treatment for the improvement of dyspepsia.

기능성 소화불량증이 호전된 소양인 비수한표한병 환자 3례 보고 (A Clinical Case Report of Soyangin Patients with Functional Dyspepsia)

  • Objectives The purpose of this study was to report significant improvement of functional dyspepsia in 3 patients, who were diagnosed with 'Soyangin Spleen Cold-based Exterior Cold disease' and applied medication based on Sasang Constitutonal medicine.Methods The patients were treated with herbal medications, according to their own constitution. The objective outcome was measured by body composition analyzer and Gastrointestinal Symptom Rating Scale (GSRS), and the subjective sensation was assessed by using Visual Analogue Scale (VAS).Results The symptoms of functional dyspepsia improved remarkedly in all respects, without any side effects. Original symptoms also changed for the better.Conclusions This results show that it is available that functional dyspepsia is treated with medicines according to Sasang Constitutional medicine.

  • Objective: This case presents the effects of Gyejibanhasaenggang-tang, acupuncture, and moxibustion on functional dyspepsia.Method: A patient was treated with herbal medicine, acupuncture, and moxibustion. Functional dyspepsia-related quality of life (FD-QoL), the gastrointestinal symptom rating scale (GSRS), and the gastrointestinal scale (GIS) were assessed for evaluation.Results: Symptoms of functional dyspepsia improved after the 22 days of Gyejibanhasaenggang-tang treatment.Conclusion: This case suggests that the combination of Gyejibanhasaenggang-tang, acupuncture, and moxibustion could be effective in treating functional dyspepsia.

  • Objectives: This case study reports the effectiveness of Korean medical treatment in a patient with cyclic vomiting syndrome. Methods: A 29-year-old female Korean patient with cyclic vomiting syndrome received acupuncture, electroacupuncture, herbal medicine, and moxibustion for 2 weeks in hospital. Changes in symptoms were evaluated using the Gastrointestinal Symptom Rating Scale (GSRS), Visual Analog Scale (VAS), Index for Nausea, Vomiting, and Retching (INVR), Nausea Severity Scale (NSS), Nepean Dyspepsia Index-Korean version (NDI-K), quality of life using the Functional Dyspepsia Related Quality of Life questionnaire (FD-QoL), and gastric motility using electrogastrography (EGG). Results: Post-treatment, the patient showed high satisfaction and improvement in symptoms of nausea and vomiting. The following changes were observed in scores: GSRS: 23 to 19; VAS of nausea: 88 to 95; VAS of dyspepsia: 95 to 12; INVR: 13 to 1; NSS: 17 to 5; NDI-K: 107 to 78; and FD-QoL 84 to 27. We also found positive results in Channel 3 of EGG parameters, implying the improvement of gastric motility disorder. Conclusion: Korean medical treatment can be a therapeutic option for cyclic vomiting syndrome.

한의 복합치료로 호전된 불응성 위식도역류질환 (PPI-refractory GERD) 환자 1례 - 복진소견 평가를 중심으로 - (Clinical Case Report of PPI-refractory Gastro-Esophageal Reflux Disease (GERD) with Korean Medicine Treatment - A Focus on Evaluation of the Abdominal Examination)

  • This case report describes a 60-year-old man who had reflux symptoms despite treatment with proton pump inhibitors (PPIs). Korean traditional medicine, including a herbal prescription, acupuncture, and moxibustion was administered to the patient during his 19 days of hospitalization. The visual analogue scale (VAS), gastrointestinal scale (GIS), and gastrointestinal symptom rating scale (GSRS) scores were assessed on the dates of admission and discharge. An abdominal examination, which is one of the Korean medicine diagnostic tools, was performed every day to evaluate the treatment progress. During the treatment, the improvement of symptoms was clinically correlated with abdominal examination outcomes. We suggest that administration of traditional Korean Medicine on PPI-refractory GERD patients could be effective and we recommend utilization of the abdominal examination as an assessment tool for Korean Medicine treatment.

  • Objectives : This study was performed to report the effect of oriental treatments for panic disorder with functional gastrointestinal disturbance. Methods : In this case, a 30 year-old female patient has been suffered by panic disorder with functional gastrointestinal disturbance for 10 years. This case can be caused by KanKiUlKyul(肝氣鬱結), KanKiBeomYi(肝氣犯胃) and KanKiSeungBi(肝氣乘脾). We treated the patient with Herbal medication, Cognitive-behavioral therapy and Relaxation therapy. The effects of treatment were measured by GSRS(Gastrointestinal Symptom Rating Scale), BPI(Brief Pain Inventory) and VAS(Visual Analog Scale) Results : As a result of oriental treatments, the symptoms of Panic Disorder with Functional Gastrointestinal Disturbance was improved. Conclusions : These results suggest that oriental treatments have an effect on Panic Disorder with Functional Gastrointestinal Disturbance.
