뇌졸중 환자에 대한 기능적 보행평가의 신뢰도 (Reliability of the Functional Gait Assessment in Patients With Stroke)

  • After stroke, many people have problems with balance during movement. Balance is essential for the optimal functioning of the locomotor system and the performance of many activities of daily living. The Functional Gait Assessment (FGA) is a clinical tool for evaluating balance ability during walking. The test consists of ten tasks, seven tasks of the Dynamic Gait Index and three additional tasks. The purpose of this study was to evaluate the reliability and internal consistency of data obtained with the Korean version of the FGA when used with people after suffering a stroke. One-hundred participants, at least three months poststroke and able to walk at least six meters with or without a walking aid, participated in the study (age range=30~83 years; $mean{\pm}SD=58.8{\pm}10.9$). Two physical therapists and two physical therapy students rated the FGA. Intrarater and interrater reliability of the FGA were assessed using kappa statistic and intraclass correlation coefficients (2,1). The internal consistency of the FGA was assessed using the Cronbach alpha. The Cronbach alpha was good (${\alpha}$=.86~.93). The intrarater (intraclass correlation coefficient=.92~.95) and interrater reliability (intraclass correlation coefficient=.91, .95) of the total scores administered by the therapists and students were good, whereas the reliability for single item scores when administered by the physical therapists was moderate to good (kapa value=.42~.97). This study found that intrarater and interrater reliability for total FGA scores and internal consistency were good. Therefore, the Korean version of the FGA can be used as a reliable tool to assess the functional gait performance of patients after stroke.

Design of Simple-Structured Fuzzy Logic Systems for Segway-Type Mobile Robot

  • Studies on the control of the inverted pendulum type system have been widely reported. This is because it is a typical complex nonlinear system and may be a good model for verifying the performance of a proposed control system. In this paper, we propose the design of some fuzzy logic control (FLC) systems for controlling a Segway-type mobile robot, which is an inverted pendulum type system. We first derive a dynamic model of the Segway-type mobile robot and then analyze it in detail. Next, we propose the design of some FLC systems that have good performance for the control of any nonlinear system. Then, we design two conventional FLC systems for the position and balance control of the Segway-type mobile robot, and we demonstrate their usefulness through simulations. Next, we point out the possibility of simplifying the design process and reducing the computational complexity,, which results from the skew symmetric property of the fuzzy control rule tables. Finally, we design two other FLC systems for position and balance control of the Segway-type mobile robot. These systems have only one input variable in the FLC systems. Furthermore, we observe that they offer similar control performance to that of the conventional two-input FLC systems.

The effect of focus of attention by electroencephalogram-feedback on balance in young adults

  • Objective: Electroencephalogram (EGG)-feedback is a training procedure aimed at altering brain activity, and is used as a treatment for disorders like attention. The purpose of this study was to determine the effects of external focus of attention by EGG-feedback on balance in young adults. Design: Cross-sectional study. Methods: Subject were students in Sahmyook University. Fifty young adults in their twenties and thirties. Subjects were performed both with and without external focus of attention by EEG-feedback on the posture of standing and tandem standing. Participants were educated effort to maintain static posture when they were under internal focus of attention. Good Balance System was used for measurement of postural consistency upon the following force platforms. Results: Body sway decreased significantly both normal standing and tandem standing with external focus of attention by EEG-feedback (p<0.05). Conclusions: The results demonstrate that the benefits of an external attentional focus are generalizable to young adults. The external focus of attention outperformed the internal focus of attention on the postural balance (p<0.05). It is showed that external focus of attention significant effects on balance by revoked automatic postural control of movement. Furthermore balance might be improved by training with an external focus. Further study is required to develop for training as a method of preventing fall in elderly peoples.

코어 프로그램이 20대 여성의 정적균형에 미치는 영향 (Effects of Core Program Exercise on Static Balance of Females in Their Twenties)

  • Purpose : The purpose of this study was to investigate the effects of core exercise on the static balance for females in their twenties. Methods : The core exercise program was conducted 3 times per week for 4 weeks. The result is based on the analysis of measuring static balance before and after the core exercise program. By using the GOOD BALANCE system, the assessment of the static balance ability was made before and after the exercise of 6 postures: normal standing, one left leg?and one right leg standing with eyes open and closed respectively. For each case, the experimental data were obtained in 3 items: mean X speed, mean Y speed, and velocity moment. Statistical analysis was performed using SPSS/Win(10.0) version and Willcoxon signed rank test. Results : From the result of this study, we found that mean X speed, mean Y speed and velocity moment of total postures significantly increased after the 4 week program of core exercises. Conclusion : These finding indicate that core exercises could be beneficial to females in their twenties due to the effect of increasing static balance.

Family 금형 제작에서의 유동 밸런스 및 휨특성에 관한 연구 (A Study on flow Balance and Warpage Characteristics in Manufacturing of Plastic Injection Family Mould)

  • In the present study, the characteristic of warpage and flow balance for family mould, which is able to mold parts with different shapes in a mold, is considered. To obtain an optimal gate and runner system, plastic injection molding analysis with commercial code is performed. Design and manufacturing of family mould is then carried out on the basis of this computer aided engineering result. Flow balance and warpage comparisons between experiment and numerical analysis give good agreement with each other. However, it was shown that results of warpage measured by CMM was about $20\~55\%$ lower than those of numerical analysis.

Effects of balance imagery of semi-tandem stance on a flat floor and balance beam for postural control: a comparison between older and younger adults

  • Objective: Balance is a preceding task for functional activities in daily activities as well as community-dwelling activities. To learn skilled and functional activities, it is also necessary to imagine an appropriate and effective movement representation used to plan and execute the functional activities. The purpose of this study was to evaluate the effects of balance imagery of semi-tandem stance on a flat floor and balance beam on balance abilities for elderly and young adults. Design: Cross-sectional study. Methods: Fifteen elderly and thirty-four young adults were enrolled in this study. In order to determine whether there is a change in postural control ability according to the different imagery training methods used, standing static balance measurements were performed. According to the therapist's instructions, participants were to stand in a semi-tandem position on the Good Balance System for 1 minute while imagining that they were standing on a balance beam, and while the postural control abilities was assessed. Results: Postural control was significantly different in balance ability of semi-tandem stance on a flat floor compared to on a balance beam in both geriatrics and young adults. Postural sway was more significantly decreased in young adults than older adults during balance imagery of semi-tandem stance on a flat floor as well as on balance beam (p<0.05). Conclusions: The results of this study suggest that the ability to mentally represent their actions was similar in older adults compared to young adults, although older adults showed a drop in efficiency of postural control more than young adults.

힘평형 서보 가속도계의 질량지지 장치 설계 및 제작 (Design and Fabrication of a Mass-spring System for the Force-balance Servo Accelerometer)

  • 힘평형 서보 가속도계의 성능특성을 결정하는 4개의 팔로 구성된 질량지지 장치를 설계·제작하였다. 팔의 두께와 인가질량의 변화에 따른 질량지지 장치의 응답특성을 유한 요소법을 이용하여 계산하고, 레이저 간섭계를 이용하여 간섭무의 수의 변화와 응답 전압값을 이용하여 가속도계의 응답특성을 측정하였다 팔의 길이와 두께가 변화함에 따라 응답특성은 크게 변화하였으며, 인가질량의 변화는 질량지지 장치의 응답특성변화에 큰 영향을 미치지 못함을 알 수 있었다. 수치해석과 레이저 간섭계로 측정한 공진주파수가 5%차이의 근사한 값을 얻을 수 있었다.

Influence of Unilateral Muscle Fatigue in Knee and Ankle Joint on Balance and Gait in Healthy Adults

  • Purpose: This study was to investigate the effects of unilateral muscle fatigue in knee and ankle joints on balance and gait in healthy adults. Methods: Exercise inducing muscle fatigue in the knee joint consisted of concentric and eccentric contraction of dominant knee extensors in healthy adults by using the Leg Extension Rehap exercise machine (HUR, Finland). Exercise inducing muscle fatigue in the ankle joint was composed of voluntary contractions and forced contractions of the dominant plantar flexors in healthy adults. Exercises to induce muscle fatigue in the knee and ankle joints were performed until the subject complained of fatigue or pain, the occurrence of muscle fatigue was confirmed by electromyography. We measured static and dynamic balance using the Good Balance system and gait performance by RS-scan. Results: Static and dynamic balance ability and spatial-temporal gait decreased significantly after muscle fatigue in knee and ankle joint. Conclusion: These results show that unilateral muscle fatigue of the lower extremities affected postural control and gait. Therefore, therapists and sport trainers should minimize the risks of fall and injuries related to unilateral muscle fatigue.

Effects of Compelled Weight Shift on Balance Ability in Patients with Stroke

  • Purpose: This study aimed to investigate the effects of compelled weight shift in paretic lower limb training on balance ability in patients with stroke. Methods: Thirty-six individuals with hemiparesis, who were randomly assigned to a 10CWST (10 mm constrained -weight shift training) group, a 5CWST (5 mm constrained-weight shift training) group, and a control group participated in this study. Compelled weight shift training was performed in 3 sets of 5 min with a rest period of 1 min between sets. Both the 5CWST and 10CWST groups performed 5 times per week for 4 weeks. Static (mediolateral and anteroposterior sway velocities) and dynamic balance (mediolateral and anteroposterior distances) was assessed using the Good Balance system. Results: Significant differences were found in the M-L and A-P sway velocities, and the M-L sway distance. The M-L and A-P sway velocities, and M-L sway distance showed significantly large group effects (p<0.05), time effects (p<0.05), and group-by-time interaction (p<0.05). The post hoc analyses indicated that, following intervention, the 10CWST group showed more significant changes in the M-L and A-P sway velocities, and the M-L sway distance than the control group. Conclusion: These results suggest that the use of compelled weight shift in paretic lower limb training may be an effective method to improve balance ability in patients with stroke.

뇌졸중 환자의 트레드밀 훈련 시 손잡이 유무 및 위치가 보행 및 균형에 미치는 영향 (The Effects of Handrails during Treadmill Gait Training in Stroke Patients)

  • Purpose: The purpose of this study was to examine the influence of a handrail (presence and position) on treadmill gait and balance in stroke patients during gait training. Methods: 39 patients with stroke (male 31, female 8) participated in this study. The training groups were classified into a no-handrail group (NHG), front handrail group (FHG), and bilateral handrail group (BHG). Each group comprised 13 subjects. The subjects were trained to walk in a straight path 30 minutes per day for 8 weeks. The Good Balance System was used to measure static balance and dynamic balance. To measure walking ability, timed up and go (TUG) was also assessed. Results: The NHG showed no significant differences in static balance, dynamic balance, and TUG. The FHG was significantly different in their medial-lateral speed of static balance, dynamic balance, and TUG. The BHG was significantly different in their static balance, dynamic balance, and TUG. Conclusion: These findings consider the effects of holding handrails concomitantly with changes in postural stability. We conclude that for training stroke patients, treadmill walking while holding handrails improves balance and gait more than treadmill walking without holding handrails. The resulting changes in muscle activity patterns may facilitate the transfer to a gait pattern. The results of this study suggest methods for training treadmill walking in stroke patients.
