Korean Red Ginseng increases defective pol gene in peripheral blood mononuclear cells of HIV-1-infected patients; inhibition of its detection during ginseng-based combination therapy

  • Background: We have reported that defective nef and gag genes are induced in HIV-1-infected patients treated with Korean Red Ginseng (KRG). Methods: To investigate whether KRG treatment and highly active antiretroviral therapy (HAART) affect genetic defects in the pol gene, we amplified and sequenced a partial pol gene (p-pol) containing the integrase portion (1.2 kb) by nested PCR with sequential peripheral blood mononuclear cells over 20 years and compared it with those patients at baseline, in control patients, those taking ginseng-based combination therapy (GCT; KRG plus combinational antiretroviral therapy) and HAART alone. We also compared our findings to look for the full-length pol gene (pol) (3.0-kb) Results: Twenty-patients infected with subtype B were treated with KRG for $116{\pm}58months$ in the absence of HAART. Internal deletion in the pol gene (${\Delta}pol$) was significantly higher in the KRG group (11.9%) than in the control group and at baseline; its detection was significantly inhibited during GCT as much as during HAART. In addition, the ${\Delta}pol$ in p-pol significantly depended on the duration of KRG treatment. In pol, the proportion of ${\Delta}pol$ was significantly higher in the KRG group (38.7%) than in the control group, and it was significantly inhibited during GCT and HAART. In contrast, the proportion of stop codon appeared not to be affected by KRG treatment. The PCR success rate was significantly decreased with longer GCT. Conclusion: The proportion of ${\Delta}pol$ depends on template size as well as KRG treatment. HAART decreases the detection of ${\Delta}pol$.

A Rare Case of Mediastinal Granular Cell Tumor

  • The granular cell tumor (GCT) occurs extremely rarely in the mediastinum. Few mediastinal GCT cases have been reported in Japan or other countries. Here, we report a case of a 24-year-old man with superior mediastinal GCT. The mass was located just above the aortic arch. It was firm, oval in shape, and well encapsulated. The tumor was removed completely with video-assisted thoracoscopic surgery, but we had to resect the vagus nerve, which was already included in the tumor, along with the tumor. After the operation, the patient recovered without any specific complications except for a mild degree of hoarseness.

  • Esophageal granular cell tumor (GCT) is a rare neoplasm originating from the Schwann cells of the submucosal neuronal plexus. Histology is the gold standard for its diagnosis. Endoscopic resection or surgical excision should be considered, depending on the potential for malignancy. Here, we report a case of an esophageal GCT in an adolescent. A 12-year-old boy presented with a 1-year history of dysphagia and vomiting. Upper gastrointestinal endoscopic examination and esophagography showed narrowing of the midesophagus, and computed tomography angiography of the thoracic aorta revealed an esophageal or periesophageal mass posterior to the paratracheal segment of the esophagus. The tumor was surgically excised, and based on the pathological findings, esophageal GCT was diagnosed.

Stimulation of Osteogenic Differentiation in Stromal Cells of Giant Cell Tumour of Bone by Zoledronic Acid

  • Therapeutic effects of zoledronic acid (ZOL) on giant cell tumour of bone (GCT) have been proven. Apoptosis induction was considered to be one of the mechanisms of ZOL tumour inhibition. In this study, we presented the possibility of an osteogenic differentiation stimulation mechanism of ZOL and further investigated dosage and time effects. We treated stromal cells of GCT (GCTSC) with ZOL for 48 hours at different concentrations ($0{\mu}M$, $0.01{\mu}M$, $0.1{\mu}M$, $1{\mu}M$, 5${\mu}M$, $30{\mu}M$) and assessed apoptotic and osteogenic differentiation markers with immunohistochemical techniques and real-time quantitative RT-PCR. Our results suggested that ZOL enhanced mRNA expression of Cbfa-1, osterix and osteocalcin genes with a maximum effect at $1{\mu}M$ in GCTSC. Time course experiments indicated a time dependent osteogenic differentiation effect. In conclusion, ZOL may be considered as an adjuvant in the treatment of GCT not only by inducing apoptosis but also by stimulating osteogenic differentiation of remaining tumor stromal cells after surgery.

Giant cell tumor of temporomandibular joint masquerading as temporomandibular joint pain dysfunction syndrome: a rare case report

  • Giant cell tumor (GCT) of the craniofacial bones has been reported but they are not common. This tumor occurs more often in women than in men and predominantly affects patients around the third to fifth decade of life. GCTs are generally benign but can be locally aggressive as well. We report a case of GCT involving the temporomandibular joint (TMJ), which was initially thought to be temporomandibular disorder (TMD). A 22-year-old female presented with swelling and pain over the right temporal region for 18 months associated with jaw locking and clicking sounds. On examination, her jaw deviated to the right during opening and there was a $2{\times}2$ cm swelling over the right temporal region. Despite routine treatment for TMD, the swelling increased in size. Computed tomography and magnetic resonance imaging of the brain and TMJ revealed an erosive tumor of the temporal bone involving the TMJ which was displacing the temporal lobe. Surgical excision was done and the tumor removed completely. Histopathological examination was consistent with a GCT. No clinical or radiological recurrence was detected 10 months post-surgery.

방사선 조사 없이 속발한 거대 세포종의 악성 전환 (Malignant Transformation of Giant Cell Tumor Not Associated with Radiotherapy)

  • 목적 : 거대 세포종이 방사선 조사 없이 악성 전환하는 것은 매우 드문 경우로 저자들이 경험한 4례를 분석하여, 흔하지 않은 악성 전환된 거대 세포종의 진단과 치료의 방침을 제시하는데 도움이 되고자 한다. 대상 및 방법 : 1985년 9월부터 2001년 1월까지 본원 정형외과에 내원하여, 수술적 치료를 시행했던 거대 세포종 112례 중 방사선 조사 없이 악성화한 4례를 대상으로 하였으며, 악성전환 후 평균 추시 기간은 2.4년(최저 1.3년, 최고 4년)이었다. 결과 : 거대 세포종의 진단 후 악성 전환 때까지의 기간은 평균 6.9년(최저 2.2년, 최고 13.5년)이었고, 근위 상완부 연부 조직, 근위 대퇴골, 원위 대퇴골, 근위 경골에 각각 1례씩 발생하였다. 3례에서는 골 육종으로, 1례에서는 악성 섬유성 조직구종으로 악성 전환이 일어났으며, 국소 재발은 1례, 폐 전이는 골 육종으로 악성 전환한 3례에서 모두 관찰되었다. 결론 : 방사선 조사 없이 속발한 거대 세포종의 악성 전환을 진단하기 위해서는 병리 조직의 철저한 채취가 매우 중요한 요소라 사료되며, 방사선 소견상 악성 전환이 의심될 때는 부적절한 절제술을 시행하기보다는 절개 생검을 먼저 시행한 후 치료 방침을 정해야 할 것으로 사료된다.

  • Purpose: Synthetic absorbable monofilaments offer excellent glide characteristics and cause minimal tissue trauma as a result of their smooth monofilament structure and gradual absorption within the healing tissues. For these reasons, these suture materials are commonly used in various surgical fields such as gastroenterology, urology, gynecology, and plastic surgery. The aim of this study was to evaluate the safety and usefulness of a new synthetic absorbable monofilament, Glycoside-..-caprolactone-trimethylene carbonate interpolymer (GCT), in gastrointestinal anastomosis and closure. Materials and Methods: We evaluated 55 gastrointestinal anastomoses and closures using GCT $MONOSYN^{R}$, B. Braun, Germany) in 47 patients who underwent gastric surgery between December 2001 and May 2002 at Seoul National University Hospital. Patient's characteristics, operative procedure, surgeon's opinion of handling properties of GCT, and suture-related complications were analyzed. Results: There were 34 males and 13 females (M:F= 2.6:1) with an average age of 54.2 years old. Forty-five cases of gastrointestinal anastomosis (20 gastrojejunostomies and 25 jejunojejunostomies) and 10 cases of intestinal closure (7 gastrostomy closures and 3 duodenal stump closures) were performed in 41 cases of stomach cancer, three of peptic ulcer disease, two of GIST, and one MALToma. The handling properties of GCT according to the criteria of knot breaking load, knot security, and placing property were always scored with 7 to 9 points (10=excellent, 1=very poor). Two cases of postoperative complications ($3.6\%$) were noted. One was a leak of the gastrojejunostomy site which was successfully managed conservatively, and the other was a stricture of the gastrojejunostomy site which was managed by reoperation (side-to-side jejunojejunostomy). Conclusion: GCT seems to be an applicable suture material for various gastrointestinal anastomoses and closures.

남성유방에서 과립세포종양의 증례 보고 (A Case of Granular Cell Tumor of the Breast in a Male Patient)

  • 52세 남자 환자가 우측 유방의 바깥부위에서 무통의 단단한 종괴를 호소하였다. 흉부 CT에서 3.3 cm 크기의 원형의 미세소엽상 경계의 조영증강되는 종괴가 확인되었고, 초음파에서는 미세소엽상 경계를 보이는 저에코성 종괴로, 종괴의 내부에는 석회화와 혈류가 보였다. 중심부바늘생검에서 종괴는 양성 과립세포종양(granular cell tumor; 이하 GCT)으로 확진되었다. 환자는 타병원으로 전원되어 병변의 수술적 제거를 시행하였다. 과립세포종양은 슈반세포(schwann cell)에 유래하는 종양으로, 유방에서는 드물고 대부분은 양성이다. 과립세포종양의 임상적 및 방사선학적 특징은 악성 종양과 유사한 소견을 보여 감별이 어려운 경우가 있으며, CT 영상은 거의 보고된 바가 없다. 본 증례를 통해 임상의와 방사선 전문의가 드물지만 양성종양인 GCT의 영상의학적 특징을 알고, 이를 악성종양과 감별하여 과도한 치료를 피해야 할것이다.

  • 원발성 전 종격동 종양은 종격동 종양의 반이상을 차지하고 그 종류도 다양하다. 흉선 상피 종양이 가장 흔하고 악성 흉선종은 드물다. 생식세포종은 두 번째로 흔한 전 종격동 종양으로 보다 어린나이에 발생하며 대부분 양성이다. 임파종은 대부분 전신성 질환의 흉부 침습 형태로 나타나나 원발성 흉부 임파종은 Hodgkin씨 병이 많다. 저자는 대표적인 전종격동 종양의 임상증상과 영상의학적 소견을 소개한다.
