A Study on the Analysis of Physical Function in Adults with Sarcopenia (근감소증 성인의 신체 기능 분석)

  • Purpose : This study used a sarcopenia diagnostic algorithm proposed by the Asia working group in adults over 50 to diagnose sarcopenia and analyze body function. The purpose of this study is to prepare basic data for the management and prevention of sarcopenia. Methods : We performed a diagnostic evaluation of sarcopenia in 97 adults over the age of 50 years with the cooperation of the Seongnam senior experience complex in Seongnam-si, Gyeonggi-do. As a result of the diagnostic process, 24 subjects were placed into the sarcopenia group, while 73 subjects were placed into the normal group. We measured each subject's body, performed the timed up and go test to evaluate functional mobility, and conducted a questionnaire on the pre-symptom of locomotive syndrome and locomotive syndrome. Results : There were statistically significant differences in height, weight, and skeletal muscle mass between the two groups. There was also a statistically significant difference in the timed up and go test, which confirmed the difference in functional mobility between the two groups. In addition, there was a statistically significant difference between the two groups in the proportion and the mean score of subjects with pre-symptom of locomotive syndrome and locomotive syndrome. In the correlation analysis, grip strength was statistically significantly correlated with height, weight, skeletal muscle mass, waist circumference, timed up and go test, pre-symptom of locomotive syndrome and locomotive syndrome. Gait speed was significantly correlated with the timed up and go test and locomotive syndrome. Appendicular skeletal muscle index was significantly correlated with height, weight, waist circumference, hip circumference, and the pre-symptom of locomotive syndrome. Conclusion : In conclusion, sarcopenia is closely related to height, weight, skeletal muscle mass and functional mobility, as well as the pre-symptom of locomotive syndrome and, locomotive syndrome. In consideration of this, the prevention and management of sarcopenia should be made accordingly.

The Effect of GEIK Programs for the Gifted Children upon Logical Thinking and Creativity. (영재교육 프로그램이 논리적 사고와 창의성에 미치는 효과)

  • The purpose of the study is to examine the effects of Gifted Education Isnstitute of Korea (GEIK) programs for gifted children especially in the areas of reasoning skills and creativity, thereby proving the effectiveness of the program. The subjects are 136 (103 boys and 33 girls) fourth, fifth, and sixth grade gifted children, who have participated in GEIK programs for more than six moths. They were stratified by the length of participation in GEIK programs. Ninety four children have participated for more than one year. Forty-two children have participated for less than one year. Both groups are rather homogeneous in IQ scores and school achievement levels at the time of enterance into GEIK programs. Both a Group Assessment of Logical Thinking (GAIT) and a Creativity test were used for the study on reasoning skills and creativity. GALT, developed by V. Roadranka, R. H. Yeany and M. J. Padilla in 1983, consists of 12 questions. It is classified into six subscales: conservation, proportional reasoning, controlling variables, provability reasoning, correlational reasoning, and combinatorial reasoning. The reliability of this test is .85. This test recommends to classify the stages of child development as follows according to the total test score. 0-4 point: Concrete Stage, 5-7 points: Transitional Stage, and 8 and above points: Formal stage. The Creativity Test was developed by Y. Lee and W. Chung (1971). It consists of four components: fluency, flexibility, originality, and openness. Only both fluency and openness were used in this study. In order to analyze data, T-Test, Intercorrelational Analyses, ANOVA, and Nultiple Regression were used. Followings are the results deduced from the above analoyses of the data. First, 43.48% of the subjects were on Concrete Stage, 36.78% were on the Transitional Stage, and 19.86% were on the Formal Stage in the developmental level classified by Piaget. Second, the students who have participated in GEIK programs more than one year acquired significantly higher score in GALT than the students who have participated in GEIK programs less than one year. Third, boys showed higher score in GALT than girls did. Fourth, there were statistically significant intercorrelations between six subscales of GALT. Fifth, the students who have participated in GEIK programs more than one year acquired significantly higher score in openness of creativity test than the students who have participated in GEIK programs less than one year. There were no significant differences in openness of creativity test between boys and girls. Sixth, the students who have participated in GEIK programs more than one year acquired significantly higher score in fluency of creativity test than the students who have participated in GEIK programs less than one year. Girls showed higher score in fluency of creativity test than boys did. Seventh, the students who acquired higher score in GALT showed higher score in both openness and fluency of creativity test. Followings are the conclusions deduced form the above results. First, the developmental level of reasoning skills of the fourth grade students participationg in GEIK programs is the same as that of 7th grade of normal Korean students and the same as those of 10th grade of U.S.A. and Philipoine students. Second, the GEIK programs are effective in improving reasoning skills. Third, the GEIK programs are effective in improving creativity. Fouth, reasoning skills and creativity can be improved by well planned programs. In conclusion, this study suggests that beyond reasoning skills and creativity, other areas such as areas in science skills, mathmatical skills, or verbal skills, etc., should be studied in the future.

A Comparative Analysis on Changes of Foot Pressure by Shoe Heel Height during Walking (하이힐 굽 높이에 따른 보행 시 족저압 변화 비교 분석)

  • We aimed to determine the effect of heel height on foot pressure by comparing and analyzing data on foot pressure in shoes with different heel heights. Qn the basis of a previous study, we selected 3cm and 7cm as the shoe heel heights preferred by female college students. We divided 10 female students into forefoot and hindfoot to measure vertical force, maximum pressure, and average pressure. The average pressure on the forefoot was higher and that on the hindfoot was lower in the case of 7cm high-heeled shoes. The maximum pressure on the forefoot was significantly higher in the case of the 7cm heel height (p<.05). The vertical force, maximum pressure, and average pressure on the hindfoot were also significantly higher in the case of the 7cm heel height (p<.05). The results showed that wearing 7cm high-heeled shoes exerted greater maximum pressure on the forefoot and greater vertical force, maximum pressure, and average pressure on the Hndfoot. This leads to increase in confining pressure caused by high pressure distribution over the forefoot and increase in the pressure on the hindfoot, which may cause deformation of toes and heel pain over a long period. Therefore, female college students who wish to wear high heels are recommended to wear 3cm high-heeled shoes rather than 7cm high-heeled shoes.

A Study on the Design Criteria of Pedestrian Facility (Stairs) by Motion Analysis of Walking Parameters in the Elderly (고령자 보행변수 실측을 통한 보행시설물 설계기준 정립 1: 고령자 보행특성을 고려한 계단 챌면 높이 연구)

  • In Korea, the number of elderly has been increasing rapidly. So it is also expected that the economic activity and the trip frequency of the elderly will increase. On the other hand, elderly related accidents such as falls during walking are steadily increasing and the satisfaction about pedestrian environment of elderly is very low. In this paper, we found one of the reasons for these dissatisfaction in pedestrian facility, which is not considering the walking ability (about 75% of non-elderly person) of the elderly. So, we analyze the kinematic walking characteristics of the elderly with the motion analysis system, when the elderly use stairs. As a result of analysis of various walking variables, the current standard for stairway height in Korean law (18cm) requires excessive force to elderly so it was difficult for elderly to keep the balance of the body in ascending and descending walk of stairs. In this paper, we propose the stair design criteria through the cluster analysis of walking parameters reflecting the gait characteristics of the elderly. This change is not a big for non-elderly person, but it can promote more socioeconomic activities for the elderly.

A Clinically Diagnosed Case of Multiple Epiphyseal Dysplasia (임상적으로 진단된 다발성 골단이형성증 1례)

  • Multiple epiphyseal dysplasia (MED) is one of the more common skeletal dysplasias. MED is characterized by joint pain and stiffness, a waddling gait, and/or mild short stature in childhood. Radiographic findings include delayed and irregular ossification of the epiphyses in multiple joints. Mutations in at least six different genes (COMP, MATN3, COL9A1, COL9A2, COL9A3, and DTDST) can cause MED, and it can be either dominant or recessive inheritance. Molecular diagnosis is important for accurate prognosis and genetic counselling. COMP mutation is the most common form of MED in Western. But, MATN3 mutation was reported as the most common type of MED in Korea. Here, we describe a boy who was diagnosed as MED by clinical and radiological features. Hip radiograph of the patient was suggested MATN3 mutation. But knee radiograph was suggested COMP mutation. MATN3 and COMP mutations direct sequencing, but were no mutation. So we tested whole exome sequencing, but significant variant was not detected as known MED six genes mutations. The patient was diagnosed as having MED clinically and radiologically. Further study to identify the other responsible genes for MED is needed.

Development of Closed-loop Control Type FES System for Restoration of Gait in Patients with Foot Drop (족하수 환자의 보행보조를 위한 피드백 제어형 전기자극기 개발)

  • The purpose of this study was to develop a portable and convenient closed-loop contrel type electrical stimulator for patients with foot drop. This system restores walking movement as well as prevents from atrophy or necrosis of lower limb muscles and increases blood circulation in hemiplegic patients caused by traffic accident, industrial disaster or stoke. This system detects the changes of the ankle joint angle during walking, and then controls the stimulus intensity automatically to maintain the programmed level of the ankle joint angle. Also, this automatic system controls the stimulus intensity which is affected by increased electrode impedance resulting from long time use. The system detects the joint angle by an optical sensor and includes modified PID control which adjusts the stimulus intensity if the joint angle deviates from the preset value. Stimulus parameters are 30~80 volt, 40 Hz, and 0.2 ms. The system was applied to five hemiplegic patients for 42 days. Duration of stimulation was 15 min/day for the first week and then the duration was gradually increased to 30, 60, 90 and 120 min/day. The muscle force was increased up to 29.7%, muscle fatigue was decreased compared with the level before stimulation and the pattern of locomotion was improved. These results suggest that the electrical stimulator with closed-loop control type is more convenient and effective in restoration of locomotion of patients with foot drop than open-loop system.

A Study on the Evacuation Characteristics Junction Indication Method of Disaster Information Map (재해정보지도의 교차경로 표시방법에 따른 피난특성에 관한 연구)

  • The purpose of this study was to assess the evacuation route in a disaster information map to evacuate people to a shelter quickly and precisely during a disaster situation. For this purpose, this study examined the current status of domestic and international disaster information maps and related laws and conducted experiments to derive effective types of intersecting routes. The problems of the disaster information map were obtained through the first preliminary experiment and a survey. Based on these problems, experiments for each type were conducted to provide effective indication information for a disaster information map. To investigate the gait characteristics according to the number of crossing paths, the reduction rate in the crossing path was derived for each type by comparing the previous speed and passing speed. This will enable suggestions to judge the route quickly and accurately when determining the intersecting route in the search for an evacuation route. In conclusion, the experiment of this paper aims to make rapid and accurate evacuations using the disaster information map in response to disasters, and provide guidelines to citizens to contribute to a reduction of casualties.

The Change of ${\alpha}$-motor neuron excitability in Spastic Stroke Patients by Pre-tibia Muscle Isometric Contraction (전경골근 등척성 수축에 의한 경직성 뇌졸중 환자의 비복근 ${\alpha}$-운동 신경원 흥분 변화)

  • Spasticity has been defined as "a motor disorder characterized by a velocity-dependent increased in tonic stretch reflexes with exaggerated tendon jerks, resulting from hyperexcitability of the stretch reflex, as one components of the upper motorneuron syndrome". Spasticity is one of the common symptoms of stroke patients and frequently interferes with the motor functions such as gait, posture and activities of daily living. Therefore, its management is becoming a major issue in physical therapy. The purpose of this study was to determined the effects of reciprocal inhibition by isometric contraction of pre-tibia muscle on spasticity in hemiplegic patients through Hoffmann reflex. The subjects were consisted 45 patients who had hemiplegia due to stroke. All subjects randomly assigned to 3 group: manual reciprocal inhibition program group(manual group), neuromuscular electrical stimulation group(NMES group) and control group. The manual group received voluntary isometric contraction of pre-tibia muscle. The NMES group received neuromuscular electrical stimulation on tibialis anterior. The control group was not received any therapeutic intervention. Before and after experiments, Hoffmann reflex, M-wave and Modified Ashworth scale was measure in all patients. The data of 30 patients who complete experimental course were statistically analysed. Modified Ashworth scale were significantly decreased after experiment in manual group(p<.01). The Hmax/Mmax ratios were significantly decreased after experiment in manual group(p<.o1). There were no statistical difference between pre-test and post-test with modified Ashworth scale in NMES group(p>.01). There were no statistical difference between pre-test and post-test with Hmax/Mmax ratios in NMES group(p>.01). There were no statistical difference between pre-test and post-test with modified Ashworth scale in control group(p>.01). There were no statistical difference between pre-test and post-test with Hmax/Mmax ratios in control group(p>.01). The present results revealed that reciprocal inhibition which produced by voluntary isometric contraction of pre-tibia muscle can be reduce spasticity of gastrocnemius. Therefore, reciprocal inhibition is useful to improve functional activities in hemiplegic patient. Further study should be done to analyse the effects of intervention duration of reciprocal inhibition, appropriate muscle contraction, optimal time to apply the reciprocal inhibition in more long period.

Study on Asymmetrical EMG Activation Pattern of Selected Trunk and Thigh Muscles on Gait Velocity of Individuals With Post-Stroke Hemiparesis During Sit-to-Stand Movement (일어서기 동작시 편마비 환자의 보행 속도에 따른 체간과 하지 근 활성 형태의 비대칭성에 관한 연구)

  • This study aimed to identify the asymmetry observed in the electromyography (EMG) activity patterns of selected trunk and thigh muscles between the affected and unaffected sides during the sit-to-stand movement in ambulatory patients with post-stroke hemiparesis. This study included 20 patients with post-stroke hemiparesis. The differences between stroke fast walkers (${\geq}8m/s$, 11 subjects) and stroke slow walkers (<8 m/s, 9 subjects) were compared. The activation magnitude and onset time of the multifidus, lumbar erector spinae, hamstrings, and quadriceps during the sit-to-stand movement were recorded through surface EMG. Moreover, the EMG activation magnitude and onset time ratios of each bilateral corresponding muscle from the trunk and leg were measured by dividing the relevant values of the unaffected side by those of the affected side. In all the subjects, the activation magnitudes of the multifidus, hamstring, and quadriceps on the affected side significantly decreased compared to those on the unaffected side (p<.05). The onset time of muscle activity in the affected side was markedly delayed for the multifidus and quadriceps during the task (p<.05). The activation magnitude ratios of the quadriceps were markedly decreased in the stroke slow walkers as compared to those in the stroke fast walkers. These findings indicate that the asymmetry in the multifidus, hamstring, and quadriceps muscle activation patterns in patients with post-stroke hemiparesis may be due to the excessive muscle activation in the unaffected side to compensate for the weakened muscle activity in the affected side. Our findings may provide researchers and clinicians with information that can be useful in rehabilitation therapy.

Effect of Dietary Zinc and Phytic Acid Levels on Protein Metabolism in Rats (아연과 Phytic Acid 수준을 달리한 식이가 흰쥐의 단백질 대사에 미치는 영향)

  • In order to investigate the effect of dietary zinc and phytic acid levels on protein metabolism in rats, male rats of Sprague-Dawley strains weighing approximately $60\~74g$ were fed different diets which contained 0, 0.35 and $1.05\%$ phytic acid each at 3 levels of zinc(0, 30 and 1,500 ppm zinc) for 28 days. Result obtained in this experiment are summarized as follows; 1. Body weight gait food consumption food efficiency ratio and protein efficiency ratio were lower in the rats fed zinc deficient diet(0 ppm zinc) than in those consuming 30 or 1,500 ppm dietary zinc, and the additional effect of phytic acid were not observed in all of then 2. Liver weight was lower in the rats fed 30 ppm zinc diet than in those fed 0 or 1,500 ppm-zinc diet but kidney and spleen weights were lower in the rats fed zinc deficient diet than in those fed 30 or 1,500 ppm-zinc diet Among organs measured only the liver appeared to be influenced by dietary phytic acid: the more the dietary phytic acid, the more the weight of liver, 3. Fecal nitrogen was decreased in the rats fed zinc deficient diet compared with those fed 30 or 1,500 ppm dietary zinc. Urinary nitrogen was increased in the rats fed $1.05\%$ dietary phytic acid compared with those fed 0.35 or $0\%$ dietary phytic acid Nitrogen retention of rat was influenced by neither dietary zinc nor phytic acid. 4. Urea nitrogen was decreased with increasing dietary zinc levels, and creatinine and uric acid levels were increased with increasing dietary zinc concentration or with additional quantity of phytic acid. Uric acid appeared to be influenced by zinc x phytic acid interaction; especially, the presence of phytic acid in the 30 ppm-zinc diet had significant effect on uric acid content. 5. Hemoglobin concentrations and hematocrit ratio were higher in the rats fed 30 ppm dietary zinc than in those fed 0 or 1,500 ppm-zinc diet Serum zinc concentration was increased with increasing dietary zinc levels. The content of total protein albumin and BUN and the ratio of albumin to globulin in serum, and protein content in liver were influenced by neither dietary zinc nor phytic acid.
