Effects of Oviductal Fluid, Culture Media and Zona Pellucida Removal on the Development of Porcine Embryos by Nuclear Transfer

  • The aim of this study was to compare the effects of oviductal fluid, porcine zygote medium (PZM)-3, PZM-4 and PZM-5, and modified PZM-5 culture media, and determine the effects of zona pellucida (ZP) removal on the development of nuclear transfer (NT) embryos. There were no significant differences in the rates of fusion and cleavage among the five different oviductal fluid concentrations. However, the rates of blastocyst formation and the cell numbers per blastocyst were high in the embryos at the 14 and 28 $\mu{g}$/ml concentrations of oviductal fluid compared to the 0, 56 and 100 $\mu{g}$/ml concentrations. The rates of cleavage and blastocyst formation, and the cell numbers per blastocyst were higher in the PZM-3, PZM-5 and modified PZM-5 media than in the PZM-4 medium. However, there were no significant differences in the fusion rates of oocytes among the four culture media. The cell numbers per blastocyst in the embryos without ZP were significantly greater than those with ZP. However, there were no significant differences in the rates of fusion, cleavage and blastocyst formation between the embryos with and without ZP. In conclusion, we improved blastocyst development and the quality of NT embryos by replacing PVA with 3 mg/ml of BSA in PZM-5 medium and supplementing the PZM-5 medium with 14 $\mu{g}$/ml oviductal fluid. The NT embryos produced by the zona-free NT method had a high rate of blastocyst formation in the modified PZM-5 medium.

Multi-Attribute Data Fusion for Energy Equilibrium Routing in Wireless Sensor Networks

  • Data fusion is an attractive technology because it allows various trade-offs related to performance metrics, e.g., energy, latency, accuracy, fault-tolerance and security in wireless sensor networks (WSNs). Under a complicated environment, each sensor node must be equipped with more than one type of sensor module to monitor multi-targets, so that the complexity for the fusion process is increased due to the existence of various physical attributes. In this paper, we first investigate the process and performance of multi-attribute fusion in data gathering of WSNs, and then propose a self-adaptive threshold method to balance the different change rates of each attributive data. Furthermore, we present a method to measure the energy-conservation efficiency of multi-attribute fusion. Based on our proposed methods, we design a novel energy equilibrium routing method for WSNs, viz., multi-attribute fusion tree (MAFT). Simulation results demonstrate that MAFT achieves very good performance in terms of the network lifetime.

Minimally Invasive Transforaminal Lumbar Interbody Fusion Using a Single Interbody Cage and a Tubular Retraction System : Technical Tips, and Perioperative, Radiologic and Clinical Outcomes

  • Objective : A minimally invasive transforaminal lumbar interbody fusion (MIS TLlF) has recently been introduced. However, MIS TLlF is a technically challenging procedure. The authors performed retrospective analysis about MIS TLlF using a single interbody cage. Methods : Twenty-eight consecutive patients were treated by MIS TLlF. Of these 28 patients, 20 patients were included in this retrospective study. Perioperative, clinical, and radiologic outcomes were assessed. Clinical outcomes were assessed using Oswestry Disability Index (ODI) and Visual Analogue Scores (VAS). Fusion rates and cross-sections of operated spinal canals were assessed by CT. Results : Twelve patients underwent MIS TLlF at one segment and 8 patients at two segments (L3/4: 4, L4/5: 17, L5/S1: 7). Operation time for a single segment was 131.7 min and for two segment was 201.4 min, and corresponding blood losses were 208.3 mL and 481.2 mL, respectively. ODI and VAS scores were significantly improved at 6 months postop (ODI from 30.32 to 15.54, VAS from 7.80 to 2.20, p = 0.001) Twenty-two segments (78.6%) achieved grade I fusion, 4 segments (14.3%) achieved grade II, 2 segments (7.1%) achieved grade III and 0 segments achieved grade IV at 12 months. Postoperatively at 12 months, spinal canal cross sectional areas at disc spaces significantly increased from 157.5 to $294.3\;mm^2$ (p = 0.012). Conclusion : MIS TLlF achieved good clinical outcomes and high fusion rates. Our findings show that MIS TLlF performed with a single Interbody cage and a tubular retractor system can be used as a standard MIS TLlF technique.

FusionScan: accurate prediction of fusion genes from RNA-Seq data

  • Identification of fusion gene is of prominent importance in cancer research field because of their potential as carcinogenic drivers. RNA sequencing (RNA-Seq) data have been the most useful source for identification of fusion transcripts. Although a number of algorithms have been developed thus far, most programs produce too many false-positives, thus making experimental confirmation almost impossible. We still lack a reliable program that achieves high precision with reasonable recall rate. Here, we present FusionScan, a highly optimized tool for predicting fusion transcripts from RNA-Seq data. We specifically search for split reads composed of intact exons at the fusion boundaries. Using 269 known fusion cases as the reference, we have implemented various mapping and filtering strategies to remove false-positives without discarding genuine fusions. In the performance test using three cell line datasets with validated fusion cases (NCI-H660, K562, and MCF-7), FusionScan outperformed other existing programs by a considerable margin, achieving the precision and recall rates of 60% and 79%, respectively. Simulation test also demonstrated that FusionScan recovered most of true positives without producing an overwhelming number of false-positives regardless of sequencing depth and read length. The computation time was comparable to other leading tools. We also provide several curative means to help users investigate the details of fusion candidates easily. We believe that FusionScan would be a reliable, efficient and convenient program for detecting fusion transcripts that meet the requirements in the clinical and experimental community. Comparison of the Effects of Sufentanil and Fentanyl Intravenous Patient Controlled Analgesia after Lumbar Fusion

  • Objective : Postoperative pain is one of the major complaints of patients after lumbar fusion surgery. The authors evaluated the effects of intravenous patient controlled analgesia (IV-PCA) using fentanyl or sufentanil on postoperative pain management and pain-related complications. Methods : Forty-two patients that had undergone surgery with lumbar instrumentation and fusion at single or double levels constituted the study cohort. Patients were equally and randomly allocated to a sufentanil group (group S) or a fentanyl group (group F) for patient controlled analgesia (PCA). Group S received sufentanil at a dose of $4{\mu}g/kg$ IV-PCA and group F received fentanyl $24{\mu}g/kg$ IV-PCA. A numeric rating scale (NRS) of postoperative pain was applied before surgery, and immediately and at 1, 6, and 24 hours (hrs) after surgery. Oswestry disability index (ODI) scores were obtained before surgery and one month after surgery. Opioid-related side effects were also evaluated. Results : No significant intergroup difference was observed in NRS or ODI scores at any of the above-mentioned time points. Side effects were more frequent in group F. More specifically, nausea, vomiting rates were significantly higher (p=0.04), but pruritus, hypotension, and headache rates were non-significantly different in the two groups. Conclusion : Sufentanil displayed no analgesic advantage over fentanyl postoperatively. However, sufentanil should be considerable for patients at high risk of GI issues, because it had lower postoperative nausea and vomiting rates than fentanyl.

The Impact of Menopause on Bone Fusion after the Single-Level Anterior Cervical Discectomy and Fusion

  • Objective : To evaluate the successful fusion rate in postmenopausal women with single-level anterior cervical discectomy and successful fusion (ACDF) and identify the significant factors related to bone successful fusion in pre- and postmenopausal women. Methods : From July 2004 to December 2010, 108 consecutive patients who underwent single-level ACDF were prospectively selected as candidates. Among these, the charts and radiological data of 39 women were reviewed retrospectively. These 39 women were divided into two groups : a premenopausal group (n=11) and a postmenopausal group (n=28). To evaluate the significant factors affecting the successful fusion rate, the following were analyzed : the presence of successful fusion, successful fusion type, age, operated level, bone mineral density, graft materials, stand-alone cage or plating with autologous iliac bone, subsidence, adjacent segment degeneration, smoking, diabetes mellitus, and renal disease. Results : The successful fusion rates of the pre- and postmenopausal groups were 90.9% and 89.2%, respectively. There was no significant difference in the successful fusion rate or successful fusion type between the two groups. In the postmenopausal group, three patients (10.8%) had successful fusion failure. In the postmenopausal group, age and subsidence significantly affected the successful fusion rate (p=0.016 and 0.011, respectively), and the incidence of subsidence in patients with a cage was higher than that in patients with a plate (p=0.030). Conclusion : Menopausal status did not significantly affect bone successful fusion in patients with single-level ACDF. However, in older women with single-level ACDF, the combination of use of a cage and subsidence may unfavorably affect successful fusion.

세포 융합액 중의$Ca^{2+}$ 농도가 소 체세포 핵이식란의 융합, 핵형 및 체외발육에 미치는 영향 (Effect of $Ca^{2+}$ Concentration in Fusion Medium on the Fusion, Nuclear Morphology and Development of Bovine Somatic Cell Nuclear Transfer Embryos)

  • 소 체세포 핵이식 시 융합액 내 $Ca^{2+}$ 농도에 따른 융합율, 핵형의 변화, 배 발달율을 검토한 결과는 다음과 같다. 1. 융합액 내 $Ca^{2+}$ 농도를 0.05, 0.1, 0.5 및 1.0 mM로 각기 다르게 처리한 결과, 0.5와 1.0 mM의 CaC$_2$에서 융합율이 80.5 와 84.3%로 나타나 0.05 mM CaCL$_2$에서의 융합율 56.6%에 비하여 유의적(P<0.01)으로 높았다. 2. 융합액 내 $Ca^{2+}$ 농도에 따른 재구축배의 핵형을 검토한 결과, CaCL$_2$농도 0.05와 0.1 mM 에서는 88.4와 84.0%의 난자가 PCC이후 염색질괴를 형성한 반면, 0.5와 1.0 mM 에서는 54.5와 59.3%가 PCC 형태를 거치지 않고 직접 전핵을 형성하여, $Ca^{2+}$ 농도가 증가함에 따라 PCC 형태를 거치지 않고 직접 전핵을 형성하는 난자의 비율이 증가하였다. 3. 핵이식란의 체외 발육율을 검토한 결과, 1.0 mM CaCl$_2$에서는 배반포 발육율이 30.6%로 나타난 반면, 0.1 mM CaCl$_2$에서 는 20.0%로 나타나 유의적(P<0.05)인 차이를 나타냈다. 본 연구의 결과는 융합액 내 $Ca^{2+}$농도의 증가가 소 체세포 핵이식란의 융합율 및 배반포 발육율을 향상시킬 수 있음을 시사한다.

Long Term Efficacy of Posterior Lumbar Interbody Fusion with Standard Cages alone in Lumbar Disc Diseases Combined with Modic Changes

  • Objective : Posterior lumbar interbody fusion (PLIF) is considered to have the best theoretical potential in promoting bony fusion of unstable vertebral segments by way of a load sharing effect of the anterior column. This study was undertaken to investigate the efficacy of PLIF with cages in chronic degenerative disc disease with Modic degeneration (changes of vertebral end plate). Methods : A total of 597 patients underwent a PLIF with threaded fusion cages (TFC) from 1993 to 2000. Three-hundred-fifty-one patients, who could be followed for more than 3 years, were enrolled in this study. Patients were grouped into 4 categories according to Modic classification (no degeneration : 259, type 1 : 26, type 2 : 55, type 3 : 11). Clinical and radiographic data were evaluated retrospectively. Results : The clinical success rate according to the Prolo's functional and economic outcome scale was 86% in patients without degeneration and 83% in patients with Modic degeneration. The clinical outcomes in each group were 88% in type 1, 84% in type 2, and 73% in type 3. The bony fusion rate was 97% in patients without degeneration and 83% in patients with Modic degeneration. The bony fusion rate in each group was 81% in type 1, 84% in type 2, and 55% in type 3. The clinical success and fusion rates were significantly lower in patients with type 3 degeneration. Conclusion : The PLIF with TFC has been found to be an effective procedure for lumbar spine fusion. But, the clinical outcome and bony fusion rates were significantly low in the patients with Modic type 3. The authors suggest that PLIF combined with pedicle screw fixation would be the better for them.

Comparison of Fusion with Cage Alone and Plate Instrumentation in Two-Level Cervical Degenerative Disease

  • Objective : This study assessed the efficacy of anterior cervical discectomy and fusion (ACDF) with cage alone compared with ACDF with plate instrumentation for radiologic and clinical outcomes in two-level cervical degenerative disease. Methods : Patients with cervical degenerative disc disease from September 2004 to December 2009 were assessed retrospectively. A total of 42 patients received all ACDF at two-level cervical lesion. Twenty-two patients who underwent ACDF with cage alone were compared with 20 patients who underwent ACDF with plate fixation in consideration of radiologic and clinical outcomes. Clinical outcomes were assessed using Robinson's criteria and posterior neck pain, arm pain described by a 10 point-visual analog scale. Fusion rate, subsidence, kyphotic angle, instrument failure and the degenerative changes in adjacent segments were examined during each follow-up examination. Results : VAS was checked during each follow-up and Robinson's criteria were compared in both groups. Both groups showed no significant difference. Fusion rates were 90.9% (20/22) in ACDF with the cage alone group, 95% (19/20) in ACDF with the plate fixation group (p = 0.966). Subsidence rates of ACDF with cage alone were 31.81% (7/22) and ACDF with plate fixation were 30% (6/20) (p = 0.928). Local and regional kyphotic angle difference showed no significant difference. At the final follow-up, adjacent level disease developed in 4.54% (1/22) of ACDF with cage alone and 10% (2/20) of ACDF with plate fixation (p= 0.654). Conclusion : In two-level ACDF, ACDF with cage alone would be comparable with ACDF with plate fixation with regard to clinical outcome and radiologic result with no significant difference. We suggest that the routine use of plate and screw in 2-level surgery may not be beneficial.

한우 체세포를 이용한 핵이식에서 전기융합 방법이 융합율 및 배발달율에 미치는 영향 (Effect of Electric fusion Methods on Cell Fusion Rate and Embryo Development by Somatic Cell Nuclear Transfer in Korean Native Cattle(KNC))

  • 복제수정란 생산에 있어서 수핵란 내 체세포 주입 후 전기적인 융합은 필수과정인데, 이 과정을 거치는 동안 많은 수의 체세포 주입 난자가 융합에 실패하거나 lysis가 일어나게 된다. 본 실험에서는 한우 체세포를 이용하여 핵이식을 실시한 후 수핵세포질과 응합을 시도할 때 전기융합 방법에 따른 융합율과 배발달율을 검토하고자 실시하였다. 공여세포는 한우 귀 세포조직을 채취하여 0.05% trypsin과 EDTA가 첨가된 D-PBS로 세포를 분리한 후 DMEM 배양액으로 계대배양을 실시하여 사용하였다. 핵이식을 위하여 체외성숙시킨 난자를 탈핵용 micro pipette을 이용하여 투명대를 절개하고 수핵 난자의 극체 와 핵을 제거한 후 공여세포를 주입하였으며, 핵이식 수정란은 직접, 간접적인 전기적 자극으로 융합을 실시한 후 calcium iono-phore와 6-DMAP를 이용하여 활성화를 유도하였다. 활성화된 수정란은 38.5$^{\circ}C$, 90% $N_2$, 5% $CO_2$로 조정된 배양기에서 처음 3일은 0.3% BSA가 첨가된 CR1-aa 배양액에서, 배양 4일째부터는 10% FBS가 첨가된 CR1-aa 배양액에서 배양을 실시하였다. 본 연구결과를 요약하면 다음과 같다. 전기자극 융합방법에 따른 수핵난자의 융합율과 lysis율은 electric chamber를 이용하여 간접융합을 실시하였을 경우 51.6%와 10.7%를 보였고, needle을 이용하여 직접 융합을 실시하였을 경우 68.9%의 융합율과 11.5%의 lysis율을 보임으로써 needle을 이용한 직접융합 방법이 유의적으로 높은 융합율을 보였다(P<0.05). 융합 후 체외 발달과정을 살펴보면 난할율에 있어서 needle을 이용했을 시 80.3%로 chamber를 이용했을 시 73.2%보다 유의적으로 높은 난할율을 보였다. 초기배 발달단계인 2∼4세포기의 발달율 역시 needle을 사용한 구가 61.1%로 chamber를 사용한 구 54.1%보다 유의적으로 높은 차이를 보였다. 상실배 단계는 chamber를 사용한 구가 6.7%로 needle을 사용한 구 6.2%보다 약간 높았지만 유의적인 차이는 없었다. 하지만 이식 가능한 단계인 배반포배 발달율에 있어서는 needle을 사용하여 융합을 시도한 구가 26.3%로 chamber를 사용한 구 18.4%보다 유의적으로 높은 발달율을 보였다(P<0.05). electric chamber를 이용하여 전기융합을 시도시전류가 흐르는 wire와 주입된 체세포가 직각을 이루도록 정렬을 시키느라 많은 시간이 소요되고, 주입한 체세포가 세포질과 떨어진 난자는 전기자극을 주어도 융합이 일어날 수 없으며, 높은 전압을 사용하기 때문에 융합된 복제란의 lysis가 많이 발생하는게 가장 큰 단점으로 꼽을 수 있다. 하지만 미세조작기와 needle 방법을 이용하면 낮은 전압을 이용하여 융합을 시도하기 때문에 복제란의 lysis를 줄일 수 있고, 전극과 체세포 주입란을 정렬시키는 과정이 생략되어 시간이 절약되며, 결정적으로 주입된 체세포가 세포질과 떨어져 있더라도 미세조작기로 약간의 압력을 가하여 전기자극을 가할 수 있어서 보다 높은 융합율과 배반포배 발달율을 얻을 수 있기 때문에 needle을 이용한 직접적인 전기융합 방법이 체세포 복제수정란 생산에 효과적이라고 사료된다.
