Concomitant use of corticosteroid and antimicrobials for liver abscesses in patients with chronic granulomatous disease

  • Chronic granulomatous disease (CGD) is a rare inherited disorder caused by defective nicotinamide adenine dinucleotide phosphate oxidase enzyme and characterized by recurrent bacterial and fungal infections. Although liver abscess is a common manifestation of CGD, its management in CGD patients is not well-defined. In addition, the generalized guidelines for treating liver abscesses do not necessarily apply to CGD patients. Corticosteroids are commonly used to control granulomatous complications, such as inflammatory gastrointestinal and genitourinary lesions, in patients with CGD, Corticosteroids have also been used in combination with antimicrobials to treat refractory infections in patients with CGD. Because corticosteroids are capable of suppressing symptomatic inflammation, all potential infections must be adequately controlled prior to corticosteroid initiation. We report 3 typical CGD cases with liver abscesses refractory to conventional treatments that were successfully treated with the concomitant use of corticosteroid and antimicrobials. It remains unclear whether corticosteroid therapy is required for liver abscesses in CGD refractory to conventional treatments. However, based on our observations, use of corticosteroids in combination with optimal antimicrobials should be considered for refractory liver abscesses in CGD.

  • The maxilla rarely undergoes necrosis due to its rich vascularity. Maxillary necrosis can occur due to bacterial infections such as osteomyelitis. viral infections such as herpes zoster and fungal infections such as mucormycosis, aspergillosis etc. Herpes zoster is a common viral infection, the oral soft tissue manifestations of which are widely known and recognized. Extremely rare complications such as osteonecrosis, and secondary osteomyelitis in maxilla were observed. But, reports of spontaneous tooth exfoliation and jaw osteonecrosis following herpes zoster infection in the distribution of the trigeminal nerve are extremely rare in the literature. We report a case of maxillary necrosis by herpes zoster in an uncontrolled diabetic patient. There was extensive necrosis of the buccal and palatal mucoperiosteum and exposure of the alveolar bone. This patient was successfully treated using a removal of necrotic bone and nasolabial flap. We briefly discuss different diseases which can lead to maxillary necrosis and a review. Analysis of the pathogenesis of herpes zoster and bone necrosis are discussed.

국내 닭 전염성 질병에 관한 역학적 조사 연구 (Studies on Epidemiological Survey of Infectious Disease of Chicken in Korea)

  • 한국 양계의 성장기이었던 1971년부터 1988년까지 최근 18연간 국내 양계농가로부터 농촌진흥청 가축위생연구소에 의뢰된 닭 가검물의 병성감정결과 질병검색된 총 9,012건 중 전염성 질병으로 진단된 6,181건에 대하여 검색 연도별, 계절별 및 일영별 검색상황을 조사 분석하였던 바 다음과 같은 결과를 얻었다. 1. 연도별 전염성 질병의 검색률은 49.3%(1973연)∼78.3%(1985년)로써 연평균 68.6%로 나타나 비전염성 질병(31.42%)보다는 월등히 높았다. 2. 전염성 질병 중에는 세균성이 32.6%(16.2∼42.5%)로 가장 높았고 다음 바이러스성 질병이 26.3%(21.8∼38.1%)이었으며 기생충성 및 곰팡이성 질병은 각각 7.7% 및 2.1%의 낮은 검색율을 나타내었다. 3. 세균성 질병으로는 마이코프라스마병, 대장균증 및 포도상구균증의 연평균검색률이 각각 8.8%, 8.5%, 5.8%로 가장 높았으며, 다음 살모넬라균증, 추백리, 난황염 및 수란괌염이 각각 0.8∼1.5% 이었고 피부염(0.2%) 및 연쇄상구균증(0.1%)은 매우 낮은 빈도로 검색되었다. 4. 바이러스성 질병중에는 임파성 백혈병 및 마렉병이 각각 연평균 7.5% 및 7.2%로 가장 많았고 다음 뉴캣슬병(4.4%), 전염성 후두기관염(2.0%), 전염성 낭병(1. 7%) 및 조뇌척수염(1.0%)의 순이었으며 그 외 전염성 기관지염, 산란저하증 '76 및 봉입체간염 등이 각각 1.0%, 미만으로 검색되었다. 5. 기생충성 질병의 검색률에 있어서는 닭 콕시듐병이 연평균 4.5%로 가장 높았고 다음 회염증이 1.4%이었다. 그 외에 류코싸이토준병 흑두병, 맹장충증 및 외부기생충증이 각각 0.2∼0.7%의 낮은 비율로 검색되었다. 곰팡이성 질병으로는 곰팡이성 폐렴이 연평균 2.0%, 다음으로 캔디다증이 0.1% 검색되었다. 6. 계절별 전염성 질병의 검색은 하계 및 추계절에 각각 27.7% 검색되어 가장 높았으며 다음 동계(23.5%) 및 춘계절의 순으로 낮았다. 이들중 세균성, 바이러스성 및 곰팡이성 질병은 이와 유사한 경향이었으나 기생충성 질병은 하계 및 추계절에서의 검색률이 각각 34.3% 및 39.5%로 다른 계절에 비하여 월등히 높았다. 7. 세균성 포도장구균증은 추계절에서의 검색율이 가장 높았으며, 바이러스성 질병으로는 마렉병, 전염성 후두기관염 및 전염성 낭병은 하계절에, 그리고 임파성백혈병, 계두 및 산란저하증 '76은 추계절에서 가장 높았으며 전염성 기관지염은 동계절에 최고빈도로 검색되었다. 기생충성 질병에 있어서는 콕시듐병을 위시하여 회충증, 류코싸이토준병 및 흑두병등 대부분의 주요질병이 하계절 및 추계절에 가장 많이 검색되었다. 8. 닭 일영별 전염성 질병의 검색률은 육성계(30∼150일영)에서 47.4%의 가장 높은 검색률을 나타내었고 다음 성계(150일영이상)에서 35.3% 및 유추(30 일영미만)에서 17.3%의 순이었다. 이중 세균성 및 기생충성 질병은 육성계에서의 검색률이 가장 높았으나, 바이러스성 질병은 육성계 및 성계에서 거의 같은 비율로 높았으며 곰팡이성 질병은 유추에서 가장 높은 검색률을 나타내었다.

Cervical Spondylodiscitis Caused by Candida Albicans in Non-Immunocompromised Patient

  • Fungal infections of the spine are relatively uncommon. Moreover, cervical spondylodiscitis due to Candida albicans in non-immunocompromised patient is very rare. We report a case of Candida spondylodiscitis in a 64-year-old woman who complained of neck pain. The clinical feature and treatment option are presented with a review of pertinent literatures.

Enhanced Tolerance of Chinese Cabbage Seedlings Mediated by Bacillus aryabhattai H26-2 and B. siamensis H30-3 against High Temperature Stress and Fungal Infections

  • Two rhizobacteria Bacillus aryabhattai H26-2 and B. siamensis H30-3 were evaluated whether they are involved in stress tolerance against drought and high temperature as well as fungal infections in Chinese cabbage plants. Chinese cabbage seedlings cv. Ryeokgwang (spring cultivar) has shown better growth compared to cv. Buram-3-ho (autumn cultivar) under high temperature conditions in a greenhouse, whilst there was no difference in drought stress tolerance of the two cultivars. In vitro growth of B. aryabhattai H26-2 and B. siamensis H30-3 were differentially regulated under PEG 6000-induced drought stress at different growing temperatures (30, 40 and $50^{\circ}C$). Pretreatment with B. aryabhattai H26-2 and B. siamensis H30-3 enhanced the tolerance of Chinese cabbage seedlings to high temperature, but not to drought stress. It turns out that only B. siamensis H30-3 showed in vitro antifungal activities and in planta crop protection against two fungal pathogens Alternaria brassicicola and Colletotrichum higginsianum causing black spots and anthracnose on Chinese cabbage plants cv. Ryeokgwang, respectively. B. siamensis H30-3 brings several genes involved in production of cyclic lipopeptides in its genome and secreted hydrolytic enzymes like chitinase, protease and cellulase. B. siamensis H30-3 was found to produce siderophore, a high affinity iron-chelating compound. Expressions of BrChi1 and BrGST1 genes were up-regulated in Chinese cabbage leaves by B. siamensis H30-3. These findings suggest that integration of B. aryabhattai H26-2 and B. siamensis H30-3 in Chinese cabbage production system may increase productivity through improved plant growth under high temperature and crop protection against fungal pathogens.

Use of an Artificial Neural Network to Construct a Model of Predicting Deep Fungal Infection in Lung Cancer Patients

  • Background: The statistical methods to analyze and predict the related dangerous factors of deep fungal infection in lung cancer patients were several, such as logic regression analysis, meta-analysis, multivariate Cox proportional hazards model analysis, retrospective analysis, and so on, but the results are inconsistent. Materials and Methods: A total of 696 patients with lung cancer were enrolled. The factors were compared employing Student's t-test or the Mann-Whitney test or the Chi-square test and variables that were significantly related to the presence of deep fungal infection selected as candidates for input into the final artificial neural network analysis (ANN) model. The receiver operating characteristic (ROC) and area under curve (AUC) were used to evaluate the performance of the artificial neural network (ANN) model and logistic regression (LR) model. Results: The prevalence of deep fungal infection from lung cancer in this entire study population was 32.04%(223/696), deep fungal infections occur in sputum specimens 44.05%(200/454). The ratio of candida albicans was 86.99% (194/223) in the total fungi. It was demonstrated that older (${\geq}65$ years), use of antibiotics, low serum albumin concentrations (${\leq}37.18g/L$), radiotherapy, surgery, low hemoglobin hyperlipidemia (${\leq}93.67g/L$), long time of hospitalization (${\geq}14$days) were apt to deep fungal infection and the ANN model consisted of the seven factors. The AUC of ANN model($0.829{\pm}0.019$)was higher than that of LR model ($0.756{\pm}0.021$). Conclusions: The artificial neural network model with variables consisting of age, use of antibiotics, serum albumin concentrations, received radiotherapy, received surgery, hemoglobin, time of hospitalization should be useful for predicting the deep fungal infection in lung cancer.

한방병원에 내원한 노장층 피부과 환자에 관한 임상적 고찰 (A Clinical Study of Elderly Patients with Skin Diseases)

  • Background: It is axiomatic that average life span is prolonged in proportion to the advance of socio-economic improvements and medical science. Consequently, the number of elderly patients with skin diseases is increasing. Objective: This study was performed to examine the patterns of skin diseases in elderly patients that had visited Dongguk Oriental Medical hospital, Dept. of Dermatology. We also compared these patients to the young generational groups. Methods: We studied 67 elderly patients with complaints of skin diseases who had visited our hospital from March 2000 to February 2002. We analysed their skin diseases and compared them with the younger group's. Results: The results were as follows; 1. The common disease groups were eczema(32.8$\%$); pruritus(23.9$\%$); viral infections (13.4$\%$); psoriasis(9.0$\%$); fungal infections and urticaria(6.0$\%$). 2. The disorders prevalent in men were eczema(30.0$\%$); herpes zoster & pruritus(23.3$\%$); fungal infection(13.3$\%$); psoriasis (3.3$\%$); melasma(6.7$\%$). 3. The common diseases of women's group were observed in the following order; eczema(35.l$\%$); pruritus(24.3$\%$); psoriasis(13.5$\%$); urticaria(10.8$\%$); other dermatoses(8.1$\%$); herpes zoster(5.4$\%$). 4. The most frequent skin disease for those in their 50's was eczema(32.3$\%$); 60's, pruritus(36.8$\%$); 70's, eczema(50.0); and over eighties., eczema and pruritus(40.0$\%$). Conclusion: This study suggests that dermatologic problems in the elderly are very common, especially eczema, pruritus. It is a very different pattern from the younger group's. And we have to better consider more effective management and treatment for them; especially further dermatologic studies including substantial medical care. Usually Oriental medicine is good at treating chronic diseases and less harmful. So we expect it to be generally better for elderly patients than western medicine.

Chemical, Pharmacological and Computerized Molecular Analysis of Stem's Extracts of Bauhinia scandens L. Provide Insights into the Management of Diarrheal and Microbial Infections

  • Bauhinia scandens L. (Family: Fabaceae) is commonly used to treat cholera, diarrhea, asthma, and diabetes disorder in integrative medicine. This study aimed to screen the presence of phytochemicals (preliminary and UPLC-QTOF-M.S. analysis) and to examine the pharmacological activities of Bauhinia scandens L. stems (MEBS) stem extracts. Besides, in silico study was also implemented to elucidate the binding affinity and drug capability of the selected phytochemicals. In vivo anti diarrheal activity was investigated in mice models. In vitro, antibacterial and antifungal properties of MEBS against several pathogenic strains were evaluated using the disc diffusion method. In addition, in silico study has been employed using Discovery studio 2020, UCFS Chimera, PyRx autodock vina, and online tools. In the anti-diarrheal investigation, MEBS showed a significant dose-dependent inhibition rate in all three methods. The antibacterial and antifungal screening showed a remarkable zone of inhibition, of the diameter 14-26 mm and 12-28 mm, by MEBS. The present study revealed that MEBS has remarkable anti-diarrheal potential and is highly effective in wide-spectrum bacterial and fungal strains. Moreover, the in silico study validated the results of biological screenings. To conclude, MEBS is presumed to be a good source in treating diarrhea, bacterial and fungal infections.

The Major Postharvest Disease of Onion and Its Control with Thymol Fumigation During Low-Temperature Storage

  • Onion (Allium cepa L.) is one of the major vegetable crops in Korea that are damaged and lost by pathogenic fungal infection during storage due to a lack of proper storage conditions. The aim of this study was to determine an appropriate control measure using thymol to increase the shelf life of onions. To control fungal infections that occur during low-temperature storage, it is necessary to identify the predominant fungal pathogens that appear in low-temperature storage houses. Botrytis aclada was found to be the most predominant fungal pathogen during low-temperature storage. The antifungal activity of the plant essential oil thymol was tested and compared to that of the existing sulfur treatments. B. aclada growth was significantly inhibited up to 16 weeks with spray treatments using a thymol solution. To identify an appropriate method for treating onions in a low-temperature storage house, thymol was delivered by two fumigation treatment methods, either by heating it in the granule form or as a solution at low-temperature storage conditions (in vivo). We confirmed that the disease severity was reduced up to 96% by fumigating thymol solution compared to the untreated control. The efficacy of the fumigation of thymol solution was validated by testing onions in a low-temperature storage house in Muan, Jeollanam-do. Based on these results, the present study suggests that fumigation of the thymol solution as a natural preservative and fungicide can be used as an eco-friendly substitute for existing methods to control postharvest disease in long-term storage crops on a commercial scale.
