기능적 전기 자극을 병행한 과제 지향적 훈련이 경수 손상 환자의 손 기능에 미치는 영향: 개별사례 연구 (Effects of Task-Oriented Training With Functional Electrical Stimulation on Cervical Spinal Cord Injury Patients' Hand Function: A Single-Subject Experimental Design)

  • 목적 : 본 연구는 기능적 전기 자극을 병행한 과제 지향적 훈련이 불완전 경수 손상 환자의 손 기능에 미치는 영향을 알아보고자 하였다. 연구방법 : 대상자는 불완전 경수 손상 진단을 받은 성인 3명으로, ABA 설계를 사용하였으며, 연구기간 동안 기초선(A1) 5회기, 중재기(B) 20회기, 재기초선(A2) 5회기로 총 30회기 실시하였다. 중재기(B) 동안 기능적 전기 자극을 병행한 과제 지향적 훈련이 제공되었고, 모든 회기에는 손 기능을 평가하기 위해 상자와 나무토막 검사(Box and Block Test), 떨어지는 막대 잡기(Grip the falling bar)와 먹기 흉내 내기(Simulated feeding)를 측정하였으며, 기초선(A1) 전, 재기초선(A2) 후로 Canadian Occupational Performance Measure(COPM), Jebsen-Taylor Hand Function Test(JTHFT)와 Wolf Motor Function Test(WMFT)를 실시하여 중재 효과를 살펴보았다. 결과 : 모든 대상자는 상자와 나무토막 검사(Box and Block Test), 떨어지는 막대 잡기(Grip the falling bar)와 먹기 흉내 내기(Simulated feeding)를 통해 기초선(A1)보다 중재기(B)에서 우세 손 기능이 향상되었고, 재기초선(A2)에서 그 효과가 유지되었다. 재기초선(A2) 후 COPM, JTHFT와 WMFT의 결과, 우세 손 기능의 향상과 과제 수행도와 만족도의 향상을 보였다. 결론 : 기능적 전기 자극을 병행한 과제 지향적 훈련은 경수 손상 환자의 손 기능뿐만 아니라, 대상자가 선택한 훈련 과제의 과제 수행도와 만족도 향상에 효과적이다.

반복적 양측 운동학습에 따른 대뇌 및 소뇌 피질 활성화 (Activations of Cerebral and Cerebellar Cortex Induced by Repetitive Bilateral Motor Excercise)

  • The aim of this study was to evaluate effects of short-tenn repetitive-bilateral excercise on the activation of motor network using functional magnetic resonance imaging (fMRI). The training program was performed at 1 hr/day, 5 days/week during 6 weeks. Fugl-Meyer Assessments (FMA) were performed every two weeks during the training. We compared cerebral and cerebellar cortical activations in two different tasks before and after the training program: (1) the only unaffected hand movement (Task 1); and (2) passive movements of affected hand by the active movement of unaffected hand (Task 2). fMRI was performed at 3T with wrist flexion-extension movement at 1 Hz during the motor tasks. All patients showed significant improvements of FMA scores in their paretic limbs after training. fMRI studies in Task 1 showed that cortical activations decreased in ipsilateral sensorimotor cortex but increased in contralateral sensorimotor cortex and ipsilateral cerebellum. Task 2 showed cortical reorganizations in bilateral sensorimotor cortex, premotor area, supplemetary motor area and cerebellum. Therefore, this study demonstrated that plastic changes of motor network occurred as a neural basis of the improvement subsequent to repetitive-bilateral excercise using the symmetrical upper-limb ann motion trainer.

The Biofeedback Scapular Stabilization Exercise in Stroke Patients Effect of Muscle Activity and Function of the Upper Extremity

  • Purpose: This study attempts to understand the effect of stabilization exercise of biofeedback scapular on muscle activity and functional evaluation of the upper extremity in stroke patients. Methods: Patients were divided into two groups; a biofeedback scapular stabilization exercise group comprised of 8 patients and a task-oriented training group including another 8 patients, and 30-minute exercise was performed 5 times a week for 8 weeks. Electromyogram was used to measure muscular activity of lower trapezius, deltoid middle, and serratus anterior. Fugl-Meyer Assessment and Manual Function Test were used to evaluate functions of the muscles mentioned. Results: Significant difference was observed in the comparison group before and after exercise in muscular activity of lower trapezius, deltoid middle, and serratus anterior, Fugl-Meyer Assessment, and Manual Function Test. Conclusion: Therefore, we could see that biofeedback scapular stabilization exercise is more effective than task-oriented training in facilitating muscle activation and functional capacity of upper limb.

Comparison of Usability and Prefrontal Cortex Activity of Cognitive-Motor Training Programs using Sensor-Based Interactive Systems

  • Objective: Cognitive-motor trainings had a positive impact on cognitive function and dual-task trainings led to improvements of global cognitive function. The brain activity of the prefrontal cortex (PFC) is another indicator that can infer cognitive function. This study aims to confirm the usability of the interactive system cognitive-motor training program and the changes in the prefrontal cortex through training. Design: Cross-sectional study Methods: In this study, two cognitive tasks were randomly applied to 20 adults as cognitive-motor training using an interactive system, and the same task was performed using the original method. During all tasks, the brain activity of the prefrontal cortex was measured by the change in oxyhemoglobin (HbO) in real-time using Functional Near-Infrastructure. After performing the tasks, the usability of the developed interactive system was evaluated by a usability questionnaire which consists of five items, and each item consists of a 7-point Likert scale that responds from 1 point to 7 points. Results: The HbO levels were increased during cognitive task performance than at the resting phase. And evaluating the usefulness of the interactive system, a questionnaire result showed that it would be useful for cognitive-motor programs. Conclusions: The cognitive-motor training using the interactive system increased the activity of the prefrontal cortex, and the developed wearable sensor-based interactive system confirmed its usefulness.

An Exploration of a Performer's Organic Action

  • This thesis explores the principle of a performer's organic action by means of his/her bodily responses on stage. This research has been developed to define the nature of a performer's central task in order to constitute empirical understanding of acting and the purpose of training in addressing the question of what sort of qualitative bodily training is necessary to be in a state of the full body involvement. This study investigates to articulate a performer's fundamental task at the most rudimentary level by utilizing those theatre artists' concepts with practical assumptions. In particular, the key terms, happen and change signifies the quality of a performer's body that has to fit into the given environment in which the performer's body can be subordinated through the moment on stage. Here, we argue that a performer's essential task parallel to make the following moment to happen and change by means of progressing a set of the next moment. In this manner, we also argue that a moment of displaying the performer's conscious effort, forceful and externalizing the visible elements under the use of erroneous language leads his/her body not to function on stage, a state of disengagement from his/her body. Finally, we provide a way to facilitate a performer's organic action by focused on his/her lived experience to create the functional moment which is opposite to the predominance of a representation, maintaining the performer's intellectual sense.

FES와 과제지향 훈련이 뇌졸중 환자의 복부근 두께와 균형에 미치는 영향 (The Effect of Task Oriented Exercise and Abdominal Muscle Contraction using FES on Abdominal Muscle Thickness and Balance of Stroke Patients)

  • Purpose: The purpose of this study was to evaluate the effect of task-oriented exercise and abdominal muscle contraction using functional electrical stimulation (FES) on abdominal muscle thickness and balance of stroke patients. Methods: Ten stroke patients who met the selection criteria were assigned randomly into two groups of five. One group received FES therapy before task-oriented training (experimental group), while the other group received a FES placebo before task-oriented training (control group). The Mann-Whitney U test was used to compare the groups, and the Wilcoxon Signed-ranks test was used to compare differences between the groups before and after intervention. The Mann-Whitney U test was used to compare the rate changes of each item before and after intervention, between the two groups. Results: In the rectus abdominalis and external oblique muscle thickness tests that used ultrasound, there was a statistically significant difference in the experimental group (p<.05),but no significant difference in the control group (p>.05). There was also a significant difference between the groups (p<.05). In the Balance test that used the Berg Balance Scale (BBS) and timed up and go test (TUG), there was a statistically significant difference in the experimental and control groups (p<.05), but there was no significant difference between the groups (p>.05). Conclusions: FES therapy before task-oriented training increases the thickness of abdominal muscles and improves balance abilities.

  • The purpose of this study was to characterize lesion development, neural plasticity, and motor learing after spinal cord injury. Facilitatory intervention such as weight bearing and locomotor training after SCI may be more effective than compensatory strategies at inducing neuroplasticity and motor recovery. Minimal tissue sparing has a profound impact on segmental systems and recovery of function Spinal animal could functional locomotion when subjected to repetitive stimulation. task-specific learning of isolated lumbar spinal could improve motor performance more then other task learning.

Effects of Elastic Resistance Training on Biomechanical Ability in College Taekwondo Athletes

  • Purpose: This study compared the effects of the elastic resistance and general training for college Taekwondo athletes, and investigated the biomechanical ability and lower extremity function according to the movement speed of the knee joint. Methods: Twenty university student Taekwondo athletes participated voluntarily in this study. The subjects performed general resistance training for four weeks, followed by elastic resistance training for another four weeks. The biomechanical parameters during the fast and slow isokinetic conditions, Y-balance ability, and vertical jump ability were measured three times: before training, after general resistance training, and after elastic resistance training. Statistical analysis was performed under isokinetic conditions and the intervention effect. Results: The biomechanical ability differed significantly between fast and slow isokinetic conditions (p<0.05). An analysis of the training method revealed a significant difference in the maximum knee extension speed and the posterior-lateral direction of the Y-balance test (p<0.05). Conclusion: Elastic resistance training had a more positive effect on the functional task performance than biomechanical factors related to muscular strength compared to general resistance training. In addition, speed-related knee biomechanical information of subjects with strong physical abilities could be utilized in sports physiotherapy.

아급성기 뇌졸중 환자의 이중 과제 수행이 보행의 시·공간적 변수에 미치는 영향 (Effects of Performing Dual Task on Temporospatial Gait Variables in Subjects With Subacute Stroke)

  • Purpose: The purpose of this study was to examine the effects of performing a dual task on gait velocity, temporospatial variables, and symmetry in subjects with subacute stroke. Methods: The study included 14 independent community ambulators with gait velocity of 0.8m/s. The Korean mini-mental state examination, the Berg balance scale, the Trunk impairment scale, and the Fugl-Meyer assessment scale were used to recruit homogeneous subjects. Subjects performed a single task (10m ambulation at a comfortable speed) and a dual task (10m ambulation at a comfortable speed while carrying a water-filled glass). Gait variables were examined with the OptoGait system. Results: The findings of this study were as follows: 1) Gait velocity decreased significantly in the dual-task condition as compared to the single task condition. 2) There were no significant differences between the paretic and non-paretic stances. 3) Paretic swing decreased significantly in the dual-task condition as compared to the single task condition. 4) The non-paretic, double-limb support phase increased significantly in the dual-task condition as compared to the single- task condition. 5) There was no significant difference in temporal symmetry. 6) Non-paretic step length decreased significantly in the dual-task condition as compared to the single-task condition. 7) There was no significant difference in spatial symmetry. Conclusion: Performing dual tasks decreases gait velocity, paretic swing phase, and non-paretic step length, while it increases non-paretic double limb support. In addition, although there is no difference in temporospatial symmetry, there is high inter-subject variability in temporospatial symmetry. Thus, dual tasks should be selected in accordance with the functional level of the hemiplegic patient, and inter-subject variability of the individual should be considered when dual tasks are considered for gait-training of hemiplegic patients.

과제지향적순환훈련이 만성 뇌졸중 환자의 운동기능, 일상생활동작 및 삶의 질에 미치는 영향 : 사례연구 (Effects of Group Task-Oriented Circuit Training on Motor Function, ADLs and Quality of Life in Individuals with Chronic Stroke: A Case Study)

  • 본 연구의 목적은 만성뇌졸중 환자에게 운동학습이론을 기초한 물리치료와 작업치료를 병행한 집단 과제지향적 순환훈련 프로그램을 실시하여 운동기능, 일상생활동작과 삶의 질적인 변화를 알아보고자 하였다. 6명의 만성 뇌졸중 환자에게 31주간 실시하였다. Chedoke-McMaster 뇌졸중평가, 균형검사, 일상생활활동 운동 처리기술 평가(Assessment of Motor and Process Skills: AMPS)와 뇌졸중 영향척도 측정을 치료 전과 후에 수집하였다. 연구결과, 운동기능 손상영역, 균형과 뇌졸중 영향척도의 기분과 정서영역에서 통계적으로 유의하게 향상되었다. 일상생활활동 운동 처리기술 평가는 운동(motor)영역에서 6명 중 3명에게 처리(process)영역은 4명에서 훈련 후 향상되었다. 본 연구결과를 바탕으로 집단 과제지향적 순환훈련은 지역사회 만성 뇌졸중 환자의 신체적 기능 향상에 이점이 있다고 제언하는 바이다.
