The Abnormality of Posterior Default Mode Network in Medication-Naïve Attention-Deficit Hyperactivity Disorder Children : Resting State fMRI Study (약물 복용력이 없는 주의력결핍 과잉행동장애 아동에서의 뒤쪽 내정상태회로 이상 : 휴식상태 기능적 뇌자기공명영상 연구)

  • Objectives : Characteristic symptoms, including hyperactivity and easy distractibility, in children with attention-deficit hyperactivity disorder (ADHD) suggest that their brain status, even at rest, might differ from that of healthy children. This study was conducted in order to determine whether resting state brain activity is compromised in medication-naive children with ADHD. Methods : Twenty medication-naive children with ADHD (mean age $10.3{\pm}2.5$) and 28 age- and gender-matched healthy volunteers (mean age $10.3{\pm}2.0$) underwent measurements for resting state brain activity using functional magnetic resonance imaging (fMRI). Among resting state related-independent components (RSICs) extracted from fMRI data using independent component analysis, a significant difference in RSICs was observed between groups, using a mixed Gaussian/gamma model. Results : Except for IQ, which was higher in the healthy control group, no demographic difference was observed between the two groups (p<.001). Significantly less activation of one RSIC, which includes the bilateral precuneus/posterior cingulate cortex, occipito-temporal junction, and anterior cingulate cortex, was observed in the ADHD group, compared with the control group (p<.05). Conclusion : An abnormal RSIC, posterior default mode network (DMN), was observed in the medication-naive ADHD group. Results of our study suggest that abnormality of posterior DMN is one of the main pathophysiologies of ADHD.

Survival analysis of dental implants in maxillary and mandibular molar regions; A 4$\sim$5 year report ($\cdot$하악 대구치 부위에 식립된 임플란트의 생존율에 대한 후향적 연구)

  • Dental Implants have been proved to be successful prosthetic modality in edentulous patients for 10 years. However, there are few reports on the survival of implant according to location in molar regions. The purpose of this study was to evaluate the $4{\sim}5$ years' cumulative survival rate and the cause of failure of dental implants in different locations for maxillary and mandibular molars. Among the implants placed in molar regions in Gwangju Mir Dental Hospital from Jan. 2001 to Jun. 2002, 473 implants from 166 patients(age range; $26{\sim}75$) were followed and evaluated retrospectively for the causes of failure. We included 417 implants in 126 periodontally compromised patients, 56 implants in 40 periodontal healthy patients, and 205 maxillary and 268 mandibular molar implants. Implant survival rates by various subject factors, surgical factors, fixture factors, and prosthetic factors at each location were compared using Chi-square test and Kaplan-Meier cumulative survival analysis was done for follow-up(FU) periods. The overall failure rate at 5 years was 1O.2%(subject level) and 5.5%(implant level). The overall survival rates of implants during the FU periods were 94.5% with 91.3% in maxillary first molar, 91.1% in maxillary second molar, 99.2% in mandibular first molar and 94,8% in mandibular second molar regions. The survival rates differed significantly between both jaws and among different implant locations(p<0.05), whereas the survival rates of functionally loaded implants were similar in different locations. The survival rates were not different according to gender, age, previous periodontal status, surgery stage, bone graft type, or the prosthetic type.   • Purpose: To evaluate the result and complication of treatment using Locking Compression Plate (LCP) for fracture of proximal humerus. Materials and Methods: Between 2004 and 2006, 21 patients with two-part and three-part fractures of the proximal humerus were treated by LCP fixation. Their average age was 54.9 years. Postoperative mean follow-up period was 22.9 months. The reduction was qualified and complication were assessed with final radiographs. The functional outcome was evaluated by Neer's rating system. Results: By Neer's functional evaluation, mean score of shoulder function was 86.3 and 18 case (86%) had excellent or satisfactory results. There was one case of nonunion but no infection or avascular necrosis of the humeral head. No correlation was found between the final result and the type of fracture, age, gender or quality of reduction. Conclusion: We obtained satisfactory result of LCP fixation for fracture of proximal humerus in this study. LCP fixation for proximal humerus fracture is a reliable method to obtain satisfactory reduction, rigid fixation and early exercise.

Evaluation of the qualite of life related to oral health among elderly people in some elderly care facilities by OHIP-14 (OHIP-14를 이용한 일부 요양기관 노인들의 구강건강관련 삶의 질)

  • Objectives : The growing elderly population and social changes have fueled a rapid increase in demand for elderly care facilities, but health care services for the elderly, especially oral health services, have long been overlooked. The purpose of this study was to examine the oral health care of elderly residents in elderly care facilities in an effort to provide some information on the elderly's oral health care and the development of dental care programs geared toward institutionalized elderly people. Methods : The subjects in this study were 230 elderly people who were in elderly care facilities in Jeolla Province. An individual interview was held, and they got a dental checkup. As for data analysis, t-test and ANOVA were utilized, and Scheffe post-hoc test was employed. Results : 1. In relation to the subfactors of the quality of life related to oral health, the elderly people investigated got a mean of 4.58, 4.47, 4.38, 4.09, 3.94, 3.91 and 3.76 respectively in activity disorder, mental disorder, social disorder, mental inconvenience, functional disorder, physical pain and physical disorder. 2. Concerning the overall quality of life related to oral health, there were statistically significant gaps in this aspect according to gender, age, presence or absence of systemic disorder, presence or absence of eating difficulties, subjective health status, subjective oral health state and oral health concern. 3. The presence or absence of root caries had a significant relationship to physical pain and social disorder among the subfactors of the OHIP-14, but that had nothing to do with functional disorder, mental inconvenience, physical disorder, mental disorder and activity disorder. Conclusions : The oral health indexes of the institutionalized elderly people in Jeolla Province were measured, and what factors affected the subfactors of oral health was checked. As a result, there appeared a close relation ship between oral health and the quality of life. Therefore effective oral health plans that cater to the elderly should be carried out to improve elderly people's quality of life related to oral health who stay in long-term elderly care facilities. In the future, prolonged research should be implemented from diverse angles for the sake of institutionalized elderly people.

Reduction Loss after Extension Block Kirschner Wire Fixation for Treatment of Bony Mallet Finger (골성 추지 신전제한 K 강선 고정술 시행 후 정복소실)

  • Purpose: We investigated occurrence of reduction loss after extension block (EB) Kirschner wire fixation or additional interfragmentary fixation (AIF) and clinical results including extension lag of the distal interphalangeal joint for treating bony mallet finger. Methods: Forty-six patients were included with a mean follow-up of 28 months (range, 12-54 months). Twenty-seven patients were treated with EB K-wire fixation (Group A) while 19 patients were treated with AIF (Group B). We checked radiologic factors, such as amount of articular involvement, volar subluxation, mallet fragment angle, reduction loss, range of motion including extension lag, and functional outcomes using Crawford's criteria. Results: Reduction loss occurred in eight patients (17%). Differences in mean extension lag, age, preoperative volar subluxation and mallet fragment angle between patients with reduction loss and those with reduction maintaining were significant. However, there were no significant differences in gender, hand dominance, amount of articular involvement, AIF, or further flexion between reduction loss and reduction maintaining. As for patterns of displacement, there was a significant relationship between gap or step-off and extension lag. Using Crawford's evaluation criteria, functional outcomes were excellent in 31, good in 10, fair in 3, and poor in 2 patients. Conclusion: Reduction loss should be careful in older age, smaller mallet fragment angle and preoperative volar subluxation.

A Study on the Multi-dimensional Greening Preferential Factors for Securing Green Spaces in Cities (도시 녹지 공간 확보를 위한 건물의 입체녹화 선호요인에 관한 연구)

  • With a view to respond to urban climate change issues and improve the urban environment in a qualitative manner, the strategy of multi-dimensional greening of the traditional markets in the City of Seoul to secure the multi-dimensional green spaces can result in a significant effect in terms of landscaping, environment, and spatial aspect. The purpose of this study is to examine the elements of multi-dimensional greening for the multi-dimensional greening of buildings in order to enhance the greening function within urban phenomena and, with this, establish the criteria for success introduction of multi-dimensional greening and its qualitative expansion. For this, a total of 569 consumers of traditional markets have been interviewed, and the data from their questionnaires were analyzed through SPSS and AMOS software. The result of the analysis showed that the elements of the multi-dimensional greening of traditional markets, such as the economical, physical, functional, visual, and facility aspects all affected the consumer expectation in a positive manner. Also, depending on the gender of the customers of traditional markets, the ecological aspect among the elements of multi-dimensional greening had a significant difference. And, by the age, the functional, physical, and visual aspects of the elements of multi-dimensional greening showed a significant different. And, the expectation on the multidimensional greening of traditional markets also showed a statistically significant difference. By the level of education, there were significant differences in terms of the physical and visual aspects of the multi-dimensional greening elements. And, the level of expectation on the multidimensional greening also differed significantly.

A study on pattern of changes of Mibyeong recognition, prevalence and its management plan in Korean public : national survey (일반인의 미병 인식 변화 양상과 관리방안 조사)

  • Objectives : The purposes of this study were to investigate the recognition of Mibyeong, symptom, management plan and correlation between quality of life and health condition, and to suggest a plan for managing Mibyeong status. Methods : Participants were recruited based on the same sampling methods used on the previous study performed in 2013 based on area, gender and age. Questionnaires were collected by Gallup Korea professional surveyor through face-to-face interviews. The questionnaires contain questions about the recognition of Mibyeong, symptom, management plan and the quality of life in accordance with the relevant health conditions. Descriptive statistics were used for data analysis and the results were expressed as percentage ratios (%). Results : 1,100 of people were acquired in 2015. The responses for "First time to hear of Mibyeong" have shown that the percentage were reduced from 80% to 67% compared with 2013 data. The ratio of Mibyeong's symptoms in 2015 were similar to 2013. we suggested some ways to deal with the Mibyeong status including behavior adjustment (non smoking, non alcohol, control sleep pattern), herbal tea, health functional food, exercise, preventive health care (qi-gong, yoga), meditation, home health care medical device (hot-pack, seat device for fumigation, massager), and medical service (hospital, medical clinic oriental medical clinic). Almost people showed that positive opinion with them. There were significant correlation between quality of life with the health status rather than Mibyeong or disease group. Conclusions : This study was performed through scientific questionnaires collected in 2013 and 2015 to investigate people's understanding of Mibyeong as a present condition in Korean public. Some questions had significantly different responds between both years while others showed similar trends for both years. These results suggest that the concept of Mibyeong in oriental medicine could provide a management mechanisms that help people to manage the Mibyeong status.

  • Objectives : The purpose of this study was to reveal the prevalence of Mibyeong and its symptoms including fatigue, pain, sleep disturbance, dyspepsia, depression, anxiety and anger by using the national survey. Methods : Questionnaires were collected by Gallup Korea. Participants were chosen through stratified sampling method based on area, gender and age. Questionnaire was designated to confirm the recognition, managing of Mibyeong, investigation of life habit, medical history, basic information, QoL questionnaires (Short Form-12, EuroQol-5D) and understanding of Mibyeong medical service conditions. Generally all questionnaires were used for survey the Mibyeong status in public except QoL questionnaires. Questionnaires were fulfilled by professional surveyor as face to face interview. Descriptives was used for data analysis and the results were expressed as percentage ratios (%) Results : 1,101 of people were acquired in this study. Eighty point two (80.2%) percent of participants did not know the concept of Mibyeong accurately even though 80.6% complained of Mibyeong related symptoms. Among them, fatigue was accounted for the highest response (70.7%)in this study. Sixty point four percent of participants identified non-smoking, stop drinking, eating habits and sleeping habits as a way to manage their Mibyeong related symptoms. In addition, exercising (60.8%), visiting medical institution (58.4%) and taking health functional food (52.7%) were presented. Only 23.1% among people with symptoms Mibyeong visited medical facilities. Moreover, the quality of life was found to be significantly correlated with health status. Conclusions : This study could contribute to express the importance of announcing the concept of Mibyeong and status to Korean public. Moreover, more Mibyeong studies should be conducted in the future to evaluate the Mibyeong status objectively.

Study on the Factors Influencing Food Consumption by Food Frequency Qustionnaire of University Students in Taejon (대전지역 대학생의 식품섭취빈도에 영향을 미치는 요인 연구)

  • The purpose of this study is to analyze the foods which 424(male 171, female 253) university students surveyed consumed frequently and to evaluate and the factors affecting their food consumption patterns. The survey was conducted at the beginning of nutrition courses each semester, March and September, 1998, with the questionnaire composed of general information, food, drinking and smoking habits, nutrition knowledge/attitude and food frequency questionnaire. The rates of alcohol drinking in male and female students were 89.2% and 78.1% respectively, and the rates of smoking were 68.1% and 1.6% in males and female. The nutrition knowledge score was higher in females than in males, but the nutrition attitude score was not significantly different between the male and female groups. The foods frequently consumed among students were cooked rice(14.3/week), kimchi(11.1/week), coffee(5.7/week), vegetables in soup, jjigae and jorim(3.5/week), carbonated beverages(3.3/week), cooked mixed rice(3.2/week). Ramyun and chocolate·candies·biscuits were frequently consumed, too. There were several factors influencing food consumption patterns. These were gender, regularity of mealtimes, the status of alcohol drinking and smoking, residence type and the levels of nutrition knowledge and attitude. Males ate more frequently cooked rice, coffee, carbonated beverages, ramyun, functional beverages and ham·sausage, while females ate more frequently cooked mixed rice and fruits. Those who had the habits of irregular mealtimes seemed to eat more soft drinks, instant foods and snacks. These trends were also found in the alcohol drinking and smoking groups. High level groups for nutrition knowledge of attitude score chose raw yellow green and green vegetables, cooked mixed rice, soybeans and seaweeds more frequently than the other groups. On the other hand, low level groups for nutrition knowledge or attitude score were apt to eat carbonated beverages and ramyun more frequently. Therefore, more attention should be taken to males, having habits of irregular mealtimes, alcohol drinking and smoking, and low level groups for nutrition knowledge or attitude score so as to improve their health.

Design Development of the Recuperation Clothing using Polygonum Indigo and Traditional Design for New Silver Generation (발효쪽 소재와 전통디자인을 활용한 뉴실버세대의 요양복 개발)

  • In Korea, to deal with the housing needs of the elderly, the government operates free nursing homes and residential homes. And to deal with the needs for daily necessities, imported products or products for the disabled have been supplied for the elderly. The government has recognized the importance of silver industry in this rapidly aging society and has established strategies to vitalize industries related to seniors to deal with the seniors' need for food, clothing, and shelter. This study examined the necessity of nursing home gowns, and the development of nursing home gowns that are environmentally friendly, beautiful and functional for the new silver generation. For this study, literature review and Internet search were conducted regarding the status of silver fashion, necessity of nursing home gowns, emergence of new silver generation and change in lifestyle, and value and utilization of natural dye and traditional patterns. To produce nursing home gowns, fabric was dyed using fermented indigo dying, and 3 styles of nursing home gowns for each gender were designed in consideration of seasons. Lastly, these gowns were evaluated by fashion experts, medical personnel, hospital gown and silver wear experts, and natural dye experts. The results showed that the traditional Korean designs developed in this study were excellent and the fermented indigo dye was appropriate for the nursing home gowns. The development of nursing home gowns in this study will be utilized as basic material for the development of silver wear and nursing home gowns to improve the quality of life for the seniors.
