Attainment of the goals of functional electrical stimulation(FES) applications such as strengthening of muscle, increasing muscular endurance, improvement in joint range of motion or the reduction of spasticity does not insure that patients will be able to produce voluntary muscle contraction sufficient to maintain posture or produce purposeful movements. The purpose of this study is to see the effect of functional electrical stimulation on forced vital capacity and alternating motion rate in children with spastic cerebral palsy. This study divided 20 children with spastic cerebral palsy into two groups; functional electrical stimulation treatment group and control group. Functional electrical stimulation treatment group had 20min per day treatment three times a week for four weeks and the control group did not have any treatment. Before and after intervention, this study measured forced vital capacity and alternate motion rate(/peo/,/teo/) for all children. Forced vital capacity showed statistically significant increase for the group with functional electrical stimulation(p<.05) while the control group did not show any significant increase(p>.05). Alternate motion rate showed statistically significant increase for the group with functional electrical stimulation(p<.05) while the control group did not show any significant increase(p>.05). This result shows that functional electrical stimulation affected the ability of the children with spastic cerebral palsy who have decreased breathing and phonation capability.
Background: The purpose of this study is to investigate the effect of the intensive functional electrical stimulation(FES) on the improvement of the gait pattern of the chronic hemiplegic patients. Method: Six hemiplegic patients, who could walk independently but have equinovarus deformity during the gait cycle, participated in this study. The affected peroneus longus and tibialis anterior muscles of all subjects were stimulated for 2 weeks period (20 minutes duration, 6 times/day). We measured the activities (mean voltage) of those muscles during the walking, using dynamic EMG. Results: After treatment, there were significant improvements in the strength of peroneus longus and tibialis anterior muscles and the gait speed, but there was no interval change of the spasticity of plantar flexor. The mean voltages of two muscles are significantly increased in all the patients (p<0.05). Conclusion: The results showed that the intensive FES on affected peroneus longus and tibialis anterior muscles in chronic hemiplegic patients could be useful for the improvement of functional gait.
Objective: Bilateral movement training is an effective method for upper extremity rehabilitation of stroke. An approach to induce bilateral movement through functional electrical stimulation is attempted. The purpose of this study is to develop an EMG-triggered functional electrical stimulation device for upper extremity bilateral movement training in stroke patients and test its feasibility. Design: Feasibility and Pilot study design. Methods: We assessed muscle activation and kinematic data of the affected and unaffected upper extremities of a stroke patient during wrist flexion and extension with and without the device. Wireless EMG was used to evaluate muscle activity, and 12 3D infrared cameras were used to evaluate kinematic data. Results: We developed an EMG-triggered functional electrical stimulation device to enable bilateral arm training in stroke patients. A system for controlling functional electrical stimulation with signals received through a 2-channel EMG sensor was developed. The device consists of an EMG sensing unit, a functional electrical stimulation unit, and a control unit. There was asymmetry of movement between the two sides during wrist flexion and extension. With the device, the asymmetry was lowest at 60% of the threshold of the unaffected side. Conclusions: In this study, we developed an EMG-triggered FES device, and the pilot study result showed that the device reduces asymmetry.
기능적 전기자극(FES)에 의한 사지운동의 효과적인 제어를 위해서는, 전기자극을 입력으로 하여 근력 및 운동을 정확히 출력하는 근골격모델이 요망된다. 이 연구에서는 FES에 의한 근력 및 운동을 보다 정확히 예측할 수 있는 모델을 작성하기 위하여, 기존의 근육모델에서는 포함되지 않았던 근력의 점진적 강화현상에 대한 기초적 성질을 조사하는 것을 목적으로 한다. 구체적으로는, 일정강도의 표면자극에 대한 근력의 강화현상이 주파수, 자극이력, 근육길이에 어떻게 의존하는지를 조사하였다. 실험결과로부터, 자극의 주파수가 높을수록 초기근력에 대한 자극중의 근력의 증가도는 작아지고 근력의 피크에 도달하는 시간이 짧아지는 것을 알 수 있었다. 선행 자극에 의해 근육의 내부적인 강화상태가 포화되면 근력은 추가적인 자극에 대해서도 더 이상 증가하지 않았다. 자극시의 근육의 길이는 근력강화에 큰 영향을 미쳤으며, 근육의 길이가 짧을수록 증가도가 컸다. 장래에는 이러한 결과를 토대로 한 새로운 근력강화의 모델이 요망된다.
Two patients, diagnosed as cervical spinal cord injury, were hospitalized and treated by Korean medical interventions with Functional Electrical Stimulation (FES). The range of motion of the shoulder joint, manual muscle test were adopted to measure the resulting motor recovery after treatment. And the Korean version of Barthel index (K-MBI) was adopted as a method of measuring the improvement of quality of life after treatment. The range of motions and muscle strengths of the shoulder joints were improved. The activities of daily living were slightly improved in case 2, but showed no improvement in case 1. FES was effective for upper extremities rehabilitation of spinal cord injured patients. Further studies are needed to set up an Korean medical protocol for spinal cord injury rehabilitation.
In this paper, a DSP and microcomputer-based EMG controlled functional electrical stimulation (FES) system, for restoring walking of paraplegics at the patients' own command, is presented. The above-lesion EMG is a time-varying nonstationary signal and its autoregressive (AR) parameters are identified by the nonstationary identification algorithm using a DSP chip. The identified AR parameters are used for the cloassification of the function and the control of the movement. The below-lesion response-EMG signal is used as a measure of muscle fatigue. This FES system is designed to measure muscle fatigue and control the stimulation intensity according to the amplitude of the response-EMG signal. While the automatic electrical intensity control is obtained by identifying the movement, the proposed FES system is suitable for the automatic control of paraplegic walking.
Purpose : This study was to investigate the prevention of spasticity with modified Ashworth scale(MAS) and range of motion(ROM) increase with goniometer in ankle joint by functional electrical stimulation(FES) and exercise for 4 weeks in chronic stroke patients. Methods : 60 chronic stroke patients participated in this study. The subjects were divided into 3 groups, FES group(n=20), FES+exercise group(n=20) and control croup(n=20). After FES application on tibialis anterior muscle by 35Hz for 4 weeks, the change of ankle joint movement was measured by goniometer. Results : The spasticity in ankle joint was decreased greatly in FES and FES+excercise groups(p<.05), and the ROM of ankle joint was increased greatly in FES and FES +exercise groups(p<.05) than control group(p>.05). Conclusion : It shows that FES made the angle of dorsiflexion in spastic ankle joint increase with functional improvement of tibialis anterior muscle in chronic stroke patients. This show that the FES is avaliable for facilitation of ROM and decrease of spasticity as a therapeutic tool.
Foot drop is a common symptom in stroke patients due to central nervous system (CNS) damage, which causes walking disturbances. Functional electrical stimulation (FES) is an effective rehabilitation method for stroke patients with CNS damage. Aim of this study was to determine the effectiveness of 6 weeks FES walking training based lower limb muscle synergy of stroke patients. Lower limb muscle synergies were extracted from electromyography (EMG) using a non-negative matrix factorization algorithm (NMF) method. Cosine similarity and cross correlation were calculated for similarity comparison with healthy subjects. In both stroke patients, the similarity of leg muscle synergy during walking changed to similar to that of healthy subjects due to a decrease in foot drop during. FES walking intervention influenced the similarity of muscle synergies during walking of stroke patients. This intervention has an effective method on foot drop and improving the gait performance of stroke patients.
