Comparison of Dysphagia Rehabilitation Therapy with Natural Recovery on Swallowing Ability among Acute Stroke Patients (연하재활치료의 효과와 자연회복과의 비교연구 - 급성기 뇌졸중 환자를 중심으로 -)

  • The purpose of this study was to compare the effectiveness of rehabilitation therapy with natural recovery in patients who had dysphagia due to stroke. Swallowing functions were examined on a total of 98 patients with stroke-caused dysphagia based on a videofluoroscopic swallowing study (VFSS) for 8 weeks. The each group was evaluated with DOSS, DSS and FOSS to assess swallowing ability in the first week and re-evaluated in the $8^{th}$ week during the session for investigating the recovery status. The author compared the above initial data and follow-up data using a Mann-Whitney U test, Kruskal-Wallis test and Wilcoxon's Signed Rank test, ANCOVA to confirm statistically controlling for the effects of other continuous variables, performed by IBM SPSS Statistics 20.0 for windows. There was statistically significant recovery in the experimental group except measure of DOSS (p>0.05). In the control group, it showed improvement in all the scales(p<0.05). After terminating each session, the comparison of the two group of patients showed statistical significant differences in DSS and FOSS(p<0.05). These results suggest that possibility of natural neurologic recovery has positive effects on early stage of dysphagia caused by stroke.

Morphologic Alterations in Amygdala Subregions of Adult Patients with Bipolar Disorder

  • Objectives Previous studies have revealed inconsistent results on amygdala volume in adult bipolar disorder (BD) patients compared to healthy controls (HC). Since the amygdala encompasses multiple subregions, the subtle volume changes in each amygdala nucleus might have not been fully reflected in the measure of the total amygdala volume, causing discrepant results. Thus, we aimed to investigate volume changes in each amygdala subregion and their association with subtypes of BD, lithium use and clinical status of BD. Methods Fifty-five BD patients and 55 HC underwent T1-weighted structural magnetic resonance imaging. We analyzed volumes of the whole amygdala and each amygdala subregion, including the anterior amygdaloid area, cortico-amygdaloid transition area, basal, lateral, accessory basal, central, cortical, medial and paralaminar nuclei using the atlas in the FreeSurfer. The volume difference was analyzed using a one-way analysis of covariance with individual volumes as dependent variables, and age, sex, and total intracranial volume as covariates. Results The volumes of whole right amygdala and subregions including basal nucleus, accessory basal nucleus, anterior amygdaloid area, and cortico-amygdaloid transition area in the right amygdala of BD patients were significantly smaller for the HC group. No significant volume difference between bipolar I disorder and bipolar II disorder was found after the Bonferroni correction. The trend of larger volume in medial nucleus with lithium treatment was not significant after the Bonferroni correction. No significant correlation between illness duration and amygdala volume, and insignificant negative correlation were found between right central nucleus volume and depression severity. Conclusions Significant volume decrements of the whole amygdala, basal nucleus, accessory basal nucleus, anterior amygdaloid area, and cortico-amygdaloid transition area were found in the right hemisphere in adult BD patients, compared to HC group. We postulate that such volume changes are associated with altered functional activity and connectivity of amygdala nuclei in BD.

The impact of nutrition literacy on the food habits among young adults in Turkey

  • BACKGROUND/OBJECTIVES: Youth is the crucial stage between childhood and adulthood during which an individual acquires new behavior and practices including dietary habits, that may be influenced by his peers, social circle, nutrition knowledge level and other factors. The increase in awareness of young adults is of special importance from the perspective of health and prevention of obesity related chronic diseases. To measure the nutrition literacy level of university students using the Turkish version of Adolescent Nutrition Literacy Scale (ANLS). We evaluated their food habits using the Adolescent Food Habit Checklist (AFHC) tool and analysed it, if the eating habits were affected by nutrition literacy level. SUBJECTS/METHODS: A cross-sectional descriptive study was conducted on randomly selected 276 students studying in the Faculty of Health Sciences of Istanbul Aydin University, Turkey. Data was collected by means of ANLS and AFHC tools by face to face interview method. SPSS statistical sofware (IBM SPSS Statistics 19) was used to analyse the obtained data. RESULTS: Participants comprised of 47.1% males and 52.9% females with mean age of $20.0{\pm}1.60years$. Mean body mass index (BMI) was determined as $22.4{\pm}3.76$ ($23.6{\pm}3.63$ males $21.3{\pm}3.57$ females). Functional, interactive and critical nutritional literacy scores for male participants were $22.49{\pm}5.71$, $17.45{\pm}4.84$, and $28.28{\pm}7.13$ respectively (total $64.98{\pm}10.15$). For females the values were $24.66{\pm}5.13$, $20.17{\pm}4.28$, and $32.20{\pm}5.65$ (total $69.72{\pm}8.59$) respectively. For all the three sub-categories of nutrition literacy, the scores acquired by females were significantly higher than males (P < 0.001). In the AFHC tool, the mean score acquired by males were $9.26{\pm}4.18$ whereas for females it was $10.37{\pm}3.40$ significantly higher as compared to males (P = 0.007). CONCLUSION: The food habits of female participants were better than males; in accordance with their nutrition literacy status. It may be concluded that food habits are influenced by nutrition literacy and therefore stress much be given to increase the nutrition awareness among children youth as well as adults.

An Integrated Review on Main Caregiver's Burden of Elderly in Korean Nursing Home (한국 요양시설 노인의 주 돌봄자 부담감에 대한 통합적 고찰)

  • The purpose of this study is to investigate the burden of caring for the care-givers of the elderly in Korea by using an integrated literature review method. A total of 23 studies were analyzed using a search database. When care-givers had higher sense of filial and guilt or more than two diseases, they showed high level of feeling of burden. Also, they had high level of feeling of physical burden by their oldness, service period (especially at the period of 1-3 years). In the feeling of economic burden, they had high level of feeling of burden by their oldness, or elderly's disease periods. The feeling of burden by psychological condition was found in elderly in aged and the beginning of admission of nursing home. The feeling of burden by environment situation was found when the functional status of the elderly was bad. Therefore, we need to concern care-givers's feeling of burden with elderly people in the nursing home. In the future, I believe that the findings of this study will be helpful for development of the intervention program for alleviate burden for the care-giver.

A Study on the Special Needs of the Hearing-Impaired Person for Disaster Response (청각장애인 재난대응 욕구에 관한 연구)

  • This study evaluated the actual status of special needs of the hearing-impaired person for disaster response. The analysis revealed a significant level of unmet needs in disaster response for hearing-impaired person. The 5 special needs in disaster response include: 1) communication needs, which involve securing the means to make an emergency rescue request and communicating information during the rescue process; 2) transportation needs, which indicate the effective evacuation capacity and the level of training; 3) medical needs, which address the degree of preparedness for physical and mental emergency measures and the delivery of health information for rescue and first aid process; 4) maintaining functional independence needs, which refer to the level of self-preparedness to minimize damage in disaster situations, and; 5) supervision needs, which correspond to a personalized support system provided to disaster-vulnerable groups.

A Study on Current Status and Improvement Tasks of Records Classification in Government-funded Research Institutes (정부출연연구기관의 기록분류 현황과 개선과제)

  • The purpose of this study is to find out the level of the classification system of the government-funded research institutes and to search the future direction of the classification scheme of these research institutes. As a result of analyzing the classification system and regulations of the 23 government-funded research institutes, it is found that the regulations of the records management of most institutions are basically based on other regulations before Public Records Act. In addition, there are not many institutions in which the record production system that reflects the Public Records Act is introduced, and the production and classification of the records are performed separately, and the classification scheme in which the organization classification and the functional classification are mixed is often used only as a reference tool for setting retention period. To overcome this situation, it is necessary to improve the regulation, classification scheme and system. The revision of the law at the national level and establishment of records management regulations at the institutional level should be made. A special classification scheme for 'research' which is a core function of the research institute should be designed and this classification scheme must be applied from the registration stage of records. The fact that the record management obligation is ignored and the production records are not properly managed is to deny the value of the research field records by themselves. I hope that the records management of government-funded research institutes will be moved in place as soon as possible and practical issues will be communicated to academia and good ideas for better development will be returned to the field of practice.

Factors Affecting Emotional Status and Suicidal Risk in Hospitalized Medical Patients (일 종합병원 입원 환자의 자살 위험도 및 정서 상태에 영향을 미치는 요인 연구)

  • Objectives : This study aimed to identify the group with high risk of suicide in general hospital inpatient by comparing the depression, anxiety, and distress and their clinical characteristics. Methods : The study included all patients admitted to a general hospital between January 2016 and December 2017, and the final 41,249 patients were analyzed. Distress thermometer (DT) and hospital anxiety-depression scale (HADS) were used for emotional state evaluation. Group with high risk of suicide was defined as showing more than 4 in the DT and more than 8 in one of the two subscales (anxiety and depression) of the HADS. Results : Among the subjects, 3,603 patients (8.7%) met the high-risk criteria for suicide. The group with high risk of suicide experienced more severe pain than the control patient group, and it was found that there was more underlying disease. Patients who reported pain showed higher scores than those who reported no pain on the DT and HADS subscales. Conclusions : Group with high risk of suicide showed more severe pain and more underlying disease. Therefore, suicide prevention program specifically targeted for the group with high risk of suicide should be developed according to the risk of the patient evaluated at the time of the admission.

Status and prospect of safety evaluation of genetically modified microorganism (GMM) for domestic and foreign food application (국내·외 식품용 유전자변형미생물 안전성 심사 현황 및 전망)

  • With the breathtaking stride beingmade in the field of biotechnology, biocatalyst research using genetically modified microorganism (GMM) is actively being pursued in food industry. However, domestic food and food additive regulation standards and the number of examination management examples currently used in industry is lacking significantly. Up till now, there are only 6 examples of domestic GMM examination and approval cases for food production purposes and furthermore they are limited to the production of functional sweeteners. Domestically, although GMM is developed as a processing aid (contained use), if they are used in the production of food, the safety of GMM, including environmental safety, is evaluated. Also the produced food or food additives using GMM need to be separately examined and approved as a novel food. On the other hand, imported products produced using GMM need to gain approval for the final product only. Thus the expense and the time to obtain regulatory approval is advantageous for imported products versus domestically produced products. This commentary is written to create the opportunity to reform the current domestic food GMM regulation by comparing and discussing domestic and foreign case analyses of safety evaluation of GMM and related regulations.

Negative Regulation of Tumor Suppressor p53 at the Promoter Regions of Oncogenic SETDB1 and FosB Genes (암종양유전자 SETDB1과 FosB 발현에 대한 p53의 음성 조절기작)

  • Treatment with anticancer drugs changes the expression of multiple genes related to cell proliferation, migration, and drug resistance. These changes in gene expression may be connected to regulatory networks for each other. This study showed that doxorubicin treatment induces the expression of oncogenic FosB and decreases the expression of oncogenic SETDB1 in A549 and H1299 human lung cancer cells, which are different in tumor suppressor p53 status. However, a small difference was detected in the quantitative expression of those proteins in the two kinds of cells. To examine the potential regulation of SETDB1 and FosB by p53, we predicted putative p53 binding sites on the genomic DNA of SETDB1 and FosB using a TF motif binding search program. These putative p53 binding sites were identified as 18 sites in the promoter regions of SETDB1 and 21 sites in the genomic DNA of FosB. A luciferase assay confirmed that p53 negatively regulated the promoter activities of SETDB1 and FosB. Furthermore, the results of RT-PCR, western blot, qPCR, and immunostaining experiments indicated that the transfection of exogenous p53 decreases the expression of SETDB1 and FosB in H1299 cells. This indicates that p53 negatively regulates the expression of SETDB1 and FosB at the transcriptional level. Collectively, the downregulation of SETDB1 and FosB by p53 may provide functional networks for apoptosis and for the survival of cancer cells during anticancer drug treatment.

Current status and prospects of chitosan for industrial applications (키토산 관련 산업의 현황과 전망)

  • For the development of the chitosan industry in Korea, the catch of red snow crabs caught on the east coast is rapidly decreasing. Therefore, it is urgent to develop raw materials that can replace the red snow crab as the top priority to solve the supply and demand problems, as well as wastewater treatment costs account for a large proportion of the cost of chitosan. In order to solve the problems, continuous research on biological extraction methods such as enzymatic extraction and microbial fermentation will increase production efficiency and lower unit cost as well as chemical extraction methods. Further efficient manufacturing method can be established. Establishing of novel techniques is indispensable for production of high-purity chitosan and the ability to regulate and separate the molecular weight, as well as joint research with industry, academia and research institute for the research and development of high-functional chitosan derivatives.
